Extra-hepatic fascioliasis with peritoneal malignancy tumor feature
Fascioliasis is a zoonose parasitic disease
caused by Fasciola hepatica and Fasciola gigantica and is
widespread in most regions of the world. Ectopic fascioliasis
usually caused by juvenile Fasciola spp., but in
recent years a few cases of tissue-embedded ova have been
reported from different endemic areas. A 79-year-old Iranian
man resident in Eird-e-Mousa village from Ardabil
Province, north-west of Iran, complained with abdominal
pain, nausea, and intestinal obstruction symptoms referred
to Ardabil Fatemi hospital. In laparotomy multiple intestinal
masses with peritoneal seeding resembling of a
malignant lesion were seen. After appendectomy and peritoneal
mass biopsy with numerous intraperitoneal adenopathy,
paraffin embedded blocks were prepared from
each tissues. A blood sample was taken from the patient
5 months later for serological diagnosis. Histopathological
examination of sections showed fibrofatty stroma with
dense mixed inflammatory cells infiltration and fibrosis in
peritoneal masses. Large numbers of ova of Fasciola spp.
were noted with typical circumscribed granulomas. Despite
of anti-fasciola treatment, IHA test for detecting anti F.
hepatica antibodies was positive 5 months after surgery
with a titer of 1/128. Due to multiple clinical manifestation
of extra-hepatic fascioliasis, its differential diagnosis from
intraperitoneal tumors or other similar diseases should be
considered
Assessing the methodological strengths and limitations of the Spanish Society of Medical Oncology (SEOM) guidelines : a critical appraisal using AGREE II and AGREE-REX tool
Altres ajuts: acords transformatius de la UABThe Spanish Society of Medical Oncology (SEOM) has provided open-access guidelines for cancer since 2014. However, no independent assessment of their quality has been conducted to date. This study aimed to critically evaluate the quality of SEOM guidelines on cancer treatment. Appraisal of Guidelines for Research and Evaluation II (AGREE II) and AGREE-REX tool was used to evaluate the qualities of the guidelines. We assessed 33 guidelines, with 84.8% rated as "high quality". The highest median standardized scores (96.3) were observed in the domain "clarity of presentation", whereas "applicability" was distinctively low (31.4), with only one guideline scoring above 60%. SEOM guidelines did not include the views and preferences of the target population, nor did specify updating methods. Although developed with acceptable methodological rigor, SEOM guidelines could be improved in the future, particularly in terms of clinical applicability and patient perspectives

Cost-effectiveness of adjuvant therapy for hepatocellular carcinoma during the waiting list for liver transplantation.
Background: Survival after liver transplantation for early hepatocellular carcinoma (HCC) is worsened by the increasing dropout rate while waiting for a donor.
Aims: To assess the cost effectiveness of adjuvant therapy while waiting for liver transplantation in HCC patients.
Method: Using a Markov model, a hypothetical cohort of cirrhotic patients with early HCC was considered for: (1) adjuvant treatment—resection was limited to Child-Pugh's A patients with single tumours, and percutaneous treatment was considered for Child-Pugh's A and B patients with single tumours unsuitable for resection or with up to three nodules < 3 cm; and (2) standard management. Length of waiting time ranged from six to 24 months.
Results: Surgical resection increased the transplantation rate (>10%) and provided gains in life expectancy of 4.8–6.1 months with an acceptable cost (74 000/life of year gained) for shorter waiting times or high dropout rate scenarios. Percutaneous treatment increased life expectancy by 5.2–6.7 months with a marginal cost of approximately $20 000/year of life gained in all cases, remaining cost effective for all waiting times.
Conclusions: Adjuvant therapies for HCC while waiting for liver transplantation provide moderate gains in life expectancy and are cost effective for waiting lists of one year or more. For shorter waiting times, only percutaneous treatment confers a relevant survival advantage
Extinction law variations and dust excitation in the spiral galaxy NGC 300
We investigate the origin of the strong radial gradient in the
ultraviolet-to-infrared ratio in the spiral galaxy NGC 300, and emphasize the
importance of local variations in the interstellar medium geometry, concluding
that they cannot be neglected with respect to metallicity effects. This
analysis is based upon a combination of maps from GALEX and Spitzer, and from
the ground (UBVRI, Halpha and Hbeta). We select ionizing stellar clusters
associated with HII regions of widely varying morphologies, and derive their
fundamental parameters from population synthesis fitting of their spectral
energy distributions, measured to eliminate local backgrounds accurately. From
these fits, we conclude that the stellar extinction law is highly variable in
the line of sight of young clusters of similar ages. In the particular model
geometry that we consider most appropriate to the sampled regions, we checked
that our findings are not significantly altered by the correct treatment of
radiative transfer effects. The variations are systematic in nature: extinction
laws of the Milky Way or LMC type are associated with compact HII regions (the
compacity being quantified in two different ways), while clusters surrounded by
diffuse HII regions follow extinction laws of the 30 Doradus or SMC type. The
Calzetti starburst attenuation law, although most often degenerate with the 30
Doradus extinction law, overpredicts ionizing photon fluxes by large amounts.
We also find that the extinction law variations are correlated with the column
density of dust species emitting in the near- and mid-infrared. Finally, we
briefly discuss the nebular to stellar extinction ratios, and the excitation of
aromatic band carriers, invalidating their claimed association with cold dust.Comment: accepted for publication in ApJ -- figure 6 abridged her
The Alcock–Paczyński effect from Lyman-α forest correlations: analysis validation with synthetic data
The three-dimensional distribution of the Ly α forest has been extensively used to constrain cosmology through measurements of the baryon acoustic oscillations (BAO) scale. However, more cosmological information could be extracted from the full shapes of the Ly α forest correlations through the Alcock–Paczyński (AP) effect. In this work, we prepare for a cosmological analysis of the full shape of the Ly α forest correlations by studying synthetic data of the extended Baryon Oscillation Spectroscopic Survey (eBOSS). We use a set of 100 eBOSS synthetic data sets in order to validate such an analysis. These mocks undergo the same analysis process as the real data. We perform a full-shape analysis on the mean of the correlation functions measured from the 100 eBOSS realizations, and find that our model of the Ly α correlations performs well on current data sets. We show that we are able to obtain an unbiased full-shape measurement of DM/DH(zeff), where DM is the transverse comoving distance, DH is the Hubble distance, and zeff is the effective redshift of the measurement. We test the fit over a range of scales, and decide to use a minimum separation of rₘᵢₙ = 25 h−¹Mpc. We also study and discuss the impact of the main contaminants affecting Ly α forest correlations, and give recommendations on how to perform such analysis with real data. While the final eBOSS Ly α BAO analysis measured DM/DH(zeff = 2.33) with 4 per cent statistical precision, a full-shape fit of the same correlations could provide an ∼2 per cent
measurement
Detection of an optical transient following the 13 March 2000 short/hard gamma-ray burst
We imaged the error box of a gamma-ray burst of the short (0.5 s), hard type
(GRB 000313), with the BOOTES-1 experiment in southern Spain, starting 4 min
after the gamma-ray event, in the I-band. A bright optical transient (OT
000313) with I = 9.4 +/- 0.1 was found in the BOOTES-1 image, close to the
error box (3-sigma) provided by BATSE. Late time VRIK'-band deep observations
failed to reveal an underlying host galaxy. If the OT 000313 is related to the
short, hard GRB 000313, this would be the first optical counterpart ever found
for this kind of events (all counterparts to date have been found for bursts of
the long, soft type). The fact that only prompt optical emission has been
detected (but no afterglow emission at all, as supported by theoretical models)
might explain why no optical counterparts have ever been found for short, hard
GRBs.This fact suggests that most short bursts might occur in a low-density
medium and favours the models that relate them to binary mergers in very
low-density enviroments.Comment: Revised version. Accepted for publication in Astronomy and
