What Contribution Did Economic Evidence Make to the Adoption of Universal Newborn Hearing Screening Policies in the United States?
Universal newborn hearing screening (UNHS), when accompanied by timely access to intervention services, can improve language outcomes for children born deaf or hard of hearing (D/HH) and result in economic benefits to society. Early Hearing Detection and Intervention (EHDI) programs promote UNHS and using information systems support access to follow-up diagnostic and early intervention services so that infants can be screened no later than 1 month of age, with those who do not pass their screen receiving diagnostic evaluation no later than 3 months of age, and those with diagnosed hearing loss receiving intervention services no later than 6 months of age. In this paper, we first document the rapid roll-out of UNHS/EHDI policies and programs at the national and state/territorial levels in the United States between 1997 and 2005. We then review cost analyses and economic arguments that were made in advancing those policies in the United States. Finally, we examine evidence on language and educational outcomes that pertain to the economic benefits of UNHS/EHDI. In conclusion, although formal cost-effectiveness analyses do not appear to have played a decisive role, informal economic assessments of costs and benefits appear to have contributed to the adoption of UNHS policies in the United States
Assessing Impact of COVID-19 Pandemic on Receipt and Timeliness of Newborn Hearing Screening and Diagnostic Services Among Infants Born in Four States
The study compares receipt and timeliness of newborn hearing screening and follow-up diagnostic services between the pre-pandemic birth cohort and the pandemic birth cohort in four participating states. Findings from this study will help inform state Early Hearing Detection and Intervention (EHDI) programs in the future should major public health event occur again
Likely Impact of the COVID-19 Pandemic on Newborn Hearing Screening and Follow-up Services in the United States in 2020
This perspective aims to highlight aspects of the Early Hearing Detection and Intervention (EHDI) newborn hearing screening and follow-up processes that were impacted by the COVID-19 pandemic and considers factors that likely impacted follow-up after newborn hearing screening among infants born in the United States during 2020. Efforts to minimize the potential impact of missed or delayed identification of hearing loss in infants and young children will also be discussed to help guide future program improvement activities
A mathematical model of tumour & blood pHe regulation: The HCO-3/CO2 buffering system
Malignant tumours are characterised by a low, acidic extracellular pH (pHe) which facilitates invasion and metastasis. Previous research has proposed the potential benefits of manipulating systemic pHe, and recent experiments have highlighted the potential for buffer therapy to raise tumour pHe, prevent metastases, and prolong survival in laboratory mice. To examine the physiological regulation of tumour buffering and investigate how perturbations of the buffering system (via metabolic/respiratory disorders or changes in parameters) can alter tumour and blood pHe, we develop a simple compartmentalised ordinary differential equation model of pHe regulation by the View the MathML source buffering system. An approximate analytical solution is constructed and used to carry out a sensitivity analysis, where we identify key parameters that regulate tumour pHe in both humans and mice. From this analysis, we suggest promising alternative and combination therapies, and identify specific patient groups which may show an enhanced response to buffer therapy. As metagenomic studies move to increasing numbers of samples, communities like the human gut may benefit more from the assembly of abundant microbes in many samples, rather than the exhaustive assembly of fewer samples. We term this approach leaderboard metagenome sequencing. To explore protocol optimization for leaderboard metagenomics in real samples, we introduce a benchmark of library prep and sequencing using internal references generated by synthetic long-read technology, allowing us to evaluate high-throughput library preparation methods against gold-standard reference genomes derived from the samples themselves. An integrated framework for sustainable development goals
The United Nations (UN) Rio+20 summit committed nations to develop a set of universal sustainable development goals (SDGs) to build on the millennium development goals (MDGs) set to expire in 2015. Research now indicates that humanity’s impact on Earth’s life support system is so great that further global environmental change risks undermining long-term prosperity and poverty eradication goals. Socioeconomic development and global sustainability are often posed as being in conflict because of trade-offs between a growing world population, as well as higher standards of living, and managing the effects of production and consumption on the global environment. We have established a framework for an evidence-based architecture for new goals and targets. Building on six SDGs, which integrate development and environmental considerations, we developed a comprehensive framework of goals and associated targets, which demonstrate that it is possible, and necessary, to develop integrated targets relating to food, energy, water, and ecosystem services goals; thus providing a neutral evidence-based approach to support SDG target discussions. Global analyses, using an integrated global target equation, are close to providing indicators for these targets. Alongside development-only targets and environment-only targets, these integrated targets would ensure that synergies are maximized and trade-offs are managed in the implementation of SDGs
Effect of Re-impacting Debris on the Solidification of the Lunar Magma Ocean
Anorthosites that comprise the bulk of the lunar crust are believed to have
formed during solidification of a Lunar Magma Ocean (LMO) in which these rocks
would have floated to the surface. This early flotation crust would have formed
a thermal blanket over the remaining LMO, prolonging solidification.
Geochronology of lunar anorthosites indicates a long timescale of LMO cooling,
or re-melting and re-crystallization in one or more late events. To better
interpret this geochronology, we model LMO solidification in a scenario where
the Moon is being continuously bombarded by returning projectiles released from
the Moon-forming giant impact. More than one lunar mass of material escaped the
Earth-Moon system onto heliocentric orbits following the giant impact, much of
it to come back on returning orbits for a period of 100 Myr. If large enough,
these projectiles would have punctured holes in the nascent floatation crust of
the Moon, exposing the LMO to space and causing more rapid cooling. We model
these scenarios using a thermal evolution model of the Moon that allows for
production (by cratering) and evolution (solidification and infill) of holes in
the flotation crust that insulates the LMO. For effective hole production,
solidification of the magma ocean can be significantly expedited, decreasing
the cooling time by more than a factor of 5. If hole production is inefficient,
but shock conversion of projectile kinetic energy to thermal energy is
efficient, then LMO solidification can be somewhat prolonged, lengthening the
cooling time by 50% or more
Ludic maps and capitalist spectacle in Rio de Janeiro
Rio de Janeiro is undergoing a makeover that is undeniably spectacular. Redevelopment schemes are dramatically rearranging the urban landscape, and crucially, this economic growth hinges on the production and circulation of images of the city. This paper explores a site of alterity and resistance where a favela youth collective has re-created Rio from its margins. This miniature world, known as Morrinho and built of bricks, mortar, and re-used materials, hosts a role-playing game featuring thousands of inch-tall avatars. This paper argues that re-visioning the world anew through play makes the society of the spectacle inhabitable and thus contestable. How does the society of the spectacle become a terrain for struggle in Rio? Locating spectacle production in nation-state formation and the urban process, the paper provides a genealogy of the spectacle beyond the modern North Atlantic metropole. Locating the favela within a Brazilian geographical imagination frames ethnographic data collected as an observer and participant in the Morrinho game. While the spectacle may hinge on the relationship between visuality and power, this essay observes how signs take on material lives through ludic re-appropriation. Play becomes a form of commentary, an alternative mode of knowledge about the city, and functions dually as both description of and participant in the social world in which it is embedded
Expression of estrogen receptor beta in the breast carcinoma of BRCA1 mutation carriers
<p>Abstract</p> <p>Background</p> <p>Breast cancers (BC) in women carrying mutations in BRCA1 gene are more frequently estrogen receptor negative than the nonhereditary BC. Nevertheless, tamoxifen has been found to have a protective effect in preventing contralateral tumors in BRCA1 mutation carriers. The identification of the second human estrogen receptor, ERβ, raised a question of its role in hereditary breast cancer. The aim of this study was to assess the frequency of ERα, ERβ, PgR (progesterone receptor) and HER-2 expression in breast cancer patients with mutated <it>BRCA1 </it>gene and in the control group.</p> <p>Methods</p> <p>The study group consisted of 48 women with <it>BRCA1 </it>gene mutations confirmed by multiplex PCR assay. The patients were tested for three most common mutations of BRCA1 affecting the Polish population (5382insC, C61G, 4153delA). Immunostaining for ERα, ERβ and PgR (progesterone receptor) was performed using monoclonal antibodies against ERα, PgR (DakoCytomation), and polyclonal antibody against ERβ (Chemicon). The EnVision detection system was applied. The study population comprised a control group of 120 BC operated successively during the years 1998–99.</p> <p>Results</p> <p>The results of our investigation showed that <it>BRCA1 </it>mutation carriers were more likely to have ERα-negative breast cancer than those in the control group. Only 14.5% of <it>BRCA1</it>-related cancers were ERα-positive compared with 57.5% in the control group (<it>P </it>< 0.0001). On the contrary, the expression of ERβ protein was observed in 42% of <it>BRCA1</it>-related tumors and in 55% of the control group. An interesting finding was that most hereditary cancers (75% of the whole group) were triple-negative: ERα(-)/PgR(-)/HER-2(-) but almost half of this group (44.4%) showed the expression of ERβ.</p> <p>Conclusion</p> <p>In the case of <it>BRCA1</it>-associated tumors the expression of ERβ was significantly higher than the expression of ERα. This may explain the effectiveness of tamoxifen in preventing contralateral breast cancer development in <it>BRCA1 </it>mutation carriers.</p
