    ¿Son los debates electorales la nueva Eurovisión? Análisis del seguimiento de dos eventos masivos a través de Twitter

    Las redes sociales han transformado el modo en que seguimos los contenidos televisivos. De esta manera, asistimos a la expansión de la televisión social y al aumento del consumo de doble pantalla. También presenciamos un proceso de mediatización de la política, que cada vez comparte más códigos con el lenguaje televisivo de masas. El objetivo de este trabajo es estudiar los paralelismos en el consumo a través de Twitter de dos de los eventos con mayor audiencia social de 2016: el debate electoral a cuatro del 13 de junio y la nal de Eurovisión. El estudio parte de la premisa de que, a pesar de tratarse de dos formatos de naturaleza distinta, el seguimiento de ambos a través de las redes sociales presenta dinámicas semejantes y re eja que los espectadores buscan en ellos un espacio de ocio. Así, seleccionamos a un conjunto de ?tuiteros estrella? y estudiamos los mensajes que publicaron en ambas citas. Para ello, combinamos un análisis del discurso de corte cualitativo con un análisis de contenido de carácter cuantitativo. Entre los elementos estudiados destacan el contenido textual generado, el tono de los mensajes, el uso de recursos retóricos como la ironía y el material grá co. Nuestro análisis permite con rmar tendencias como la interrelación entre los formatos políticos y los de entretenimiento, de forma que durante la emisión de Eurovisión son numerosas las referencias de los tuiteros a la actualidad política y en el debate electoral proliferan los guiños a los productos de la cultura popular contemporánea

    La siguiente investigación se titula: “Funcionamiento familiar y consumo de sustancias psicoactivas en jóvenes estudiantes del centro de desarrollo y formación “Juventud”, ubicado en las Jaguitas, Managua, en el II semestre del año 2014”. Para la realización de este estudio, se trabajó en el barrio Grenada y Pantasma ubicados en el distrito V de la ciudad de Managua, con una muestra poblacional de veinte personas conformada por adolescentes y jóvenes, en un rango promedio de edad de 16 a 18 años los adolescentes y de 19 a 24 años los jóvenes, siendo diez personas de sexo femenino y diez personas de sexo masculino. Esta investigación es de tipo cuantitativo con enfoque descriptivo de corte transversal, debido a que valoraron los factores que están influyendo en un fenómeno específico y se midieron las variables a través de un análisis estadístico tomando en cuenta las siguientes variables: desintegración familiar, consumo de sustancias, factores socioeconómicos, factores sociodemográficos, comunicación, adaptación y cohesión familiar. Se obtuvo como resultado que los factores más significativos que inducen a los adolescentes y jóvenes al consumo están ligados al sexo, donde tiene mayor predominio el sexo masculino, por su parte refiriéndose a desintegración familiar los encuestados afirman que las familias desintegradas no es una causal que oriente a los estudiantes al consumo , sin embargo , por medio de la observación, se encontró que las familias monoparentales y extensas son las que tienen un porcentaje mayor de adolescentes y jóvenes en cuanto al consumo de sustancias psicoactivas

    Primary melanoma of the nasal mucosa: case presentation and systematic review of the literature

    Introducción: Los melanomas de mucosa nasosinusales, representa una de las patologías de menor proporción en las tumoraciones de nariz y senos paranasales y como consecuencia diagnósticos tardíos. Reporte de caso: paciente masculino de la sexta década de la vida, que inicio con cuadro de obstrucción nasal y epistaxis leve, fue enviado para su valoración nuestro hospital de tercer nivel, donde se sometió a biopsia, presentando el resultado histopatológico de melanoma de mucosa nasal. Conclusión: las lesiones se presentaron principalmente como epistaxis y obstrucción nasal, involucraron a la cavidad nasal y senos paranasales y el lado izquierdo fue el más común involucrado. Los estadios de la enfermedad avanzada y metástasis a distancia se presentaron con gran frecuencia y en general con una pobre supervivencia a 5 años.Background: Mucosal melanoma represents one of the pathologies with a lower proportion among tumors of the nose and paranasal sinuses and as a consequence late diagnoses. Objective: To present the case of a male patient in the sixth decade of life with nasal mucosal melanoma and a systematic review of studies reported from 2016 to 2021. Results: The systematic search was carried out from January 2016 to December 2021 in PubMed, Google Scholar and Elsevier; 147 articles that meet the 19 criteria were located. Conclusion: The injury was prevalent in women, there was a higher prevalence in the seventh and eighth decades. The main lesions were epistaxis and nasal obstruction, involved the nasal cavity and paranasal sinuses and the left side was the most common involved. There was high frequency of advanced disease stages and distant metastases and generally with poor 5-year survival

    The combination of sirolimus plus tacrolimus improves outcome after reduced-intensity conditioning, unrelated donor hematopoietic stem cell transplantation compared with cyclosporine plus mycofenolate

    Different types of graft-versus-host disease prophylaxis have been proposed in the setting of reduced intensity and non-myeloablative allogeneic stem cell transplantation. An alternative combination with sirolimus and tacrolimus has recently been tested although comparative studies against the classical combination of a calcineurin inhibitor and mycophenolate mofetil or methotrexate are lacking. We describe the results of a prospective, multicenter trial using sirolimus + tacrolimus as immunoprophylaxis, and compare this approach with our previous experience using cyclosporine + mycophenolate in the setting of unrelated donor transplantation setting after reduced-intensity conditioning. Forty-five patients received cyclosporine + mycophenolate between 2002 and mid-2007, while the subsequent 50 patients, who were transplanted from late 2007, were given sirolimus + tacrolimus. No significant differences were observed in terms of hematopoietic recovery or acute graft-versus-host disease overall, although gastrointestinal acute graft-versus-host disease grade ≥2 was more common in the cyclosporine + mycophenolate group (55% versus 21%, respectively, P=0.003). The 1-year cumulative incidence of chronic graftversus-host disease was 50% versus 90% for the patients treated with the sirolimus- versus cyclosporine-based regimen, respectively (P<0.001), while the incidence of extensive chronic disease was 27% versus 49%, respectively (P=0.043). The 2-year non-relapse mortality rate was 18% versus 38% for patients receiving the sirolimus- versus the cyclosporine-based regimen, respectively (P=0.02). The event-free survival and overall survival at 2 years were 53% versus 29% (P=0.028) and 70% versus 45% (P=0.018) among patients receiving the sirolimus- versus the cyclosporine-based regimen, respectively. In conclusion, in the setting of reduced intensity transplantation from an unrelated donor, promising results can be achieved with the combination of sirolimus + tacrolimus, due to a lower risk of chronic graft-versus-host disease and non-relapse mortality, which translates into better event-free and over all survival rates, in comparison with those achieved with cyclosporine + mycophenolate

    Experimental and genetic evidence for the impact of CD5 and CD6 expression and variation in inflammatory bowel disease

    Crohn's disease (CD) and ulcerative colitis (UC) are inflammatory bowel diseases (IBD) resulting from the interaction of multiple environmental, genetic and immunological factors. CD5 and CD6 are paralogs encoding lymphocyte co-receptors involved in fine-tuning intracellular signals delivered upon antigen-specific recognition, microbial pattern recognition and cell adhesion. While CD5 and CD6 expression and variation is known to influence some immune-mediated inflammatory disorders, their role in IBD remains unclear. To this end, Cd5- and Cd6-deficient mice were subjected to dextran sulfate sodium (DSS)-induced colitis, the most widely used experimental animal model of IBD. The two mouse lines showed opposite results regarding body weight loss and disease activity index (DAI) changes following DSS-induced colitis, thus supporting Cd5 and Cd6 expression involvement in the pathophysiology of this experimental IBD model. Furthermore, DNA samples from IBD patients of the ENEIDA registry were used to test association of CD5 (rs2241002 and rs2229177) and CD6 (rs17824933, rs11230563, and rs12360861) single nucleotide polymorphisms with susceptibility and clinical parameters of CD (n=1352) and UC (n=1013). Generalized linear regression analyses showed association of CD5 variation with CD ileal location (rs2241002CC) and requirement of biological therapies (rs2241002C-rs2229177T haplotype), and with poor UC prognosis (rs2241002T-rs2229177T haplotype). Regarding CD6, association was observed with CD ileal location (rs17824933G) and poor prognosis (rs12360861G), and with left-sided or extensive UC, and absence of ankylosing spondylitis in IBD (rs17824933G). The present experimental and genetic evidence support a role for CD5 and CD6 expression and variation in IBD's clinical manifestations and therapeutic requirements, providing insight into its pathophysiology and broadening the relevance of both immunomodulatory receptors in immune-mediated disorders

    Performance of Screening Strategies for Latent Tuberculosis Infection in Patients with Inflammatory Bowel Disease: Results from the ENEIDA Registry of GETECCU

    (1) Aims: Patients receiving antitumor necrosis factor (anti-TNF) therapy are at risk of developing tuberculosis (TB), usually due to the reactivation of a latent TB infection (LTBI). LTBI screening and treatment decreases the risk of TB. This study evaluated the diagnostic performance of different LTBI screening strategies in patients with inflammatory bowel disease (IBD). (2) Methods: Patients in the Spanish ENEIDA registry with IBD screened for LTBI between January 2003 and January 2018 were included. The diagnostic yield of different strategies (dual screening with tuberculin skin test [TST] and interferon-gamma-release assay [IGRA], two-step TST, and early screening performed at least 12 months before starting biological treatment) was analyzed. (3) Results: Out of 7594 screened patients, 1445 (19%; 95% CI 18-20%) had LTBI. Immunomodulator (IMM) treatment at screening decreased the probability of detecting LTBI (20% vs. 17%, p = 0.001). Regarding screening strategies, LTBI was more frequently diagnosed by dual screening than by a single screening strategy (IGRA, OR 0.60; 95% CI 0.50-0.73, p < 0.001; TST, OR 0.76; 95% CI 0.66-0.88, p < 0.001). Two-step TST increased the diagnostic yield of a single TST by 24%. More cases of LTBI were diagnosed by early screening than by routine screening before starting anti-TNF agents (21% [95% CI 20-22%] vs. 14% [95% CI 13-16%], p < 0.001). The highest diagnostic performance for LTBI (29%) was obtained by combining early and TST/IGRA dual screening strategies in patients without IMM. (4): Conclusions: Both early screening and TST/IGRA dual screening strategies significantly increased diagnostic performance for LTBI in patients with IBD, with optimal performance achieved when they are used together in the absence of IMM

    [EN] Innocampus Explora aims to show the students of the Burjassot-Paterna campus of the Universitat de València how the different scientific degrees are interrelated. To do this we propose activities in which students and teachers work together to cover the interdisciplinary nature of science, both in everyday and professional issues. Throughout this course the activities developed relate to new ways to communicate science. With the development of this project we contribute to a transversal quality education for all the participating students.[ES] Innocampus Explora tiene por objetivo mostrar a los estudiantes del campus de Burjassot-Paterna de la Universitat de València cómo los diferentes grados científicos están interrelacionados. Para ello proponemos actividades en las que estudiantes y profesores trabajen conjuntamente para abarcar la interdisciplinariedad de la ciencia, tanto en temas cotidianos como profesionales. A lo largo de este curso las actividades desarrolladas se relacionan con las nuevas formas de comunicar ciencia.     Effectiveness and Safety of the Sequential Use of a Second and Third Anti-TNF Agent in Patients With Inflammatory Bowel Disease: Results From the Eneida Registry

    Background: The effectiveness of the switch to another anti-tumor necrosis factor (anti-TNF) agent is not known. The aim of this study was to analyze the effectiveness and safety of treatment with a second and third anti-TNF drug after intolerance to or failure of a previous anti-TNF agent in inflammatory bowel disease (IBD) patients. Methods: We included patients diagnosed with IBD from the ENEIDA registry who received another anti-TNF after intolerance to or failure of a prior anti-TNF agent. Results: A total of 1122 patients were included. In the short term, remission was achieved in 55% of the patients with the second anti-TNF. The incidence of loss of response was 19% per patient-year with the second anti-TNF. Combination therapy (hazard ratio [HR], 2.4; 95% confidence interval [CI], 1.8-3; P < 0.0001) and ulcerative colitis vs Crohn's disease (HR, 1.6; 95% CI, 1.1-2.1; P = 0.005) were associated with a higher probability of loss of response. Fifteen percent of the patients had adverse events, and 10% had to discontinue the second anti-TNF. Of the 71 patients who received a third anti-TNF, 55% achieved remission. The incidence of loss of response was 22% per patient-year with a third anti-TNF. Adverse events occurred in 7 patients (11%), but only 1 stopped the drug. Conclusions: Approximately half of the patients who received a second anti-TNF achieved remission; nevertheless, a significant proportion of them subsequently lost response. Combination therapy and type of IBD were associated with loss of response. Remission was achieved in almost 50% of patients who received a third anti-TNF; nevertheless, a significant proportion of them subsequently lost response
