    Motor Task Processing After Constraint- Induced Movement Therapy in Children With Cerebral Palsy: A Case Series

    Constraint-induced movement therapy (CIMT) has shown positive results in children with hemiplegic cerebral palsy (CP). However, studies on neural basis of such functional gains are limited. This study reports the event-related potential (ERP) changes in two children with hemiplegic CP after receiving CIMT for three weeks. Both cases were nine years old, had a diagnosis of left hemiplegic CP, had normal intelligence, and were able to extend the wrist at least 20° and the metacarpophalangeal joint at least 10° from full flexion. Before and after the three-week intervention, the children participated in ERP sessions with a choice reaction task to capture the changes in neural mechanism after intervention. Both children exhibited improvement in reaction time (RT) in both hand tasks after the intervention. The improvement was larger in the affected hand than the unaffected hand. Improved accuracy rate (AC) and shortened P300 latencies in the affected hand were also demonstrated in both cases. Topographical maps showed that in centro-parietal regions, patterns shifted from central and left-lateralized to more central and right-lateralized. CMIT was a useful method in improving upper limb function in our cases

    Effectiveness of a Multifaceted Cognitive Training Programme for People with Mild Cognitive Impairment: A One-Group Pre- and Posttest Design

    SummaryObjectivesThe effectiveness of a cognitive training programme in enhancing the functional abilities of elderly persons with mild cognitive impairments was tested in an integrated home and institutional training programme focused on performing daily tasks.MethodsTwenty elderly participants were taught cognitive stimulation and memory encoding strategies for 10 weeks by an occupational therapist, or by nonprofessionals and community caregivers. The programme consisted of attention and memory stimulation, association-based and imagery-based strategies. Functional assessment (Chinese version of the disability assessment for dementia instrument and the instrumental activities of daily living scale) and neuropsychological tests (digit span forward test, word list memory subtest of the neuropsychological test battery developed by the Consortium to Establish a Registry for Alzheimer's Disease, Cognistat) were administered before and after the programme.ResultsAfter the 10-week programme the participants showed significant improvements in average attention and memory. The participants showed improved memory (word list memory: p ≀ .001) and other cognitive function as measured by the naming (p ≀ .001), construction (p ≀ .001), memory (p ≀ .001) and similarities (p ≀ .001) subtests of the Cognistat.ConclusionThese results provide initial evidence supporting the use of daily tasks as the context in teaching cognitive stimulation and memory encoding strategies to mildly impaired elderly people

    Clinical effectiveness of mindfulness-based music therapy on improving emotional regulation in blind older women: A randomized controlled trial

    BackgroundThis study aimed to investigate clinical effectiveness of a structured eight-week mindfulness-based music therapy (MBMT) program on improving mood regulation in older women with blindness. This investigation compared a MBMT group with a mindfulness intervention (MI) group and a control group.MethodsNinety-two older females with blindness from a residential setting in Hong Kong were recruited and randomly allocated to a MBMT (n=31), MI (n=30), or control (n=31) group. Psychological measurements regarding mood regulation and general mood states (namely, Difficulties in Emotion Regulation Scale [DERS], Geriatric Depression Scale [GDS], and Depression Anxiety Stress Scales-21), were taken at pretest and posttest. Outcome assessors were blinded to group assignment.ResultsData was analyzed based on intention-to-treat basis. At posttest, DERS scores in the MBMT group (mean differences and 95% confidence interval: 12.1, 5.5 to 18.8) and the MI group (7.2, 0.5 to 13.8) were lower than that in the control group. GDS scores in the MBMT group (2.9, 1.7 to 4.0) and the MI group (1.7, 0.6 to 2.9) were lower than those in the control group. Compared with the MI group, the MBMT group improved emotional awareness sub-scores in DERS (2.1, 0.2 to 4.1) and appeared to lower depression in GDS scores (1.1, -0.0 to 2.3; p = .053).ConclusionMBMT seems more beneficial than MI alone for improving emotional regulation in older women with blindness. The combination of mindfulness and music can generate a synergetic effect by enhancing both attention and appraisal components within the emotional-regulation process

    Multi-messenger Observations of a Binary Neutron Star Merger

    International audienceOn 2017 August 17 a binary neutron star coalescence candidate (later designated GW170817) with merger time 12:41:04 UTC was observed through gravitational waves by the Advanced LIGO and Advanced Virgo detectors. The Fermi Gamma-ray Burst Monitor independently detected a gamma-ray burst (GRB 170817A) with a time delay of ∌1.7 s\sim 1.7\,{\rm{s}} with respect to the merger time. From the gravitational-wave signal, the source was initially localized to a sky region of 31 deg(2) at a luminosity distance of 40−8+8{40}_{-8}^{+8} Mpc and with component masses consistent with neutron stars. The component masses were later measured to be in the range 0.86 to 2.26  M⊙\,{M}_{\odot }. An extensive observing campaign was launched across the electromagnetic spectrum leading to the discovery of a bright optical transient (SSS17a, now with the IAU identification of AT 2017gfo) in NGC 4993 (at ∌40 Mpc\sim 40\,{\rm{Mpc}}) less than 11 hours after the merger by the One-Meter, Two Hemisphere (1M2H) team using the 1 m Swope Telescope. The optical transient was independently detected by multiple teams within an hour. Subsequent observations targeted the object and its environment. Early ultraviolet observations revealed a blue transient that faded within 48 hours. Optical and infrared observations showed a redward evolution over ∌10 days. Following early non-detections, X-ray and radio emission were discovered at the transient’s position ∌9\sim 9 and ∌16\sim 16 days, respectively, after the merger. Both the X-ray and radio emission likely arise from a physical process that is distinct from the one that generates the UV/optical/near-infrared emission. No ultra-high-energy gamma-rays and no neutrino candidates consistent with the source were found in follow-up searches. These observations support the hypothesis that GW170817 was produced by the merger of two neutron stars in NGC 4993 followed by a short gamma-ray burst (GRB 170817A) and a kilonova/macronova powered by the radioactive decay of r-process nuclei synthesized in the ejecta
