    Cuidado de la salud del adulto mayor en una comunidad semirural desde una metodológica de participación acción investigación centrada en la ocupación

    The objective of the study was to identify the health care needs of the elderly in a semi - rural community, their existing support networks and to develop a healthcare project for this population from an occupational perspective. A mixed, three-phase methodology was used. In Phase I, a descriptive, transverse and correlational design was employed, followed by a combined quantitative and qualitative design in Phase II and a qualitative methodology in Phase III. In the latter two phases, a Participatory–Action-Research Methodology and Community Based Rehabilitation was used. Results. The occupational resources and health needs of the elderly in the community were identified. These resources and needs became the basis for the development of an awareness and training program for the caregivers. A rehabilitation support program was also implemented. Social network resources and needs were identified and generated a support group for the elderly in a vulnerable situation. This project allowed to connect different actors involved in the healthcare of the elderly. It also raised awareness among the elderly regarding the value of their own knowledge and their contribution to the social fabric of the community. The involvement of undergraduate and graduate interns in occupational therapy throughout the project facilitated covering different types of health needs as well as to expand the number of beneficiaries. It also gave students the opportunity to get involved and to work directly in the community through the identification of the occupational needs of the population as a means to intervene more effectively.El objetivo del estudio fue identificar las necesidades del cuidado de la salud de los adultos mayores en una comunidad semi – rural, las redes de apoyo existentes y desarrollar un proyecto de cuidado de salud dirigido a esta población desde un enfoque ocupacional. Se siguió una metodología mixta de tres fases: En la Fase I se utilizó un diseño descriptivo, transversal y correlacional. En la Fase II se utilizó un diseño mixto cuantitativo y cualitativo y en la Fase III una metodología cualitativa. En las Fases II y III se utilizó una metodología de Investigación-Acción-Participación y Rehabilitación Basada en la Comunidad. Resultados. Se identificaron los recursos y necesidades ocupacionales y de salud de los adultos mayores residentes en la comunidad. Estos recursos y necesidades sirvieron de base para realizar un programa de sensibilización, capacitación y atención rehabilitatoria a adultos mayores, implementado con la participación de cuidadores primarios. Se identificaron los recursos y necesidades comunitarias en cuanto a sus redes de apoyo, esto generó la creación de un grupo de apoyo para los adultos mayores en situación de vulnerabilidad. Esta experiencia permitió vincular a diferentes actores en el cuidado de la salud del adulto mayor, creando conciencia del valor de su propio conocimiento y su aportación al entretejido social de la comunidad. El involucramiento de los pasantes de licenciatura y maestría de terapia ocupacional a lo largo del proyecto, permitió ampliar el número de beneficiarios, capacitar en rehabilitación basada en la comunidad a cuidadores primarios, además de darle a los estudiantes la oportunidad de involucrarse y colaborar directamente en la comunidad, identificando las necesidades ocupacionales de la población para intervenir en forma más efectiva

    Gasdermin B over-expression modulates HER2-targeted therapy resistance by inducing protective autophagy through Rab7 activation

Decoding the molecular mechanism of GSDMB-mediated drug resistance is crucial to identify novel effective targeted treatments for HER2/GSDMB aggressive tumors. Methods Different in vitro approaches (immunoblot, qRT-PCR, flow cytometry, proteomic analysis, immunoprecipitation, and confocal/electron microscopy) were performed in HER2 breast and gastroesophageal carcinoma cell models. Results were then validated using in vivo preclinical animal models and analyzing human breast and gastric cancer samples. Results GSDMB up-regulation renders HER2 cancer cells more resistant to anti-HER2 agents by promoting protective autophagy. Accordingly, the combination of lapatinib with the autophagy inhibitor chloroquine increases the therapeutic response of GSDMB-positive cancers in vitro and in zebrafish and mice tumor xenograft in vivo models. Mechanistically, GSDMB N-terminal domain interacts with the key components of the autophagy machinery LC3B and Rab7, facilitating the Rab7 activation during pro-survival autophagy in response to anti-HER2 therapies. Finally, we validated these results in clinical samples where GSDMB/Rab7/LC3B co-expression associates significantly with relapse in HER2 breast and gastric cancers. Conclusion Our findings uncover for the first time a functional link between GSDMB over-expression and protective autophagy in response to HER2-targeted therapies. GSDMB behaves like an autophagy adaptor and plays a pivotal role in modulating autophagosome maturation through Rab7 activation.     Psychological and Sleep Effects of Tryptophan and Magnesium-Enriched Mediterranean Diet in Women with Fibromyalgia

    Anxiety, mood disturbance, eating and sleep disorders, and dissatisfaction with body image are prevalent disorders in women with fibromyalgia. The authors of this study aimed to determine the effects of tryptophan (TRY) and magnesium-enriched (MG) Mediterranean diet on psychological variables (trait anxiety, mood state, eating disorders, self-image perception) and sleep quality in women with fibromyalgia (n = 22; 49 ± 5 years old). In this randomized, controlled trial, the participants were randomly assigned to the experimental group and the placebo group. The intervention group received a Mediterranean diet enriched with high doses of TRY and MG (60 mg of TRY and 60 mg of MG), whereas the control group received the standard Mediterranean diet. Pittsburgh Sleep Quality Questionnaire, Body Shape Questionnaire, State–Trait Anxiety Inventory (STAI), Profile of Mood States (POMS-29) Questionnaire, Eating Attitudes Test-26, and Trait Anxiety Inventory were completed before and 16 weeks after the intervention. Significant differences were observed between groups after the intervention for the mean scores of trait anxiety (p = 0.001), self-image perception (p = 0.029), mood disturbance (p = 0.001), and eating disorders (p = 0.006). This study concludes that tryptophan and magnesium-enriched Mediterranean diet reduced anxiety symptoms, mood disturbance, eating disorders, and dissatisfaction with body image but did not improve sleep quality in women with fibromyalgia

    Local use of rock materials – production and utilization State-of-the-art

    During excavation of tunnels, large amounts of rock material are produced. This excavated rock material is utilized to a varying extent for road-, railway- and concrete purposes, but significant amounts are used as deposits on land, in fjords or lakes. For both economic and environmental reasons there is a great potential in increasing the utilization of excavated rock material locally in the same project or in neighboring projects. Initially, this report presents a brief introduction to the geology in Norway and some comparable European countries. This may give an indication which main rock type to expect in different areas during tunnelling. Further, the development within the main directions of tunnelling is discussed. Also, an overview of equipment and crushing technology is given. The boundaries of this report are excavated material used in asphalt, concrete, road construction and railway construction. For each area of utilization, a presentation of the currenttechnical requirements for various uses of the material in Norway is given. Examples of projects where excavated material is utilized are presented. Finally, possibilities and obstacles when it comes to utilizing excavated rock material are discussed, and further work is proposed.publishedVersio

    Effects of ethyl pyruvate on leukocyte-endothelial interactions in the mesenteric microcirculation during early sepsis treatment

    OBJECTIVES: Experimental studies on sepsis have demonstrated that ethyl pyruvate is endowed with antioxidant and anti-inflammatory properties. This study aimed to investigate the effects of ethyl pyruvate on leukocyte-endothelial interactions in the mesenteric microcirculation in a live Escherichia coli-induced sepsis model in rats. METHODS: Male Wistar rats were administered an intravenous suspension of E. coli bacteria or were subjected to a sham procedure. Three hours after bacterial infusion, the rats were randomized into the following groups: a control group without treatment, a group treated with lactated Ringer’s solution (4 mL/kg, i.v.), and a group treated with lactated Ringer’s solution (4 mL/kg, i.v.) plus ethyl pyruvate (50 mg/kg). At 24 h after bacterial infusion, leukocyte-endothelial interactions were investigated using intravital microscopy, and the expression of P-selectin and intercellular adhesion molecule-1 was evaluated via immunohistochemistry. White blood cell and platelet counts were also determined at baseline and 3 h and 24 h after E. coli inoculation. RESULTS: The non-treated and lactated Ringer’s solution-treated groups exhibited increases in the numbers of rolling leukocytes (∼2.5-fold increase), adherent cells (∼3.0-fold), and migrated cells (∼3.5-fold) compared with the sham group. In contrast, treatment with Ringer’s ethyl pyruvate solution reduced the numbers of rolling, adherent and migrated leukocytes to the levels observed in the sham group. Additionally, the expression of P-selectin and intercellular adhesion molecule-1 was significantly increased on mesenteric microvessels in the non-treated group compared with the sham group (
