    Introduction There is growing concern about the aggressiveness of cancer care at the end of life (ACCEoL), defined as overly aggressive treatments that compromise the quality of life at its end. Recognising the most affected patients is a cornerstone to improve oncology care. Our aim is to identify factors associated with ACCEoL for patients with cancer dying in hospitals. Methods All adult patients with cancer who died in public hospitals in mainland Portugal (January 2010 to December 2015), identified from the hospital morbidity database. This database provided individual clinical and demographic data. We obtained hospital and region-level variables from a survey and National Statistics. The primary outcome is a composite ACCEoL measure of 16 indicators. We used multilevel random effects logistic regression modelling (p14 days in the hospital (43%, 42-43) and surgery (28%, 28-28) in the last 30 days. Older age (p<0·001), breast cancer (OR 0·83; 95% CI 0·76 to 0·91), and metastatic disease (0·54; 95% CI 0·50 to 0·58) were negatively associated with ACCEoL. In contrast, higher Deyo-Charlson Comorbidity Index (p<0·001), gastrointestinal and haematological malignancies (p<0·001), and death at cancer centre (1·31; 95% CI 1·01 to 1·72) or hospital with medical oncology department (1·29; 95% CI 1·02 to 1·63) were positively associated with ACCEoL. There was no association between hospital palliative care services at the hospital of death and ACCEoL. Conclusion Clinical factors related to a better understanding of disease course are associated with ACCEoL reduction. Patients with more comorbidities and gastrointestinal malignancies might represent groups with complex needs, and haematological patients may be at increased risk because of unpredictable prognosis. Improvement of hospital palliative care services could help reduce ACCEoL, particularly in cancer centres and hospitals with medical oncology department, as those services are usually under-resourced, thus reaching few.publishersversionpublishe

    Ergonews: um telejornal para o ensino de ergonomia em Odontologia

    During their undergraduate course, dentists need to be aware about the risk they are exposed to in their work environment so they can avoid them. Thus, we proposed a TV newscast entitled "Ergonews" which was presented to the 2nd-year undergraduate Dentistry students at Araraquara Dental School – UNESP, in order to expand and enhance their knowledge in the discipline of Ergonomics in Dentistry I. The newscast has included a reporter and a "philosopher", the latter having the function to issue philosophical opinions not always correct about the concepts of ergonomics. The topics covered were: i. the importance of rationalizing work to increase productivity and quality of life of the dentists; ii. requirements of ergonomic posture, occupational health, and the importance of cross infection control in dental practice; iii. dental equipment and delegation of functions - four hands work. Each “Ergonews” episode was prepared previously to the class and all recordings were edited using the Movie Maker. The newscast was presented to students during the Ergonomics in Dentistry I classes, every 15 days, using multimedia projection, with an average duration of 10 minutes. After the presentation of each "Ergonews" episode, students were asked to evaluate critically the concepts of ergonomics issued by "philosopher" in the newscast. It can be concluded that the "Ergonews" was an auxiliary method to promote learning about ergonomics in dentistry.Los cirujanos dentales requieren ser concientizados durante su formación sobre los riesgos a que están expuestos en su ambiente de trabajo para que puedan así evitarlos. Por lo tanto, se propuso producir y presentar un noticiero titulado "Ergonews", dirigido a los estudiantes del segundo año de Licenciatura de Odontología, Facultad de Odontología de Araraquara - UNESP con el fin de ampliar y mejorar sus conocimientos en la disciplina de Ergonomía en Odontología I. El noticiero contaba con un reportero y también con un "filósofo", cuya función era la de emitir opiniones filosóficas, no siempre correctas, acerca de los conceptos de ergonomía. Los temas tratados fueron los siguientes: (a) la importancia de racionalizar el trabajo para aumentar la productividad y la calidad de vida del dentista; (b) requisitos de una postura ergonómica, salud ocupacional, la importancia del control de las infecciones cruzadas en la práctica dental; (c) equipo dental y la delegación de funciones, el trabajo en equipo. El diario fue grabado antes de su presentación y todas las grabaciones fueron editadas utilizando Maker. El noticiero fue presentado a los estudiantes en el aula, utilizando un proyector de video en la clase de Ergonomía en Odontología I y con una duración media de 10 minutos con periodicidad quincenal. Después de la presentación de cada noticiero "Ergonews" se les pidió a los estudiantes que evaluaran críticamente los conceptos de ergonomía emitidos por el "filósofo" del noticiero, con el objetivo de asentar dichos conocimientos adquiridos durante la clase.     Deep Eutectic Solvents (DES) based on sulphur for silicon surfaces as alternative lubricants

    Encontro realizado na Universidade Nova de Lisboa de 24-27 de junho de 2019.Deep eutectic mixtures composed of hydrogen-bond donors and hydrogen-bond acceptors, the so-called DESs, have recently being proposed as possible “green” alternatives to mineral oils and ionic liquids (ILs) in the lubrication of steel surfaces. DESs have similar physical properties to ILs but have the advantage of being cheaper and easier to prepare. In this work, new DESs containing sulphur units in their structure were prepared and tested in the lubrication of silicon surfaces which are relevant for nano/microelectromechanical systems (NEMS/MEMS). The following new DESs were prepared: dibutil-ethyl sulfonium ethylsulfate: polyethylene glycol ([S4,4,2][EtSO4]:PEG), ethyl-tetrahydrothiophenium ethylsulfate: polyethylene glycol ([C2-THT][EtSO4]:PEG, cis-1-ethyl-3methylimidazolium canforsulfonate: polyethylene glycol ([C2MIM][(S)-CSA]:PEG), and 1,3-dimethylpiridinium methyl sulfate: polyethylene glycol ([C1-3-pic][MeSO4]:PEG). Other DES, already known, were tested for comparison purposes: tetrabutylammonium bromide: ‎tetrahydrothiophene 1,1-dioxide ([N4,4,4,4][Br]:Sulfolane), choline chloride: polyethylene glycol (ChCl:PEG), and tetrabutylammonium bromide: polyethylene glycol ([N4,4,4,4][Br]:PEG). All DESs were characterized in terms of their water content, viscosity, wettability, and tribological properties. The friction coefficients were measured in a nanotribometer using steel spheres against Si surfaces. O telejornal contou com uma repórter e também um “filósofo”, cuja função foi emitir opiniões filosóficas nem sempre corretas a respeito dos conceitos de ergonomia. Evidence on the effectiveness of palbociclib plus fulvestrant is scarce, which highlights the need of additional studies. The aim of this study was to evaluate the effectiveness of palbociclib plus fulvestrant in advanced breast cancer (ABC). Materials and methods: We conducted a population-based retrospective cohort study and cases of interest were identified through the Portuguese National Cancer Registry database and additional data sources. Patients aged≥18 years, diagnosed with ABC and exposed to palbociclib plus fulvestrant between May 31, 2017 and March 31, 2019 were included. Patients were followed-up until death or cut-off date (February 28, 2021). Primary outcome was rw-progression-free survival (rwPFS). Secondary outcomes were rw-overall survival (rwOS), rw-time to palbociclib failure (rwTPF) and rw-time to next treatment (rwTTNT). Results: A total of 210 patients were included. Median age was 58 years (range 29–83) and 99.05% were female. Median follow-up time was 23.22 months and, at cut-off date, treatment had been discontinued in 189 patients, mainly due to disease progression (n = 152). Median rwPFS was 7.43 months (95% confidence interval [CI] 6.28–9.05) and 2-year rwPFS was 16.65% (95%CI 11.97–22.00). Median rwOS was 24.70 months (95%CI 21.58–29.27), median rwTPF was 7.5 months (95%CI 6.51–9.08) and median rwTTNT was 11.74 months (95%CI 10.33–14.08). Conclusion: Palbociclib plus fulvestrant seems an effective treatment for ABC in real-world context. Compared to registrations studies, rwPFS and rwOS were shorter in real-life setting.publishersversionpublishe

This cross-sectional survey-based study investigated the impact of the pandemic on clinical practices of Portuguese medical oncologists caring for patients with breast cancer. Methods: An anonymous online survey comprising 42 questions gathered information regarding COVID-19 testing, treatment in (neo)adjuvant and metastatic settings, and other aspects of breast cancer management. Practices before and during the pandemic were compared, and potential differences in outcomes according to respondents’ regions, case volumes, and practice type were explored. Results: Of 129 respondents, 108 worked in the public health system, giving a representative national picture of the impact of the COVID-19 pandemic on breast cancer management. Seventy-one percent of respondents reported a reduction in visits for new cases of breast cancer, and there was a shift towards increased use of telemedicine. Clinical decision-making was largely unaffected in the most aggressive indications (i.e., triple-negative, HER2-positive, visceral crisis). The use of neoadjuvant therapy increased when access to surgery was difficult, whereas dose-dense regimens decreased, and cyclin-dependent kinase 4/6 inhibitor treatment decreased for less aggressive disease and increased for more aggressive disease. The use of oral formulations and metronomic chemotherapy regimens increased, and clinical trial participation decreased. Some differences by respondents’ region and case volume were noted. Conclusion: Medical oncologists in Portugal implemented many changes during the COVID-19 pandemic, most of which were logical and reasonable responses to the current healthcare emergency; however, the true impact on patient outcomes remains unknown.publishersversionepub_ahead_of_prin

    Synergistic roles of the proteasome and mitochondria in alpha-synuclein oligomerization : implications in Parkinson's disease

    Trabalho final de mestrado integrado em Medicina área científica de Neurociências, apresentado à Faculdade de Medicina da Universidade de CoimbraParkinson’s disease (PD) is the most common neurodegenerative movement disorder, characterized by selective loss of nigrostriatal dopaminergic neurons and presence of intracellular insoluble proteinaceous inclusions, known as Lewy Bodies. Although PD etiopathogenesis remains elusive, the leading hypothesis establishes that mitochondrial dysfunction, protein quality control system deficiency, and protein oligomerization are major events that act synergistically to cause this devastating disease. The main goal of this work is to study ubiquitin-proteasome system (UPS) and ubiquitin dependent alpha-synuclein (aSN) clearance in different PD cellular models with mitochondrial deregulation. We used three different PD cellular models: SH-SY5Y ndufa2 knock-down (KD) cells, PD cybrids and peripheral blood mononuclear cells (PBMC) of patients with diagnostic of PD. For each model we studied proteasome activity, using fluorimetric analysis, and quantified cellular ubiquitination and aSN aggregation by Western Blot. We used lactacystin as negative control of proteasomal function. In PBMC of PD patients population we proceed to evaluate aSN secretion to plasma by Dot Blot and the influence of several demographic characteristics in the above mentioned determinations. SH-SY5Y ndufa2 KD cells shown a proteasome activity up-regulation with increased levels of total ubiquitination, ubiquitin monomers and aSN oligomers as compared with SH-SY5Y parental cells. PD Cybrids did not show differences concerning proteasome enzymatic activity. PBMC of patients do not exhibit statistical significant differences in proteasome activity compared to age-matched controls. However, there is a negative correlation of both chymotrypsin-like activity and total ubiquitin content with age in control and LOPD groups. Despite there are no statistical significant differences in ubiquitin levels between patients and controls, total ubiquitin content increases and is positively correlated with chymotrypsin-like activity and with aSN oligomers levels. aSN levels in plasma are slightly increased in LOPD and significantly increased in EOPD. Thus, we conclude that in SH-SY5Y ndufa2 KD cells there is an up-regulation of proteasomal enzymatic activity that could mean an interesting cell rescue attempt. Moreover, although ubiquitinated proteins content are increased in the other two models, proteasome activity is not significantly altered, what is compatible with an over request of UPS due to high rate of protein misfolding or somehow a compromise in the UPS pathway, upstream the enzymatic core. We also conclude that in PBMC of both patients and control individuals, there is a decrease of UPS activity with age. In addition, aSN oligomers are ubiquitinated and we identified an ubiquitin-dependent clearance insufficiency. Secretion of aSN in PBMC seems to be a cell mechanism to prevent its cellular accumulation.A Doença de Parkinson (PD) é a doença neurodegenerativa do movimento mais comum sendo caracterizada pela perda selectiva de neurónios dopaminérgicos nigro-estriatais e pela presença de inclusões proteicas intracelulares insolúveis, os Corpos de Lewy. Apesar de a etiopatogenia não estar completamente esclarecida, existem evidências que apontam para que a disfunção mitocondrial, um ineficiente sistema de controlo de qualidade proteica e a oligomerização proteica sejam eventos fundamentais que actuam de forma sinérgica causando esta doença. O principal objectivo deste trabalho é estudar o sistema ubiquitina-proteassoma (UPS) e a degradação de alfa-sinucleina (aSN) dependente de ubiquitina em diferentes modelos de PD com disfunção mitocondrial. Foram usados três modelos de PD: células SH-SY5Y ndufa2 knock-down (KD), PD Cybrids e células mononucleadas do sangue periférico (PBMC) de indivíduos com diagnóstico de PD. Para cada modelo, estudámos a actividade do proteassoma, usando análise fluorimétrica. Por Western Blot, quantificámos a ubiquitinação e agregação de aSN. Os dois primeiros modelos foram incubados com lactacistina, condição que desempenha o papel de controlo negativo para a função do proteassoma. Nas PBMC dos doentes quantificámos a secreção de aSN para o plasma através de Dot Blot e procurámos influências demográficas nas determinações acima mencionadas. As células SH-SY5Y ndufa2 KD apresentam aumento da actividade do proteassoma com níveis aumentados de ubiquitinação, monómeros de ubiquitina e oligomeros de aSN comparando com a linha parental SH-SY5Y. Os PD Cybrids não mostram diferenças no que respeita a actividade do proteassoma As PBMC dos doentes não têm diferenças estatisticamente significativas na actividade do proteassoma comparadas com as de controlos de idade aproximada. Contudo, existe uma correlação negativa com a idade nos grupos controlo e LOPD, quer da actividade “chymotrysin-like” do proteassoma, quer do conteúdo total de ubiquitina. Apesar de não existirem diferenças estatisticamente significativas nos níveis de ubiquitina entre doentes e controlos, o conteúdo total de ubiquitina está aumentado e está positivamente correlacionado com a actividade “chymotrysin-like” bem como com os níveis de oligomeros de aSN. Os níveis de aSN no plasma estão discretamente aumentados nos doentes LOPD e de forma significativa nos EOPD. Assim, concluímos que nas células SH-SY5Y ndufa2 KD existe um mecanismo de activação do proteassoma que pode significar uma tentativa interessante de sobrevivência celular. Além disso, apesar do conteúdo de proteínas ubiquitinadas também estar aumentado nos outros dois modelos, o mesmo não se pode afirmar face à actividade enzimática do proteassoma, o que pode corresponder a um aumento da função do UPS devido a uma concentração aumentada de proteínas disfuncionais ou de certa forma um compromisso da via do UPS, acima do complexo enzimático central. Concluímos também que existe diminuição da actividade do UPS com a idade nas PBMC, quer dos doentes quer dos indivíduos controlo. A aSN oligomerizada está ubiquitinada possivelmente devido a uma insuficiência da degradação dependente de ubiquitina. Por último, a secreção de aSN nas PBMC parece ser um mecanismo para prevenção da sua acumulação intracelula

    Trabalho final do 5º ano com vista à atribuição do grau de mestre no âmbito do ciclo de estudos de Mestrado Integrado em Medicina Dentária apresentado à Faculdade de Medicina da Universidade de Coimbra.Introdução: A Diabetes tipo 1 é uma condição na qual a destruição de células pancreáticas leva a uma deficiência de insulina. A Diabetes apresenta diferentes complicações micro e macrovasculares, sendo também um fator de risco para diversas complicações orais. Objetivo: Estabelecer uma relação entre a cárie dentária em diabéticos tipo 1 tratados com múltiplas injeções de insulina e em indivíduos não diabéticos. É ainda objetivo caracterizar os hábitos de higiene oral desta população. Materiais e métodos: Um estudo clínico e observacional de caráter analítico e transversal foi realizado. Trinta diabéticos tipo 1 e 30 não-diabéticos foram observados e questionados sobre informações referentes à sua histórica médica. O exame oral foi realizado de acordo com as normas da Organização Mundial de Saúde, tendo-se utilizado o índice de cárie ICDAS. A análise estatística foi realizada e foi estabelecido um nível de significância de 5%. Resultados: Os doentes diabéticos apresentaram um índice de cárie semelhante ao dos indivíduos não diabéticos. Verificou-se, ainda, um índice de placa bacteriana superior nos diabéticos. Apenas 10% dos doentes que referiam episódios de hipoglicemia noturna escovam os dentes após a ingestão de glicose. Conclusão: Não foi encontrada nenhuma associação estatisticamente significativa entre a diabetes tipo 1 e o desenvolvimento de cárie dentária. Implicações clínicas: Estão descritas diversas complicações orais associadas à diabetes, bem como vários fatores de risco para o desenvolvimento de lesões de cárie dentária. Deste modo, é fundamental que estes doentes sejam integrados num programa de controlo e prevenção rigorosos. Introduction: Type 1 diabetes is a condition characterized by the presence of abnormal levels of plasma glucose and in which the destruction of pancreatic cells leads to an insulin deficiency. In addition to macro and microvascular complications that are associated with the disease, hyperglycaemia is also a risk factor for several oral complications. Objective: To establish a relationship between dental caries in type 1 diabetics treated with multiple insulin injections and that of non-diabetics. It is also an aim to characterize the oral hygiene habits of this population. Materials and methods: An observational clinical study of analytical and cross-sectional nature was conducted. Thirty type 1 diabetics and 30 non-diabetics were observed and questioned about information regarding their medical history. Oral examination was conducted according to the standards of the World Health Organization and ICDAS was used for caries detection. Statistical analysis was performed and the significance level was set at 5%. Results: Diabetic patients showed similar caries levels to that of non-diabetics. Diabetics had a higher dental plaque index. Only 10% of the diabetics having episodes of nocturnal hypoglycaemia brush their teeth after glucose intake. Conclusion: No statistically significant association was found between type 1 diabetes and dental caries. Clinical implications: There are various oral complications associated with diabetes described, as well as several risk factors for the development of dental caries.
