    Parents and school integration: the case of Tenerife (Canary Islands)

    La presente investigación pretende conocer las actitudes hacia la integración escolar de alumnos y alumnas con necesidades educativas especiales (n.e.e.) por parte de los padres y madres de los centros educativos públicos de la isla de Tenerife a los cuales acude dicho alumnado. Se ha desarrollado en dos períodos distintos de la evolución del proceso de respuesta educativa a la diversidad del alumnado; uno inicial, producido entre los años 1997-1998, y otro más reciente, del año 2002 al 2003. El estudio se realizó en los mismos centros educativos en las dos etapas, participando un total de mil sesenta y siete padres y madres que han valorado este proceso y expresado su opinión hacia el mismo.The present investigation seeks to know the attitudes toward the school integration of students with special educational needs (n.e.e.) on the part of the parents and mothers of the centers educational publics of the island of Tenerife to which this pupil goes. It has been developed in two periods different from the evolution from the process of educational answer to the diversity of the pupil, one initial, taken place among the years 1997-1998 and other more recent, of the year 2002 at the 2003.     Intelligent approach for analysis of respiratory signals and oxygen saturation in the sleep apnea/hypopnea syndrome

    doi: 10.2174/1874431101408010001This work deals with the development of an intelligent approach for clinical decision making in the diagnosis of the Sleep Apnea/Hypopnea Syndrome, SAHS, from the analysis of respiratory signals and oxygen saturation in arterial blood, SaO2. In order to accomplish the task the proposed approach makes use of different artificial intelligence techniques and reasoning processes being able to deal with imprecise data. These reasoning processes are based on fuzzy logic and on temporal analysis of the information. The developed approach also takes into account the possibility of artifacts in the monitored signals. Detection and characterization of signal artifacts allows detection of false positives. Identification of relevant diagnostic patterns and temporal correlation of events is performed through the implementation of temporal constraints

    El Efecto De La Implantación Temprana En Niños Con Hipoacusia

    Hearing loss in pediatric population is a major health concern, taking into account the immediate repercussions on the cognitive, emotional and language development (serious difficulties in communication and language development). Therefore, it is of crucial importance its early diagnosis and implantation. The objective of this study is to know the effect of cochlear implant on the development of oral language in implanted children in the Province of Santa Cruz de Tenerife (Canary Islands). In order to carry out this research, the psycholinguistic profiles of children who were implanted between 2011 and 2014 were studied, through the use of two standardized tests, Peabody Picture Vocabulary Test and The Illinois Test of Psycholinguistic Abilities (ITPA). As regards the age of implantation, we could observe a significant tendency to use the visual channel in the communication of those children who were implanted at a later age and a tendency to use the auditory channel at an early age of implantation. Children who were implanted at a later age showed a nonfunctional use of the implant and a limited development of their speaking skills

    Addressing the data bottleneck in medical deep learning models using a human-in-the-loop machine learning approach

    [Abstract]: Any machine learning (ML) model is highly dependent on the data it uses for learning, and this is even more important in the case of deep learning models. The problem is a data bottleneck, i.e. the difficulty in obtaining an adequate number of cases and quality data. Another issue is improving the learning process, which can be done by actively introducing experts into the learning loop, in what is known as human-in-the-loop (HITL) ML. We describe an ML model based on a neural network in which HITL techniques were used to resolve the data bottleneck problem for the treatment of pancreatic cancer. We first augmented the dataset using synthetic cases created by a generative adversarial network. We then launched an active learning (AL) process involving human experts as oracles to label both new cases and cases by the network found to be suspect. This AL process was carried out simultaneously with an interactive ML process in which feedback was obtained from humans in order to develop better synthetic cases for each iteration of training. We discuss the challenges involved in including humans in the learning process, especially in relation to human–computer interaction, which is acquiring great importance in building ML models and can condition the success of a HITL approach.     Identification of clinical variants beyond the exome in inborn errors of metabolism

    Inborn errors of metabolism (IEM) constitute a huge group of rare diseases affecting 1 in every 1000 newborns. Next-generation sequencing has transformed the diagnosis of IEM, leading to its proposed use as a second-tier technology for confirming cases detected by clinical/biochemical studies or newborn screening. The diagnosis rate is, however, still not 100%. This paper reports the use of a personalized multi-omics (metabolomic, genomic and transcriptomic) pipeline plus functional genomics to aid in the genetic diagnosis of six unsolved cases, with a clinical and/or biochemical diagnosis of galactosemia, mucopolysaccharidosis type I (MPS I), maple syrup urine disease (MSUD), hyperphenylalaninemia (HPA), citrullinemia, or urea cycle deficiency. Eight novel variants in six genes were identified: six (four of them deep intronic) located in GALE, IDUA, PTS, ASS1 and OTC, all affecting the splicing process, and two located in the promoters of IDUA and PTS, thus affecting these genes’ expression. All the new variants were subjected to functional analysis to verify their pathogenic effects.     Resultados clínicos iniciales y variables pronósticas en la implementación de un Código Sepsis en un Hospital Universitario de alta complejidad

    Objective: To assess the impact of the first months of application of a Code Sepsis in a high complexity hospital, analyzing patient´s epidemiological and clinical characteristics and prognostic factors. Methods: A long-term observational study was carried out throughout a consecutive period of seven months (February 2015 - September 2015). The relationship with mortality of risk factors, and analytic values was analyzed using uni- and multivariate analyses. Results: A total of 237 patients were included. The in-hospital mortality was 24% at 30 days and 27% at 60 days. The mortality of patients admitted to Critical Care Units was 30%. Significant differences were found between the patients who died and those who survived in mean levels of creatinine (2.30 vs 1.46 mg/dL, p 65 years (OR 5.33, p 3 mmol/L (OR 5,85, p 1,2 mgr /dL (OR 4,54, p <0,05) and shock (OR 6,57, P <0,05). Conclusions: The epidemiological, clinical and mortality characteristics of the patients in our series are similar to the best published in the literature. The study has identified several markers that could be useful at a local level to estimate risk of death in septic patients. Studies like this one are necessary to make improvements in the Code Sepsis programsObjetivo. Evaluar el impacto de un programa educativo y organizativo llamado Código Sepsis, en los primeros siete meses de su aplicación en un hospital de alta complejidad. Material y métodos. Se realizó un estudio observacional durante un período consecutivo de siete meses (Febrero 2015-Septiembre 2015). Se analizó la relación con la mortalidad de los factores de riesgo y los valores analíticos usando análisis uni y multivariante. Resultados. Se incluyeron un total de 237 pacientes. La mortalidad intrahospitalaria a los 30 días fue del 24 % y del 27% a los 60 días. La mortalidad de los pacientes ingresados en Unidades de Cuidados Críticos fue del 30%. Se encontraron diferencias significativas entre los pacientes que murieron y los que sobrevivieron en sus valores medios de creatinina (2,30 vs 1,46 mg/dL, p <0,05), ácido láctico (6,10 vs 2,62 mmol/L, p <0,05) y procalcitonina (23,27 vs 12,73 mg/dL, p <0,05). Se encontró una tendencia lineal estadísticamente significativa entre los valores de la escala SOFA y la mortalidad (p <0,05). En el análisis multivariante se identificaron otros factores de riesgo independientes asociados con la muerte: edad > 65 años (OR 5,33, p 3 mmol/L (OR 5,85, p <0,05), creatinina > 1,2 mgr/dL (OR 4,54, p <0,05) y el shock (OR 6,57, P <0,05). Conclusiones. La mortalidad en este estudio se encuentra dentro de los límites de los ensayos clínicos más recientes de sepsis. El estudio ha identificado varios marcadores que podrían ser útiles a nivel local para estimar el riesgo en pacientes sépticos.     Las nuevas tendencias en tecnologías blandas o administrativas introducen una buena dosis de cambio, oportunidad y pragmatismo. Nuestra disciplina contable parece no atinar hacia nuevos desarrollos que satisfaga estas necesidades y cada vez más está quedando en la puerta trasera del desarrollo. Otras áreas diferentes a la propiamente contable como los desarrollos en sistemas de información gerencial, avances informáticos en aplicaciones comerciales, nuevos conceptos de medición, evaluación y control producto de enfoques administrativos como la calidad total y gerencia de la velocidad, están motivando cambios estructurales a lo contable. La pasividad frente a estas situaciones motivadas talvez por una pésima interpretación del principio contable de la prudencia, debe ser revaluada en forma urgente en el seno de gremios profesionales y círculos académicos e investigativos. En Colombia la nueva constitución política abrió un horizonte completamente diferente a la disciplina contable. Ya no lo tradicional operativo del presupuesto oficial Sino nuevos conceptos como auditoría de gestión, vigilancia del medio ambiente, agresividad en la participación de la auditoría oficial frente a las necesidades del país. El nuevo auditor fiscal tiene un enfoque interdisciplinario, moderno pragmático. Las Facultades de Contaduría Pública del país están obligadas a mirar por la ventana del cambio y no el encerramiento actual bajo una tradición completamente obsoleta.PRESENTACION. In Colombia, the new political constitution opened a completely different horizon for accounting discipline. No longer the traditional operating of the official budget, but new concepts such as management audit, environmental surveillance, aggressiveness in the participation of the official audit against the needs of the country. the new listener prosecutor has an interdisciplinary, modern pragmatic approach. The Public Accounting Faculties of the country are obliged to look through the window of change and not the current closure under a completely obsolete tradition

    Effectiveness and safety of integrase strand transfer inhibitors in Spain: a prospective real-world study

    IntroductionSecond-generation integrase strand transfer inhibitors (INSTIs) are preferred treatment options worldwide, and dolutegravir (DTG) is the treatment of choice in resource-limited settings. Nevertheless, in some resource-limited settings, these drugs are not always available. An analysis of the experience with the use of INSTIs in unselected adults living with HIV may be of help to make therapeutic decisions when second-generation INSTIs are not available. This study aimed to evaluate the real-life effectiveness and safety of dolutegravir (DTG), elvitegravir/cobicistat (EVG/c), and raltegravir (RAL) in a large Spanish cohort of HIV-1-infected patients.MethodsReal-world study of adults living with HIV who initiated integrase INSTIs DTG, EVG/c, and RAL-based regimens in three settings (ART-naïve patients, ART-switching, and ART-salvage patients). The primary endpoint was the median time to treatment discontinuation after INSTI-based regimen initiation. Proportion of patients experiencing virological failure (VF) (defined as two consecutive viral loads (VL) ≥200 copies/mL at 24 weeks or as a single determination of VL ≥1,000 copies/mL while receiving DTG, EVG/c or RAL, and at least 3 months after INSTI initiation) and time to VF were also evaluated.ResultsVirological effectiveness of EVG/c- and RAL-based regimens was similar to that of DTG when given as first-line and salvage therapy. Treatment switching for reasons other than virological failure was more frequent in subjects receiving EVG/c and, in particular, RAL. Naïve patients with CD4+ nadir &lt;100 cells/μL were more likely to develop VF, particularly if they initiated RAL or EVG/c. In the ART switching population, initiation of RAL and EVG/c was associated with both VF and INSTI discontinuation. There were no differences in the time to VF and INSTI discontinuation between DTG, EVG/c and RAL. Immunological parameters improved in the three groups and for the three drugs assessed. Safety and tolerability were consistent with expected safety profiles.DiscussionWhereas second-generation INSTIs are preferred treatment options worldwide, and DTG is one of the treatment of choices in resource-limited settings, first-generation INSTIs may still provide high virological and immunological effectiveness when DTG is not available

    Actas del V Congreso ISUF-H Costa Rica 2021: Ciudades espontáneas versus ciudades planificadas: distintos retos, distintas realidades

    En el año 2021 celebramos en Costa Rica la V edición del Congreso ISUF-H, los días 1, 2 y 3 de diciembre, con la Escuela de Arquitectura de la Universidad de Costa Rica como anfitriona del evento. El congreso “Ciudades espontáneas versus ciudades planificadas: distintos retos, distintas realidades” propuso como eje central una reflexión crítica sobre los procesos de urbanización planificada y urbanización espontánea, en el cual se fomente un abordaje de las ciudades como expresión de organización social, económica, ambiental y cultural, enfatizando el carácter ideológico de la urbanización y subrayando su continua construcción como resultado de construcciones complejas. La celebración de un nuevo congreso en América Latina, permitió reforzar la tradición crítica en el abordaje de las ciudades, y reforzar también la necesidad de plantear una perspectiva latinoamericana de los estudios urbanos, y por consiguiente de una teoría urbana latinoamericana. En esta ocasión el congreso se centró en ahondar en la temática de la forma urbana, desde perspectivas transversales que involucren las amplias disciplinas que asumen como objeto de discusión las problemáticas de la ciudad contemporánea y cuestionan la dicotomía planteada entre lo espontáneo y lo planificado. Para la Escuela de Arquitectura de la Universidad de Costa Rica y su Laboratorio de Ciudad y Territorio es un honor haber podido llevar a cabo esta nueva edición del congreso de la Asociación ISUF-H como segunda sede en un país latinoamericano. Relevante para fortalecer la temática de la forma urbana en la región, reforzando alianzas y estableciendo nuevas redes que permitan compartir conocimientos a partir de las experiencias de esas diversidades urbanas. Auspiciar el debate en torno a la morfología urbana y las diferencias entre esas ciudades espontáneas y las planificadas, fue una oportunidad para reunir a expertos de las distintas latitudes hispánicas.UCR::Vicerrectoría de Docencia::Ingeniería::Facultad de Ingeniería::Escuela de Arquitectur
