    Anxiety and depression in keratotic oral lichen planus: a multicentric study from the SIPMO

    Objectives: Oral lichen planus with exclusive keratotic reticular, papular, and/or plaque-like lesions (K-OLP) is a clinical pattern of OLP that may be associated with a complex symptomatology and psychological alteration. The aim of the study was to evaluate the prevalence of anxiety (A) and depression (D) in patients with K-OLP, analyzing the potential predictors which can affect mental health status. Methods: Three hundred K-OLP patients versus 300 healthy controls (HC) were recruited in 15 Italian universities. The Numeric Rating Scale (NRS), Total Pain Rating Index (T-PRI), and Hamilton Rating Scales for Depression and for Anxiety (HAM-D and HAM-A) were administered. Results: The K-OLP patients showed statistically higher scores in the NRS, T-PRI, HAM-D, and HAM-A compared with the HC (p-value < 0.001**). A and D were found in 158 (52.7%) and 148 (49.3%) K-OLP patients. Strong linear correlations were identified between HAM-A, HAM-D, NRS, T-PRI, and employment status and between HAM-D, HAM-A, NRS, T-PRI, employment status, and female gender. Multivariate logistic regression revealed that HAM-D and HAM-A showed the greatest increase in the R2 value for A and D in the K-OLP patients, respectively (DR2 = 55.5% p-value < 0.001**; DR2 = 56.5% p-value < 0.001**). Conclusions: The prevalence of A and D is higher in the K-OLP patients compared with the HC, also found in K-OLP subjects without pain, suggesting that the processing of pain may be in a certain way independent of the processing of mood. Clinical relevance: Mood disorders and pain assessment should be carefully performed in relation to K-OLP to obtain a complete analysis of the patients

    The wide world of technological telerehabilitation for pediatric neurologic and neurodevelopmental disorders – a systematic review

    IntroductionThe use of Information and Communication Technology (ICT) for assessing and treating cognitive and motor disorders is promoting home-based telerehabilitation. This approach involves ongoing monitoring within a motivating context to help patients generalize their skills. It can also reduce healthcare costs and geographic barriers by minimizing hospitalization. This systematic review focuses on investigating key aspects of telerehabilitation protocols for children with neurodevelopmental or neurological disorders, including technology used, outcomes, caregiver involvement, and dosage, to guide clinical practice and future research.MethodThis systematic review adhered to PRISMA guidelines and was registered in PROSPERO. The PICO framework was followed to define the search strategy for technology-based telerehabilitation interventions targeting the pediatric population (aged 0–18) with neurological or neurodevelopmental disorders. The search encompassed Medline/PubMed, EMBASE, and Web of Science databases. Independent reviewers were responsible for selecting relevant papers and extracting data, while data harmonization and analysis were conducted centrally.ResultsA heterogeneous and evolving situation emerged from our data. Our findings reported that most of the technologies adopted for telerehabilitation are commercial devices; however, research prototypes and clinical software were also employed with a high potential for personalization and treatment efficacy. The efficacy of these protocols on health or health-related domains was also explored by categorizing the outcome measures according to the International Classification of Functioning, Disability, and Health (ICF). Most studies targeted motor and neuropsychological functions, while only a minority of papers explored language or multi-domain protocols. Finally, although caregivers were rarely the direct target of intervention, their role was diffusely highlighted as a critical element of the home-based rehabilitation setting.DiscussionThis systematic review offers insights into the integration of technological devices into telerehabilitation programs for pediatric neurologic and neurodevelopmental disorders. It highlights factors contributing to the effectiveness of these interventions and suggests the need for further development, particularly in creating dynamic and multi-domain rehabilitation protocols. Additionally, it emphasizes the importance of promoting home-based and family-centered care, which could involve caregivers more actively in the treatment, potentially leading to improved clinical outcomes for children with neurological or neurodevelopmental conditions.Systematic review registrationPROSPERO (CRD42020210663)

    Background: Oral microbiota has been at the center of cultural attention in recent years. In daily clinical practice, orthodontic appliances may be associated with an increased cariogenic risk and a worsening of preexisting periodontal diseases. Objective: The purpose of this review is to investigate the available evidence regarding the association between orthodontic appliances and changes in the quality and quantity of the oral microbiota. Design: The research included every article published up to October 2017 featuring the keywords ‘Orthodontic appliance* AND (microbiological colonization OR periodontal pathogen* OR Streptococcus mutans OR Lactobacillus spp. OR Candida OR Tannerella forsythia OR Treponema denticola OR Fusobacterium nucleatum OR Aggregatibacter actinomycetemcomitans OR Prevotella intermedia OR Prevotella nigrescens OR Porphyromonas gingivalis)’ and was conducted in the major medical databases. The methodological quality of selected papers was scored using the ‘Swedish Council on Technology Assessment in Health Care Criteria for Grading Assessed Studies’ (SBU) method. Results: Orthodontic appliances influence the oral microbiota with an increase in the counts of S. mutans and Lactobacillus spp. and in the percentage of potentially pathogenic gram-negative bacteria. Conclusions: There is moderate/high evidence regarding the association between orthodontic appliances and changes in the oral microbiota. PROSPERO registration number CRD42018091589

    Saúde mental no Programa Saúde da Família: caminhos e impasses de uma trajetória necessária

    Esse estudo tem como objeto a atenção à saúde mental no Programa Saúde da Família (PSF) e como pressuposto a complexidade desse atendimento. Os objetivos foram analisar as condições concretas da assistência à saúde mental em unidades de PSF de Cuiabá, Mato Grosso, Brasil, descrevendo a dinâmica assistencial e identificando situações e instrumentos utilizados pelos diferentes trabalhadores para atender problemas de sofrimento mental na comunidade. Por meio de metodologia exploratório-descritiva, foram estudados os discursos de oito equipes com os recursos da análise de conteúdo. Concluiu-se que a demanda de atendimento às pessoas com sofrimento mental não era registrada nos prontuários nem integrava projeto de intervenção individual ou familiar; algumas ações eram realizadas numa concepção leiga de "doença mental" e tratamento, e o recurso do encaminhamento para a rede especializada era freqüentemente acionado, apesar da precariedade desta, refletindo a necessidade de continuidade no processo de capacitação em saúde mental

    Articulación Educativa entre la Universidad y la Secundaria abordada desde la Virología.

    Una de las preocupaciones más relevantes que manifiestan los docentes, es la escasa vinculación entre los diferentes niveles educativos. La articulación entre la Universidad y el Nivel Secundario es una temática abordada actualmente por las políticas educativas, sin embargo, la normativa vigente no condujo a un sistema común entre las instituciones. En este contexto, se concreta una experiencia de articulación entre la Universidad y la Escuela Secundaria, a través de una propuesta pedagógica entre docentes de la Facultad de Ciencias Médicas (Instituto de Virología “J. M. Vanella” y  Departamento de Admisión) y docentes del IPEM 38 “Francisco Pablo de Mauro” de la ciudad de Córdoba. El tema-eje de la propuesta pedagógica es las infecciones virales y su importancia sanitaria. Esta experiencia se propone promover la articulación educativa entre la escuela media y la universidad para favorecer la vinculación de los estudiantes de nivel medio con la actividad científica y la vida universitaria. La modalidad de trabajo implementada es el Aula-Taller. Los docentes y alumnos evaluaron, a través de encuestas, de manera satisfactoria las producciones grupales y la experiencia efectuada. Los resultados obtenidos permiten concluir que la estrategia pedagógica implementada promueve la vinculación entre los niveles educativos, facilitando a los jóvenes una aproximación a la educación superior

    Correlation between Mesio-Distal Angulation and Bucco.-Lingual Inclination of First and Second Maxillary Premolars Evaluated with Panoramic Radiography and Cone-Beam Computed Tomography

    Background: Panoramic radiography creates distortion in teeth position and inclination especially in lateral areas of the arches due to acquisition and curved shape of dental arch. The aim of this study was to evaluate panoramic distortion compared to cone-beam computed tomography. Methods: Mesiodistal angulations were measured on panoramic radiography. Mesiodistal angulations and buccolingual inclinations of the teeth were determined on cone-beam computed tomography scans. The previous measurements were then compared through Pearson correlation coefficient. Results: The results showed higher correlation between mesiodistal 2D angulation and buccolingual 3D inclination, especially for first premolars, rather than between mesiodistal angulations measured in 2D and 3D. Conclusion: Especially for the lateral areas of the dental arches, root mesiodistal angulation measured on panoramic radiography is not reliable; in fact, it is greatly influenced by buccolingual inclination. Therefore, clinical evaluation is highly recommended with panoramic radiography to provide correct diagnosis and the most satisfactory results
