    Herpes simplex virus (HSV) pneumonia in the non-ventilated immunocompromised host: Burden and predictors

    Objectives: To evaluate burden and predictors of HSV pneumonia among immunocompromised patients not undergoing invasive mechanical ventilation according to a tailored diagnostic algorithm. Methods: This prospective, observational study included immunocompromised adults with pneumonia non-responding to empirical antibiotic therapy. Bronchoalveolar lavage (BAL) specimens were cultured for bacteria, mycobacteria and fungi. Real-time PCR for Herpesviruses and other microorganisms were performed on BAL and other specimens. Cytological examination of BAL samples was carried out for identification of intranuclear inclusion bodies and immunohistochemical staining for HSV. Results: We enrolled 45 patients (mean age 64.6 years) from January 2015 to June 2016. Nineteen (42.2%) cases tested positive for HSV-1 PCR on BAL. According to our definitions, 11 (24.4%) patients had HSV- 1 pneumonia with viral loads ranging between 10 3 copies/mL and 10 7 copies/mL. HSV-1 positive throat swab (OR 85.2, 95% CI 5.83\u20131245.1, P < 0.001) and solid organ transplant (SOT) (OR 53.3, 95% CI 1.37\u20132072.8, P < 0.03) as underlying condition were found to be independently associated with HSV pneumonia by multivariable analysis. Conclusions: HSV pneumonia turned out to be relatively common and should be investigated especially in individuals with HSV positive throat swab and SOT. Interventional studies are needed to assess the real clinical impact of HSV pneumonia in immunocompromised patients

    Anxiety and Depression Prevalence Rates in Age-Related Macular Degeneration

    PURPOSE. To estimate the prevalence rates of depression and anxiety in patients with wet age-related macular degeneration (AMD) and the relationship with visual acuity and to develop a simple algorithm for depression screening. METHODS. This cross-sectional, prospective, observational, multicenter study was performed in France, Germany, and Italy. Retina specialists at 10 centers per country each enrolled 12 consecutive patients with wet ARMD. Patients were stratified into four severity groups by using best eye (BE) and worst eye (WE) visual acuity (VA) thresholds (BE:VA 20/40 and WE:VA 20/200). Patients rated themselves on the Hospital Anxiety and Depression Scale (HADS). Analysis of variance was performed to estimate the effect of VA severity levels on HADS scores adjusted on age, gender, and country. RESULTS. Patients (females 609/6) were recruited, with a mean age of 77 years and 2.3 years' disease duration. Mean BE:VA at inclusion was 0.49 logMar (logarithm of the minimum angled of resolution) and NW:VA 1.0 logMar. The prevalence of severe depression increased from 0% (BE:VA >= 20/40+WE:VA >= 20/200) to 7.6% (BE:VA < 20/40+)WE:VA < 20/200), whereas anxiety was unrelated to VA loss. Moreover, total depression scores were strongly associated with VA severity (P = 0.006), but not total anxiety scores (P = 0.840). Responses to two HADS items ("I still enjoy things I used to enjoy"; "I can enjoy a good book or radio or television program") identified 95% of severely to moderately depressed patients. CONCLUSIONS. Self-rated depression in patients with AMD was associated with VA severity level. It should, therefore, be relatively easy for ophthalmologists to implement the screening procedure and refer identified patients to psychiatrists for proper assessment and treatment

    Distribuição horizontal e vertical de fósforo em sistemas de cultivos exclusivos de soja e de integração lavoura‑pecuária‑floresta

    The objective of this work was to evaluate the horizontal and vertical distribution of soil phosphorus and soybean yield in single‑crop and in integrated crop‑livestock‑forest (ICLF) systems. The evaluated treatments were: forest planted with eucalyptus; soybean crop and off-season corn intercropped with Urochloa brizantha 'Marandu'; U. brizantha pasture; ICLF system; and two control areas, with native forest and fallow. Soil samples were taken at the depths of 0–5, 5–10, 10–20, 20–30 cm for determining chemical characteristics. Evaluations were done in the second crop year after the implementation of the experiment. In the ICLF system, soil and yield were evaluated in four equidistant transects, at the center of the tree rows, and at 3, 6, 10, and 15 m from the rows on the northern and southern sides. Available P levels (Mehlich‑1) were greater at the upper soil layers, in the systems with soybean/off-season corn and ICLF. In ICLF, available P contents did not differ between the distances from the tree rows, and soybean yield did not differ from the one observed in the soybean/off-season corn system. Therefore, a two‑year ICLF is not yet capable of major interference on soil chemical characteristics and on soybean yield.O objetivo deste trabalho foi avaliar a distribuição horizontal e vertical do fósforo no solo e a produtividade de soja em sistemas de cultivos exclusivos e de integração lavoura‑pecuária‑floresta (ILPF). Foram avaliados os tratamentos: floresta plantada de eucalipto; lavoura de soja e milho safrinha consorciado com Urochloa brizantha 'Marandu'; pastagem de U. brizantha; sistema de ILPF; e duas áreas controle, com floresta nativa e pousio. O solo foi coletado nas camadas de 0–5, 5–10, 10–20 e 20–30 cm, para determinação das características químicas. As avaliações foram realizadas no segundo ano agrícola após a implantação do experimento. No sistema de ILPF, solo e produtividade foram avaliados em quatro transectos equidistantes, no centro do renque e a 3, 6, 10 e 15 m do renque, nas faces norte e sul. Os teores de P disponível (Mehlich‑1) foram maiores nas camadas superficiais do solo, nos sistemas com soja/milho safrinha e com ILPF. No ILPF, os teores de P disponível não diferiram entre as distâncias do renque das árvores, e a produtividade de soja não diferiu da observada no sistema com soja/milho safrinha. Portanto, com dois anos de implantação, o sistema ILPF ainda não é capaz de interferir marcadamente nas características químicas do solo e na produtividade da soja

    Transmission of the malaria parasite from the mammalian host to the mosquito vector requires the formation of adequately adapted parasite forms and stage-specific organelles. Here we show that formation of the crystalloid-a unique and short-lived organelle of the Plasmodium ookinete and oocyst stage required for sporogony-is dependent on the precisely timed expression of the S-acyl-transferase DHHC10. DHHC10, translationally repressed in female Plasmodium berghei gametocytes, is activated translationally during ookinete formation, where the protein is essential for the formation of the crystalloid, the correct targeting of crystalloid-resident protein LAP2, and malaria parasite transmission

    Randomized phase II trial of FOLFIRI-panitumumab compared with FOLFIRI alone in patients with RAS wild-type circulating tumor DNA metastatic colorectal cancer beyond progression to first-line FOLFOX-panitumumab : the BEYOND study (GEMCAD 17-01)

The value of panitumumab rechallenge is currently unknown. We assessed addition of panitumumab to FOLFIRI (P-FOLFIRI) beyond progression to P-FOLFOX in patients with no RAS mutations in liquid biopsy (LB). Methods: In this randomized phase II trial, patients were assigned (3:2 ratio) to second-line P-FOLFIRI (arm A) or FOLFIRI alone (arm B). LB for circulating tumor DNA analysis was collected at study entry and at disease progression. Primary endpoint was 6-month progression-free survival. Two-stage Simon design required 85 patients to be included (EudraCT 2017-004519-38). Results: Between February 2019 and November 2020, 49 patients were screened (16 RAS mutations in LB detected) and 31 included (18 assigned to arm A and 13 to arm B). The study was prematurely closed due to inadequate recruitment. Serious adverse events were more frequent in arm A (44% vs. 23%). Overall response rate was 33% (arm A) vs. 7.7% (arm B). Six-month progression-free survival rate was 66.7% (arm A) and 38.5% (arm B). Median progression-free survival was 11.0 months (arm A) and 4.0 months (arm B) (hazard ratio, 0.58). At disease progression, RAS or BRAF mutations in LB were found in 4/11 patients (36%) in arm A and 2/10 (20%) in arm B. Conclusions: The BEYOND study suggests a meaningful benefit of P-FOLFIRI beyond progression to P-FOLFOX in metastatic colorectal cancer patients with WT RAS status selected by LB. This strategy deserves further investigation

    ColoLipidGene: Signature of lipid metabolism-related genes to predict prognosis in stage-II colon cancer patients

    Lipid metabolism plays an essential role in carcinogenesis due to the requirements of tumoral cells to sustain increased structural, energetic and biosynthetic precursor demands for cell proliferation. We investigated the association between expression of lipid metabolism-related genes and clinical outcome in intermediate-stage colon cancer patients with the aim of identifying a metabolic profile associated with greater malignancy and increased risk of relapse. Expression profile of 70 lipid metabolismrelated genes was determined in 77 patients with stage II colon cancer. Cox regression analyses using c-index methodology was applied to identify a metabolic-related signature associated to prognosis. The metabolic signature was further confirmed in two independent validation sets of 120 patients and additionally, in a group of 264 patients from a public database. The combined analysis of these 4 genes, ABCA1, ACSL1, AGPAT1 and SCD, constitutes a metabolic-signature (ColoLipidGene) able to accurately stratify stage II colon cancer patients with 5-fold higher risk of relapse with strong statistical power in the four independent groups of patients.
