    Society for Endocrinology (SfE) guidelines for the diagnosis and management of post bariatric hypoglycaemia

    Post-bariatric hypoglycaemia (PBH) is typically a post-prandial hypoglycaemia occurring about 2–4 h after eating in individuals who have undergone bariatric surgery. PBH develops relatively late after surgery and often after discharge from post-surgical follow-up by bariatric teams, leading to variability in diagnosis and management in non-specialist centres.AimThe overall aim was to improve and standardise clinical practice in the diagnosis and management of PBH. The objectives were: (1) to undertake an up-to-date review of the current literature; (2) to formulate practical and evidence-based guidance regarding the diagnosis and treatment of PBH; (3) to recommend future avenues for research in this condition.MethodA scoping review was undertaken after an extensive literature search. A consensus on the guidance and confidence in the recommendations was reached by the steering group authors prior to review by key stakeholders.OutcomeWe make pragmatic recommendations for the practical diagnosis and management of PBH, including criteria for diagnosis and recognition, as well as recommendations for research areas that should be explored.Plain English summaryPost-bariatric hypoglycaemia (PBH) is a condition that commonly affects people who have undergone weight loss surgery. In this condition, people develop low blood sugar occurring about 2–4 h after meals, leading to debilitating symptoms such as hunger, sweating, anxiety, palpitations and even blackouts and fainting. PBH is becoming more common as weight loss surgery is being taken up by more people to help with their weight and to help with diabetes. The condition often develops after the patient has been discharged from follow-up after their surgery, which can lead to inconsistent diagnosis and treatment in non-specialist healthcare centres. The lack of clear information and evidence in the existing scientific literature further contributes to the variation in care. To address this problem, the Society for Endocrinology has created new guidelines to help healthcare professionals accurately diagnose and manage this condition. The guidelines were developed with input from dietitians, surgeons and doctors specialising in weight loss, and hormone specialists

    Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050.     Overview of Synthetic Cannabinoids ADB-FUBINACA and AMB-FUBINACA: Clinical, Analytical, and Forensic Implications

    ADB-FUBINACA and AMB-FUBINACA are two synthetic indazole-derived cannabinoid receptor agonists, up to 140- and 85-fold more potent, respectively, than trans-∆9-tetrahydrocannabinol (∆9-THC), the main psychoactive compound of cannabis. Synthesised in 2009 as a pharmaceutical drug candidate, the recreational use of ADB-FUBINACA was first reported in 2013 in Japan, with fatal cases being described in 2015. ADB-FUBINACA is one of the most apprehended and consumed synthetic cannabinoid (SC), following AMB-FUBINACA, which emerged in 2014 as a drug of abuse and has since been responsible for several intoxication and death outbreaks. Here, we critically review the physicochemical properties, detection methods, prevalence, biological effects, pharmacodynamics and pharmacokinetics of both drugs. When smoked, these SCs produce almost immediate effects (about 10 to 15 s after use) that last up to 60 min. They are rapidly and extensively metabolised, being the O-demethylated metabolite of AMB-FUBINACA, 2-(1-(4-fluorobenzyl)-1H-indazole-3-carboxamide)-3-methylbutanoic acid, the main excreted in urine, while for ADB-FUBINACA the main biomarkers are the hydroxdimethylpropyl ADB-FUBINACA, hydroxydehydrodimethylpropyl ADB-FUBINACA and hydroxylindazole ADB-FUBINACA. ADB-FUBINACA and AMB-FUBINACA display full agonism of the CB1 receptor, this being responsible for their cardiovascular and neurological effects (e.g., altered perception, agitation, anxiety, paranoia, hallucinations, loss of consciousness and memory, chest pain, hypertension, tachycardia, seizures). This review highlights the urgent requirement for additional studies on the toxicokinetic properties of AMB-FUBINACA and ADB-FUBINACA, as this is imperative to improve the methods for detecting and quantifying these drugs and to determine the best exposure markers in the various biological matrices. Furthermore, it stresses the need for clinicians and pathologists involved in the management of these intoxications to describe their findings in the scientific literature, thus assisting in the risk assessment and treatment of the harmful effects of these drugs in future medical and forensic investigations

    Formulación y optimización de un medio de cultivo económico para Lactobacillus con potencial probiótico aislado del pulque

    Pulque is a pre-Hispanic beverage obtained from the fermentation of the agave mead. From this drink is possible to isolate yeasts and bacteria. Previously, the work team performed the isolation and identification of potential probiotic bacteria in several samples of pulque distributed in Mexico City, among the isolates, a strain of the genus Lactobacillus which has probiotic potential in the digestive tract was obtained, allowing the possibility of development of products with Lactobacillus (probiotics) that are typical and indigenous from Mexican culture. However, for the establishment of the strain to a product, is necessary the formulation, optimization of culture medium and incubation conditions based on the needs of the strain to obtain a good performance and a broth culture that is economical and conducive to the growth of the strain. Kinetics studies were performed with different formulations for the strain of Lactobacillus. Kinetic studies performed were: peptone water as negative control, MRS medium (DeMan, Rogosa & Sharpe) as positive control, formulations of milk and whey, with different concentrations of nutrients in the medium MRS. Further growth of Lactobacillus was observed in the broth culture: milk with MRS medium salts at 10% w/v, pH 6.4, 7% w/v lactose, 29 ° C at 18 h of culture.El pulque es una bebida prehispánica obtenida de la fermentación del aguamiel del agave. A partir de esta bebida es posible aislar levaduras y bacterias. Anteriormente, se realizó el aislamiento y la identificación de bacterias con potencial probiótico en varias muestras de pulque distribuido en la Ciudad de México; entre los microorganismos aislados se obtuvo una cepa del género Lactobacillus la cual tiene potencial probiótico en el tracto digestivo, lo que permite la posibilidad de desarrollo de productos de consumo con Lactobacillus (probióticos) que sean típicos y autóctonos de la cultura mexicana. Sin embargo, para el establecimiento de la cepa a un producto de consumo, es necesario realizar la formulación, la optimización del medio de cultivo y la determinación de las condiciones de incubación con base en las necesidades del lactobacilo para obtener un buen rendimiento, así como la formulación de un medio de cultivo que sea económico y propicie el crecimiento de la cepa. Se realizaron estudios cinéticos de crecimiento con diferentes formulaciones para la cepa de lactobacilo. Los estudios cinéticos realizados fueron los siguientes: Agua peptonada como control negativo, medio de cultivo MRS (DeMan, Rogosa y Sharpe) como control positivo, formulaciones de leche y suero de leche, con diferentes concentraciones de nutrientes del medio MRS. Se observó el mayor crecimiento del lactobacilo en el medio de leche con sales del medio MRS al 10% p/v, pH 6.4, 7% p/v lactosa, 29°C a las 18 h de cultivo

    Neotropical freshwater fisheries : A dataset of occurrence and abundance of freshwater fishes in the Neotropics

    The Neotropical region hosts 4225 freshwater fish species, ranking first among the world's most diverse regions for freshwater fishes. Our NEOTROPICAL FRESHWATER FISHES data set is the first to produce a large-scale Neotropical freshwater fish inventory, covering the entire Neotropical region from Mexico and the Caribbean in the north to the southern limits in Argentina, Paraguay, Chile, and Uruguay. We compiled 185,787 distribution records, with unique georeferenced coordinates, for the 4225 species, represented by occurrence and abundance data. The number of species for the most numerous orders are as follows: Characiformes (1289), Siluriformes (1384), Cichliformes (354), Cyprinodontiformes (245), and Gymnotiformes (135). The most recorded species was the characid Astyanax fasciatus (4696 records). We registered 116,802 distribution records for native species, compared to 1802 distribution records for nonnative species. The main aim of the NEOTROPICAL FRESHWATER FISHES data set was to make these occurrence and abundance data accessible for international researchers to develop ecological and macroecological studies, from local to regional scales, with focal fish species, families, or orders. We anticipate that the NEOTROPICAL FRESHWATER FISHES data set will be valuable for studies on a wide range of ecological processes, such as trophic cascades, fishery pressure, the effects of habitat loss and fragmentation, and the impacts of species invasion and climate change.
