    Cirurgia parendodôntica – técnica de curetagem e alisamento radicular: revisão de literatura

    Introdução: a cirurgia parendodôntica é um procedimento fundamental para o tratamento de lesões periapicais refratárias que não responderam às terapias endodônticas convencionais. Essa abordagem busca eliminar infecções persistentes e promover a neoformação óssea, possibilitando a restauração da saúde bucal e a qualidade de vida dos pacientes. A técnica cirúrgica de curetagem e alisamento radicular (CAR) tem se destacado como uma modalidade eficiente nesse contexto. Objetivo: esta revisão de literatura tem como objetivo analisar estudos que investigaram a eficácia da técnica de curetagem e alisamento radicular na cirurgia parendodôntica, com foco em avaliar sua efetividade na cicatrização periapical, controle de infecção e taxa de sucesso do tratamento. Metodologia: esta revisão de literatura foi conduzida de acordo com uma metodologia narrativa para identificar estudos relevantes sobre a eficácia da técnica de curetagem e alisamento radicular em cirurgia parendodôntica. A estratégia de busca incluiu as principais bases de dados científicas, como PubMed, Scopus e Web of Science, com artigos publicados nos últimos 05 anos. Resultados: os estudos analisados nesta revisão indicaram que a técnica de curetagem e alisamento radicular demonstrou ser eficaz na remoção de tecidos inflamados e contaminados no ápice da raiz. Houve uma melhora significativa na cicatrização periapical e no controle de infecção em pacientes submetidos a esse procedimento. Conclusão: com base nas evidências disponíveis, a técnica de curetagem e alisamento radicular mostra-se uma abordagem promissora na cirurgia parendodôntica. A remoção adequada de tecidos inflamados e contaminados no ápice da raiz contribui para a cicatrização periapical e o controle da infecção, resultando em uma alta taxa de sucesso do tratamento

    Miltefosine in the Treatment of Cutaneous Leishmaniasis Caused by Leishmania braziliensis in Brazil: A Randomized and Controlled Trial

    Cutaneous leishmaniasis (CL) is characterized by skin ulcerations and occurs in rural poor areas of developing countries. It is treated with daily injections of antimony for 20 days, which is associated with irregular use and increasingly lower cure rates. Miltefosine is an oral medication with activity against the agent of CL (Leishmania). We have studied the efficacy and safety of miltefosine compared with antimony in patients with CL caused by Leishmania braziliensis in Bahia, Brazil. A total of 90 patients participated; 60 received miltefosine and 30 were treated with antimony. Six months after treatment, 75% of patients treated with miltefosine were cured, compared with 53% of the patients in the antimony group, a difference considered significant (p = 0.04). We also found that miltefosine was more effective than antimony in adults than in children. The incidence of side effects was similar with both drugs (76.7% vs. 78.3%), but all patients were able to finish the treatments. Our study shows that miltefosine is more effective than antimony for the treatment of CL in Bahia, Brazil and can contribute to the control of this disease due to its activity and easier administration

    Immunodominant Antigens of Leishmania chagasi Associated with Protection against Human Visceral Leishmaniasis

    One of the most striking features of infection by Leishmania chagasi is that infection leads to a spectrum of clinical outcomes ranging from asymptomatic infection to active disease. The existence of asymptomatic infected people has served as an incentive to believe that an effective vaccine is possible, but unfortunately no successful immunological characterization of such cases was obtained. Patients recovered from visceral leishmaniasis show a similar immunological profile to asymptomatic infected individuals and both exhibit a strong cell-mediated immune response against Leishmania antigens and are resistant to disease. Since the past decade several approaches were undertaken to try to shed light on the immunological profile associated with such “resistance” to infections, notwithstanding antigenic recognition profile associated to resistance to infection was not successfully explored. In the present manuscript we describe a specific IgG recognizing pattern associated with resistant individuals (asymptomatic infected people and recovery patients to visceral leishmaniasis). These data highlight the possibility of using specific proteins in serological tests for the identification of asymptomatic infected individuals

Una cartografia completa de les experiències en mediació en tots els àmbits socials, de les escoles als hospitals, de les empreses als nuclis familiars, de la mediació comunitària als conflictes de consum o laborals, de la mediació penal a la mediambiental. També s'hi ha incorporat una anàlisi dels costos de la mediació i de la seva configuració jurídica. La conjunció de les fotografies en relleu i dels estudis més teòrics han fet possible la reflexió ulterior, les interpretacions crítiques i, en darrer terme, les conclusions i les recomanacions, que ens ajudaran a progressar. La mediació permet detenir l'escalada dels conflictes i sostreure'ls de la resolució judicial, per implicar les parts i fer-les protagonistes actives de les solucions a què arribin. Des del Departament de Justícia, ens interessa superar l'excessiva judicialització dels conflictes -insatisfactòria per a tothom- i promoure instruments que facilitin, de manera àgil però amb totes les garanties, la intel·ligència dels problemes i, a partir d'aquí, la fixació de les millors solucions per a les parts implicades, que elles mateixes hauran construït

    Dans un contexte d’interrogation sur les modèles agricoles et de profondes transformations des agricultures et des marchés, cet ouvrage s’attache à revisiter la diversité des formes familiales de production et leurs mutations de par le monde. Dès lors, l’analyse intègre les liens aux marchés, aux territoires et à l’Ailleurs — par le fait migratoire — les enjeux d’autonomie et de sécurité alimentaire, les stratégies de survie et d’accumulation ainsi que les formes d’action collective et politique. L’ouvrage est construit autour de dix-huit études de cas, menées dans les cinq continents. Elles ont toutes été conduites avec un cadre méthodologique, identique et original, inspiré du Sustainable Rural Livehoods (cadre d’identification des moyens de subsistance durables en milieu rural). Revisitée pour cet ouvrage, cette méthode d’observations et d’analyses permet aux auteurs de préciser finement « ce qui fait famille », d’analyser les adaptations du travail des actifs familiaux et les mettre en perspective avec le contexte territorial et les politiques publiques de chaque pays. Écrit à plusieurs mains, par un réseau de chercheurs, cet ouvrage contribue autant à l’approfondissement des savoirs scientifiques sur les agricultures familiales dans le monde qu’à la mise à l’épreuve d’un cadre méthodologique d’analyse et d’observations en milieu rural. Il vise un public de chercheurs, d’enseignants et d’étudiants, agronomes, économistes, sociologues et historiens. Les experts du développement agricole et rural y trouveront un grand intérêt. Plus largement, toute personne qui s’intéresse aux agricultures familiales et à leurs évolutions dans divers contextes sociaux trouvera avantage à cette lecture. Les chercheurs qui ont coordonné cet ouvrage sont agronomes, économistes et géographes, au Cirad ou à l’Inra où ils mènent des recherches sur les systèmes agricoles familiaux à des échelles locales, nationales ou internationales. Cet ouvrage est le fruit d’une démarche de recherche collective et partagée permettant à de nombreux scientifiques des pays du Sud d’exprimer la diversité des réalités agraires contemporaines.     A cintilografia óssea como exame complementar no diagnóstico da necrose avascular de sesamoide

    OBJETIVO: Este estudo objetiva apresentar sete casos de necrose avascular de sesamoide e relatar o papel da cintilografia óssea no diagnóstico desses pacientes. MÉTODOS: Sete pacientes com suspeita clínica de necrose avascular de sesamoide submetidos a cintilografia óssea trifásica com 30mCi de MDP-99mTc. RESULTADOS: A maioria dos pacientes eram adultos jovens, do sexo feminino, com queixas de dor limitante em antepé, que faziam uso de calçados inadequados e/ou apresentavam relato de trauma associado ou não à fratura. Não houve predileção por um dos pés ou entre sesamoides tibial ou femoral. Dois (28,57%) pacientes apresentavam sesamoides tibiais bipartidos e um (14,29%) apresentava bipartição de sesamoides tibial e fibular. Em 100% dos pacientes a cintilografia óssea trifásica, associada a outros métodos propedêuticos, mostrou-se fundamental para o diagnóstico. A conduta inicial em todos os casos foi conservadora. Nos quatro casos (57,14%) em que não houve remissão dos sintomas, excisão cirúrgica do tecido sesamoideo necrotizado foi realizada. Em todos os pacientes a terapia utilizada se mostrou eficaz, havendo completa remissão dos sintomas sem complicações ou deformidades do antepé. CONCLUSÕES: A cintilografia óssea trifásica torna-se angular na propedêutica da necrose avascular do sesamoide, concorrendo com diagnóstico precoce acurado e permitindo ao especialista tratamento adequado.OBJECTIVE: This study aimed to present seven cases of avascular necrosis of the sesamoid end report the role of bone scintigraphy in the diagnosis of these patients. METHODS: Seven patients with clinical suspicion of avascular necrosis of the sesamoid underwent three-phase bone scintigraphy with 30 mCi of 99mTc-MDP. RESULTS: Most of the patients were young female adults with complaints of limiting pain in the forefoot, who were making use of inappropriate footwear and/or had a history of injury with or without fracture. There was no predominance of either of the feet or between the femoral or tibial sesamoid. Two patients (28.57%) had a bipartite tibial sesamoid and one (14.29%) had splitting of the tibial and fibular sesamoids. In 100% of the patients, three-phase bone scintigraphy, combined with other propaedeutic methods, proved to be crucial for the diagnosis. The initial procedure in all cases was conservative. In four cases (57.14%), there was no remission of symptoms, and surgical excision of the necrotized sesamoid tissue was performed. In all the patients, the therapy used was effective, with complete remission of symptoms, without complications or deformities of the forefoot. CONCLUSIONS: Three-phase bone scintigraphy becomes a cornerstone of the propaedeutics when avascular necrosis of the sesamoid is suspected, through contributing towards early and accurate diagnosis and enabling allowing appropriate specialized treatment
