    Survey Tendency to Leave Service and its Related some Factors Aamong Nurses in Zabol University Hospitals

    زمینه و هدف: افزایش میزان خدمات فوریتی، بار‌کاری زیاد و کمبود تعداد پرستاران، تأثیر به‌سزایی بر کیفیت مراقبت‌های پرستاری دارد. همزمان با این کمبود جهانی پرستاری، بسیاری از آن‌ها ممکن است شغل خود را ترک‌کرده و در قسمت‌های دیگری، خارج از حرفه پرستاری مشغول به‌کار‌شوند. مطالعه حاضر با هدف تعیین میزان تمایل به ترک ‌خدمت و برخی عوامل مرتبط با آن در پرستاران شاغل بیمارستان‌های تابع دانشگاه علوم‌پزشکی زابل در سال 1394 انجام پذیرفت. روش ‌بررسی: در این پژوهش توصیفی همبستگی، 196 پرستار شاغل در دو بیمارستان دانشگاهی زابل به‌صورت سرشماری شرکت داشتند. جمع‌آوری داده‌ها با استفاده از پرسشنامه مشخصات فردی (9 عبارت) و پرسشنامه پیش‌بینی ترک خدمت محقق ساخته (45 عبارت) پس از انجام روایی و پایایی انجام شد. برای بررسی داده‌ها از نرم‌افزار SPSS و آزمون های آماری فراوانی، درصد، میانگین و انحراف استاندارد، t مستقل، آنالیز واریانس یک‌طرفه و تعقیب رنج توکی، من‌ویتنی‌یو و کروسکال‌والیس استفاده شد. یافته‌‌ها: میانگین نمره تمایل به ترک خدمت پرستاران (9/29±128) بود. اکثر واحدهای پژوهش (9/69) تمایل به ترک‌ خدمت متوسط داشتند. در این بین عوامل اجتماعی با میانگین نمره 63/3 از 5 بیشترین تمایل به ترک خدمت را ایجاد کرده‌بود. میانگین ماهیت حرفه، محیط شغلی و عوامل اجتماعی مرتبط با تمایل به ترک ‌خدمت در پرستاران مونث نسبت به مذکر به‌طور‌ معنی‌داری بیشتر بود (052/0=P). پرستاران مورد مطالعه از تعلق شغلی متوسطی (8/63) برخوردار بودند. نتیجه‌‌گیری: نتایج تحقیق حاضر بیانگر تمایل متوسط به ترک خدمت پرستاران و بالا‌بودن میانگین نمره عوامل اجتماعی است؛ لذا توجه مسئولین را به بهبود و تغییر نوع نگاه جامعه به پرستاری با همکاری رسانه‌های جمعی جلب می‌کنیم

    Nurses' Perception of Patient Education in Critical Care Units: A Content Analysis

    Introduction: Patient education is the most fundamental role of nurses and an important factor in raising awareness and changing health behaviors toward various diseases, particularly coronary artery diseases. This study aims to define nurses' perception of patient education in the CCU ward of Valiasr Hospital, Birjand, Iran. Method: This was a qualitative study performed in 2011. In the present study, 11 nurses of CCU wards were recruited by purposeful sampling. Data were collected through semi-structured interviews, and were analyzed using content analysis. Results: Three themes have emerged including culture, knowledge of the patient's background, and education process suited to patients’ culture and society. Each theme has some subcategories. Conclusion: According to the results, it is necessary to design the patient education process based on culture and conditions of the patients’ community in order for that process to be effective. Methods Three databases were searched for relevant studies from launch through September of 2017 and included MEDLINE, Scopus and Web of science. Results The average knowledge of CAM therapies by nurses was 62.2% with attitude about use averaging 65.7%. Close to two-thirds (65.9%) reported use of CAM therapies with patients. The primary reasons nurses suggested use of CAM were for stress and anxiety reduction and health improvement. Conclusion Current evidence demonstrates the need for nurse education programs to integrate and strengthen CAM content into existing curricula. Similarly, documentation of the nature and extent of nurse use of CAM therapies in the clinical setting, as well as patient-reported use and preferences for CAM therapies, would provide valuable prospective data

    Problem-based learning versus reflective practice on nursing students’ moral sensitivity

    Abstract Background Moral sensitivity is one of the prerequisites for nurses’ professional competency and patient care. It is critical to teach professional ethics in a student-centered manner in order to increase students’ moral sensibility. This study evaluated the effects of professional ethics education via problem-based learning and reflective practice on nursing students’ moral sensitivity. Methods This experimental study was performed on 74 nursing students who were randomly divided into three groups of problem-based learning, reflective practice and control. Principles of professional ethics were presented for the two intervention groups in four 2-hour sessions using ethical dilemmas scenarios. Participants completed the Moral Sensitivity Questionnaire before, immediately, and three months after the intervention. Data were analyzed using SPSS16. Results Demographic characteristics of the three groups were similar (p > 0.05). The total moral sensitivity score significantly differed between the groups immediately and three months after the intervention (p < 0.001). The total mean score for moral sensitivity was significantly different between the two groups of problem-based learning and reflective practice, with the mean scores in the problem-based learning group being higher (p = 0.02). The mean score of moral sensitivity decreased statistically significantly in both experimental groups three months after the intervention as compared to immediately after the intervention (p < 0.001). Conclusion Nursing students’ moral sensitivity can be increased through reflective practice and problem-based learning. While the results indicated that problem-based learning was more successful than reflective practice, additional research is recommended to confirm the influence of these two strategies on moral sensitivity

    Nursing Students\' and Instructors\' Experience about Nursing Fundamental Course: A Qualitative Study

    Introduction: The situations affecting teaching and learning nursing fundamental course may influence acquiring competencies in primary clinical skills. Several reasons have been mentioned for this problem. The aim of this study was to describe the experiences and viewpoints of nursing students and instructors on nursing fundamental course. Methods: This was a qualitative study using thematic analysis method. Fifteen nursing students, 4 nursing instructors, the head nurse of operating ward, and two nurses graduated from the university in which the study was performed, were selected through purposive sampling and interviewed. All interviews were transcribed, analyzed. Results: Using thematic analysis, a significant number of preliminary themes and 7 main themes each having few other sub-themes, were extracted. The main themes included educational factors, not respecting the standards in performing the techniques, the gap between theory and practice, motivation, human resources, communications, management staff, equipments, and physical environment. These main themes and other sub themes have been explained in the main article using direct statements of study samples. Conclusion: The results of this study revealed problems in communication in educational environment and interaction between instructor and students as well as lack of motivation among nursing instructors and students. These are the main principles of the appropriate teaching and learning. It is recommended that nursing education administrators pay more attention to these aspects of educational process and remove the causing factors

    Supportive activities in oncological wards during the COVID-19 pandemic: a qualitative study

    Introduction. The oncology ward is a challenging and unique workplace due to physical and psychological stress that staff experience and the need for their support. Cancer patients and oncology nurses have many needs, and support is one of the basic ones. This study aimed to explore supportive activities in the oncology ward during the COVID-19 pandemic. Material and methods. This qualitative study was conducted in Eastern and Southeastern Iran in 2020 and 2021 through a conventional content analysis approach. The participants included 21 (10 oncology nurses, 5 managers, and 6 cancer patients), who were selected through purposive sampling. To collect data, in-depth semi-structured face-to-face interviews were done. Interviews were continued until data saturation was achieved. After transcribing the interviews, the data were analyzed according to the steps proposed by Graneheim &amp; Lundman.  Results. The results consisted of three main themes and nine categories, namely the perceive of threat in supportive atmosphere in the oncology ward (cancer patients’ sense of desperation and need for support, difficulty of working in the department, close relationships governing the ward), Seeking support in the oncology ward (Professional support, patient advocacy), and supportive divergence (poor family support, perceived poor social support, unsupportive behaviors, Being far from the supportive standards of working in an oncology ward).  Conclusions. The results of the study have shown that the supportive activities in the oncology ward during the COVID-19 pandemic are affected by various factors. The experiences of participants provide new insight into supportive activities around managing oncology wards supportive needs during such stressful times

    Causes of Chronic Kidney Disease in Iranian Children: A Meta-Analysis and Systematic Review

    This study aimed to determine the causes of chronic kidney disease (CKD) in Iranian children. In this systematic review and meta-analysis study, international (PubMed, Web of Science, Scopus, and Google Scholar) and national (SID, Magiran) databases were searched for articles published through December 30, 2017. The quality of the studies was determined using the Hoy instrument. Out of 2,117 initial studies, 13 studies performed on a total of 3,596 children were included in the final stage of the study. Based on the results of the random effects method (95% CI), the main causes of chronic kidney disease in stages 1-4 (CKD) were CAKUT (37%) and glomerulonephritis (19.96%); in stage 5 (ESRD) they were CAKUT (40.82%) and urological disorders (27.44%). Considering the high prevalence of CAKUT, glomerulonephritis and urinary problems, the use of comprehensive approaches can be very effective in enhancing the knowledge of patients about the causes of kidney disease. The results obtained from the present study can assist policymakers in more accurately planning screenings of the causes of CKD in Iranian children
