    Complicaciones en el uso de aloinjertos intercalares tras resección de tumores óseos

    La reconstrucción ósea mediante aloinjertos intercalares es una técnica de uso frecuente tras resecciones tumorales, que presenta -a largo plazo- mejores resultados que las endoprótesis diafisarias o los autoinjertos vascularizados. Sin embargo, entre las complicaciones más frecuentes destacan las infecciones y las fracturas del aloinjero asociadas a roturas o aflojamientos de la osteosíntesis. Se presentan dos casos tratados con reconstrucciones de este tipo en los que el sistema de osteosíntesis utilizado fracasó, en uno de ellos de forma repetida.The reconstruction of skeletal defects using bone allografts is a frequent procedure after tumoral resections associated to good long-term results, usually better than diaphyseal endoprostheses or autologous grafts.     Influencia biomecánica de la artroplastia total de rodilla en el apoyo metatarsal

    Las articulaciones de la extremidad inferior forman una cadena cinemática y tanto desde el punto de vista normal como patológico se influyen mutuamente. El objetivo de nuestro trabajo es analizar las características del apoyo plantar en 100 pacientes subsidiarios de prótesis total de rodilla que presentan trastornos en la alineación del miembro antes y después de la corrección del eje de la extremidad que supone la cirugía. Además se compara el patrón de apoyo metatarsal con sujetos sanos de características antropométricas similares (grupo control).El genu varo preoperatorio se relaciona con un hiperapoyo de la paleta externa y un hipoapoyo de las paletas central e interna del antepié. En cambio el valgo fisiológico conseguido en el postoperatorio se corresponde con un patrón de apoyo metatarsal al año y a los dos años similar al del grupo control con pequeñas variaciones.Peer Reviewe

    Tilting mutation of weakly symmetric algebras and stable equivalence

    We consider tilting mutations of a weakly symmetric algebra at a subset of simple modules, as recently introduced by T. Aihara. These mutations are defined as the endomorphism rings of certain tilting complexes of length 1. Starting from a weakly symmetric algebra A, presented by a quiver with relations, we give a detailed description of the quiver and relations of the algebra obtained by mutating at a single loopless vertex of the quiver of A. In this form the mutation procedure appears similar to, although significantly more complicated than, the mutation procedure of Derksen, Weyman and Zelevinsky for quivers with potentials. By definition, weakly symmetric algebras connected by a sequence of tilting mutations are derived equivalent, and hence stably equivalent. The second aim of this article is to study these stable equivalences via a result of Okuyama describing the images of the simple modules. As an application we answer a question of Asashiba on the derived Picard groups of a class of self-injective algebras of finite representation type. We conclude by introducing a mutation procedure for maximal systems of orthogonal bricks in a triangulated category, which is motivated by the effect that a tilting mutation has on the set of simple modules in the stable category.Comment: Description and proof of mutated algebra made more rigorous (Prop. 3.1 and 4.2).     Concurrent validity of the Alcohol Purchase Task for measuring the reinforcing efficacy of alcohol: an updated systematic review and meta-analysis

    Background and aims: An early meta-analysis testing the concurrent validity of the Alcohol Purchase Task (APT), a measure of alcohol''s relative reinforcing value, reported mixed associations, but predated a large number of studies. This systematic review and meta-analysis sought to: (1) estimate the relationships between trait-based alcohol demand indices from the APT and multiple alcohol indicators, (2) test several moderators and (3) analyze small study effects. Methods: A meta-analysis of 50 cross-sectional studies in four databases (n = 18 466, females = 43.32%). Sex, year of publication, number of APT prices and index transformations (logarithmic, square root or none) were considered as moderators. Small study effects were examined by using the Begg–Mazumdar, Egger''s and Duval & Tweedie''s trim-and-fill tests. Alcohol indicators were quantity of alcohol use, number of heavy drinking episodes, alcohol-related problems and hazardous drinking. APT indices were intensity (i.e. consumption at zero cost), elasticity (i.e. sensitivity to increases in costs), Omax (i.e. maximum expenditure), Pmax (i.e. price associated to Omax) and breakpoint (i.e. price at which consumption ceases). Results: All alcohol demand indices were significantly associated with all alcohol-related outcomes (r = 0.132–0.494), except Pmax, which was significantly associated with alcohol-related problems only (r = 0.064). The greatest associations were evinced between intensity in relation to alcohol use, hazardous drinking and heavy drinking and between Omax and alcohol use. All the tested moderators emerged as significant moderators. Evidence of small-study effects was limited. Conclusions: The Alcohol Purchase Task appears to have concurrent validity in alcohol research. Intensity and Omax are the most relevant indices to account for alcohol involvement

    Injuries in Traditional Wrestling: The Leonese Wrestling Case Study (2005-2015)

    El objetivo de este estudio fue describir la localización anatómica, el tipo y la gravedad de las lesiones ocurridas en las competiciones oficiales de las ligas de verano masculinas (2005-2015) de Lucha Leonesa. Al objeto de realizar un análisis más profundo, se tuvo en cuenta, el perfil del luchador. Se produjeron un total de 401 lesiones, siendo los luchadores de mayor nivel los que menos número de lesiones sufrieron. Las contusiones fueron las lesiones más frecuentes (41,4%) pero entre las graves fueron los esguinces (42,3%). Los miembros inferiores fueron la localización más frecuente en el total de lesiones (38,2%) y entre las graves (45,4%). La rodilla, el tórax y el hombro acumulaban casi la mitad de las lesiones y el 68% de las lesiones graves. Los resultados obtenidos sugieren que las características de las lesiones que se pueden observar en la lucha leonesa son similares a las de otros deportes de combateThis study aimed at describing the anatomical location, type and severity of the sport injuries observed during the Leonese Wrestling “masculine summer leagues” official tournaments (2005-2015). The wrestler’s profile was taken into account as a way to achieve a deep analysis. A total of 401 injuries were registered. The best wrestlers were the ones that suffered the least number of injuries. Contusions were the type of injuries more frequently registered (41.4%), while sprains were the most common cause of severe injury (42.3%). The most frequent anatomical location affected and the one in which a great number of severe injuries occurred were the lower limbs (38.2% and 45.4%). The knee, thorax, and shoulder accounted for almost half of the total injuries and represented 68% of the severe injuries.     Are protected areas covering important biodiversity sites? An assessment of the nature protection network in Sicily (Italy)

    GIS spatial analysis of three indicators (vegetation value, faunal richness and landscape heterogeneity) was used to detect and map High-Value Biodiversity Areas (HVBAs), estimate the coverage of biodiversity in the Sicilian protected areas network, and identify new priority areas that could improve long-term biodiversity conservation outcomes. Findings indicated that only 32% of HVBAs are currently covered by the protected areas network. Hotspot analysis revealed that a modest expansion (less than 1%) in the current extent of protected areas would include a disproportionate amount (56%) of biodiversity hotspots, and identified prioritized candidates HVBAs for designation of new protected areas. © 2018 Elsevier Lt

    Prevalence of metabolic syndrome in scholars from Bucaramanga, Colombia: a population-based study

    Background: Obesity and metabolic syndrome are strongly associated with type 2 diabetes mellitus and cardiovascular diseases, thus the increasing trend in their prevalence among children and adolescents from developing countries requires a further understanding of their epidemiology and determinants. Methods and design: A cross-sectional study was designed to determine the prevalence of metabolic syndrome among 6–10 year-old children from Bucaramanga, Colombia. A two-stage random-cluster (neighborhoods, houses) sampling process was performed based on local city maps and local statistics. The study involves a domiciliary survey; including a comprehensive sociodemographic, nutritional and physical activity characterization of the children that participated in the study, followed by a complete clinical examination; including blood pressure, anthropometry, lipid profile determination, fasting glucose and insulin levels. The prevalence of metabolic syndrome will be determined using definitions and specific percentile cut-off points for this population. Finally, the association between components of metabolic syndrome and higher degrees of insulin resistance will be analyzed through a multivariable logistic regression model. This study protocol was designed in compliance with the Helsinki declaration and approved by the local ethics board. Consent was obtained from the children and their parents/guardians. Discussion: A complete description of the environmental and non-environmental factors underlying the burden of metabolic syndrome in children from a developing country like Colombia will provide policy makers, health care providers and educators from similar settings with an opportunity to guide primary and secondary preventive initiatives at both individual and community levels. Moreover, this description may give an insight into the pathophysiological mechanisms mediating the development of cardio-metabolic diseases early in life

    Participatory coastal management through elicitation of ecosystem service preferences and modelling driven by coastal squeeze

    The Baixo Vouga Lagunar (BVL) is part of Ria de Aveiro coastal lagoon in Portugal, which is classified as a Special Protection Area under the European Habitats and Birds Directives. This part of the system, corresponding to the confluence of the Vouga River with the lagoon, is very important culturally and socioeconomically for the local communities, taking place several human activities, especially agriculture. To prevent salt water intrusion from the Ria de Aveiro into agriculture fields, a floodbank was initiated in the 90's. In frame of ongoing changes in Ria de Aveiro hydrodynamics, the existing floodbank will be now extended, introducing further changes in the ecological dynamics of the BVL and its adjacent area. As a consequence, the water level in the floodbank downstream side is expected to rise, increasing the submersion period in tidal wetlands, and leading to coastal squeeze. The aim of this study is to apply an ecosystem based-management approach to mitigate the impacts on biodiversity resulting from the management plan. To do so, we have modelled the implications of the changes in several hydrological and environmental variables on four saltmarsh species and habitats distribution, as well as on their associated ecosystem services, both upstream and downstream of the floodbank. The ecosystem services of interest were prioritized by stakeholders' elicitation, which were then used as an input to a spatial multi-criteria analysis aimed to find the best management actions to compensate for the unintended loss of biodiversity and ecosystem services in the BVL. According to our results, the main areas to be preserved in the BVL were the traditional agricultural mosaic fields; the freshwater courses and the subtidal estuarine channels.
