    Total Quality Management System in an Education Environment: The Case of a Private University in Bahrain

    The study aims to analyze and explain the effectiveness and efficiency of implementing total quality management principles in private educational institutions. The context of the study narrowed down the areas of comparisons to tutorial conducts, student affairs and infrastructure. A detailed analysis of the existing total quality management currently in place at the selected university was duly comprehended. This resulted in finding the flaws/weaknesses in the system of universities in the kingdom of Bahrain generally. Findings through a survey and interview sessions indicated that teachers were not consulted for any changes in curriculum which leads to a lack of co-operation between management and teachers. Another problem in regards to the total quality management implementation was that students perceived the university as not being concerned with maintenance of the premises. Moreover, student affairs/services section was seriously lacking sport facilities, limited training sessions, poor equipment maintenance

    Association of Physical Activity with Co-morbid Conditions in Geriatric Population

    To find out association of physical activity with co-morbid conditions in geriatric population, a cross-sectional study was conducted in different cties of Pakistan in 2015. A total of 114 participants were inducted by non-probability convenience sampling technique. Data was collected after informed verbal consent by a validated questionnaire that is Rapid Assessment of Physical Activity (RAPA). Participants were categorized into two groups i.e. physically active and physically inactive. Data was entered and analyzed in SPSS version 20. There were 66 (57.9%) males and 48 (42.1%) females with mean age of 57.04±7.348 years. Among hypertensive individuals (n=43, 37.7%) there were 39 (90.7%) physically inactive, among individuals having angina (n=17, 14.9%) there were 15 (88.2%) physically inactive. Out of 37 (32.5%) diabetics, 35 (94.6%) were physically inactive. Among individuals suffering from arthritis (n=40, 35.1%), there were 38 (95%) physically inactive. A significant association was found between physical activity and diabetes and arthritis with p-value of 0.048 and 0.029 respectively. Physical activity is significantly associated with diabetes and arthritis in geriatric population. Adequate physical activity should be performed to reduce the risk of co-morbid conditions and improve the quality of life in geriatric population

    Efficacy and safety of panchgavya GHRIT along with flunarizine in prophylaxis for migraine patients: a comparitive study

    Background: Propranolol and flunarizine have proven to be useful tools in migraine prophylaxis. This trial aims the comparison of the efficacy of flunarizine, flunarizine and placebo and flunarizine and panchgavya ghrit in migraine prophylaxis.Methods: The present study was a prospective, randomized, open-label, blinded-endpoint trial. Patients with chronic migraine were randomized (1:1:1) to flunarizine and flunarizine and placebo and flunarizine and panchgavya ghrit in three treatment groups. The study was carried out in outdoor patients in the department of Psychiatry, T.S. Mishra Medical College and Hospital, Lucknow and K.G.M.U, Ayush Department, Lucknow after clearance from Institutional Ethical Committee. Data was analysed using SPSS software.Results: The prevalence of migraine was found to be higher in the age group greater than 30 years and females. Overall there was more reduction in CGI scores in flunarizine with panchgavya ghrit and the other two groups equally at the end of 4, 6 8 and 10 weeks. Decrease in MIDAS score was observed after the therapy. Clinical Global Impression rating scale employed revealed that to start with subjects scored 7 which stands for pathology interfering in many life functions which reduced drastically in Group C as compared to Group B and Group A in descending order. Pain scales namely VAS (visual analogue scale), NPRS (Numeric Pain Rating Scale), VRS (verbal rating scale) when employed denoted there was decreased migraine frequency, decreased perception of pain, less intake of abortive medication consumed by subjects implying there were reduction in number of migraine days and there was decrease in the abortive medications taken for the same. Group C scored better on pain scales followed by Group B and Group A. Lower proportion of individuals in group C had Behavioural Toxicity and Neurological Side effects as compared to Group A and B.Conclusions: Panch gavya ghrit when administered along with flunarizine was more efficacious and safe when compared with other two groups. However large multicentric RCTs of long duration and involving more number of subjects are required to ascertain these facts

    Synthesis and liquid crystalline behaviour of substituted (E)-phenyl-4-(phenyldiazenyl) benzoate derivatives and their photo switching ability

    Azobenzene derivatives containing phenyl/4-halogen-phenyl 4-{(E)-[4-(pent-4-en-1-yloxy)phenyl] diazenyl}benzoate group with different electronegative substituent (H, F, Cl, Br and I) at other end was synthesised. These azo-based benzoate derivatives have been characterised by FTIR, 1H-NMR, 13C-NMR, elemental analyser, POM and UV-Vis spectroscopy. Photosaturation at 358 nm obtained after 82 s of UV irradiation and the longest thermal back relaxation time of 45 h recorded by UVVis. The azo derivative could be possible photolock under UV light, as observed by the improved thermal back relaxation time. The resulting photolockable chain of azobenzene might prove valuable in the development of optical device application. These azobenzene moieties also exhibit liquid crystalline behaviour with respect to the halogen substitution as an electron withdrawing group shows that strong structure property relationship exists among them

    Natural or man‐made disasters are dreadful incidents that devastate lives, disturb the socioeconomic and socio-political structure of a society and preserve or erase developments and gains based on decades, within few minutes. A catastrophe has the capacity to affect existing general population to their base, parting an occasion for self-investigation and reassessment of their framework and composition. This study signifies the Radar Digital Elevation Model centered pattern of drainage network to appraise the catastrophic landslide events due to the 2005 earthquake in Neelum-Jhelum Valley in SE-Indus Kohistan Zone north of Pakistan. This investigation highlights zones affected by the earthquake and vulnerable to landslides by utilizing Hypsometric integrals (HI values) and Hack SL-gradient techniques that are proficient in detecting erosion, land mass and tectonic movements. Dataset principally includes “Shuttle Radar Topography Mission (SRTM)” Digital Elevation Model having pixel resolution of 90 meters. Hypsometric investigation brings evidence related to the deformation periods of a geographical stage. To accomplish this objective, D8 method was used, 355 subbasins of 4th Strahler order, from 5th Strahler order 75 subbasins and from 6th Strahler order 15 subbasins were delineated. To appraise the indentations of erosional scarps, Hypsometric curves (HC) and Hypsometric integrals (HI) for all distinct subbasins were computed. Variable topographic elevations (Maximun, minimum and mean) were determined to decipher the HI values. The HCs are characterized as convex up, S shaped and concave down curves. Curvature of convex up symbolizes a lesser amount of eroded or deformed subbasins (comparatively young geography), and are located in conjunction of the North-Eastern anticline side of the Muzaffarabad that indicates the tectonic behavior of HKS, however S-shaped curvatures denote the transitional stage between the convex up and concave down deformational stage. The curvatures of concave down represents extreme deformation phase and are located west and eastern side of HKS

    We report 13 cases of Naegleria fowleri primary amebic meningoencephalitis in persons in Karachi, Pakistan, who had no history of aquatic activities. Infection likely occurred through ablution with tap water. An increase in primary amebic meningoencephalitis cases may be attributed to rising temperatures, reduced levels of chlorine in potable water, or deteriorating water distribution systems

    Upfront dexrazoxane for the reduction of anthracycline-induced cardiotoxicity in adults with preexisting cardiomyopathy and cancer: a consecutive case series

    Abstract Background Cardiotoxicity associated with anthracycline-based chemotherapies has limited their use in patients with preexisting cardiomyopathy or heart failure. Dexrazoxane protects against the cardiotoxic effects of anthracyclines, but in the USA and some European countries, its use had been restricted to adults with advanced breast cancer receiving a cumulative doxorubicin (an anthracycline) dose > 300 mg/m2. We evaluated the off-label use of dexrazoxane as a cardioprotectant in adult patients with preexisting cardiomyopathy, undergoing anthracycline chemotherapy. Methods Between July 2015 and June 2017, five consecutive patients, with preexisting, asymptomatic, systolic left ventricular (LV) dysfunction who required anthracycline-based chemotherapy, were concomitantly treated with off-label dexrazoxane, administered 30 min before each anthracycline dose, regardless of cancer type or stage. Demographic, cardiovascular, and cancer-related outcomes were compared to those of three consecutive patients with asymptomatic cardiomyopathy treated earlier at the same hospital without dexrazoxane. Results Mean age of the five dexrazoxane-treated patients and three patients treated without dexrazoxane was 70.6 and 72.6 years, respectively. All five dexrazoxane-treated patients successfully completed their planned chemotherapy (doxorubicin, 280 to 300 mg/m2). With dexrazoxane therapy, changes in LV systolic function were minimal with mean left ventricular ejection fraction (LVEF) decreasing from 39% at baseline to 34% after chemotherapy. None of the dexrazoxane-treated patients experienced symptomatic heart failure or elevated biomarkers (cardiac troponin I or brain natriuretic peptide). Of the three patients treated without dexrazoxane, two received doxorubicin (mean dose, 210 mg/m2), and one received daunorubicin (540 mg/m2). Anthracycline therapy resulted in a marked reduction in LVEF from 42.5% at baseline to 18%. All three developed symptomatic heart failure requiring hospitalization and intravenous diuretic therapy. Two of them died from cardiogenic shock and multi-organ failure. Conclusion The concomitant administration of dexrazoxane in patients with preexisting cardiomyopathy permitted successful delivery of anthracycline-based chemotherapy without cardiac decompensation.     Outcomes of High-Dose-Rate Interstitial Brachytherapy in the Treatment of Locally Advanced Cervical Cancer: Long-term Results

    PURPOSE: The purpose of this study was to determine locoregional control (LRC), disease-free survival (DFS), and toxicity of high-dose-rate interstitial brachytherapy (HDR-ISBT) in the treatment of locally advanced cervical cancer. METHODS AND MATERIALS: Between March 1996 and May 2009, 116 patients with cervical cancer were treated. Of these, 106 (91%) patients had advanced disease (International Federation of Gynecology and Obstetrics stage IIB-IVA). Ten patients had stage IB, 48 had stage II, 51 had stage III, and 7 had stage IVA disease. All patients were treated with a combination of external beam radiation therapy (EBRT) to the pelvis (5040 cGy) and 2 applications of HDR-ISBT to a dose of 3600 cGy to the implanted volume. Sixty-one percent of patients also received interstitial hyperthermia, and 94 (81%) patients received chemotherapy. RESULTS: Clinical LRC was achieved in 99 (85.3%) patients. Three-year DFS rates were 59%, 67%, 71%, and 57% for patients with stage IB, II, III, and IVA disease, respectively. The 5-year DFS and overall survival rates for the entire group were 60% and 44%, respectively. Acute and late toxicities were within acceptable limits. CONCLUSIONS: Locally advanced cervical cancer patients for whom intracavitary BT is unsuitable can achieve excellent LRC and OS with a combination of EBRT and HDR-ISBT
