    Seudomicetoma dermatofítico en un gato persa : aspectos clínicos, patológicos y evolutivos

    En este trabajo presentamos los aspectos clínicos, anatomopatológicos y evolutivos de un caso de dermatitis piogranulomatosa múltiple causada por hongos del género Microsporum, también conocida como seudomicetoma dermatofítico, en un gato persa, no castrado, de un año de edad al comienzo de la enfermedad. Las lesiones se caracterizaron macroscópicamente por la formación de numerosos nódulos de diferentes tamaños, localizados en la cabeza y antebrazo derecho, extendiéndose posteriormente a zona dorso lateral del tronco. La evolución de estos nódulos, que se generalizaron en algo menos de un mes, era hacia la coalescencia y fistulización, con descarga de un material grumoso blanco-grisáceo. El estudio histopatológico reveló una dermatitis piogranulomatosa, supeficial y profunda, caracterizada por presentar en el centro de los granulomas agregados de hongos septados inmersos en una matriz acidófila. La extirpación quirúrgica de un gran número de nódulos proporcionó mejorías transitorias, aunque reaparecieron nuevas lesiones. En este trabajo se describe la evolución tras dos años de seguimiento clínico. Tras el tratamiento con griseofulvina no se detectan nuevas lesiones después de 6 meses de su aplicación y seguimiento.This paper reports the clinical and histopathological features of a case of pyogranulomatous dermatitis caused by Microsporum fungi, a condition also known as dermatophyte pseudomycetoma, in a eneyear-old Persian cat. Grossly apparent fistulized nodules of varying sizes displaying a granular discharge were localized on the head, back, elbow and tail. Microscopic analysis revealed a deep piogranulomatous dermatitis characterized by centrally-located clusters of fungi within an acidophilic matrix. In just under a month, nodules had beco me generalized. Partial surgical excision and non-specific treatment provided temporary improvement, but new lesions subsequentlyappeared

    Relation between plasma antioxidant vitamin levels, adiposity and cardio-metabolic profile in adolescents: Effects of a multidisciplinary obesity programme

    Background & aims In vivo and in vitro evidence suggests that antioxidant vitamins and carotenoids may be key factors in the treatment and prevention of obesity and obesity-associated disorders. Hence, the objective of the present study was to determine the relationship between plasma lipid-soluble antioxidant vitamin and carotenoid levels and adiposity and cardio-metabolic risk markers in overweight and obese adolescents participating in a multidisciplinary weight loss programme. Methods A therapeutic programme was conducted with 103 adolescents aged 12–17 years old and diagnosed with overweight or obesity. Plasma concentrations of a-tocopherol, retinol, ß-carotene and lycopene, anthropometric indicators of general and central adiposity, blood pressure and biochemical parameters were analysed at baseline and at 2 and 6 months of treatment. Results Lipid-corrected retinol (P < 0.05), ß-carotene (P = 0.001) and a-tocopherol (P < 0.001) plasma levels increased significantly, whereas lipid-corrected lycopene levels remained unaltered during the treatment. Anthropometric indicators of adiposity (P < 0.001), blood pressure (P < 0.01) and biochemical parameters (P < 0.05) decreased significantly, whereas fat free mass increased significantly (P < 0.001). These clinical and biochemical improvements were related to changes in plasma lipid-corrected antioxidant vitamin and carotenoid levels. The adolescents who experienced the greatest weight loss also showed the largest decrease in anthropometric indicators of adiposity and biochemical parameters and the highest increase in fat free mass. Weight loss in these adolescents was related to an increase in plasma levels of lipid-corrected a-tocopherol (P = 0.001), ß-carotene (P = 0.034) and lycopene (P = 0.019). Conclusions Plasma lipid-soluble antioxidant vitamin and carotenoid levels are associated with reduced adiposity, greater weight loss and an improved cardio-metabolic profile in overweight and obese adolescents

However, studies on conceptions and practices of teaching-learning show that these still largely focus on transmitting the musical and technical knowledge needed to produce the correct sound. Our main aim was to study the conceptions of teaching-learning held by 53 string teachers at elementary levels, and to test how they are affected by the variable teaching experience (in three groups: [a] less than 7 years; [b] 7-14 years; and [c] more than 14 years). We also wanted to determine whether these conceptions give rise to consistent profiles in three pedagogical dimensions: teaching, learning, and evaluation. We collected data by means of a multiple-choice questionnaire, and applied cluster analysis, correlations, analysis of variance (ANOVA), and post hoc tests. Agreeing with prior research, we found three distinct profiles in the answers to the questionnaire: direct, interpretative, and constructive. Teachers' beliefs were more constructive regarding teaching and evaluation than regarding learning, although younger teachers held more complex positions regarding teaching and learning music, in contrast to much research on teaching-expertise

    M1-78: a nitrogen-rich Galactic compact HII region beyond the Perseus arm

    There is considerable controversy surrounding the nature of M1-78, a compact nebula located beyond the Perseus arm. It was first classified as a planetary nebula and is nowadays generally considered to be a compact HII region. To investigate the nature M1-78 further, we present a detailed spectroscopic study of M1-78 in the optical and near-infrared. M1-78 is a high-density nebula with substantial physical differences between its two main morphological zones: a bright arc to the SW and a blob of emission in the NE. Specifically, the blob in the NE has a higher electron temperature and visual extinction than the SW arc. The most important result, however, is the confirmation of a nitrogen enrichment in M1-78. This enrichment is stronger at the location of the NE blob and is correlated with a defficiency in the O abundance and a (dubious) He enrichment. Such an abundance pattern is typical of ejecta nebulae around evolved massive stars such as Wolf-Rayet and Luminous Blue Variable stars. The spatial variations in the physical conditions and chemical abundances and the presence of more than one possible ionizing source indicates, however, that M1-78 is better described as a combination of a compact HII region + ejecta. Finally, we detect H2 emission that extends over a large (~30 arcsec) area around the ionized nebula. Analysis of the near-infrared H2 lines indicates that the excitation mechanism is UV fluorescence.Comment: 14 pages, 8 figures. Accepted for publication in A&

    Diet quality index as a predictor of treatment efficacy in overweight and obese adolescents: The EVASYON study

    Background & aim: A diet quality index (DQI) is a tool that provides an overall score of an individual''s dietary intake when assessing compliance with food-based dietary guidelines. A number of DQIs have emerged, albeit their associations with health-related outcomes are debated. The aim of the present study was to assess whether adherence to dietary intervention, and the overall quality of the diet, can predict body composition changes. Methods: To this purpose, overweight/obese adolescents (n = 117, aged: 13–16 years; 51 males, 66 females) were recruited into a multi-component (diet, physical activity and psychological support) family-based group treatment programme. We measured the adolescents’ compliance and body composition at baseline and after 2 months (intensive phase) and 13 months (extensive phase) of follow-up. Also, at baseline, after 6 months, and at the end of follow-up we calculated the DQI. Results: Global compliance with the dietary intervention was 37.4% during the intensive phase, and 14.3% during the extensive phase. Physical activity compliance was 94.1% at 2-months and 34.7% at 13months and psychological support compliance were growing over the intervention period (10.3% intensive phase and 45.3% during extensive phase). Adolescents complying with the meal frequency criteria at the end of the extensive phase had greater reductions in FMI z-scores than those did not complying (Cohen''s d = 0.53). A statistically significant association was observed with the diet quality index. DQI-A variation explained 98.1% of BMI z-score changes and 95.1% of FMI changes. Conclusions: We conclude that assessment of changes in diet quality could be a useful tool in predicting body composition changes in obese adolescents involved in a diet and physical activity intervention programme backed-up by psychological and family support

    La leptospirosis es una antigua enfermedad producida por diversas serovares de Leptospira interrogans, espiroqueta ampliamente distribuida en la naturaleza y que afecta a varias especies animales domésticos, silvestres y accidentalmente al hombre. Se presume que es la zoonosis de mayor difusión en el mundo. Se conocen unas 260 serovares reunida en 25 serogrupos. La transmisión se debe a aguas, alimentos, orinas o secreciones contaminados. Las leptospiras penetran en el organismo a través de la piel (heridas y erosiones), o de las mucosas de boca, ojos y nariz. Los reservorios son los animales, siendo los de mayor importancia los roedores y mamíferos domésticos. La provincia de Buenos Aire, cuenta con áreas rurales cuyas poblaciones (humana y animal) están expuestas a los agentes causales de esta enfermedad zoonótica. hasta el momento las políticas de salud pública y de desarrollo pecuario locales no han incluido el abordaje integral de ésta enfermedad. La importancia radica no tan solo por las perdidas productivas debidas a abortos en los animales, sino en la afectación del ser humano a través de sus diversas manifestaciones clínicas. Esta enfermedad está clasificada en el grupo a (notificación inmediata) dentro del listado de enfermedades de denuncia obligatoria (ley 15465 - d. r. 2126). Nuestro propósito fue determinar la seroprevalencia de Leptopirosis bovina en un estudio transversal de tipo no probabilístico en 3 áreas productivas de la Provincia de Buenos Aires (Cuenca del salado, La Plata y Gran La Plata).Facultad de Ciencias Veterinaria

    La transfusión de sangre alogénica (TSA) no es inocua, y como consecuencia han surgido múltiples alternativas a la misma (ATSA). Existe variabilidad respecto a las indicaciones y buen uso de las ATSA. Dependiendo de la especialidad de los médicos que tratan a los pacientes, el grado de anemia, la política transfusional, la disponibilidad de las ATSA y el criterio personal, estas se usan de forma variable. Puesto que las ATSA tampoco son inocuas y pueden no cumplir criterios de coste-efectividad, la variabilidad en su uso es inaceptable. Las sociedades españolas de Anestesiología y Reanimación (SEDAR), Hematología y Hemoterapia (SEHH), Farmacia Hospitalaria (SEFH), Medicina Intensiva y Unidades Coronarias (SEMICYUC), Trombosis y Hemostasia (SETH) y Transfusiones Sanguíneas (SETS) han elaborado un documento de consenso para el buen uso de la ATSA. Un panel de expertos de las 6 sociedades ha llevado a cabo una revisión sistemática de la literatura médica y elaborado el 2013. Documento Sevilla de Consenso sobre Alternativas a la Transfusión de Sangre Alogénica. Solo se contempla las ATSA dirigidas a disminuir la transfusión de concentrado de hematíes. Se definen las ATSA como toda medida farmacológica y no farmacológica encaminada a disminuir la transfusión de concentrado de hematíes, preservando siempre la seguridad del paciente. La cuestión principal que se plantea en cada ítem se formula, en forma positiva o negativa, como: «La ATSA en cuestión reduce/no reduce la tasa transfusional». Para formular el grado de recomendación se ha usado la metodología Grades of Recommendation Assessment, Development and Evaluation (GRADE)
