    Avant les falaises ; suivi de L'apostrophe et la majuscule : écrire en héros

    « Le froid dans le dos », première partie du roman Avant les falaises, relate à la première personne les péripéties intérieures d'un jeune Québécois ayant quitté sa patrie pour s'établir dans une ville italienne coupée des lois de l'État. Poussé par le spectre de son frère jumeau, qu'il voit mener une bande d'homosexuels fanatiques vers cette ville qu'ils croient être leur terre promise, il s'établira dans la famille du maire lors de la tenue de célébrations en l'honneur de Leonardo da Vinci. Ainsi, il servira d'intermédiaire entre les bouleversements qui surviennent à San Leonardo et l'intimité de ses hôtes, dans laquelle il s'insère graduellement. Comme simple indication, je me permets d'ajouter que la seconde partie du roman, « Les mains à la pâte », raconte les actions posées par les fanatiques pour s'établir dans la ville qu'ils prennent pour leur terre d'élection, ainsi que les mesures prises par le maire pour se débarrasser d'eux. Tiraillés entre un passé taillé dans la pierre et un avenir qui s'amène en prophète et s'amuse à l'effriter, les Léonardiens se désunissent avec l'arrivée des deux frères et remettent en doute les principes qui font l'indépendance de leur ville. En filigrane s'élabore une relation entre le narrateur et le fils du maire, qui tentent d'échapper à la folie du monde extérieur. À l'intérieur du volet réflexif de ce mémoire, L'apostrophe et la majuscule, j'explore la figure du héros et les rapports qu'elle entretient avec l'écriture romanesque. La mort de Dieu, tel que proposée par la pensée nietzschéenne, aurait permis l'apparition du héros-narrateur dans le territoire romanesque, ce je dont l'agir n'est plus voué au monumental et au divin, mais à l'humain et à l'introspection. Cet héroïsme moderne, lié à la remise en question de l'agir dans le monde et à la quête de liberté, se construit dans un rapport à la filiation littéraire et au temps, de manière à résister à l'autarcie et à l'individualisme contemporains. Par la narration romanesque, l'écrivain se crée une communauté langagière dédiée à une mission éthique paradoxale: compléter le monde, en soutenir une fraction, sans toutefois l'achever. ______________________________________________________________________________ MOTS-CLÉS DE L’AUTEUR : Héros, Héroïsme, Roman, Narration, Liberté

    Critical Appraisal of C-Reactive Protein Throughout the Spectrum of Cardiovascular Disease

    Clinicians involved in the care of patients with cardiovascular conditions have recently been confronted with an important body of literature linking inflammation and cardiovascular disease. Indeed, the level of systemic inflammation as measured by circulating levels of C-reactive protein (CRP) has been linked to prognosis in patients with atherosclerotic disease, congestive heart failure, atrial fibrillation, myocarditis, aortic valve disease and heart transplantation. In addition, a number of basic science reports suggest an active role for CRP in the pathophysiology of cardiovascular diseases. This article explores the potential role of CRP in disease initiation, progression, and clinical manifestations and reviews its role in the prediction of future events in clinical practice. Therapeutic interventions to decrease circulating levels of CRP are also reviewed

    Beneficial effects of reconstituted high-density lipoprotein (rHDL) on circulating CD34+ cells in patients after an acute coronary syndrome

    Background: High-density lipoproteins (HDL) favorably affect endothelial progenitor cells (EPC). Circulating progenitor cell level and function are impaired in patients with acute coronary syndrome (ACS). This study investigates the short-term effects of reconstituted HDL (rHDL) on circulating progenitor cells in patients with ACS. Methods and Findings: The study population consisted of 33 patients with recent ACS: 20 patients from the ERASE trial (randomized to receive 4 weekly intravenous infusions of CSL-111 40 mg/kg or placebo) and 13 additional patients recruited as controls using the same enrolment criteria. Blood was collected from 16 rHDL (CSL-111)-treated patients and 17 controls at baseline and at 6–7 weeks (i.e. 2–3 weeks after the fourth infusion of CSL-111 in ERASE). CD34+ and CD34+/kinase insert domain receptor (KDR+) progenitor cell counts were analyzed by flow cytometry. We found preserved CD34+ cell counts in CSL-111-treated subjects at follow-up (change of 1.6%), while the number of CD34+ cells was reduced (-32.9%) in controls (p = 0.017 between groups). The level of circulating SDF-1 (stromal cell-derived factor-1), a chemokine involved in progenitor cell recruitment, increased significantly (change of 21.5%) in controls, while it remained unchanged in CSL-111-treated patients (p = 0.031 between groups). In vitro exposure to CSL-111 of early EPC isolated from healthy volunteers significantly increased CD34+ cells, reduced early EPC apoptosis and enhanced their migration capacity towards SDF-1. Conclusions: The relative increase in circulating CD34+ cells and the low SDF-1 levels observed following rHDL infusions in ACS patients point towards a role of rHDL in cardiovascular repair mechanisms

    Post-operative atrial fibrillation: a maze of mechanisms

    Post-operative atrial fibrillation (POAF) is one of the most frequent complications of cardiac surgery and an important predictor of patient morbidity as well as of prolonged hospitalization. It significantly increases costs for hospitalization. Insights into the pathophysiological factors causing POAF have been provided by both experimental and clinical investigations and show that POAF is ‘multi-factorial’. Facilitating factors in the mechanism of the arrhythmia can be classified as acute factors caused by the surgical intervention and chronic factors related to structural heart disease and ageing of the heart. Furthermore, some proarrhythmic mechanisms specifically occur in the setting of POAF. For example, inflammation and beta-adrenergic activation have been shown to play a prominent role in POAF, while these mechanisms are less important in non-surgical AF. More recently, it has been shown that atrial fibrosis and the presence of an electrophysiological substrate capable of maintaining AF also promote the arrhythmia, indicating that POAF has some proarrhythmic mechanisms in common with other forms of AF. The clinical setting of POAF offers numerous opportunities to study its mechanisms. During cardiac surgery, biopsies can be taken and detailed electrophysiological measurements can be performed. Furthermore, the specific time course of POAF, with the delayed onset and the transient character of the arrhythmia, also provides important insight into its mechanisms

    Aim High-density lipoproteins (HDLs) have several potentially protective vascular effects. Most clinical studies of therapies targeting HDL have failed to show benefits vs. placebo. Objective: To investigate the effects of an HDL-mimetic agent on atherosclerosis by intravascular ultrasonography (IVUS) and quantitative coronary angiography (QCA). Design and setting A prospective, double-blinded, randomized trial was conducted at 51 centres in the USA, the Netherlands, Canada, and France. Intravascular ultrasonography and QCA were performed to assess coronary atherosclerosis at baseline and 3 (2–5) weeks after the last study infusion. Patients Five hundred and seven patients were randomized; 417 and 461 had paired IVUS and QCA measurements, respectively. Intervention Patients were randomized to receive 6 weekly infusions of placebo, 3 mg/kg, 6 mg/kg, or 12 mg/kg CER-001. Main outcome measures The primary efficacy parameter was the nominal change in the total atheroma volume. Nominal changes in per cent atheroma volume on IVUS and coronary scores on QCA were also pre-specified endpoints. Results: The nominal change in the total atheroma volume (adjusted means) was −2.71, −3.13, −1.50, and −3.05 mm3 with placebo, CER-001 3 mg/kg, 6 mg/kg, and 12 mg/kg, respectively (primary analysis of 12 mg/kg vs. placebo: P = 0.81). There was also no difference among groups for the nominal change in per cent atheroma volume (0.02, −0.02, 0.01, and 0.19%; nominal P = 0.53 for 12 mg/kg vs. placebo). Change in the coronary artery score was −0.022, −0.036, −0.022, and −0.015 mm (nominal P = 0.25, 0.99, 0.55), and change in the cumulative coronary stenosis score was −0.51, 2.65, 0.71, and −0.77% (compared with placebo, nominal P = 0.85 for 12 mg/kg and nominal P = 0.01 for 3 mg/kg). The number of patients with major cardiovascular events was 10 (8.3%), 16 (13.3%), 17 (13.7%), and 12 (9.8%) in the four groups. Conclusion: CER-001 infusions did not reduce coronary atherosclerosis on IVUS and QCA when compared with placebo. Whether CER-001 administered in other regimens or to other populations could favourably affect atherosclerosis must await further study.
