    Chemotherapy Induced Neutropenia and Increased Risk for Sepsis

    Available treatment options for patients diagnosed with cancer include surgery, the administration of chemotherapeutic agents, radiation therapy, or a combination of these modalities. Chemotherapeutic agents utilized to treat cancer have a variety of potential side effects. One of the most common and well known side effects associated with the administration of chemotherapeutic agents is neutropenia. Neutropenia places oncology patients at an increased risk of contracting infections which may lead to a life threatening complication known as sepsis. Sepsis is an extremely dangerous oncologic emergency requiring prompt medical treatment. Failure to recognize symptoms associated with sepsis in the neutropenic patient and initiate appropriate medical treatment significantly increases the risk of patient mortality

    Transcriptomes of parents identify parenting strategies and sexual conflict in a subsocial beetle

Such flexibility is expected to involve specialized behaviour by the two sexes under biparental conditions. Here, we show that offspring fare equally well regardless of the sex or number of parents present. Comparing transcriptomes, we find a largely overlapping set of differentially expressed genes in both uniparental and biparental females and in uniparental males including vitellogenin, associated with reproduction, and takeout, influencing sex-specific mating and feeding behaviour. Gene expression in biparental males is similar to that in non-caring states. Thus, being ‘biparental’ in N. vespilloides describes the family social organization rather than the number of directly parenting individuals. There was no specialization; instead, in biparental families, direct male parental care appears to be limited with female behaviour unchanged.     Predicting Discharge Walking Function With High-Intensity Stepping Training During Inpatient Rehabilitation in Nonambulatory Patients Poststroke

    Objective This cohort investigation identified primary predictors of discharge walking function of nonambulatory individuals poststroke with high-intensity training (HIT) during inpatient rehabilitation. Design Observational cohort investigation. Setting Inpatient rehabilitation. Participants Data were collected from individuals (N=257) <6 months poststroke who required assistance to walk at admission. Intervention Clinical physical therapy interventions attempted to maximize stepping practice at higher intensities. Main Outcome Measures Primary outcomes included the discharge level of assistance required during walking (minimal or no assistance) and attainment of specific gait speed thresholds (0.4 and 0.8 m/s) during the 10-m walk test. Independent predictors were demographics, training interventions (including steps/day), baseline Berg Balance Scale (BBS), and paretic leg strength. Results Participants performed a median (interquartile range) of 1270 (533-2297) steps per day throughout inpatient rehabilitation, with significant differences between those who walked with versus without assistance at discharge. Logistic regressions indicate steps per day was a primary predictor of unassisted walking recovery; removal of steps per day resulted in primary predictors of baseline BBS and strength. Receiver operating characteristic (ROC) analyses indicate significant areas under the curve for BBS and relatively low cutoff scores of 5.5 points at admission to walk without assistance at any speed. ROC analyses performed using 1-week outcomes indicate BBS scores of 5-17 points were needed to achieve locomotor thresholds. Conclusion Stepping activity, BBS, and paretic leg strength were primary predictors of walking outcomes in patients performing HIT, and ROC analyses indicated recovery of independent walking could be achieved in low functioning patients early poststroke

    What kind of a problem is loneliness? Representations of connectedness and participation from a study of telepresence technologies in the UK

    Loneliness is represented in UK policy as a public health problem with consequences in terms of individual suffering, population burden and service use. However, loneliness is historically and culturally produced; manifestations of loneliness and social isolation also require social and cultural analysis. We explored meanings of loneliness and social isolation in the UK 2020–2022 and considered what the solutions of telepresence technologies reveal about the problems they are used to address. Through qualitative methods we traced the introduction and use of two telepresence technologies and representations of these, and other technologies, in policy and UK media. Our dataset comprises interviews, fieldnotes, policy documents, grey literature and newspaper articles. We found loneliness was represented as a problem of individual human connection and of collective participation in social life, with technology understood as having the potential to enhance and inhibit connections and participation. Technologically-mediated connections were frequently perceived as inferior to in-person contact, particularly in light of the enforced social isolation of the COVID-19 pandemic. We argue that addressing loneliness requires attending to other, related, health and social problems and introducing technological solutions requires integration into the complex social and organisational dynamics that shape technology adoption. We conclude that loneliness is primarily understood as a painful lack of co-presence, no longer regarded as simply a subjective experience, but as a social and policy problem demanding resolution

    From the Wash to the White Cliffs:The Contribution of the Heritage Sector

    This report reviews the contribution of heritage to the region defined by the counties of Kent, Essex, Suffolk and Norfolk. It identifies four key themes that link the heritage in the region: coastal defence; Christian heritage; historic houses; and historic landscapes and natural heritage. The region contains one UNESCO World Heritage Site at Canterbury. Heritage is supported by the development of several Heritage Action Zones and High Street Heritage Action Zones across the four counties. Heritage features in the strategies for the two regional Local Enterprise Partnerships (LEP), as well as countywide and local authority heritage and cultural strategies. The report identifies examples of good practice. Several research themes have been identified that link to the interests of the three sponsoring universities of East Anglia, Essex, and Kent. Coastal heritage across the four counties is facing the threat of the climate crisis and assets are being lost due to coastal erosion. The impact of rising sea levels is also assessed. Heritage and cultural property crime affects the sustainability of heritage and cultural property across the region. Five case studies are presented: damage to churches, including lead roof theft; illegal metal-detecting and the disposal of finds; architectural theft; vandalism; and the use of technology to facilitate crime against heritage assets. The third research theme relates to the way that the DCI sector works with heritage organisation to record and interpret assets. The development of a county based Digital Heritage Strategy for Suffolk is highlighted. The economic benefits of heritage are explored through the award of National Lottery Heritage Fund (NLHF) grants to heritage projects. Between 2013 and 2020 the EARC region was awarded over £190 million for heritage projects by NLHF. In addition, the report explores visitor trends and identifies the impact of COVID-19 on the tourism economy for the region. Historic England estimates that the heritage sector accounted for 140,000 jobs in the south east, and eastern England in 2019. The social benefits of heritage align with the UK Government’s Levelling-Up agenda. This is explored through a number of sub-themes: health and well-being; pride in place; digital connectivity; education and skills. The report concludes with a reflection on the challenges facing heritage across the region. This includes encouraging public participation with museums and archives

    Terpene polyacrylate TPA5 shows favorable molecular hydrodynamic properties as a potential bioinspired archaeological wood consolidant

    There is currently a pressing need for the development of novel bioinspired consolidants for waterlogged, archaeological wood. Bioinspired materials possess many advantages, such as biocompatibility and sustainability, which makes them ideal to use in this capacity. Based on this, a polyhydroxylated monomer was synthesised from α-pinene, a sustainable terpene feedstock derived from pine trees, and used to prepare a low molar mass polymer TPA5 through free radical polymerisation. This polymer was extensively characterised by NMR spectroscopy (chemical composition) and molecular hydrodynamics, primarily using analytical ultracentrifugation reinforced by gel filtration chromatography and viscometry, in order to investigate whether it would be suitable for wood consolidation purposes. Sedimentation equilibrium indicated a weight average molar mass Mw of (4.3 ± 0.2) kDa, with minimal concentration dependence. Further analysis with MULTISIG revealed a broad distribution of molar masses and this heterogeneity was further confirmed by sedimentation velocity. Conformation analyses with the Perrin P and viscosity increment ν universal hydrodynamic parameters indicated that the polymer had an elongated shape, with both factors giving consistent results and a consensus axial ratio of ~ 4.5. These collective properties—hydrogen bonding potential enhanced by an elongated shape, together with a small injectable molar mass—suggest this polymer is worthy of further consideration as a potential consolidant
