    Comparative US-MRI evaluation of the Insall–Salvati index

    Purpose: To investigate whether the universally accepted range of normal patellar height ratio derived from MRI for the Insall–Salvati (IS) method could be similarly applied to ultrasound (US). Materials and methods: This study included 52 patients (age range 11–75 years) who underwent a bi-modality (US and MRI) examination, with a total of 60 knees evaluated. IS index (ratio of the patella tendon length to length of the patella) was acquired with both methods. Two operators, with different experiences of musculoskeletal imaging and blinded to the results of other investigators, separately performed the MRI and US measurements. Results: For the two operators, MRI reported a mean value of patellar height ratio of 1.10 Â± 0.16 (mean Â± standard deviation SD), while US a mean value of 1.17 Â± 0.16 (mean Â± SD). For comparable results, the small addition of 0.16 is needed for the measurements on US compared with MRI. Inter-observer agreements using intra-class correlation coefficient (ICC) was, respectively, 0.97 for MRI and 0.98 for US. The difference of mean values in patellar height ratios between MRI and US was not statistically significant (p = 0.15). The ICC between the two modalities was 0.94. Conclusion: According to our experience, IS index can be appropriately evaluated on US images, reducing the need of other imaging techniques

    Energy stores, oxidative balance and sleep in migratory Garden Warblers (Sylvia borin) and Whitethroats (Sylvia communis) at a spring stopover site

    Little is known about how songbirds modulate sleep during migratory periods. Due to the alternation of nocturnal endurance flights and diurnal refueling stopovers, sleep is likely to be a major constraint for many migratory passerine species. Sleep may help to increase the endogenous antioxidant capacity that counteracts free radicals produced during endurance flight and reduces energy expenditure. Here, we investigated the relationship between sleep behavior, food intake, and two markers of physiological condition—the amount of energy reserves and oxidative status—in two migratory songbird species, the garden warbler (Sylvia borin) and the whitethroat (Sylvia communis). In garden warblers, birds with high energy stores were more prone to sleep during the day, while this condition-dependent sleep pattern was not present in whitethroats. In both species, birds with low energy stores were more likely to sleep with their head tucked in the feathers during nocturnal sleep. Moreover, we found a positive correlation between food intake and the extent of energy reserves in garden warblers, but not in whitethroats. Finally, we did not find significant correlations between oxidative status and sleep, or oxidative status and energy stores. Despite our study was not comparative, it suggests that different species might use different strategies to manage their energy during stopover and, additionally, it raises the possibility that migrants have evolved physiological adaptations to deal with oxidative damage produced during migration

    Brief communication: Co-seismic displacement on 26 and 30 October 2016 ( M w = 5.9 and 6.5) – earthquakes in central Italy from the analysis of a local GNSS network

    Abstract. On 24 August 2016 a strong earthquake (Mw = 6.0) affected central Italy and an intense seismic sequence started. Field observations, DInSAR (Differential INterferometry Synthetic-Aperture Radar) analyses and preliminary focal mechanisms, as well as the distribution of aftershocks, suggested the reactivation of the northern sector of the Laga fault, the southern part of which was already rebooted during the 2009 L'Aquila sequence, and of the southern segment of the Mt Vettore fault system (MVFS). Based on this preliminary information and following the stress-triggering concept (Stein, 1999; Steacy et al., 2005), we tentatively identified a potential fault zone that is very vulnerable to future seismic events just north of the earlier epicentral area. Accordingly, we planned a local geodetic network consisting of five new GNSS (Global Navigation Satellite System) stations located a few kilometres away from both sides of the MVFS. This network was devoted to working out, at least partially but in some detail, the possible northward propagation of the crustal network ruptures. The building of the stations and a first set of measurements were carried out during a first campaign (30 September and 2 October 2016). On 26 October 2016, immediately north of the epicentral area of the 24 August event, another earthquake (Mw = 5.9) occurred, followed 4 days later (30 October) by the main shock (Mw = 6.5) of the whole 2016 summer–autumn seismic sequence. Our local geodetic network was fully affected by the new events and therefore we performed a second campaign soon after (11–13 November 2016). In this brief note, we provide the results of our geodetic measurements that registered the co-seismic and immediately post-seismic deformation of the two major October shocks, documenting in some detail the surface deformation close to the fault trace. We also compare our results with the available surface deformation field of the broader area, obtained on the basis of the DInSAR technique, and show an overall good fit

    "Solid state charge trapping": Examples of polymer systems showing memory effect

    The paper reports on a characteristic property of electroactive materials bearing an electron-rich and an electron-poor moiety, known as charge trapping. As examples of materials that exhibit this phenomenon, films of poly(4,4"-dipentoxy-4\u27-(2,2\u27-dicyano)ethenyl-2,2\u27:5\u27,2"-terthiophene), poly(2,3-dihexylthieno[3,4-b]pyrazine) and a blend between a fulleropyrrolidine derivative and poly(3-hexylthiophene) were investigated by cyclic voltammetry, spectroelectrochemistry and electrochemical quartz crystal microbalance. In the cyclic voltammetry, the reduction processes show the reverse oxidation potential about 1 V higher than the expected value, indicating a strong stabilization of the corresponding anion species. The mechanism leading to the stabilisation of the anions is discussed and the results indicate that the investigated materials exhibit a remarkable and quite stable memory effect

    Multiple micronutrients and docosahexaenoic acid supplementation during pregnancy : A randomized controlled study

    Maternal dietary intake during pregnancy needs to meet increased nutritional demands to maintain metabolism and to support fetal development. Docosahexaenoic acid (DHA) is essential for fetal neuro-/visual development and in immunomodulation, accumulating rapidly within the developing brain and central nervous system. Levels available to the fetus are governed by the maternal diet. In this multicenter, parallel, randomized controlled trial, we evaluated once-daily supplementation with multiple micronutrients and DHA (i.e., multiple micronutrient supplementation, MMS) on maternal biomarkers and infant anthropometric parameters during the second and third trimesters of pregnancy compared with no supplementation. Primary efficacy endpoint: change in maternal red blood cell (RBC) DHA (wt% total fatty acids) during the study. Secondary variables: other biomarkers of fatty acid and oxidative status, vitamin D, and infant anthropometric parameters at delivery. Supplementation significantly increased RBC DHA levels, the omega-3 index, and vitamin D levels. Subscapular skinfold thickness was significantly greater with MMS in infants. Safety outcomes were comparable between groups. This first randomized controlled trial of supplementation with multiple micronutrients and DHA in pregnant women indicated that MMS significantly improved maternal DHA and vitamin D status in an industrialized setting\u2014an important finding considering the essential roles of DHA and vitamin D

    The impact of previous pregnancy loss on lactating behaviors and use of herbal medicines during breastfeeding: A Post Hoc analysis of the herbal supplements in breastfeeding InvesTigation (HaBIT)

    Introduction. Complementary and alternative medicines (CAMs) are commonly used among lactating women, despite the poor knowledge of these products and of their safety. Perception of pregnancy- and breastfeeding-related difficulties and consequent use of CAMs may differ in bereaved women, by force of the distress related to previous loss, although no literature evidence is available. This Herbal supplements in Breastfeeding InvesTigation (HaBIT) post hoc analysis explored the impact of previous pregnancy loss on lactating behaviors and on use of CAMs during breastfeeding. Methods. A web-based survey was conducted among lactating women with no previous alive child, resident in Tuscany (Italy). Data on lactating behavior and on CAMs use were collected and evaluated among women with previous pregnancy loss as compared to control women. Results. Out of 476 women answering the questionnaire, 233 lactating women with one child were considered. Of them, 80 had history of pregnancy loss. Cesarean birth was significantly more frequent among women with history of pregnancy loss as compared to controls (41% versus 22%; p=0.004). Proportion, length of exclusive breastfeeding, and occurrence of breastfeeding-related complications were comparable among the two cohorts. More than half of women used CAMs during breastfeeding. Use of CAMs was more frequent among women with previous pregnancy loss (54% versus 68%; p=0.050), specifically considering herbal preparations (16% versus 30%; p=0.018). Major advisors for CAMs use were midwives. 18% and 23% of women without and with history of pregnancy loss declared no clear perception on CAMs efficacy and safety. Conclusion. Overcoming the social taboo of pregnancy loss and training healthcare professionals for an adequate management of the perinatal period are essential for an effective and safe care. Despite the common use and advice on CAMs use during breastfeeding, it is important to acknowledge that limited evidence supports their safety and efficacy during such critical period

    Fullerene-driven encapsulation of a luminescent Eu(III) complex in carbon nanotubes

    A novel CNT-based hybrid luminescent material was obtained via encapsulation of a C60-based Eu(III) complex into single-, double- and multi-walled carbon nanotubes (SWCNTs, DWCNTs and MWCNTs, respectively). Specifically, a luminescent negatively charged Eu(III) complex, electrostatically bonded to an imidazolium-functionalized fullerene cage, was transported inside CNTs by exploiting the affinity of fullerenes for the inner surface of these carbonaceous containers. The filling was performed under supercritical CO2 (scCO2) conditions to facilitate the entrapment of the ion-paired assembly. Accurate elemental, spectroscopic and morphological characterization not only demonstrated the efficiency of the filling strategy, but also the occurrence of nano-ordering of the encapsulated supramolecular luminophores when SWCNTs were employed

    Patterns and trends of utilization of incretin-based medicines between 2008 and 2014 in three Italian geographic areas

    Background: The incretin-based medicines GLP1 analogues (GLP1a) and dipeptidyl peptidase-4 inhibitors (DPP4i) are hypoglycaemic agents licensed for the treatment of type 2 diabetes mellitus (T2DM). Although these drugs possess comparable efficacy and low risk of hypoglycaemia, differences in terms of route of administration (subcutaneous versus oral), effect on body weight and gastrointestinal tolerabily can impact their actual use in clinical practice. This study aimed to describe the real-world utilization of incretin-based medicines in the Italian clinical practice. Methods: A multi-database, population-based, descriptive, cohort study was performed using administrative data collected between 2008 and 2014 from three Italian geographic areas. Subjects aged 6518 were selected. New users were defined as those with 651 dispensing of GLP1a or DPP4i during the year of interest and none in the past. Trends of cumulative annual incidence of use in the general adult population were observed. New users of GLP1a or DPP4i were respectively described in terms of demographic characteristics and use of antidiabetic drugs during 1 year before and after the first incretin dispensing. Results: The overall study population included 4,943,952 subjects. A total of 7357 new users of GLP1a and 41,907 of DPP4i were identified during the study period. Incidence of use increased between 2008 (0.2\u2030 for both GLP1a and DPP4i) and 2011 (GLP1a = 0.6\u2030; DPP4i = 2.5\u2030) and slightly decreased thereafter. In 2014, 61% of new GLP1a users received once-daily liraglutide while 52% of new DPP4i users received metformin/DPP4i in fixed-dose. The percentage of new DPP4i users older than 65 years of age increased from 30.9 to 62.6% during the study period. Around 12% of new users had not received any antidiabetic before starting an incretin. Conclusions: During the study period, DPP4i rapidly became the most prescribed incretin-based medicine, particularly among older new user. The choice of the specific incretin-based medicine at first prescription appeared to be directed towards those with higher convenience of use (e.g. oral DPP4i rather than subcutaneous GLP1a, once-daily liraglutide rather than twice-daily exenatide). The non-negligibile use of incretin-based medicines as first-line pharmacotherapy for T2DM warrants further effectiveness and safety evaluations to better define their place in therapy
