    Association Between Early Hyperoxia Exposure After Resuscitation From Cardiac Arrest and Neurological Disability: Prospective Multicenter Protocol-Directed Cohort Study

    BACKGROUND: Studies examining the association between hyperoxia exposure after resuscitation from cardiac arrest and clinical outcomes have reported conflicting results. Our objective was to test the hypothesis that early postresuscitation hyperoxia is associated with poor neurological outcome. METHODS: This was a multicenter prospective cohort study. We included adult patients with cardiac arrest who were mechanically ventilated and received targeted temperature management after return of spontaneous circulation. We excluded patients with cardiac arrest caused by trauma or sepsis. Per protocol, partial pressure of arterial oxygen (Pao2) was measured at 1 and 6 hours after return of spontaneous circulation. Hyperoxia was defined as a Pao2 >300 mm Hg during the initial 6 hours after return of spontaneous circulation. The primary outcome was poor neurological function at hospital discharge, defined as a modified Rankin Scale score >3. Multivariable generalized linear regression with a log link was used to test the association between Pao2 and poor neurological outcome. To assess whether there was an association between other supranormal Pao2 levels and poor neurological outcome, we used other Pao2 cut points to define hyperoxia (ie, 100, 150, 200, 250, 350, 400 mm Hg). RESULTS: Of the 280 patients included, 105 (38%) had exposure to hyperoxia. Poor neurological function at hospital discharge occurred in 70% of patients in the entire cohort and in 77% versus 65% among patients with versus without exposure to hyperoxia respectively (absolute risk difference, 12%; 95% confidence interval, 1-23). Hyperoxia was independently associated with poor neurological function (relative risk, 1.23; 95% confidence interval, 1.11-1.35). On multivariable analysis, a 1-hour-longer duration of hyperoxia exposure was associated with a 3% increase in risk of poor neurological outcome (relative risk, 1.03; 95% confidence interval, 1.02-1.05). We found that the association with poor neurological outcome began at ≥300 mm Hg. CONCLUSIONS: Early hyperoxia exposure after resuscitation from cardiac arrest was independently associated with poor neurological function at hospital discharge

    Principles and practices of ESL teachers: A study of adults and children

    The study reported here aimed to identify the particular practices and uncover the particular principles of a group of ESL teachers. Some of the teachers were working with adult learners and some were working with young children. The research aimed to answer three main questions: 1. What are the classroom practices of this group of language teachers? 2. What principles do they identify as underlying their work in the classroom? 3. What is the relationship between the principles they held and the practices they adopted? The study also aimed to find out if there were similarities or differences between teachers of adults and teachers of children in both the practices they adopted and the principles which motivated their work. In other words, we wanted to know whether teachers\u27 thinking and behaviour in the classroom were influenced by the age of the learners whom they taught. The study further aimed to consider if variation among the teachers in their practices and principles was due to the teachers\u27 professional experiences, their own language backgrounds, and the different institutional contexts in which they were teaching

    Dry Matter Yield of Perennial Ryegrass Cultivars under Mechanical Cutting and Animal Grazing

    Perennial ryegrass evaluation trials are essential to identifying and promoting the most productive cultivars for use at farm level to maximise sward productivity (Grogan and Gilliland 2011). Cultivar testing is predominantly conducted under simulated grazing trials to predict dry matter yield (DMY) performance under animal grazing. Previous studies have shown a high correlation in DMY between these two defoliation methods (Camlin and Stewart 1975; Creighton et al. 2010). In contrast, Binnie and Chestnutt (1991) demonstrated that swards grazed by animals had higher DMY performance than those exposed to simulated grazing managements. Animal pressures such as pulling, treading and nutrient return are not present in a simulated grazing management.     A Model for the Propagation of Sound in Granular Materials

    This paper presents a simple ball-and-spring model for the propagation of small amplitude vibrations in a granular material. In this model, the positional disorder in the sample is ignored and the particles are placed on the vertices of a square lattice. The inter-particle forces are modeled as linear springs, with the only disorder in the system coming from a random distribution of spring constants. Despite its apparent simplicity, this model is able to reproduce the complex frequency response seen in measurements of sound propagation in a granular system. In order to understand this behavior, the role of the resonance modes of the system is investigated. Finally, this simple model is generalized to include relaxation behavior in the force network -- a behavior which is also seen in real granular materials.     'Against the World': Michael Field, female marriage and the aura of amateurism'

    This article considers the case of Katherine Bradley and Edith Cooper, an aunt and niece who lived and wrote together as ‘Michael Field’ in the fin-de-siècle Aesthetic movement. Bradley’s bold statement that she and Cooper were ‘closer married’ than the Brownings forms the basis for a discussion of their partnership in terms of a ‘female marriage’, a union that is reflected, as I will argue, in the pages of their writings. However, Michael Field’s exclusively collaborative output, though extensive, was no guarantee for success. On the contrary, their case illustrates the notion, valid for most products of co-authorship, that the jointly written work is always surrounded by an aura of amateurism. Since collaboration defied the ingrained notion of the author as the solitary producer of his or her work, critics and readers have time and again attempted to ‘parse’ the collaboration by dissecting the co-authored work into its constituent halves, a treatment that the Fields too failed to escape

    The Prevalence and Volumetry of Pituitary Cysts in Children with Growth Hormone Deficiency and Idiopathic Short Stature

    Background Pituitary cysts have been speculated to cause endocrinopathies. We sought to describe the prevalence and volumetry of pituitary cysts in patients with growth hormone deficiency (GHD) and idiopathic short stature (ISS). Methods Six hundred and eighteen children evaluated for growth failure at the Division of Pediatric Endocrinology at New York Medical College between the years 2002 and 2012, who underwent GH stimulation testing and had a brain magnetic resonance imaging (MRI) prior to initiating GH treatment were randomly selected to be a part of this study. High resolution MRI was used to evaluate the pituitary gland for size and the presence of a cyst. Cyst prevalence, cyst volume and percentage of the gland occupied by the cyst (POGO) were documented. Results Fifty-six patients had a cyst, giving an overall prevalence of 9.1%. The prevalence of cysts in GHD patients compared to ISS patients was not significant (13.5% vs. 5.7%, p=0.46). Mean cyst volume was greater in GHD patients than ISS patients (62.0 mm3 vs. 29.4 mm3, p=0.01). POGO for GHD patients was significantly greater (p=0.003) than for ISS patients (15.3%+/-12.8 vs. 7.1%+/-8.0). Observers were blinded to patient groups. Conclusions GHD patients had a significantly greater volume and POGO compared to ISS patients. This raises the question of whether cysts are implicated in the pathology of growth failure

    An Efficient PCA Ensemble Learning Approach for Prediction of RNA-Seq Malaria Vector Gene Expression Data Classification

    Malaria parasites adopt outstanding variation of life phases as they evolve through manifold mosquito vector atmospheres. Transcriptomes of thousands of individual parasites exists. Ribonucleic acid sequencing (RNA-seq) is a widespread method for gene expression which has resulted into improved understandings of genetical queries. RNA-seq compute transcripts of gene expressions. RNA-seq data necessitates analytical improvements of machine learning techniques. Several learning approached have been proposed by researchers for analysing biological data. In this study, PCA feature extraction algorithm is used to fetch latent components out of a high dimensional malaria vector RNA-seq dataset, and evaluates it classification performance using an Ensemble classification algorithm. The effectiveness of this experiment is validated on aa mosquito anopheles gambiae RNA-Seq dataset. The experiment result achieved a relevant performance metrics with a classification accuracy of 93.3%

    A fossil peat deposit from the Late Triassic (Carnian) of Zimbabwe with preserved cuticle of Pteridospermopsida and Ginkgoales, and its geological setting

    Well-preserved cuticular material of Pteridospermopsida and Ginkgoales from the Late Triassic of Zimbabwe is described here for the first time. It is preserved within a brown peat-like lens in the Upper Karoo Angwa Sandstone Formation. The locality is on the Manyima River in the lower portion of the mid-Zambezi Valley of Zimbabwe. Using SEM and light microscopy to identify the taxa, the fragmentary cuticles are of Pteridospermopsida type and have been assigned to Lepidopteris sp. (Peltaspermales) and Dicroidium sp. A, B, (Corystospermales). Cuticles of the ginkgoalean leaf genus, Sphenobaiera, are also described. Well-preserved ovules were found in close association with the cuticles, but as the stomata are not visible they cannot be assigned to any genus. Based on their close similarity to the Dicroidium flora of the South African Upper Karoo, the plants are considered to be equivalent to the South African Molteno Formation in age (Carnian). The palynoflora supports this age bracket, as does fauna preserved nearby.
