    The Health Enhancement of the Rural Elderly (HERE) project is a federally funded grant project designed to empower the rural elderly in Butler and Edmonson counties to maximize their use of the health care system. This project was developed after identifying the needs of approximately 25% of the population in the U.S. that reside in rural areas. Of this population in rural Kentucky, 14-15% is 65 years or older (U.S. Bureau of Census, 2000). A higher incidence of chronic diseases, disabilities, difficulties with daily living activities, and cutbacks in social services have been identified as factors causing this population to be compromised in their success with health care

    What Would I Know About Mercy? Faith and Optimistic Expectancies Among African Americans

    A small body of research has begun to explore the association between faith and optimism among African Americans. However, missing from the extant work is an examination of the extent to which traditional indices of religious commitment work together with beliefs about God to shape optimism. The present study examines the utility of indices of social location, religious commitment (i.e., early and current religious service attendance, subjective religiosity), belief about the quality of one’s relationship with God (i.e., a belief that one is connected to a loving God), and beliefs about being the recipient of divine forgiveness for predicting dispositional optimism among a sample of community residing African American adults (N = 241). Age, subjective religiosity, and organizational religiosity were positively related to optimism in bivariate analyses. Hierarchical regression analyses demonstrated a significant association between age, subjective religiosity, and optimism; however, those associations were eliminated once relationship with God and belief in one’s forgiveness by God were entered into the model. Only belief in God’s love predicted optimism in multivariate analyses. Serial mediation analyses revealed that beliefs about the quality of one’s relationship with God and belief in divine forgiveness fully mediated the relationship between subjective religiosity and optimism, but that the relationship is driven largely by relationship with God. Implications of these findings are discussed

    Repeatable use assessment of silicon carbide as permanent susceptor bed in ex situ microwave remediation of petroleum-impacted soils

    Efficiency of microwave-enabled ex situ soil remediation can be improved by dielectric susceptors. Cost, and environmental burden of these susceptors can be minimized if they are used repeatedly in a permanent bed set-up. In this study, carbon nanofibers, activated carbon, magnetite, and silicon carbide were tested at the lab scale for repeated use in permanent microwave-induced thermal soil remediation. Despite their superior ability to convert microwaves into heat, carbon nanofibers experienced electrokinesis and activated carbon partially combusted in the microwave cavity, which complicates their pragmatic use in remediation applications. Magnetite was also able to convert microwaves into heat effectively and it was relatively more stable; however, repeated heating/and cooling cycles changed its physicochemical properties, which was attributed to oxidation of iron oxides at the air-soil interface. Silicon carbide, on the other hand, was an efficient heating agent and was stable during repeatable heating and cooling cycles. Through 25 heating/cooling cycles, an average peak temperature of 329 ± 55°C was achieved for a 29 cm3 sample and analysis of dielectric properties after every 10th and 25th cycle indicated that there were no significant losses in thermal conductivity or permittivity of the material. Subsequent remediation experiments with silicon carbide demonstrated that between 89 and 97% of the total petroleum hydrocarbons were removed from soil including a marked fraction of heavy hydrocarbons when 20.2 kJ g−1 of microwave energy was introduced. In addition, post-treatment experiments demonstrated that soil conditions were capable of supporting seed germination indicating that some conditions of soil were recovered after microwave remediation

    Advancing Equity In The Pandemic Treaty

    There is a broad consensus around equity’s importance. Even countries that hoarded supplies during the acute phase of COVID-19 seem to understand that the international community must find a means to ensure fairer allocation of medical resources when the next health crisis hits. But there has been little agreement about the concrete steps needed to operationalize fairer access and benefit sharing. That is, what are the workable mechanisms that could reduce the divide between richer and poorer populations? The World Health Assembly, the governing body of the World Health Organization, has appointed an Intergovernmental Negotiating Body to develop a pandemic convention, agreement, or other instrument under the WHO constitution. The February 2023 draft is designed “to achieve greater equity … through the fullest national and international cooperation.” It is important that the negotiators develop specific, measurable metrics that directly impact equity. The mechanisms and metrics agreed upon should allow the public to evaluate whether a more equitable system is emerging through this new regime. Equity won’t just happen. We need to plan and prepare for equity, and we need international norms with which nations must comply to achieve the fairness we strive for

    The writing on the wall? John Ingram’s verse and the dissemination of Catholic prison writing

    The strong association between prison writing and writing on walls, whether by graffiti or carving, is as true of Tudor and Stuart England as of other times and places. Yet even if prison-writers associated themselves with the idea of writing on a wall, they need not have done so in reality. This article considers the topos in the writings and afterlife of the Catholic priest, poet and martyr John Ingram, and asks whether it is to be taken at face value. Ingram’s verse, composed in Latin and mostly epigrammatic, survives in two contemporary manuscripts. The notion that the author carved his verses with a blunt knife on the walls of the Tower of London while awaiting death derives from a previous editorial interpretation of a prefatory sentence within the more authoritative manuscript of the two, traditionally held to be autograph. However, though several Tudor and Stuart inscriptions survive to this day on the walls of the Tower of London, no portions of Ingram’s verse are among them, nor any inscriptions of similar length and complexity. Ingram might instead have written his verse down in the usual way, using wall-carving as a metaphor for the difficulty of writing verse when undergoing incarceration and torture

    Influenza Vaccination and Mortality: Differentiating Vaccine Effects From Bias

    It is widely believed that influenza (flu) vaccination of the elderly reduces all-cause mortality, yet randomized trials for assessing vaccine effectiveness are not feasible and the observational research has been controversial. Efforts to differentiate vaccine effectiveness from selection bias have been problematic. The authors examined mortality before, during, and after 9 flu seasons in relation to time-varying vaccination status in an elderly California population in which 115,823 deaths occurred from 1996 to 2005, including 20,484 deaths during laboratory-defined flu seasons. Vaccine coverage averaged 63%; excess mortality when the flu virus was circulating averaged 7.8%. In analyses that omitted weeks when flu circulated, the odds ratio measuring the vaccination-mortality association increased monotonically from 0.34 early in November to 0.56 in January, 0.67 in April, and 0.76 in August. This reflects the trajectory of selection effects in the absence of flu. In analyses that included weeks with flu and adjustment for selection effects, flu season multiplied the odds ratio by 0.954. The corresponding vaccine effectiveness estimate was 4.6% (95% confidence interval: 0.7, 8.3). To differentiate vaccine effects from selection bias, the authors used logistic regression with a novel case-centered specification that may be useful in other population-based studies when the exposure-outcome association varies markedly over time

    Probiotic Bacteria Produce Conjugated Linoleic Acid Locally in the Gut That Targets Macrophage PPAR γ to Suppress Colitis

    Inflammatory bowel disease (IBD) therapies are modestly successful and associated with significant side effects. Thus, the investigation of novel approaches to prevent colitis is important. Probiotic bacteria can produce immunoregulatory metabolites in vitro such as conjugated linoleic acid (CLA), a polyunsaturated fatty acid with potent anti-inflammatory effects. This study aimed to investigate the cellular and molecular mechanisms underlying the anti-inflammatory efficacy of probiotic bacteria using a mouse model of colitis. The immune modulatory mechanisms of VSL#3 probiotic bacteria and CLA were investigated in a mouse model of DSS colitis. Colonic specimens were collected for histopathology, gene expression and flow cytometry analyses. Immune cell subsets in the mesenteric lymph nodes (MLN), spleen, blood and colonic lamina propria cells were phenotypically and functionally characterized. Fecal samples and colonic contents were collected to determine the effect of VSL#3 and CLA on gut microbial diversity and CLA production. CLA and VSL#3 treatment ameliorated colitis and decreased colonic bacterial diversity, a finding that correlated with decreased gut pathology. Colonic CLA concentrations were increased in response to probiotic bacterial treatment, but without systemic distribution in blood. VSL#3 and CLA decreased macrophage accumulation in the MLN of mice with DSS colitis. The loss of PPAR γ in myeloid cells abrogated the protective effect of probiotic bacteria and CLA in mice with DSS colitis. Probiotic bacteria modulate gut microbial diversity and favor local production of CLA in the colon that targets myeloid cell PPAR γ to suppress colitis
