    Recurrent anal fistulae: Limited surgery supported by stem cells

    AIM: To study the results of stem-cell therapy under a Compassionate-use Program for patients with recurrent anal fistulae. METHODS: Under controlled circumstances, and approved by European and Spanish laws, a Compassionate-use Program allows the use of stem-cell therapy for patients with very complex anal fistulae. Candidates had previously undergone multiple surgical interventions that had failed to resolve the fistulae, and presented symptomatic recurrence. The intervention consisted of limited surgery (with closure of the internal opening), followed by local implant of stem cells in the fistula- tract wall. Autologous expanded adipose-derived stem cells were the main cell type selected for implant. The first evaluation was performed on the 8th postoperative week; outcome was classified as response or partial response. Evaluation one year after the intervention confirmed if complete healing of the fistula was achieved. RESULTS: Ten patients (8 male) with highly recurrent and complex fistulae were treated (mean age: 49 years, range: 28-76 years). Seven cases were non- Crohn’s fistulae, and three were Crohn’s-associated fistulae. Previous surgical attempts ranged from 3 to 12. Two patients presented with preoperative incontinence (Wexner scores of 12 and 13 points). After the intervention, six patients showed clinical response on the 8th postoperative week, with a complete cessation of suppuration from the fistula. Three patients presented a partial response, with an evident decrease in suppuration. A year later, six patients (60%) remained healed, with complete reepithelization of the external opening. Postoperative Wexner Scores were 0 in six cases. The two patients with previous incontinence improved their scores from 12 to 8 points and from 13 to 5 points. No adverse reactions or complications related to stem-cell therapy were reported during the study period. CONCLUSION: Stem cells are safe and useful for treating anal fistulae.     Using one-dimensional modeling to analyze the influence of the use of biodiesels on the dynamic behavior of solenoid-operated injectors in common rail systems: Results of the simulations and discussion

    The influence of using biodiesel fuels on the hydraulic behavior of a solenoid operated common rail injection system has been explored by means of a one-dimensional model. This model has been previously obtained, including a complete characterization of the different components of the injector (mainly the nozzle, the injector holder and the electrovalve), and extensively validated by means of mass flow rate results under different conditions. After that, both single and multiple injection strategies have been analyzed, using a standard diesel fuel and rapeseed methyl ester (RME) as working fluids. Single long injections allowed the characterization of the hydraulic delay of the injector, the needle dynamics and the discharge capability of the couple injector-nozzle for the two fuels considered. Meanwhile, the effect of biodiesel on main plus post injection strategies has been evaluated in several aspects, such as the separation of the two injections or the effect of the main injection on the post injection fueling.     Na+-dependent D-mannose transport at the apical membrane of rat small intestine and kidney cortex

    The presence of a Na+/D-mannose cotransport activity in brush-border membrane vesicles (BBMV), isolated from either rat small intestine or rat kidney cortex, is examined. In the presence of an electrochemical Na+ gradient, but not in its absence, D-mannose was transiently accumulated by the BBMV. D-Mannose uptake into the BBMV was energized by both the electrical membrane potential and the Na+ chemical gradient. D-Mannose transport vs. external D-mannose concentration can be described by an equation that represents a superposition of a saturable component and another component that cannot be saturated up to 50 μM D-mannose. D-Mannose uptake was inhibited by D-mannose ≫ D-glucose > phlorizin, whereas for α-methyl glucopyranoside the order was D-glucose = phlorizin ≫ D-mannose. The initial rate of D-mannose uptake increased as the extravesicular Na+ concentration increased, with a Hill coefficient of 1, suggesting that the Na+ :D-mannose cotransport stoichiometry is 1:1. It is concluded that both rat intestinal and renal apical membrane have a concentrative, saturable, electrogenic and Na+-dependent D-mannose transport mechanism, which is different from SGLT1.Dirección General de Investigaciones Científicas y Técnicas PM99-012

    Self-efficacy and sports injuries: is this a risk or a protective factor?

    The present paper analyses the effect of self-efficacy in sports injury and to what extent such effect is mediated by coping strategies to competitive situations shown by athletes. 245 sportspeople were assessed regarding the number of injuries suffered the last 12 months, General selfefficacy and coping strategies. regression analysis proved that two coping strategies, emotional calming and risk behavior, predicted the suffering of less or more injuries, respectively. A path analysis showed that self-efficacy was not directly related to the number of injuries but indirectly through coping strategies. Unexpectedly, the greater self-efficacy induces both coping strategies oriented to trying new options and risky alternatives (risk behavior) and coping strategies directed to control negative emotions (emotional calming) which would supposedly be related to least risk-taking behaviors, the former increasing and the later decreasing vulnerability to sports injuryEl presente estudio analiza el efecto de la autoeficacia en la lesión deportiva y en qué medida ese efecto se ve mediado por las estrategias de afrontamiento ante la situación competitiva que los deportistas exhiban. Se evaluaron el número de lesiones en los últimos 12 meses, la autoeficacia general y las estrategias de afrontamiento de 245 deportistas. Un análisis de regresión mostró que dos de las estrategias de afrontamiento: calma emocional y tendencia al riesgo, predecían el padecimiento de un menor o mayor número de lesiones, respectivamente. Un path analysis mostró que la autoeficacia no incidía directamente en el número de lesiones sino que las estrategias de afrontamiento mediaban esa relación. Contra lo esperado, una mayor autoeficacia induce tanto estrategias que llevan a intentar nuevas alternativas y opciones arriesgadas (conductas de riesgo) como otras que intentan controlar las emociones negativas (calma emocional) que, supuestamente, estarían relacionadas con menores conductas de riesgo. En el primer caso incrementando y en el segundo decrementando la vulnerabilidad a las lesionesO presente estudo analisa o efeito da autoeficacia na lesão esportiva e em que medida este efeito é mediado por estratégias de enfrentamento à situação de concorrência que os atletas apresentam. Foram avaliados o número de lesões nos últimos 12 meses, a auto-eficácia geral e as estratégias de enfrentamento de 245 atletas. A análise de regressão mostrou que duas estratégias de enfrentamento: calma emocional e tendência ao risco previu o sofrimento de um número menor ou maior de lesões, respectivamente. A path analysis mostrou que a auto-eficácia não influencia diretamente o número de lesionados, mas as estratégias de enfrentamento mediam essa relação. em contra as expectativas, uma maior auto-eficácia induz tanto estratégias levam a tentar novas alternativas e opções arriscadas (comportamentos de risco ) assim como outras que tentam controlar as emoções negativas (emotional calma) que, supostamente, estariam relacionados com comportamentos de menor risco.     Estructuras de gobierno y previsión sucesional en las compañías familiares españolas

    Family firms are the most common business formula in the Western world, being at the core of economic activity and supporting development and social cohesion. They are currently facing a double challenge: first, the professionalization of management, and second, addressing the succession process. In the present study, an analysis has been carried on the governing bodies that characterize this type of companies and their relation to the practices they are adopting with the aim of achieving continuity. The sample was formed by a total of 69 corporations in our country, belonging to all the Autonomous Communities. From a methodological point of view, data were obtained using an instrument that was specifically elaborated for this study, assorted documents provided by the companies, and personal interviews with their main agents. Following the results of the statistical analyses performed on the recorded data, a set of recommendations for family companies are provided.Las firmas familiares constituyen la fórmula empresarial más usual en el entorno occidental; suponen el núcleo de la actividad económica y el soporte de su desarrollo y cohesión social. En la actualidad se enfrentan a un doble reto: profesionalizar la gestión y, en segundo lugar, afrontar el proceso sucesorio. En este trabajo de investigación se han analizado los órganos de gobierno de estas compañías y su relación con las prácticas que están adoptando en orden a lograr esta continuidad. Se ha trabajado con una muestra de 69 corporaciones de nuestro país, radicadas en todas las Comunidades Autónomas; desde un punto de vista de procedimiento metodológico, los datos fueron obtenidos a través de un instrumento –creado ad hoc– para este estudio, documentación de distinta naturaleza –aportada por las empresas– y entrevistas personales a sus actores principales. Realizado el análisis estadístico correspondiente, y con base en los datos obtenidos, se llevan a cabo un conjunto de recomendaciones para estas corporaciones
