    Translational research opportunities regarding homologous recombination in ovarian cancer

    Homologous recombination (HR) is a DNA repair pathway that is deficient in 50% of high-grade serous ovarian carcinomas (HGSOC). Deficient HR (DHR) constitutes a therapeutic opportunity for these patients, thanks to poly (ADP-ribose) polymerases (PARP) inhibitors (PARPi; olaparib, niraparib, and rucaparib are already commercialized). Although initially, PARPi were developed for patients with BRCA1/2 mutations, robust clinical data have shown their benefit in a broader population without DHR. This breakthrough in daily practice has raised several questions that necessitate further research: How can populations that will most benefit from PARPi be selected? At which stage of Ovarian Cancer should PARPi be used? Which strategies are reasonable to overcome PARPi resistance? In this paper, we present a summary of the literature and discuss the present clinical research involving PARPi (after reviewing ClinicalTrials.gov) from a translational perspective. Research into the functional biomarkers of DHR and clinical trials testing PARPi benefits as first-line setting or rechallenge are currently ongoing. Additionally, in the clinical setting, only secondary restoring mutations of BRCA1/2 have been identified as events inducing resistance to PARPi. The clinical frequency of this and other mechanisms that have been described in preclinics is unknown. It is of great importance to study mechanisms of resistance to PARPi to guide the clinical development of drug combinations

    Background. The open-label, single-arm, multicenter ORZORA trial (NCT02476968) evaluated the efficacy and safety of maintenance olaparib in patients with platinum-sensitive relapsed ovarian cancer (PSR OC) who had tumor BRCA mutations (BRCAm) of germline (g) or somatic (s) origin or non-BRCA homologous recombination repair mutations (HRRm) and were in response to their most recent platinum-based chemotherapy after >= 2 lines of treatment. Methods. Patients received maintenance olaparib capsules (400 mg twice daily) until disease progression. Prospective central testing at screening determined tumor BRCAm status and subsequent testing determined gBRCAm or sBRCAm status.     Factors pronĂČstics inicials i tardans en els tumors epitelials no benignes de l’ovari: des del diagnĂČstic fins a les complicacions

    Els tumors epitelials no benignes de l'ovari comprenen cĂ ncers i tumors borderline (BT) originats a l'ovari, trompes de Fal·lopi o peritoneu. Malgrat diferĂšncies histopatolĂČgiques i moleculars, comparteixen la via de disseminaciĂł transcelĂČmica, procediments diagnĂČstics i quirĂșrgics, i alguns d'ells la carcinogĂšnesi. Mentre que el cĂ ncer el formen cĂšl·lules proliferatives anĂČmales capaces d'envair altres teixits, els BT es caracteritzen per la proliferaciĂł atĂ­pica sense invasiĂł de l'estroma i un excel·lent pronĂČstic a llarg termini (malgrat que algunes pacients moriran per recaigudes invasives peritoneals). Factors pronĂČstics coneguts comuns sĂłn l'estadiatge FIGO i la presĂšncia de malaltia residual post cirurgia. L'edat, l'estat general i el CA125 inicials podrien tenir un paper diferencial entre els cĂ ncers i BT, i es desconeix el paper d'altres factors potencials. L'objectiu d'aquest treball, compost de 3 articles publicats, Ă©s l'estudi de potencials factors pronĂČstics. El primer article estudia la radicalitat de la cirurgia en els BT. Si bĂ© Ă©s prioritĂ ria en els cĂ ncers, no Ă©s clara la rellevĂ ncia de l'estadificaciĂł quirĂșrgica abdominal estĂ ndard en els BT, malgrat el risc d'obviar implants abdominals ocults en el 10-30% dels casos. A l'estudi, es van revisar 46 BT per analitzar diversos factors pronĂČstics, incloent la radicalitat de l'estadificaciĂł quirĂșrgica, observant-se que la cirurgia d'estadificaciĂł incompleta estava associada a un major risc de recaiguda (log-rank p = 0,038), perĂČ no a canvis de la supervivĂšncia global. Considerant aquests i d'altres resultats publicats posteriorment, aquesta tesi conclou que la cirurgia d'estadificaciĂł completa ha de ser considerada com a mĂ­nim en el subgrup de pacients amb major risc de recaiguda (serosos en estadis avançats, serosos sense exploraciĂł abdominal mĂ­nima, i serosos micropapil·lars i microinvasors). El segon article explora si RAP80, un gen relacionat amb la recombinaciĂł homĂČloga (HR), podria ser un factor pronĂČstic en el cĂ ncer serĂłs d'alt grau esporĂ dic (sHGSOC). Cal assenyalar que l'atles del genoma del cĂ ncer d'ovari identificĂ  que el 50% dels HGSOC tenia deficiĂšncies en la HR, principalment per mutacions patogĂšniques en els gens BRCA; a mĂ©s, el "fenotip BRCAness" es relaciona amb uns millors pronĂČstic i taxes de resposta a platĂ­. Mitjançant regressiĂł de Cox, l'estudi observĂ  una correlaciĂł entre nivells baixos d'expressiĂł de RAP80 (RNAm) i una pitjor supervivĂšncia en una mostra de 35 sHGSOC operats i exposats a platĂ­ en primera lĂ­nia (p = 0,047). Els autors hipotetitzen que malgrat la HR excessiva i ineficient associada a deficiĂšncies en RAP80, la redundĂ ncia funcional parcial dels macrocomplexes de BRCA1 podria preservar el seu paper en la supervivĂšncia cel·lular, el que suggereix que la deficiĂšncia de HR causada per una baixa expressiĂł de RAP80 no estaria associada amb la hipersensibilitat als compostos de platĂ­. El tercer article explora factors pronĂČstics de resoluciĂł de l'obstrucciĂł intestinal maligna inoperable (OIMI), una complicaciĂł greu dels tumors d'ovari. La taxa de desobstrucciĂł Ă©s baixa (30-40%) i la presa de decisions de final de vida Ă©s complicada per la manca de factors pronĂČstics coneguts. Aquest treball estudiĂ  prospectivament 45 pacients ingressats per OIMI i tractats amb antiemĂštics, esteroides, octreotida i butilbromur d'hioscina. Es van realitzar dues regressions logĂ­stiques, la primera amb dades del dia 1 d'admissiĂł i l'altra amb dades del dia 8. La freqĂŒĂšncia de desobstrucciĂł fou del 48,9%. En l'anĂ lisi del dia 1, els episodis que havien derivat de mecanismes fisiopatolĂČgics funcionals (vs. mecĂ nics o mixts) eren mĂ©s propensos a resoldre's (p <0,001, correcciĂł per comparacions mĂșltiples), el que suggereix que aquest mecanisme de desenvolupament de l'OIMI pot ser un factor pronĂČstic precoç de desobstrucciĂł. Aquests estudis generen noves hipĂČtesis i apunten futures lĂ­nies d'investigaciĂł que ajudaran a subclassificar el pronĂČstic de les pacients i eventualment guiar les decisions terapĂšutiques.Non-benign epithelial ovarian tumors comprise cancers and borderline tumors (BT) that arise from the ovary, the Fallopian tubes, or the peritoneum. Despite histopathologic and molecular differences, they share a common transcelomic spreading way, diagnostic and surgical procedures and some of them are related in their carcinogenesis. While cancers are composed of abnormal proliferative cells able to invade other tissues, BT are characterized by atypical proliferation without stromal invasion, and an overall excellent long-term prognostic (although some patients will eventually die from peritoneal invasive relapses). Common prognostic factors are FIGO staging and residual disease at surgery, whereas age, performance status and CA125 at diagnosis could have a differential role in cancers and BT. The aim of this work, composed of 3 published articles, is to study 3 potential prognostic factors. The first article is focused on the extent of surgery in BT. Contrary to its key importance in cancers, it is unclear the relevance of the standard abdominal surgical staging procedure in BT, despite the risk of occult abdominal implants in 10-30% of patients. Forty-six patients with BT were retrospectively reviewed to analyze several prognostic factors, including the completeness of standard surgical staging procedures (fertility sparing surgeries and one or two surgical times allowed in the definition). Log-rank tests corrected for multiple comparisons confirmed the association between incomplete staging surgery and shorter time to BT relapse (p=0.038), but no overall survival. Taking into account these and later published results from other authors, this thesis concludes that complete staging surgery should be at least considered in the subset of patients with higher risk of relapse (advanced stage serous BT, serous BT without minimal exploration of the abdominal cavity, and micropapillar and microinvasive serous BT). The second article explores whether RAP80, a homologous recombination (HR) related gene, may be a prognostic factor in sporadic high grade serous ovarian cancer (sHGSOC). It must be noted that the genome cancer atlas identified that 50% HGSOC had deficiencies in the HR pathway, being pathogenic BRCA mutations the first cause, and that the "BRCAness phenotype" is related to a better outcome and higher response rates to platinum. Analyzing 35 retrospectively retrieved tumor samples, we found preliminary clinical evidence that low RAP80 expression levels (mRNA) were correlated with worse overall survival in sHGSOC postoperatively exposed to platinum as first line, by means of Cox regressions (p=0.047). Authors' hypothesis argue that despite the excessive and inefficient HR associated to RAP80 deficiencies, the partial functional redundancy of the BRCA1 macrocomplexes could preserve BRCA1's role in cell survival, suggesting that HR deficiency due to low RAP80 expression is not associated with hypersensitivity to platinum compounds. The third article explores potential prognostic factors for resolution of inoperable malignant bowel obstruction (IMBO), a severe complication of ovarian tumors with a low desobstruction rate (30–40 %) in which end-of-life decision making is hampered by the lack of known prognostic factors. This work prospectively retrieved 45 in-patients with IMBO treated following a clinical protocol based on antiemetics, steroids, octreotide, and hyoscine butylbromide. Two prognostic factor analyses using logistic regressions were performed, one based on data from day 1 of admission and the other on data from day 8. Frequency of desobstruction was 48.9%. In the analysis of prognostic factors on day 1, IMBO episodes derived from functional physiopathologic mechanisms (vs. mechanic or mixed) were more prone to resolve (p < 0.001 corrected for multiple comparisons), suggesting that this mechanism of MBO development may be an early prognostic factor for desobstruction. This prognostic factors study arouses new hypothesis and addresses future research directions that will help to subclassify patients'outcome and eventually guide therapeutic decisions

    Relation of Lipoprotein(a) Levels to Incident Type 2 Diabetes and Modification by Alirocumab Treatment

    none1691siOBJECTIVE: In observational data, lower levels of lipoprotein(a) have been associated with greater prevalence of type 2 diabetes. Whether pharmacologic lowering of lipoprotein(a) influences incident type 2 diabetes is unknown. We determined the relationship of lipoprotein(a) concentration with incident type 2 diabetes and effects of treatment with alirocumab, a PCSK9 inhibitor. RESEARCH DESIGN AND METHODS: In the ODYSSEY OUTCOMES trial alirocumab was compared with placebo in patients with acute coronary syndrome. Incident diabetes was determined from laboratory, medication, and adverse event data. RESULTS: Among 13,480 patients without diabetes at baseline, 1,324 developed type 2 diabetes over a median 2.7 years. Median baseline lipoprotein(a) was 21.9 mg/dL. With placebo, 10 mg/dL lower baseline lipoprotein(a) was associated with hazard ratio 1.04 (95% CI 1.02-1.06, P < 0.001) for incident type 2 diabetes. Alirocumab reduced lipoprotein(a) by a median 23.2% with greater absolute reductions from higher baseline levels and no overall effect on incident type 2 diabetes (hazard ratio 0.95, 95% CI 0.85-1.05). At low baseline lipoprotein(a) levels, alirocumab tended to reduce incident type 2 diabetes, while at high baseline lipoprotein(a) alirocumab tended to increase incident type 2 diabetes compared with placebo (treatment-baseline lipoprotein(a) interaction P = 0.006). In the alirocumab group, a 10 mg/dL decrease in lipoprotein(a) from baseline was associated with hazard ratio 1.07 (95% CI 1.03-1.12; P = 0.0002) for incident type 2 diabetes. CONCLUSIONS: In patients with acute coronary syndrome, baseline lipoprotein(a) concentration associated inversely with incident type 2 diabetes. Alirocumab had neutral overall effect on incident type 2 diabetes. However, treatment-related reductions in lipoprotein(a), more pronounced from high baseline levels, were associated with increased risk of incident type 2 diabetes.     Lipoprotein(a) and Benefit of PCSK9 Inhibition in Patients With Nominally Controlled LDL Cholesterol

    International audienceBackground: Guidelines recommend nonstatin lipid-lowering agents in patients at very high risk for major adverse cardiovascular events (MACE) if low-density lipoprotein cholesterol (LDL-C) remains ≄70 mg/dL on maximum tolerated statin treatment. It is uncertain if this approach benefits patients with LDL-C near 70 mg/dL. Lipoprotein(a) levels may influence residual risk.Objectives: In a post hoc analysis of the ODYSSEY Outcomes (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) trial, the authors evaluated the benefit of adding the proprotein subtilisin/kexin type 9 inhibitor alirocumab to optimized statin treatment in patients with LDL-C levels near 70 mg/dL. Effects were evaluated according to concurrent lipoprotein(a) levels.Methods: ODYSSEY Outcomes compared alirocumab with placebo in 18,924 patients with recent acute coronary syndromes receiving optimized statin treatment. In 4,351 patients (23.0%), screening or randomization LDL-C was 13.7 mg/dL or ≀13.7 mg/dL; corresponding adjusted treatment hazard ratios were 0.82 (95% CI: 0.72-0.92) and 0.89 (95% CI: 0.75-1.06), with Pinteraction = 0.43.Conclusions: In patients with recent acute coronary syndromes and LDL-C near 70 mg/dL on optimized statin therapy, proprotein subtilisin/kexin type 9 inhibition provides incremental clinical benefit only when lipoprotein(a) concentration is at least mildly elevated.     Background After acute coronary syndrome, diabetes conveys an excess risk of ischaemic cardiovascular events. A reduction in mean LDL cholesterol to 1.4-1.8 mmol/L with ezetimibe or statins reduces cardiovascular events in patients with an acute coronary syndrome and diabetes. However, the efficacy and safety of further reduction in LDL cholesterol with an inhibitor of proprotein convertase subtilisin/kexin type 9 (PCSK9) after acute coronary syndrome is unknown.     Background: Apolipoprotein B (apoB) provides an integrated measure of atherogenic risk. Whether apoB levels and apoB lowering hold incremental predictive information on residual risk after acute coronary syndrome beyond that provided by low-density lipoprotein cholesterol is uncertain. Methods: The ODYSSEY OUTCOMES trial (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) compared the proprotein convertase subtilisin/kexin type 9 inhibitor alirocumab with placebo in 18 924 patients with recent acute coronary syndrome and elevated atherogenic lipoproteins despite optimized statin therapy. Primary outcome was major adverse cardiovascular events (MACE; coronary heart disease death, nonfatal myocardial infarction, fatal/nonfatal ischemic stroke, hospitalization for unstable angina). Associations between baseline apoB or apoB at 4 months and MACE were assessed in adjusted Cox proportional hazards and propensity score–matched models. Results: Median follow-up was 2.8 years. In proportional hazards analysis in the placebo group, MACE incidence increased across increasing baseline apoB strata (3.2 [95% CI, 2.9–3.6], 4.0 [95% CI, 3.6–4.5], and 5.5 [95% CI, 5.0–6.1] events per 100 patient-years in strata 35–<50, and ≀35 mg/dL, respectively). Compared with propensity score–matched patients from the placebo group, treatment hazard ratios for alirocumab also decreased monotonically across achieved apoB strata. Achieved apoB was predictive of MACE after adjustment for achieved low-density lipoprotein cholesterol or non–high-density lipoprotein cholesterol but not vice versa. Conclusions: In patients with recent acute coronary syndrome and elevated atherogenic lipoproteins, MACE increased across baseline apoB strata. Alirocumab reduced MACE across all strata of baseline apoB, with larger absolute reductions in patients with higher baseline levels. Lower achieved apoB was associated with lower risk of MACE, even after accounting for achieved low-density lipoprotein cholesterol or non–high-density lipoprotein cholesterol, indicating that apoB provides incremental information. Achievement of apoB levels as low as ≀35 mg/dL may reduce lipoprotein-attributable residual risk after acute coronary syndrome.
