    Disruption risk analysis of the overhead power lines in Portugal

    Financiado para publicación en acceso aberto: Universidade de Vigo/CISUGThe growing increase in frequency and intensity of extreme weather events (EWEs) has a wide impact on energy systems and consumers, as energy transmission infrastructures - overhead power lines (OPL). The main objective of this work is to present the methodology of risk analysis of the EWEs on OPL in Portugal. The level of risk associated with each of the identified events is classified according to the probability of occurrence and consequences, in a risk matrix, and through the cause-and-effect analysis. It is concluded that, in Portugal, the extreme wind – corresponding to level 11 of the Beaufort Wind Force Scale, that is, values equal to or higher than 105.1 km h- 1 (29.22 m s-1) – is the main factor that provoked the OPL disruption, between 28% and 40% of analyzed events associated with windstorms. Considering the occurrence of compound events - wind and rain - the probability of damage to OPL is between 21% and 30%; for wind and ice, it is 3%–5%. EWEs represent a serious risk for electrical systems, and it is necessary to develop effective solutions to minimize the associated impacts, such as the modification and upgrade of the current design and engineering standards, and electrical network monitoring.Fundação para a Ciência e a Tecnologia | Ref.     Mechanical ventilation for the treatment of severe excessive dynamic airway collapse

    [Excerpt] We read with interest the case report by Ismael et al1 describing a patient with Sjo¨gren’s syndrome and cystic lung disease who could not be weaned from a ventilator due to severe central excessive dynamic airway collapse (EDAC) of the lower part of the trachea and proximal bronchi. EDAC corresponds to the expiratory bulging of the tracheobronchial wall without known airway structural abnormalities, leading to a decrease of at least 50% in internal diameter.2 It is a rare and underdiagnosed entity, commonly confused with other respiratory diseases such as asthma and COPD.     Synoptic and dynamical characteristics of high-impact storms affecting the Iberian Peninsula during the 2018–2021 extended winters

    In the extended winters from December 2017 to April 2021, numerous high-impact storms affected the Iberian Peninsula (IP) with heavy precipitation and/or strong winds. Here, we provide a comprehensive assessment of these events, synoptic conditions, large-scale dynamics associated with storms, and a climatological analysis to improve public awareness and natural disaster prevention. Variability analysis presents that their maximum intensity ranges from 955 hPa to 985 hPa, a two-to-four-day lifetime, and the highest frequency (eight events) occurred in January. At the instant of maximum intensity, anomalies presented low MSLP values (−21.6 hPa), high values of water vapor (327.6 kg m−1s−1) and wind speed at 250 hPa (29.6 m s−1), high values of θe at 850 hPa (19.1 °C), SST (−1 °C), and QE (−150 W m−2), near Iberia. The values obtained during the storm impact days exceeded the 98th percentile values in a high percentage of days for daily accumulated precipitation (34%), instantaneous wind gusts (46%), wind speed at 10 m (47%), and concurrent events of wind/instantaneous wind gusts and precipitation (26% and 29%, respectively). These results allow us to characterize their meteorological impacts on the IP, namely those caused by heavy precipitation and wind.Fundação para a Ciência e a Tecnologia | Ref. All rights reserved.In recent years, the fight against climate change and the mitigation of the impact of fluorinated gases (F-gases) on the atmosphere is a global concern. Development of technologies that help to efficiently separate and recycle hydrofluorocarbons (HFCs) at the end of the refrigeration and air conditioning equipment life is a priority. The technological development is important to stimulate the F-gas capture, specifically difluoromethane (R-32) and 1,1,1,2-tetrafluoroethane (R-134a), due to their high global warming potential. In this work, the COSMO-RS method is used to analyze the solute-solvent interactions and to determine Henry's constants of R-32 and R-134a in more than 600 ionic liquids. The three most performant ionic liquids were selected on the basis of COSMO-RS calculations, and F-gas absorption equilibrium isotherms were measured using gravimetric and volumetric methods. Experimental results are in good agreement with COSMO-RS predictions, with the ionic liquid tributyl(ethyl)phosphonium diethyl phosphate, [P2444][C2C2PO4], being the salt presenting the highest absorption capacities in molar and mass units compared to salts previously tested. The other two ionic liquids selected, trihexyltetradecylphosphonium glycinate, [P66614][C2NO2], and trihexyl(tetradecyl)phosphonium 2-cyano-pyrrole, [P66614][CNPyr], may be competitive as far as their absorption capacities are concerned. Future works will be guided on evaluating the performance of these ionic liquids at an industrial scale by means of process simulations, in order to elucidate the role in process efficiency of other relevant absorbent properties such as viscosity, molar weight, or specific heat.publishersversionpublishe

    Global impact of bronchiectasis and cystic fibrosis

    Educational aims To recognise the clinical and radiological presentation of the spectrum of diseases associated with bronchiectasis.; To understand variation in the aetiology, microbiology and burden of bronchiectasis and cystic fibrosis across different global healthcare systems.; Bronchiectasis is the term used to refer to dilatation of the bronchi that is usually permanent and is associated with a clinical syndrome of cough, sputum production and recurrent respiratory infections. It can be caused by a range of inherited and acquired disorders, or may be idiopathic in nature. The most well recognised inherited disorder in Western countries is cystic fibrosis (CF), an autosomal recessive condition that leads to progressive bronchiectasis, bacterial infection and premature mortality. Both bronchiectasis due to CF and bronchiectasis due to other conditions are placing an increasing burden on healthcare systems internationally. Treatments for CF are becoming more effective leading to more adult patients with complex healthcare needs. Bronchiectasis not due to CF is becoming increasingly recognised, particularly in the elderly population. Recognition is important and can lead to identification of the underlying cause, appropriate treatment and improved quality of life. The disease is highly diverse in its presentation, requiring all respiratory physicians to have knowledge of the different “bronchiectasis syndromes”. The most common aetiologies and presenting syndromes vary depending on geography, with nontuberculous mycobacterial disease predominating in some parts of North America, post-infectious and idiopathic disease predominating in Western Europe, and post-tuberculosis bronchiectasis dominating in South Asia and Eastern Europe.     Influence of emphysema distribution on pulmonary function parameters in COPD patients

    Objective: To evaluate the impact that the distribution of emphysema has on clinical and functional severity in patients with COPD. Methods: The distribution of the emphysema was analyzed in COPD patients, who were classified according to a 5-point visual classification system of lung CT findings. We assessed the influence of emphysema distribution type on the clinical and functional presentation of COPD. We also evaluated hypoxemia after the six-minute walk test (6MWT) and determined the six-minute walk distance (6MWD). Results: Eighty-six patients were included. The mean age was 65.2 ± 12.2 years, 91.9% were male, and all but one were smokers (mean smoking history, 62.7 ± 38.4 pack-years). The emphysema distribution was categorized as obviously upper lung-predominant (type 1), in 36.0% of the patients; slightly upper lung-predominant (type 2), in 25.6%; homogeneous between the upper and lower lung (type 3), in 16.3%; and slightly lower lung-predominant (type 4), in 22.1%. Type 2 emphysema distribution was associated with lower FEV1 , FVC, FEV1 /FVC ratio, and DLCO. In comparison with the type 1 patients, the type 4 patients were more likely to have an FEV1 < 65% of the predicted value (OR = 6.91, 95% CI: 1.43-33.45; p = 0.016), a 6MWD < 350 m (OR = 6.36, 95% CI: 1.26-32.18; p = 0.025), and post-6MWT hypoxemia (OR = 32.66, 95% CI: 3.26-326.84; p = 0.003). The type 3 patients had a higher RV/TLC ratio, although the difference was not significant. Conclusions: The severity of COPD appears to be greater in type 4 patients, and type 3 patients tend to have greater hyperinflation. The distribution of emphysema could have a major impact on functional parameters and should be considered in the evaluation of COPD patients

    Publisher Copyright: Copyright © Ordem dos Médicos 2022.Introduction: Pre-exposure prophylaxis (PrEP) has gained relevance as a method of prevention for HIV in certain people and settings. Following the publication of the guideline on PrEP prescribing in Portugal, we aimed to assess the knowledge of Portuguese Medical Students about PrEP. Material and Methods: An online survey was sent to Medical students of Portuguese Medical Schools. We conducted a descriptive analysis of the results and an analytic cross-sectional study to identify factors associated with “knowing about PrEP”, “having had one class about PrEP” and “identifying eligible groups correctly”. Results: Of the 796 students that responded to the survey, 64.6% were aware of what PrEP is. Of these, 34.44% acquired this knowledge during their training. Out of the total amount of respondents, 4.77% could identify correctly and completely the eligible groups for PrEP. As the training years went by, the probability of being aware of PrEP, having had one class about PrEP, and identifying the eligible groups correctly, increased. Of the sixth-year students, 43.48% had had one class about PrEP and among the students that were aware of PrEP, 28% identified what the eligible groups were. After adjusting for the school year, we found differences between Medical Schools regarding the outcomes. The association between the different ways of knowing about PrEP and the ability to correctly identify eligible groups for PrEP was not statistically significant. Conclusion: The differences between Medical Schools could be harmonized through changes in the medical curricula that would allow for this topic to be addressed more often.publishersversionpublishe

A linguagem é transmitida entre as gerações e com ela transporta os sujeitos, ou, o que é dizer o mesmo, a possível interrogação: quem me fala?
