    Theory and Application of Roscoe Pound\u27s Sociological Jurisprudence: Crime Prevention or Control?

    The current interest in reforming the administration of justice has been triggered by a number of factors including the 1967 report of the President\u27s Commission on Law Enforcement and the Administration of Justice and the treatment afforded arrestees during the civil disorders of the past few years. The nation is alarmed at the reported annual increases in crime, and this alarm was manifested in the 1968 presidential election when law and order became a major issue. Superficially the answer may seem clear: more effective enforcement of the law and, when necessary, more stringent laws. The critical issue, however, is a jurisprudential-philosophical one: ought the proper approach to crime essentially be its prevention through methods such as the rehabilitation of criminal offenders, or its control through efficient administrative procedures? This is not a new question in jurisprudence, but it remains an important and unresolved one. This article will examine an analytical approach to this problem which was developed and applied by Roscoe Pound, one of America\u27s most eminent jurists. After describing and interpreting Pound\u27s concept of sociological jurisprudence, we will relate it generally to the reform of criminal justice administration and analyze Pound\u27s attempt to apply his theory as -Director of the Cleveland Crime Survey of 1921. Finally, we will compare the recommendations of that Cleveland study and the recent report of the President\u27s Commission in a modest effort to assess their impact on the administration of criminal justice and to draw some lessons for future reform endeavors

    The aim was to identify the role of nurses in preventive care for Diabetic Foot injuries. Descriptive-exploratory study, of a quantitative nature. Carried out in a health clinic located in the city of São Paulo, with 65 diabetic patients, between 18 and 90 years old. Among the study participants, the highest percentage of those who showed signs and symptoms of Diabetic Foot Syndrome were among those who had the diagnosis a longer time ago, had greater resistance to changes in habits and adherence to treatment, did not follow the guidelines on foot care and/or had other associated chronic diseases. The nurse is fundamental in early diagnosis, interventions and education on preventive care. The prevention of Diabetic Foot complications depends on the commitment, communication and training of the multidisciplinary team to reduce amputations and morbidity and mortality of diabetics.Objetivou-se identificar o papel do enfermeiro nos cuidados preventivos das lesões do Pé Diabético. Estudo descritivo-exploratório, de natureza quantitativa. Realizado em uma clínica de saúde localizada no município de São Paulo, com 65 pacientes diabéticos, entre 18 e 90 anos. Entre os participantes do estudo, a maior porcentagem dos que apresentavam sinais e sintomas da Síndrome do Pé Diabético, estavam entre os que tiveram o diagnóstico há mais tempo, possuíam maior resistência a mudanças de hábito e adesão ao tratamento, não seguiam as orientações sobre os cuidados com os pés e/ou possuíam outras doenças crônicas associadas. O enfermeiro é fundamental no diagnóstico precoce, intervenções e educação sobre cuidados preventivos.     The aim was to identify the role of teaching methodologies used in lato sensu courses in Distance Education in Health Sciences. Descriptive qualitative research. Research subjects: professors/coordinators of lato sensu courses in the areas of health in Distance Education. A form on teaching tools and methodologies and a semi-structured interview were used. For data analysis, the content analysis method was used under the concepts of Vygotsky's socio-interactionist theory. The data raised the tools used in the teaching platforms. The analysis of the interviews resulted in four categories. These data showed the difficulties, experiences and expectations of teachers/coordinators. Distance Education provides expansion and deepening of knowledge. There is concern about the quality of teaching, directed to the need to adapt the methodologies applied to the use of different tools. Distance Education provides expansion of access to professional qualification, consequently improving health care for the population.Objetivou-se identificar o papel das metodologias de ensino utilizadas em cursos lato sensu em Educação a Distância nas Ciências da Saúde. Pesquisa qualitativa descritiva. Sujeitos da pesquisa: professores/coordenadores de cursos lato sensu nas áreas da saúde em Educação a Distância. Foram utilizados formulário sobre ferramentas e metodologias de ensino e entrevista semiestruturada. Para análise dos dados foi utilizado o método de análise de conteúdo sob os conceitos da teoria sócio-interacionista de Vygotsky. Os dados levantaram as ferramentas utilizadas nas plataformas de ensino. A análise das entrevistas resultou em quatro categorias.     Neuroblastoma-secreted exosomes carrying miR‐375 promote osteogenic differentiation of bone-marrow mesenchymal stromal cells

    Bone marrow (BM) is the major target organ for neuroblastoma (NB) metastasis and its involvement is associated with poor outcome. Yet, the mechanism by which NB cells invade BM is largely unknown. Tumour microenvironment represents a key element in tumour progression and mesenchymal stromal cells (MSCs) have been recognized as a fundamental part of the associated tumour stroma. Here, we show that BM-MSCs isolated from NB patients with BM involvement exhibit a greater osteogenic potential than MSCs from non-infiltrated BM. We show that BM metastasis-derived NB-cell lines secrete higher levels of exosomal miR-375, which promotes osteogenic differentiation in MSCs. Of note, clinical data demonstrate that high level of miR-375 correlates with BM metastasis in NB patients. Our findings suggest, indeed, a potential role for exosomal miR-375 in determining a favourable microenvironment in BM to promote metastatic progression. MiR-375 may, thus, represent a novel biomarker and a potential target for NB patients with BM involvement

    Prediction of early recurrent thromboembolic event and major bleeding in patients with acute stroke and atrial fibrillation by a risk stratification schema: the ALESSA score study

    Background and Purposes—This study was designed to derive and validate a score to predict early ischemic events and major bleedings after an acute ischemic stroke in patients with atrial fibrillation. Methods—The derivation cohort consisted of 854 patients with acute ischemic stroke and atrial fibrillation included in prospective series between January 2012 and March 2014. Older age (hazard ratio 1.06 for each additional year; 95% confidence interval, 1.00–1.11) and severe atrial enlargement (hazard ratio, 2.05; 95% confidence interval, 1.08–2.87) were predictors for ischemic outcome events (stroke, transient ischemic attack, and systemic embolism) at 90 days from acute stroke. Small lesions (≤1.5 cm) were inversely correlated with both major bleeding (hazard ratio, 0.39; P=0.03) and ischemic outcome events (hazard ratio, 0.55; 95% confidence interval, 0.30–1.00). We assigned to age ≥80 years 2 points and between 70 and 79 years 1 point; ischemic index lesion >1.5 cm, 1 point; severe atrial enlargement, 1 point (ALESSA score). A logistic regression with the receiver-operating characteristic graph procedure (C statistic) showed an area under the curve of 0.697 (0.632–0.763; P=0.0001) for ischemic outcome events and 0.585 (0.493–0.678; P=0.10) for major bleedings. Results—The validation cohort consisted of 994 patients included in prospective series between April 2014 and June 2016. Logistic regression with the receiver-operating characteristic graph procedure showed an area under the curve of 0.646 (0.529–0.763; P=0.009) for ischemic outcome events and 0.407 (0.275–0.540; P=0.14) for hemorrhagic outcome events. Conclusions—In acute stroke patients with atrial fibrillation, high ALESSA scores were associated with a high risk of ischemic events but not of major bleedings

    New locus underlying auriculocondylar syndrome (ARCND): 430 kb duplication involving TWIST1 regulatory elements

    Background Auriculocondylar syndrome (ARCND) is a rare genetic disease that affects structures derived from the first and second pharyngeal arches, mainly resulting in micrognathia and auricular malformations. To date, pathogenic variants have been identified in three genes involved in the EDN1-DLX5/6 pathway (PLCB4, GNAI3 and EDN1) and some cases remain unsolved. Here we studied a large unsolved four-generation family. Methods We performed linkage analysis, resequencing and Capture-C to investigate the causative variant of this family. To test the pathogenicity of the CNV found, we modelled the disease in patient craniofacial progenitor cells, including induced pluripotent cell (iPSC)-derived neural crest and mesenchymal cells. Results This study highlights a fourth locus causative of ARCND, represented by a tandem duplication of 430 kb in a candidate region on chromosome 7 defined by linkage analysis. This duplication segregates with the disease in the family (LOD score=2.88) and includes HDAC9, which is located over 200 kb telomeric to the top candidate gene TWIST1. Notably, Capture-C analysis revealed multiple cis interactions between the TWIST1 promoter and possible regulatory elements within the duplicated region. Modelling of the disease revealed an increased expression of HDAC9 and its neighbouring gene, TWIST1, in neural crest cells. We also identified decreased migration of iPSC-derived neural crest cells together with dysregulation of osteogenic differentiation in iPSC-affected mesenchymal stem cells. Conclusion Our findings support the hypothesis that the 430 kb duplication is causative of the ARCND phenotype in this family and that deregulation of TWIST1 expression during craniofacial development can contribute to the phenotype.Molecular Technology and Informatics for Personalised Medicine and Healt

    DADA: Degree-Aware Algorithms for Network-Based Disease Gene Prioritization

    <p>Abstract</p> <p>Background</p> <p>High-throughput molecular interaction data have been used effectively to prioritize candidate genes that are linked to a disease, based on the observation that the products of genes associated with similar diseases are likely to interact with each other heavily in a network of protein-protein interactions (PPIs). An important challenge for these applications, however, is the incomplete and noisy nature of PPI data. Information flow based methods alleviate these problems to a certain extent, by considering indirect interactions and multiplicity of paths.</p> <p>Results</p> <p>We demonstrate that existing methods are likely to favor highly connected genes, making prioritization sensitive to the skewed degree distribution of PPI networks, as well as ascertainment bias in available interaction and disease association data. Motivated by this observation, we propose several statistical adjustment methods to account for the degree distribution of known disease and candidate genes, using a PPI network with associated confidence scores for interactions. We show that the proposed methods can detect loosely connected disease genes that are missed by existing approaches, however, this improvement might come at the price of more false negatives for highly connected genes. Consequently, we develop a suite called D<smcaps>A</smcaps>D<smcaps>A</smcaps>, which includes different uniform prioritization methods that effectively integrate existing approaches with the proposed statistical adjustment strategies. Comprehensive experimental results on the Online Mendelian Inheritance in Man (OMIM) database show that D<smcaps>A</smcaps>D<smcaps>A</smcaps> outperforms existing methods in prioritizing candidate disease genes.</p> <p>Conclusions</p> <p>These results demonstrate the importance of employing accurate statistical models and associated adjustment methods in network-based disease gene prioritization, as well as other network-based functional inference applications.
