    Biomedical applications of polymer and ceramic coatings: a review of recent developments

    Recent literature concerning the use of polymer and ceramic coatings for a variety of biomedical applications is surveyed in this review. Applications have been grouped into six broad categories: orthopaedic materials, cardiovascular stents, antibacterial surfaces, drug delivery, tissue engineering and biosensors. Polymer and ceramic coatings add enhanced corrosion protection, antiwear, antibacterial and biocompatibility properties to various substrates for biomedical applications. Processes favoured for polymer coating formation included dip, electrodeposition, spin (including electrospin) and spray (including electrospray and ultrasonic spray). Ceramic coatings were formed using magnetron sputtering and a combination of 3-D printing and in-situ mineralisation, among others. The review period is from 2017 to the present (mid-2021)

    In vitro in vivo relations for the parenteral liposomal formulation of Amphotericin B: A biorelevant and clinically relevant approach

    There is limited information on how to perform in vitro release tests for intravenously administered parenteral formulations and how to relate the in vitro release with an in vivo pharmacokinetic parameter after the administration of the formulation. In this study, the effect of hydrodynamics (using sample and separate and continuous flow conditions) and medium components (synthetic surfactants, albumin and buffers) on the release of Amphotericin B from the liposomal Ambisome® formulation were investigated. Pharmacokinetic modeling of plasma concentration profiles from healthy subjects administered Ambisome® was used to estimate the in vivo release rate constant of drug from the formulation in order to compare it with the in vitro release profiles. With the estimated in vivo and in vitro release rate constants, release profiles were generated. Two approaches were followed: comparison of in vivo and in vitro release rate constants and comparison of the area under the percent release-time curve from observed in vitro release data and simulated in vivo release data. Albumin was found to be most critical factor for the release of the drug by having a negative effect on the amount of Amphotericin B released. The release profiles obtained with the sample and separate method in both Krebs Ringer buffer- and Phosphate Saline buffer - albumin 4.0% w/v were predictive of the in vivo release profiles in healthy subjects. Determining the factors affecting drug release from parenteral formulations and relating the release profiles to a pharmacokinetic parameter in vivo supports the development of in vitro in vivo relations for parenteral products

    Investigation of an 11 mm diameter twin screw granulator: Screw element performance and in-line monitoring via image analysis

    As twin screw granulation (TSG) provides one with many screw element options, characterization of each screw element is crucial in optimizing the screw configuration in order to obtain desired granule attributes. In this study, the performance of two different screw elements - distributive feed screws and kneading elements - was studied in an 11 mm TSG at different liquid-to-solid (L/S) ratios. The kneading element configuration was found to break large granules more efficiently, leading to narrower granule size distributions. While pharmaceutical industry shifts toward continuous manufacturing, inline monitoring and process control are gaining importance. Granules from an 11 mm TSG were analysed using the Eyecon™, a real-time high speed direct imaging system, which has been used to capture accurate particle size distribution and particle count. The size parameters and particle count were then assessed in terms of their ability to be a suitable control measure using the Shewhart control charts. d10 and particle count were found to be good indicators of the change in L/S ratio. However, d50 and d90 did not reflect the change, due to their inherent variability even when the process is at steady state

    Physicians' guideline adherence is associated with long-term heart failure mortality in outpatients with heart failure with reduced ejection fraction: the QUALIFY international registry

    Background: Physicians' adherence to guideline-recommended therapy is associated with short-term clinical outcomes in heart failure (HF) with reduced ejection fraction (HFrEF). However, its impact on longer-term outcomes is poorly documented. Here, we present results from the 18-month follow-up of the QUALIFY registry. Methods and results: Data at 18 months were available for 6118 ambulatory HFrEF patients from this international prospective observational survey. Adherence was measured as a continuous variable, ranging from 0 to 1, and was assessed for five classes of recommended HF medications and dosages. Most deaths were cardiovascular (CV) (228/394) and HF-related (191/394) and the same was true for unplanned hospitalizations (1175 CV and 861 HF-related hospitalizations, out of a total of 1541). According to univariable analysis, CV and HF deaths were significantly associated with physician adherence to guidelines. In multivariable analysis, HF death was associated with adherence level [subdistribution hazard ratio (SHR) 0.93, 95% confidence interval (CI) 0.87–0.99 per 0.1 unit adherence level increase; P = 0.034] as was composite of HF hospitalization or CV death (SHR 0.97, 95% CI 0.94–0.99 per 0.1 unit adherence level increase; P = 0.043), whereas unplanned all-cause, CV or HF hospitalizations were not (all-cause: SHR 0.99, 95% CI 0.9–1.02; CV: SHR 0.98, 95% CI 0.96–1.01; and HF: SHR 0.99, 95% CI 0.96–1.02 per 0.1 unit change in adherence score; P = 0.52, P = 0.2, and P = 0.4, respectively). Conclusion: These results suggest that physicians' adherence to guideline-recommended HF therapies is associated with improved outcomes in HFrEF. Practical strategies should be established to improve physicians' adherence to guidelines. © 2019 The Authors. European Journal of Heart Failure © 2019 European Society of Cardiolog
