    The original intention of the writer was to make a thorough biblical study of all the passages in the letters of Paul which were concerned with his treatment of the family and its relationships. Material was gathered and the first chapter, which was to be a discussion of marriage, was prepared. However, it soon became apparent that the topic undertaken was entirely too large for the type of paper which was to be prepared. The first chapter of the original thesis covered approximately forty-five pages, and various problems in the text had been either treated lightly or completely ignored. Upon the advice of the advisor an exegetical study of the marriage passage in Ephesians, 5:21-.33, was begun. This thesis, then, is the fruit of such labors

    Caregiver strain on informal caregivers when providing care for older patients undergoing major abdominal surgery:A longitudinal prospective cohort study

    Background:  Health-care systems nowadays rely on complementary patient care by informal caregivers. The need for, and burden on, informal caregivers will likely increase in the upcoming years. This study aimed to examine the burden on caregivers when providing care for elderly patients undergoing major abdominal surgery. Methods:  A single-centre longitudinal cohort study was conducted between November 2015 and June 2018 in the Amphia hospital in Breda, the Netherlands. Patients aged 70+ undergoing elective surgery for colorectal carcinoma (CRC) or an abdominal aortic aneurysm (AAA) were included in this study. Informal caregiver burden was assessed and compared over time using the Caregiver Strain Index (CSI) at the outpatient clinic visit, at discharge, 2 weeks post-discharge and after 6 and 12 months. The effects of patient- and caregiver-related factors on the experienced caregiver strain were examined. Results: CSI scores of 248 caregivers were significantly increased at discharge (3.5 vs 2.6; p < 0.001) and 2 weeks post-discharge (3.3 vs 2.6; p < 0.001). After 12 months, scores dropped below baseline scores (1.8 vs 2.6; p = 0.012). The highest strain was observed 2 weeks post-discharge for AAA patients and at discharge for CRC patients. Older age, physical or cognitive impairment and burden of comorbidity were associated with an increased caregiver strain at baseline. Type of surgery was independently associated with the change in mean CSI scores over time; a bigger change in caregiver burden is observed after open surgery. Conclusion:  In the early postoperative period, perceived caregiver strain was significantly increased. Psychological support for caregivers may be advisable, with timing of this support depending on diagnosis and patient-related factors.     Plasma Metabolome Alterations Discriminate between COVID-19 and Non-COVID-19 Pneumonia

    Pneumonia is a common cause of morbidity and mortality and is most often caused by bacterial pathogens. COVID-19 is characterized by lung infection with potential progressive organ failure. The systemic consequences of both disease on the systemic blood metabolome are not fully understood. The aim of this study was to compare the blood metabolome of both diseases and we hypothesize that plasma metabolomics may help to identify the systemic effects of these diseases. Therefore, we profiled the plasma metabolome of 43 cases of COVID-19 pneumonia, 23 cases of non-COVID-19 pneumonia, and 26 controls using a non-targeted approach. Metabolic alterations differentiating the three groups were detected, with specific metabolic changes distinguishing the two types of pneumonia groups. A comparison of venous and arterial blood plasma samples from the same subjects revealed the distinct metabolic effects of pulmonary pneumonia. In addition, a machine learning signature of four metabolites was predictive of the disease outcome of COVID-19 subjects with an area under the curve (AUC) of 86 ± 10 %. Overall, the results of this study uncover systemic metabolic changes that could be linked to the etiology of COVID-19 pneumonia and nonCOVID-19 pneumonia

    Quantitative assessment of harmonic power doppler myocardial perfusion imaging with intravenous levovist™ in patients with myocardial infarction: comparison with myocardial viability evaluated by coronary flow reserve and coronary flow pattern of infarct-related artery

    BACKGROUND: Myocardial contrast echocardiography and coronary flow velocity pattern with a rapid diastolic deceleration time after percutaneous coronary intervention has been reported to be useful in assessing microvascular damage in patients with acute myocardial infarction. AIM: To evaluate myocardial contrast echocardiography with harmonic power Doppler imaging, coronary flow velocity reserve and coronary artery flow pattern in predicting functional recovery by using transthoracic echocardiography. METHODS: Thirty patients with anterior acute myocardial infarction underwent myocardial contrast echocardiography at rest and during hyperemia and were quantitatively analyzed by the peak color pixel intensity ratio of the risk area to the control area (PIR). Coronary flow pattern was measured using transthoracic echocardiography in the distal portion of left anterior descending artery within 24 hours after recanalization and we assessed deceleration time of diastolic flow velocity. Coronary flow velocity reserve was calculated two weeks after acute myocardial infarction. Left ventricular end-diastolic volumes and ejection fraction by angiography were computed. RESULTS: Pts were divided into 2 groups according to the deceleration time of coronary artery flow pattern (Group A; 20 pts with deceleration time ≧ 600 msec, Group B; 10 pts with deceleration time < 600 msec). In acute phase, there were no significant differences in left ventricular end-diastolic volume and ejection fraction (Left ventricular end-diastolic volume 112 ± 33 vs. 146 ± 38 ml, ejection fraction 50 ± 7 vs. 45 ± 9 %; group A vs. B). However, left ventricular end-diastolic volume in Group B was significantly larger than that in Group A (192 ± 39 vs. 114 ± 30 ml, p < 0.01), and ejection fraction in Group B was significantly lower than that in Group A (39 ± 9 vs. 52 ± 7%, p < 0.01) at 6 months. PIR and coronary flow velocity reserve of Group A were higher than Group B (PIR, at rest: 0.668 ± 0.178 vs. 0.248 ± 0.015, p < 0.0001: during hyperemia 0.725 ± 0.194 vs. 0.295 ± 0.107, p < 0.0001; coronary flow velocity reserve, 2.60 ± 0.80 vs. 1.31 ± 0.29, p = 0.0002, respectively). CONCLUSION: The preserved microvasculature detecting by myocardial contrast echocardiography and coronary flow velocity reserve is related to functional recovery after acute myocardial infarction

    Motivation profiles in sport: A self-determination theory perspective

    The present study examined the link between motivation profiles among adult sports participants and the outcomes of enjoyment, effort, positive and negative affect, attitude toward sport participation, intention to continue sport participation, satisfaction, and persistence in sport. Two samples of participants (n = 590 and n = 555) completed the Sport Motivation Scale and a range of self-report measures to assess the outcome variables. Exploratory cluster analyses applied to Sample 1 and confirmatory cluster analysis applied to Sample 2 identified two clusters of sport participants. The first comprised participants with high scores on both non self-determined and self-determined motives. The second comprised participants with high scores on self-determined motives but low scores on non self- determined motives. Participants in the first cluster scored higher on all outcome variables. The results are discussed with reference to a more in-depth understanding of the motivation dynamics of sport participation based on Self-Determination Theory

    Switching Coal-Fired Thermal Power Plant to Composite Fuel for Recovering Industrial and Municipal Waste: Combustion Characteristics, Emissions, and Economic Effect

    Combustion characteristics were studied experimentally for single droplets of fuel slurries based on wet coal processing waste with municipal solid waste components (cardboard, plastic, rubber, and wood) and used turbine oil. We established the ignition delay time for three various groups of fuel compositions in motionless air at 600–1000 °C. The minimum values are 3 s, and the maximum ones are 25 s. The maximum temperatures in the droplet vicinity reach 1300 °C during fuel combustion for compositions with 10% of used oil. The combustion temperatures of fuel compositions without oil are 200–300 °C lower. The concentrations of anthropogenic emissions in flue gases do not exceed those from dry coal combustion. Adding used oils to composite fuels reduces the concentrations of dioxins and furans in flue gases when municipal solid waste in the fuel burns out due to high combustion temperatures. Based on the experimental research findings, we have elaborated a strategy of combined industrial and municipal waste recovery by burning it as part of composite fuels, as illustrated by three neighboring regions of the Russian Federation with different industrial structures and levels of social development. This strategy suggests switching three typical coal-fired thermal power plants (one in each of the regions) to composite liquid fuel. It will reduce the hazard of waste to the environment and decrease the consumption of high-quality coals for power generation. Implementing the developed strategy for 25 years will save 145 Mt of coal and recover 190–260 Mt of waste. The positive economic effect, considering the modernization of fuel handling systems at thermal power plants and the construction of a fuel preparation plant, will make up 5.7 to 6.9 billion dollars, or 65–78%, respectively, of the main costs of three thermal power plants operating on coal within the identical period

    Virtual unfolding of folded papyri

    The historical importance of ancient manuscripts is unique since they provide information about the heritage of ancient cultures. Often texts are hidden in rolled or folded documents. Due to recent improvements in sensitivity and resolution, spectacular disclosures of rolled hidden texts were possible by X-ray tomography. However, revealing text on folded manuscripts is even more challenging. Manual unfolding is often too risky in view of the fragile condition of fragments, as it can lead to the total loss of the document. X-ray tomography allows for virtual unfolding and enables non-destructive access to hidden texts. We have recently demonstrated the procedure and tested unfolding algorithms on a mockup sample. Here, we present results on unfolding ancient papyrus packages from the papyrus collection of the Musée du Louvre, among them objects folded along approximately orthogonal folding lines. In one of the packages, the first identification of a word was achieved, the Coptic word for “Lord”
