Re-laparotomy in OBG: a clinical study
Background: If laparotomy done within 60 days of primary surgery for the original disease it is called re-laparotomy. Aim of this study was to determine the risk factors causing re-laparotomy, the indications, management and outcomes of re-laparotomy.Methods: The study was conducted in the department of Obstetrics and Gynaecology, Vijayanagar Institute of Medical Sciences Hospital, Bellary, Karnataka. It is a 2 year prospective observational study of all the patients with re-laparotomy following operation done for obstetric or gynaecological indications.Results: Total 4105 patients underwent major surgery in two years between October 2013 to September 2015. Re-laparotomy was done in 10 cases. The incidence of re-laparotomy was 0.25% which is mainly for subacute intestinal obstruction, intraperitoneal haemorrhage, burst abdomen, PPH, rectus sheath hematoma.Conclusions: Hemorrhage, burst abdomen, infections are the main reasons for re-laparotomy after obstetric and gynaecological surgeries. Though the incidence of re-laparotomy is low and the outcome is favourable, several measures must be undertaken to prevent re-laparotomy such as careful surgical technique, meticulous hemostasis and strict asepsis should be maintained
Studies on genetic variability and relationship of bael (Aegle marmelos (L) Correa) using morphological and molecular markers
Bael (Aegle marmelos (L) Correa) is an important underutilized fruit crop of India. A total of 25 bael trees were selected from 356 bael trees of Sakharayapattana in Chikkamagalur district, Karnataka, India based on the fruit morphological traits (fruit weight, pulp weight, skull thickness, seed weight per fruit, No. of seeds per fruit, No. of locules per fruit, No. of seeds per locule, pulp wt. : seed wt.). These 25 trees were evaluated for phenotypic and genotypic variations using random amplified polymorphic DNA (RAPD) and inter-simple sequence repeats (ISSR) markers. RAPD and ISSR markers showed significant polymorphism among the trees. Jaccardâs genetic similarity value of RAPD and ISSR was found in the range of 0.00â0.95 and 0.06â0.56, respectively suggesting a moderate level of genetic diversity. The present study revealed that molecular markers can be successfully utilized for determining genetic diversity and relationship of bael trees for further varietal improvement
Induced hypotension for functional endoscopic sinus surgery: comparison between dexmedetomidine and nitro-glycerine
Background: Functional endoscopic sinus surgery (FESS) is a minimally invasive technique used to restore sinus ventilation and normal function by opening sinus air cells and ostia. FESS requires bloodless field, several techniques and drugs have been used in the past for achieving controlled hypotension such as nitroglycerine, esmolol, remifentanil, dexmedetomidine, isoflurane, propofol. Therefore, this study was conducted to compare between dexmedetomidine and nitroglycerine for controlled hypotension for FESS. Primary aim to compare quality of surgical field using average category scale and surgeon satisfaction and secondary aim to compare arterial pressure and heart rate changes between dexmedetomidine and nitroglycerine when used to induce hypotension.Methods: This study was conducted in 40 consenting adult patients posted for FESS and were randomly divided into two groups, group D received dexmedetomidine 1 mcg/kg and group N received nitroglycerine 0.5 mcg/kg/min, both infusions started 10 min after induction. Parameters such as quality of surgical field by average category scale, heart rate and mean arterial pressure (MAP) recorded every 10 minutes.Results: Dexmedetomidine and nitroglycerine both had comparable quality of surgical field. ACS grading of 1 or 2 were found among both the groups. Dexmedetomidine group had better mean arterial pressure at 10th, 20th, 30th, 40th, 50th, 60th and 70th min and heart rate at 10th, 20th, 30th, 40th, 50th, 60th and 70th min when compared to nitroglycerine group.Conclusions: Dexmedetomidine and nitroglycerine both were found to be safe to use for controlled hypotension in functional endoscopic sinus surgeries
Conservation Status of the Indian Peafowl Pavo cristatus (Aves: Galliformes: Phasianidae) in Bangalore University Campus, Bengaluru, Karnataka, India
The Indian Peafowl, Pavo cristatus was abundant in Bangalore University Campus (BUC) during 1970-80. However, the peafowl population declined alarmingly in the BUC region after 1990. The present paper deals with the abundance, sex ratio of peafowls and their conservation methods in the Biodiversity Park (Bio-Park) of BUC, which had been established in 2006. A total of 117 peafowls was recorded in the BUC region in which the females outnumbered the males. The increased abundance of the peafowl population in the campus could be mainly due to the availability of suitable roosting, foraging and nesting sites. The peafowls have used 18 species of plants and trees for perching and roosting in BUC. The peafowl population increased in the BUC region due to habitat rejuvenation and protection, availability of food sources and breeding/nesting sites. The results of the present investigation constitute the baseline data for the further understanding of the ecological community of national bird in the protected habitats as hotspots for biodiversity of urban ecosystem. Conservation of microhabitats through the proper restoration of ponds, streams and water check-dams including vegetation in the BUC is responsible for the existence of peafowl population

Identification of the Begomoviruses Squash Leaf Curl Virus and Watermelon Chlorotic Stunt Virus in Various Plant Samples in North America
Geminiviruses are a group of plant-infecting viruses with single-stranded DNA genomes. Within this family, viruses in the genus Begomovirus are known to have a worldwide distribution causing a range of severe diseases in a multitude of dicotyledonous plant species. Begomoviruses are transmitted by the whitefly Bemisia tabaci, and their ssDNA genomes can be either monopartite or bipartite. As part of a viral survey, various plants including those in the families Alliaceae, Amaranthaceae, Apiaceae, Asteraceae, Brassicaceae, Cactaceae, Cucurbitaceae, Lamiaceae, Lauraceae, Malvaceae, Oleaceae and Solanaceae were sampled and screened for begomoviruses using both a high-throughput sequencing and a begomovirus-specific primer pair approach. Based on the sequences derived using these approaches, the full-length genome of various begomoviruses were amplified from plants using abutting primers. Squash leaf curl virus (SLCV) and watermelon chlorotic stunt virus (WCSV) were identified in Cactaceae (n = 25), Solanaceae (n = 7), Cucurbitaceae (n = 2) and Lamiaceae (n = 1) samples. WCSV is an Old World bipartite begomovirus that has only recently been discovered infecting watermelons in the Americas. Our discovery of WCSV in the USA is the first indication that it has reached this country and indicates that this virus might be widespread throughout North America. Phylogenetic analysis suggests WCSV was introduced to the New World twice. The detection of begomoviruses in cactus plants suggests possible spillover events from agricultural areas into native vegetation. Since WCSV and SLCV have previously been found in mixed infections, pseudo-recombination infection experiments were conducted. We demonstrate that WCSV DNA-B is successfully trans-replicated by SLCV DNA-A despite very low degree of similarity between the replication-associated iterative sequences present in their common region, an essential feature for binding of the replication associated protein. This study highlights the importance of viral surveys for the detection of spillover events into native vegetation, but also suggests the need for more surveillance of WCSV in the USA, as this virus is a serious threat to watermelon cultivation in the Middle East
The impact of ADHD on the health and well-being of ADHD children and their siblings
Childhood attention-deficit/hyperactivity disorder (ADHD) has been associated with reduced health and well-being of patients and their families. The authors undertook a large UK survey-based observational study of the burden associated with childhood ADHD. The impact of ADHD on both the patient (NÂ =Â 476) and their siblings (NÂ =Â 337) on health-related quality of life (HRQoL) and happiness was quantified using multiple standard measures [e.g. child health utility-9D (CHU-9D), EuroQol-5D-Youth]. In the analysis, careful statistical adjustments were made to ensure a like-for-like comparison of ADHD families with two different control groups. We controlled for carers' ADHD symptoms, their employment and relationship status and siblings' ADHD symptoms. ADHD was associated with a significant deficit in the patient's HRQoL (with a CHU-9D score of around 6Â % lower). Children with ADHD also have less sleep and were less happy with their family and their lives overall. No consistent decrement to the HRQoL of the siblings was identified across the models, except that related to their own conduct problems. The siblings do, however, report lower happiness with life overall and with their family, even when controlling for the siblings own ADHD symptoms. We also find evidence of elevated bullying between siblings in families with a child with ADHD. Overall, the current results suggest that the reduction in quality of life caused by ADHD is experienced both by the child with ADHD and their siblings

Indian summer monsoon onset forecast skill in the UK Met Office initialized coupled seasonal forecasting system (GloSea5-GC2)
Accurate and precise forecasting of the Indian monsoon is important for the socio-economic security of India, with improvements in agriculture and associated sectors from prediction of the monsoon onset. In this study we establish the skill of the UK Met Office coupled initialized global seasonal forecasting system, GloSea5-GC2, in forecasting Indian monsoon onset. We build on previous work that has demonstrated the good skill of GloSea5 at forecasting interannual variations of the seasonal mean Indian monsoon using measures of large-scale circulation and local precipitation. We analyze the summer hindcasts from a set of three springtime start-dates in late April/early May for the 20-year hindcast period (1992-2011). The hindcast set features at least fifteen ensemble members for each year and is analyzed using five different objective monsoon indices. These indices are designed to examine large and local-scale measures of the monsoon circulation, hydrological changes, tropospheric temperature gradient, or rainfall for single value (area-averaged) or grid-point measures of the Indian monsoon onset. There is significant correlation between onset dates in the model and those found in reanalysis. Indices based on large-scale dynamic and thermodynamic indices are better at estimating monsoon onset in the model rather than local-scale dynamical and hydrological indices. This can be attributed to the model's better representation of large-scale dynamics compared to local-scale features. GloSea5 may not be able to predict the exact date of monsoon onset over India, but this study shows that the model has a good ability at predicting category-wise monsoon onset, using early, normal or late tercile categories. Using a grid-point local rainfall onset index, we note that the forecast skill is highest over parts of central India, the Gangetic plains, and parts of coastal India - all zones of extensive agriculture in India. El Niño Southern Oscillation (ENSO) forcing in the model improves the forecast skill of monsoon onset when using a large-scale circulation index, with late monsoon onset coinciding with El Niño conditions and early monsoon onset more common in La Niña years. The results of this study suggest that GloSea5's ensemble-mean forecast may be used for reliable Indian monsoon onset prediction a month in advance despite systematic model errors
Therapeutic impact of cytoreductive surgery and irradiation of posterior fossa ependymoma in the molecular era: a retrospective multicohort analysis
PURPOSE: Posterior fossa ependymoma comprises two distinct molecular variants termed EPN_PFA and EPN_PFB that have a distinct biology and natural history. The therapeutic value of cytoreductive surgery and radiation therapy for posterior fossa ependymoma after accounting for molecular subgroup is not known. METHODS: Four independent nonoverlapping retrospective cohorts of posterior fossa ependymomas (n = 820) were profiled using genome-wide methylation arrays. Risk stratification models were designed based on known clinical and newly described molecular biomarkers identified by multivariable Cox proportional hazards analyses. RESULTS: Molecular subgroup is a powerful independent predictor of outcome even when accounting for age or treatment regimen. Incompletely resected EPN_PFA ependymomas have a dismal prognosis, with a 5-year progression-free survival ranging from 26.1% to 56.8% across all four cohorts. Although first-line (adjuvant) radiation is clearly beneficial for completely resected EPN_PFA, a substantial proportion of patients with EPN_PFB can be cured with surgery alone, and patients with relapsed EPN_PFB can often be treated successfully with delayed external-beam irradiation. CONCLUSION: The most impactful biomarker for posterior fossa ependymoma is molecular subgroup affiliation, independent of other demographic or treatment variables. However, both EPN_PFA and EPN_PFB still benefit from increased extent of resection, with the survival rates being particularly poor for subtotally resected EPN_PFA, even with adjuvant radiation therapy. Patients with EPN_PFB who undergo gross total resection are at lower risk for relapse and should be considered for inclusion in a randomized clinical trial of observation alone with radiation reserved for those who experience recurrence
New World Cactaceae Plants Harbor Diverse Geminiviruses
The family Cactaceae comprises a diverse group of typically succulent plants that are native to the American continent but have been introduced to nearly all other continents, predominantly for ornamental purposes. Despite their economic, cultural, and ecological importance, very little research has been conducted on the viral community that infects them. We previously identified a highly divergent geminivirus that is the first known to infect cacti. Recent research efforts in non-cultivated and asymptomatic plants have shown that the diversity of this viral family has been under-sampled. As a consequence, little is known about the effects and interactions of geminiviruses in many plants, such as cacti. With the objective to expand knowledge on the diversity of geminiviruses infecting cacti, we used previously acquired high-throughput sequencing results to search for viral sequences using BLASTx against a viral RefSeq protein database. We identified two additional sequences with similarity to geminiviruses, for which we designed abutting primers and recovered full-length genomes. From 42 cacti and five scale insects, we derived 42 complete genome sequences of a novel geminivirus species that we have tentatively named Opuntia virus 2 (OpV2) and 32 genomes of an Opuntia-infecting becurtovirus (which is a new strain of the spinach curly top Arizona virus species). Interspecies recombination analysis of the OpV2 group revealed several recombinant regions, in some cases spanning half of the genome. Phylogenetic analysis demonstrated that OpV2 is a novel geminivirus more closely related to viruses of the genus Curtovirus, which was further supported by the detection of three recombination events between curtoviruses and OpV2. Both OpV2 and Opuntia becurtoviruses were identified in mixed infections, which also included the previously characterized Opuntia virus 1. Viral quantification of the co-infected cactus plants compared with single infections did not show any clear trend in viral dynamics that might be associated with the mixed infections. Using experimental Rhizobium-mediated inoculations, we found that the initial accumulation of OpV2 is facilitated by co-infection with OpV1. This study shows that the diversity of geminiviruses that infect cacti is under-sampled and that cacti harbor diverse geminiviruses. The detection of the Opuntia becurtoviruses suggests spill-over events between viruses of cultivated species and native vegetation. The threat this poses to cacti needs to be further investigated
