    Institutional and matrix support and its relationship with primary healthcare

    OBJECTIVE To analyze whether the level of institutional and matrix support is associated with better certification of primary healthcare teams.METHODS In this cross-sectional study, we evaluated two kinds of primary healthcare support – 14,489 teams received institutional support and 14,306 teams received matrix support. Logistic regression models were applied. In the institutional support model, the independent variable was “level of support” (as calculated by the sum of supporting activities for both modalities). In the matrix support model, in turn, the independent variables were the supporting activities. The multivariate analysis has considered variables with p RESULTS The teams had institutional and matrix supporting activities (84.0% and 85.0%), respectively, with 55.0% of them performing between six and eight activities. For the institutional support, we have observed 1.96 and 3.77 chances for teams who had medium and high levels of support to have very good or good certification, respectively. For the matrix support, the chances of their having very good or good certification were 1.79 and 3.29, respectively. Regarding to the association between institutional support activities and the certification, the very good or good certification was positively associated with self-assessment (OR = 1.95), permanent education (OR = 1.43), shared evaluation (OR = 1.40), and supervision and evaluation of indicators (OR = 1.37). In regards to the matrix support, the very good or good certification was positively associated with permanent education (OR = 1.50), interventions in the territory (OR = 1.30), and discussion in the work processes (OR = 1.23).CONCLUSIONS In Brazil, supporting activities are being incorporated in primary healthcare, and there is an association between the level of support, both matrix and institutional, and the certification result.</sec

    An update of the Worldwide Integrated Assessment (WIA) on systemic insecticides. Part 2: impacts on organisms and ecosystems

    New information on the lethal and sublethal effects of neonicotinoids and fipronil on organisms is presented in this review, complementing the previous WIA in 2015. The high toxicity of these systemic insecticides to invertebrates has been confirmed and expanded to include more species and compounds. Most of the recent research has focused on bees and the sublethal and ecological impacts these insecticides have on pollinators. Toxic effects on other invertebrate taxa also covered predatory and parasitoid natural enemies and aquatic arthropods. Little, while not much new information has been gathered on soil organisms. The impact on marine coastal ecosystems is still largely uncharted. The chronic lethality of neonicotinoids to insects and crustaceans, and the strengthened evidence that these chemicals also impair the immune system and reproduction, highlights the dangers of this particular insecticidal classneonicotinoids and fipronil. , withContinued large scale – mostly prophylactic – use of these persistent organochlorine pesticides has the potential to greatly decreasecompletely eliminate populations of arthropods in both terrestrial and aquatic environments. Sublethal effects on fish, reptiles, frogs, birds and mammals are also reported, showing a better understanding of the mechanisms of toxicity of these insecticides in vertebrates, and their deleterious impacts on growth, reproduction and neurobehaviour of most of the species tested. This review concludes with a summary of impacts on the ecosystem services and functioning, particularly on pollination, soil biota and aquatic invertebrate communities, thus reinforcing the previous WIA conclusions (van der Sluijs et al. 2015)

    Assistência em saúde mental sustentada no modelo psicossocial: narrativas de familiares e pessoas com transtorno mental

    Esta pesquisa foi realizada com o método da história oral temática e desenvolvida no ano de 2009, em Curitiba, com oito colaboradores pertencentes a três famílias que tinham um integrante com transtorno mental. O objetivo do estudo foi descrever a percepção de familiares e de portadores de transtorno mental sobre a assistência em saúde mental sustentada no modelo psicossocial. Os dados foram obtidos por meio de entrevista semiestruturada, analisados e apresentados de maneira descritiva. Os colaboradores consideraram os serviços extra-hospitalares, como o Centro de Atenção Psicossocial e os ambulatórios de saúde mental, estratégias inovadoras, e mencionaram o atendimento por equipe multiprofissional, a mediação de conflitos familiares e o princípio de territorialidade. Destacaram o acompanhamento do portador de transtorno mental pela Unidade Básica de Saúde e ressaltaram a importância das associações na rede de saúde mental. Atribuíram à inclusão da família no tratamento a melhora na relação familiar e a aceitação da doença

    Prevenção e controle de infecção em unidade de terapia intensiva neonatal

    Objetivou-se identificar o conhecimento da equipe de enfermagem de uma Unidade de Terapia Intensiva Neonatal(UTIN) sobre o controle de infecção, identificando os fatores que facilitam ou dificultam o controle e prevençãodas Infecções Relacionadas à Assistência a Saúde (IRAS). Estudo descritivo com abordagem qualitativa, realizadocom três enfermeiras e 15 técnicas de enfermagem, que atuam em uma UTIN de uma instituição filantrópica, naregião sul do Brasil. Evidenciou-se que a equipe de enfermagem possui grande conhecimento sobre os fatores quefacilitam a prevenção e controle das IRAS em UTIN, sendo o principal, a higienização das mãos. Entre os fatoresque dificultam o controle e prevenção, estão a superlotação e a excessiva carga de trabalho. A atuação eficiente equalificada da equipe de enfermagem constitui-se em estratégia de prevenção e controle das IRAS
