    Incidental finding of lymphoma after septoplasty.

    IntroductionSeptoplasty, or surgical correction of the deviated septum, is an elective, routinely performed rhinologic procedure to address nasal airway obstruction. In many cases, resected septal cartilage and bone fragments are sent for pathologic review, although there is no consensus on this practice. We reported two cases of incidentally diagnosed lymphoma after elective septoplasty and discussed clinical presentation, diagnosis, and management.MethodsRetrospective chart review of two patients who underwent septoplasty at a tertiary academic medical center and found to have incidental lymphoma based on histopathology.ResultsTwo patients who underwent septoplasty had an incidental diagnosis of lymphoma on pathologic analysis. One patient was noted to have an S-shaped septal deviation that produced bilateral nasal obstruction. She underwent a difficult septoplasty, in which the mucoperichondrial flap was firmly adherent to the underlying septum and bone. Final pathology demonstrated diffuse large B-cell lymphoma. She was treated with chemoradiation and remained free of disease at 59 months. The other patient had a history of nasal trauma, which produced left septal deviation. He underwent an uncomplicated septoplasty, with pathology that demonstrated low-grade B-cell lymphoma. Because there was no evidence of active disease, the decision was made to not treat and to observe the patient clinically.ConclusionsThis is the first reported series of septal lymphoma incidentally diagnosed on routine septoplasty. Although histopathologic review of specimens from routine nasal and sinus surgery is not routinely performed, this report highlighted the importance of this process, on a case-by-case basis, in detecting unexpected malignancies that otherwise were clinically silent

    Alimentos ricos en selenio potencialmente útiles para controlar los niveles de mercurio entre afrocolombianos: Hacia una intervención intercultural

    Introduction: Diet-based interventions may be a culturally acceptable option to decrease mercury exposure and thus prevent the effects that this metal can have on the health of the population. Selenium is an element that can act as a chelator, decreasing mercury levels in the body. Objective: explore the relationship between the consumption of selenium-rich foods and mercury exposure in the Afro-Colombian population. Materials and methods: A cross-sectional study was carried out with 320 individuals from five municipalities of Chocó, Colombia. They were asked about the frequency of consumption of selenium-rich foods, and their association with mercury concentrations in hair was estimated. Results: Walnuts, whole wheat flour, and yeast extract were identified as foods to consider in future interventions. The median mercury in hair was 0.407 ug/g (Minimum: 0.006 - Maximum: 40.028). Conclusion: It is proposed that the banana juice, the pineapple colada, the borojó (Borojoa patinoi) sorbet, the cucas, the enyucado, and the sábida of ripe primitive banana are basic elements for a culturally acceptable intervention.Introducción: Las intervenciones basadas en la dieta pueden ser una opción culturalmente aceptable para disminuir la exposición al mercurio y prevenir así los efectos que este metal puede tener sobre la salud de la población. El selenio es un elemento que puede actuar como quelante, disminuyendo los niveles de mercurio en el organismo. Objetivo: explorar la relación entre el consumo de alimentos ricos en selenio y la exposición a mercurio en población afrocolombiana. Materiales y métodos: Se realizó un estudio transversal con 320 individuos de cinco municipios del Chocó, Colombia. Se les preguntó sobre la frecuencia de consumo de alimentos ricos en selenio y se estimó su asociación con las concentraciones de mercurio en cabello. Resultados: Las nueces, la harina de trigo integral y el extracto de levadura fueron identificados como alimentos a considerar en futuras intervenciones. La mediana de mercurio en el pelo fue de 0,407 ug/g (Mínimo: 0,006 - Máximo: 40,028). Conclusiones: Se propone que el jugo de plátano, la piña colada, el sorbete de borojó (Borojoa patinoi), las cucas, el enyucado y la sábida de plátano primitivo maduro son elementos básicos para una intervención culturalmente aceptable

    Synthesis, and spectroscopic and structural characterization of three new styrylquinoline–benzimidazole hybrids

The formyl inter­mediates and the hybrid products have all been fully characterized using a combination of IR, 1H and 13C NMR spectroscopy, and high-resolution mass spectrometry, and the structures of the three hybrid products have been determined using single-crystal X-ray diffraction. Ethyl (E)-2-(1H-benzo[d]imidazol-2-yl)-4-(4-chloro­styr­yl)quinoline-3-carboxyl­ate, C27H20ClN3O2, (IIIa), and ethyl (E)-2-(1H-ben­zo[d]imidazol-2-yl)-4-(2-meth­oxy­styr­yl)quinoline-3-carboxyl­ate, C28H23N3O3, (IIIb), both crystallize in the solvent-free form with Z′ = 1, but ethyl (E)-2-(1H-benzo[d]imidazol-2-yl)-4-(4-methyl­styr­yl)quinoline-3-carboxyl­ate, C28H23N3O2, (IIIc), crystallizes as a partial hexane solvate with Z′ = 3, and the ester group in one of the independent mol­ecules is disordered over two sets of atomic sites having occupancies of 0.765 (7) and 0.235 (7). The mol­ecules of (IIIc) enclose continuous channels which are occupied by disordered solvent mol­ecules having partial occupancy. In all of the mol­ecules of (IIIa)–(IIIc), the styryl­quinoline fragment is markedly nonplanar. Different combinations of N—H⋯O and C—H⋯π hydrogen bonds generate supra­molecular assemblies which are two-dimensional in (IIIb) and (IIIc), but three-dimensional in (IIIa).     Sorption direct air capture with CO2 utilization

    Direct air capture (DAC) is gathering momentum since it has vast potential and high flexibility to collect CO2 from discrete sources as “synthetic tree” when compared with current CO2 capture technologies, e.g., amine based post-combustion capture. It is considered as one of the emerging carbon capture technologies in recent decades and remains in a prototype investigation stage with many technical challenges to be overcome. The objective of this paper is to comprehensively discuss the state-of-the-art of DAC and CO2 utilization, note unresolved technology bottlenecks, and give investigation perspectives for commercial large-scale applications. Firstly, characteristics of physical and chemical sorbents are evaluated. Then, the representative capture processes, e.g., pressure swing adsorption, temperature swing adsorption and other ongoing absorption chemical loops, are described and compared. Methods of CO2 conversion including synthesis of fuels and chemicals as well as biological utilization are reviewed. Finally, techno-economic analysis and life cycle assessment for DAC application are summarized. Based on research achievements, future challenges of DAC and CO2 conversion are presented, which include providing synthesis guidelines for obtaining sorbents with the desired characteristics, uncovering the mechanisms for different working processes and establishing evaluation criteria in terms of technical and economic aspects

    Fishing, pollution, climate change, and the long-term decline of coral reefs off Havana, Cuba

    Understanding temporal and spatial variation of coral reef communities allows us to analyze the relative effects of local stressors, such as fishing and eutrophication, and global stressors, such as ocean warming. To test for spatial and temporal changes in coral reef communities, we combined recent benthic and fish surveys from 2016 with long-term data, dating back to the late 1990s, from four zones located at different distances from Central Havana, Cuba’s largest population center. These changes may indicate the shifting importance of local vs global stressors affecting reef communities. Regardless of the distance from Havana, we found that coral cover was uniformly low (approximately 10%), whereas macroalgal abundance was often high (approximately 65%). Similarly, fish biomass was low across zones, particularly for herbivorous fishes (approximately 12 g m−2) that are critical ecological drivers of reef structure and coral resilience. Analyses of longer-term trends revealed that coral cover near Havana has been below about 10% since at least 1995, potentially because of local stressors. In contrast, reefs farther from Havana maintained relatively high coral cover (approximately 30%) until the early 2000s, but declined more recently to approximately 15%, putting them near the Caribbean-wide average. These distinct spatial and temporal trajectories of reef communities may be the result of the expansion of local stressors away from Havana as the human population increased, or as fishers ventured farther away to exploit new resources. Alternatively, the more recent decline of reefs farther from population centers may have resulted from increasingly frequent global stressors, such as bleaching events and hurricanes

    Pheochromocytomatosis associated with a novel TMEM127 mutation

    Pheochromocytomatosis, a very rare form of pheochromocytoma recurrence, refers to new, multiple, and often small pheochromocytomas growing in and around the surgical resection bed of a previous adrenalectomy for a solitary pheochromocytoma. We here report a case of pheochromocytomatosis in a 70-year-old female. At age 64 years, she was diagnosed with a 6-cm right pheochromocytoma. She underwent laparoscopic right adrenalectomy, during which the tumor capsule was ruptured. At age 67 years, CT of abdomen did not detect recurrence. At age 69 years, she began experiencing episodes of headache and diaphoresis. At age 70 years, biochemical markers of pheochromocytoma became elevated with normal calcitonin level. CT revealed multiple nodules of various sizes in the right adrenal fossa, some of which were positive on metaiodobenzylguanidine (MIBG) scan. She underwent open resection of pheochromocytomatosis. Histological examination confirmed numerous pheochromocytomas ranging 0.1–1.2 cm in size. Next-generation sequencing of a panel of genes found a novel heterozygous germline c.570delC mutation in TMEM127, one of the genes that, if mutated, confers susceptibility to syndromic pheochromocytoma. Molecular analysis showed that the c.570delC mutation is likely pathogenic. Our case highlights the typical presentation of pheochromocytomatosis, a rare complication of adrenalectomy for pheochromocytoma. Previous cases and ours collectively demonstrate that tumor capsule rupture during adrenalectomy is a risk factor for pheochromocytomatosis. We also report a novel TMEM127 mutation in this case

    Determining social factors related to pesticide poisoning among rice farmers in Colombia

    Objetivo Caracterizar la forma en que los agricultores se exponen a los plaguicidas y se presenta la intoxicación. Materiales y Métodos Estudio multi-métodos y multinivel (individuo y comunidad), que incluyó métodos etnográficos, encuesta y medición de plaguicidas en agua y muestras humanas. Resultados Se describió el proceso productivo y se conocieron los principales factores de riesgo. Los plaguicidas son considerados el mayor peligro en el trabajo y en sus viviendas. Los trabajadores tienen precarias condiciones laborales y no están protegidos por el sistema de riesgos laborales. En las muestras de agua se encontraron azinfos-metil, endosulfán, b-BHC, bromofos-metil, bromofos-etil y 2,4-DDT. En la encuesta participaron 381 trabajadores de los cuales 12,9, 68 y 5,5 % tuvieron intoxicaciones leves, moderadas y severas, respectivamente. Los casos severos tenían menores niveles de escolaridad, menor nivel de afiliación al régimen contributivo del sistema de seguridad social y un mayor número tenía enfermedad cardiovascular, diabetes, herpes u otras infecciones virales. Conclusión Existen precarias condiciones laborales que favorecen la exposición a plaguicidas caracterizadas por la exclusión del sistema de riesgos laborales, pobreza y baja educación. Es urgente la inclusión de estos trabajadores al sistema de riesgos laborales y la mejora de las condiciones de vida. De esa manera se podrán disminuir las prácticas inseguras en el manejo de plaguicidas.Objective Large quantities of pesticides are used in rice crops. The aim of this study is to characterize how farmers are exposed to pesticides and subsequent poisoning. Materials and Methods A multilevel (individual and community) multi-method study, which included ethnographic and survey methods, as well as measurement of pesticides in water and human samples, was performed. Results The production process is described and the main risk factors are presented. Pesticides are considered the greatest danger at work and at their homes. Workers have poor working conditions and are not protected by the system of occupational risks. Azinphos-methyl, endosulfan, beta-BHC, bromophos-methyl, bromophos-ethyl and 2,4- DDT were found in water samples. The survey included 381 workers with mild (12.86 %), moderate (67.98 %) and severe (5.51 %) poisonings respectively. Severe cases presented lower levels of education, lower levels of health care access to the contributory regimen of the Colombian social security system and higher incidence of cardiovascular disease, diabetes, herpes or other viral infections. Conclusion There are precarious working conditions that favor exposure to pesticides correlated to the exclusion of farmers from the occupational risk system, to poverty and to poor education. It is urgent to include these workers to the system of occupational risk system and to improve their living conditions, thus reducing unsafe practices when handling pesticides

    Formyl Peptide Receptor as a Novel Therapeutic Target for Anxiety-Related Disorders

    Formyl peptide receptors (FPR) belong to a family of sensors of the immune system that detect microbe-associated molecules and inform various cellular and sensorial mechanisms to the presence of pathogens in the host. Here we demonstrate that Fpr2/3-deficient mice show a distinct profile of behaviour characterised by reduced anxiety in the marble burying and light-dark box paradigms, increased exploratory behaviour in an open-field, together with superior performance on a novel object recognition test. Pharmacological blockade with a formyl peptide receptor antagonist, Boc2, in wild type mice reproduced most of the behavioural changes observed in the Fpr2/3(-/-) mice, including a significant improvement in novel object discrimination and reduced anxiety in a light/dark shuttle test. These effects were associated with reduced FPR signalling in the gut as shown by the significant reduction in the levels of p-p38. Collectively, these findings suggest that homeostatic FPR signalling exerts a modulatory effect on anxiety-like behaviours. These findings thus suggest that therapies targeting FPRs may be a novel approach to ameliorate behavioural abnormalities present in neuropsychiatric disorders at the cognitive-emotional interface

    The VPOS: a vast polar structure of satellite galaxies, globular clusters and streams around the Milky Way

    It has been known for a long time that the satellite galaxies of the Milky Way (MW) show a significant amount of phase-space correlation, they are distributed in a highly inclined Disc of Satellites (DoS). We have extended the previous studies on the DoS by analysing for the first time the orientations of streams of stars and gas, and the distributions of globular clusters within the halo of the MW. It is shown that the spatial distribution of MW globular clusters classified as young halo clusters (YH GC) is very similar to the DoS, while 7 of the 14 analysed streams align with the DoS. The probability to find the observed clustering of streams is only 0.3 per cent when assuming isotropy. The MW thus is surrounded by a vast polar structure (VPOS) of subsystems (satellite galaxies, globular clusters and streams), spreading from Galactocentric distances as small as 10 kpc out to 250 kpc. These findings demonstrate that a near-isotropic infall of cosmological sub-structure components onto the MW is essentially ruled out because a large number of infalling objects would have had to be highly correlated, to a degree not natural for dark matter sub-structures. The majority of satellites, streams and YH GCs had to be formed as a correlated population. This is possible in tidal tails consisting of material expelled from interacting galaxies. We discuss the tidal scenario for the formation of the VPOS, including successes and possible challenges. The potential consequences of the MW satellites being tidal dwarf galaxies are severe. If all the satellite galaxies and YH GCs have been formed in an encounter between the young MW and another gas-rich galaxy about 10-11 Gyr ago, then the MW does not have any luminous dark-matter substructures and the missing satellites problem becomes a catastrophic failure of the standard cosmological model.Comment: 21 pages, 8 figures, 2 tables.
