    Cost-effectiveness of Alzheimer's disease CSF biomarkers and amyloid-PET in early-onset cognitive impairment diagnosis

    This study aimed at determining the cost-effectiveness of amyloid-positron emission tomography (PET) compared to Alzheimer's disease (AD) cerebrospinal fluid (CSF) biomarkers (amyloid-?42, total-Tau and phosphorylated-Tau) for the diagnosis of AD in patients with early-onset cognitive impairment. A decision tree model using a national health care perspective was developed to compare the costs and effectiveness associated with Amyloid-PET and AD CSF biomarkers. Available evidence from the literature and primary data from Hospital Clínic de Barcelona were used to inform the model and calculate the efficiency of these diagnostic alternatives. Medical visits and diagnostic procedures were considered and reported in €2020. We calculated the incremental cost-effectiveness ratio to measure the cost per % of correct diagnoses detected and we perform one-way deterministic and probabilistic sensitivity analyses to assess the uncertainty of these results. Compared with AD CSF biomarkers, Amyloid-PET resulted in 7.40% more correctly diagnosed cases of AD, with an incremental total mean cost of €146,854.80 per 100 cases. We found a 50% of probability that Amyloid-PET was cost-effective for a willingness to pay (WTP) of €19,840.39 per correct case detected. Using a WTP of €75,000, the probability that it is cost-effective reached a maximum of 76.9%, thus leading to a conclusion that Amyloid-PET is not a cost-effective technique compared to AD CSF biomarkers, unless the funder is willing to pay a minimum of €19,840.39 to detect one more correct case. Furthermore, obtaining CSF provides simultaneous information on amyloid ? and tau biomarkers and allows other biomarkers to be analyzed at a relatively low cost.© 2022.     Heavy-flavour production in Pb-Pb collisions at the LHC, measured with the ALICE detector

    We present the first results from the ALICE experiment on the nuclear modification factors for heavy-flavour hadron production in Pb-Pb collisions at sqrt{s_NN}=2.76 TeV. Using proton-proton and lead-lead collision samples at sqrt{s}=7 TeV and sqrt{s_NN}=2.76 TeV, respectively, nuclear modification factors R_AA(pt) were measured for D mesons at central rapidity (via displaced decay vertex reconstruction), and for electrons and muons, at central and forward rapidity, respectively.Comment: 8 pages, 5 figures, plenary talk at Quark Matter 2011, Annecy, Franc

    Google Health Trends performance reflecting dengue incidence for the Brazilian states

    This work is licensed under a Creative Commons Attribution 4.0 International License.Background Dengue fever is a mosquito-borne infection transmitted by Aedes aegypti and mainly found in tropical and subtropical regions worldwide. Since its re-introduction in 1986, Brazil has become a hotspot for dengue and has experienced yearly epidemics. As a notifiable infectious disease, Brazil uses a passive epidemiological surveillance system to collect and report cases; however, dengue burden is underestimated. Thus, Internet data streams may complement surveillance activities by providing real-time information in the face of reporting lags. Methods We analyzed 19 terms related to dengue using Google Health Trends (GHT), a free-Internet data-source, and compared it with weekly dengue incidence between 2011 to 2016. We correlated GHT data with dengue incidence at the national and state-level for Brazil while using the adjusted R squared statistic as primary outcome measure (0/1). We used survey data on Internet access and variables from the official census of 2010 to identify where GHT could be useful in tracking dengue dynamics. Finally, we used a standardized volatility index on dengue incidence and developed models with different variables with the same objective. Results From the 19 terms explored with GHT, only seven were able to consistently track dengue. From the 27 states, only 12 reported an adjusted R squared higher than 0.8; these states were distributed mainly in the Northeast, Southeast, and South of Brazil. The usefulness of GHT was explained by the logarithm of the number of Internet users in the last 3 months, the total population per state, and the standardized volatility index. Conclusions The potential contribution of GHT in complementing traditional established surveillance strategies should be analyzed in the context of geographical resolutions smaller than countries. For Brazil, GHT implementation should be analyzed in a case-by-case basis. State variables including total population, Internet usage in the last 3 months, and the standardized volatility index could serve as indicators determining when GHT could complement dengue state level surveillance in other countries

    Reversion of Severe Mitral Insufficiency in Peripartum Cardiomyopathy Using Levosimendan

    Idiopathic peripartum cardiomyopathy presenting with heart failure is a true diagnostic and treatment challenge. Goal oriented clinical management aims at the relapse of left ventricular systolic dysfunction. A 35-year-old patient on her 12th day post-delivery presents progressive signs of heart failure. Transthoracic echocardiography showed severe mitral insufficiency, mild left ventricular dysfunction, mild tricuspid insufficiency, severe pulmonary hypertension, and right atrial enlargement. With wet and cold heart failure signs, the patient was a candidate for inodilator cardiovascular support and volume depletion therapy. As the patient presented a persistent tachycardia at rest, levosimendan was chosen over dobutamine. Levosimendan was administered at a dose of 0.2 g/kg/min during a period of 24 hours. After inodilator therapy, the patient's signs and symptoms of heart failure began to decrease, showing improvement of dyspnea, mitral murmur grade went from IV/IV to II/IV, filling pressures and systemic and pulmonary resistance indexes decreased, arterial blood gases improved, and an echocardiography performed 72 h later showed non-dilated cardiomyopathy, mild cardiac contractile dysfunction, mild mitral insufficiency, type I diastolic dysfunction and improvement of pulmonary hypertension. Cardiovascular function in peripartum cardiomyopathy tends to go back to normality in 23-41% of the cases, but in a large group of patients, severe ventricle dysfunction remains months after initial symptoms. This article describes the diagnostic process of a patient with peripartum cardiomyopathy and a successful reversion of a severe case of mitral insufficiency using levosimendan as a new therapeutic strategy in this clinical context

    La necesidad mejorar las prácticas de Ingeniería de Requerimientos en los profesionales ha sido mencionada en la literatura y en los últimos años se ha desarrollado como tema de investigación en las carreras universitarias. Las dificultades de la elicitación de requerimientos de un sistema multiplican las dificultades de la enseñanza de esas técnicas a los estudiantes de las carreras de grado. En particular aquellas más directamente relacionadas con la interacción con los stakeholders, como es el caso de las entrevistas. Se escogió una organización del proceso de enseñanza tratando de resolver las dificultades habituales al tratar de reproducir experiencia de la vida real en el aula. La utilización de la observación como técnica de enseñanza en un curso de grado permite a los estudiantes reconocer las facetas que tiene una entrevista así como ayudarlos a reducir del nivel de ignorancia en cuanto al proceso de entrevistas.     Experimental validation of Lyot stop apodization in ground-based coronagraphy

    ABSTRACT We show that the use of apodizing functions at the coronagraph Lyot plane may be useful for improving the image contrast of ground-based coronagraphs. An experimental set-up consisting of a tip–tilt mirror, a coronagraph and a low-noiseEMCCDcamerawas implemented at theWilliam Herschel Telescope. Images were taken in the I band, which meant that the D/r0 value was around 10. Experimental results confirm that, for moderately aberrated wavefronts, our instrument works as theoretically expected, and that the contrast value attained is high enough to provide direct detection of faint companions.This research was supported by the Ministerio de Economía y Competitividad under project FIS2012-31079 and the Fundación Séneca of Murcia under projects 15419/PI/10 and 15345/PI/10

    Coronagraphs adapted to atmosphere conditions

    In this paper we show new ways to improve the performance of ground-based coronagraphy. We introduce adaptive coronagraphic masks whose profile is a binary version of the instantaneous atmospherically degraded star image. We also propose the hyper-Gaussian profile masks obtained by averaging adaptive masks. In addition, adaptive Lyot stops and hyper-Gaussian Lyot stops are analyzed. Computer simulations show that all these masks outperform the circular hard-edged mask and that a proper mask and stop combination significantly reduces the angular separation at which a faint companion can be detected
