    Beyond “Making Poland Great Again.” Nostalgia in Polish Populist and Non-populist Discourses

    Nostalgia can be triggered not only by personal recollections but also by exposure to narratives or images evoking desirable pasts, more or less fictional, and inducing feelings of longing for them. We analyze the institutional and semiotic machinery involved in the cultural construction of nostalgia in Poland and its role in generating sentiments that predispose people to support populist—particularly right-wing populist—ideology. While various political options use semiotic vehicles of nostalgia designed to engender nostalgic sentiments, we argue that there are systematic differences of content and form between politically useful invocations of nostalgia, related to the ideology of their proponents. We further posit that right-wing populist nostalgia invokes not only national greatness but also national innocence. We also identify specific features of such nostalgic discourses. The empirical material for our four cases ranges from textual and film narratives, through images and performances, to large-scale exhibitions and museal projects. To analyze their content and determine the form of politicization we build upon the conceptual apparatus developed by Svetlana Boym (reflective versus restorative nostalgia), Jan Assmann (communicative versus cultural memory), and Northrop Frye (low mimetic versus high mimetic narrative forms).     Commentary: Integrating electrodermal biofeedback into pharmacologic treatment of grand mal seizures

    Thirty percent of patients with epilepsy experience seizures despite optimal anticonvulsant drug treatment. Stress is frequently identified by patients with epilepsy as a precipitant of seizures (Spector et al., 2000; Ferlisi and Shorvon, 2014). Patients also often report using countermeasures to control the seizure onset. These are typically spontaneous and individualized such as increasing arousal by walking, breathing, standing, focusing attention, changing way of thinking, and more rarely inducing relaxation (Lee and No, 2005; Hether et al., 2013). In parallel, behavioral and psychological interventions, complementing conventional therapeutic methods for the management of epileptic seizures, have gained greater clinical attention over the past decade. Among these, Biofeedback (BFK) represents a noninvasive biobehavioral treatment that enables a patient to gain volitional control over a specific physiological process. BFK has already shown its value when applied to patients with epilepsy (Sterman and Friar, 1972; Rockstroh et al., 1993; Nagai et al., 2004a; Nagai, 2011; Micoulaud-Franchi et al., 2014a,b). Scrimali et al. (2015) have rightly pointed out the potential usefulness of electrodermal biofeedback in the management of refractory epilepsy. In a single case study, they report an effect of electrodermal activity (EDA) relaxation biofeedback in reducing seizures in a patient treated for 2 years. This case study supports the necessity to expand clinical armamentarium for treatment-resistant patients with few alternatives

    W ciągu ostatnich kilkunastu lat wykazano, że komórki tłuszczowe wydzielają wiele związków o charakterze hormonalnym, z których najlepiej poznano właściwości leptyny, adiponektyny, rezystyny i adipsyny. Obok znanych stymulatorów funkcji adipocytów, takich jak noradrenalina, insulina i glukagon, na komórki te oddziaływają też hormony tarczycy i steroidowe, hormon wzrostu, prostaglandyny, czynniki wzrostowe (IGF-1, EGF) i cytokiny (TNF, IL-6). Nowe dane pozwalają na lepsze zrozumienie patomechanizmów powstawania otyłości. U osób w wieku podeszłym otyłość można zdiagnozować za pomocą prostych metod klinicznych. Intensywne działania oświatowe umożliwiają osiągnięcie umiarkowanej redukcji masy ciała. Jak dotąd brakuje wystarczających danych, aby ocenić wartość interwencji farmakologicznej lub chirurgicznej. Na redukcji masy ciała najbardziej skorzystają osoby o znacznym ryzyku sercowo-naczyniowym. Możliwe jest też uzyskanie pewnej poprawy funkcji ruchowych. Leczenie otyłości w tej grupie wiekowej powinno obejmować zapobieganie utracie masy kości

    Galvanic skin response (GSR)/electrodermal/skin conductance biofeedback on epilepsy: a systematic review and meta-analysis

    Objectives: Dynamic changes in psychophysiological arousal are directly expressed in the sympathetic innervation of the skin. This activity can be measured as tonic and phasic fluctuations in electrodermal activity [Galvanic Skin Response (GSR)/skin conductance]. Biofeedback training can enable an individual to gain voluntary control over this autonomic response and its central correlates. Theoretically, control of psychophysiological arousal may be harnessed as a therapy for epilepsy, to mitigate pre-ictal states. Evidence is accumulating for the clinical efficacy of GSR biofeedback training in the management of drug resistant epilepsy. In this review, we analyse current evidence of efficacy with GSR biofeedback and evaluate the methodology of each study. Method: We searched published literature pertaining to interventional studies of GSR biofeedback for epilepsy, through MEDLINE and Cochrane databases (1950–2018). Using percentage seizure reduction as an indicator of therapeutic efficacy induced by GSR biofeedback, we used meta-analytic methods to summarize extant findings. We also compare and contrast study design with relevance to the interpretation of outcomes. Results: Out of 21 articles retrieved for GSR/EDA/Skin conductance biofeedback, four studies were identified as interventional trials, involving 99 patients with drug-resistant epilepsy in total. Three of these studies included a control group and a positive therapeutic effect of biofeedback was reported in each of these. The difference in seizure frequency percentage (Biofeedback—Control) was between −54.4 and −74.0% with an overall weighted mean difference of −64.3% (95% CI: −85.4 to −43.2%). The response rates (proportion of patients manifesting >50% reduction in seizure frequency) varied from 45 to 66% across studies. Significance: This timely evaluation highlights the potential value of GSR biofeedback therapy, and informs the optimal study design of larger scale studies that are now required to more definitively establish the utility of this non-invasive, non-pharmacological interventional approach for drug-resistant epilepsy

    Social competence of physicians and medical students – a preliminary report

    Background . Efficient functioning at work and in the environment depends on social and emotional competence, understood as complex skills that determine the effectiveness of behaviors in various professional and social situations. Objectives. The aim of this study was to determine the social competence of physicians and medical students with regard to the sociodemographic contributors which shape social competence. Material and methods . The study was conducted in 2015 and 2016 and it involved 90 physicians, including 25 GPs (27.8%) and 53 medical students of PMU in Szczecin. The median age of the physicians and the students was 32 and 25, respectively. The Social Competencies Questionnaire (SC Q) by Anna Matczak and a self-developed survey questionnaire were employed. Results. The ability of physicians to achieve medium and high levels of social competence increases by 8.5% with every year of seniority. Membership in scientific societies increases the odds of a high level of social competence fivefold in the ES scale and fourfold in the A scale. Physicians involved in the education of medical students were less likely to obtain medium and high scores (5 stens) in the A scale. An increase in seniority in the last workplace is accompanied by a 0.93 times lower probability of obtaining a high competence score in the A scale. Similarly, third cycle degree studies increase the odds of achieving high competence level by 7.48 times in the A scale. Conclusions . Low levels of competence can be expected from physicians with less seniority, not belonging to scientific societies, not involved in students’ education, working in only one place, and not participating in third cycle degree studies. This group should be provided with social competence training

    Epileptic Seizures are Reduced by Autonomic Biofeedback Therapy Through Enhancement of Fronto-limbic Connectivity: A Controlled Trial and Neuroimaging Study.

    BACKGROUND: Thirty-percent of patients with epilepsy are drug-resistant, and might benefit from effective noninvasive therapeutic interventions. Evidence is accumulating on the efficacy of autonomic biofeedback therapy using galvanic skin response (GSR; an index of sympathetic arousal) in treating epileptic seizures. This study aimed to extend previous controlled clinical trials of autonomic biofeedback therapy with a larger homogeneous sample of patients with temporal lobe epilepsy. In addition, we used neuroimaging to characterize neural mechanisms of change in seizure frequency following the therapy. METHODS: Forty patients with drug-resistant temporal lobe epilepsy (TLE) (age: 18 to 70years old), on stable doses of anti-epileptic medication, were recruited into a controlled and parallel-group trial from three screening centers in the UK. Patients were allocated to either an active intervention group, who received therapy with GSR biofeedback, or a control group, who received treatment as usual. Allocation to the group was informed, in part, by whether patients could travel to attend repeated therapy sessions (non-randomized). Measurement of outcomes was undertaken by an assessor blinded to the patients' group membership. Resting-state functional and structural MRI data were acquired before and after one month of therapy in the therapy group, and before and after a one-month interval in the control group. The percentage change of seizure frequency was the primary outcome measure. The analysis employed an intention-to-treat principle. The secondary outcome was the change in default mode network (DMN) and limbic network functional connectivity tested for effects of therapy. The trial was registered with the National Institute for Health Research (NIHR) portfolio (ID 15967). FINDINGS: Data were acquired between May 2014 and October 2016. Twenty participants were assigned to each group. Two patients in the control group dropped out before the second scan, leaving 18 control participants. There was a significant difference in reduction of seizure frequency between the therapy and control groups (p50%. Neuroimaging analysis revealed that post-therapy seizure reduction was linearly correlated with enhanced functional connectivity between right amygdala and both the orbitofrontal cortex (OFC) and frontal pole (FP). INTERPRETATION: Our clinical study provides evidence for autonomic biofeedback therapy as an effective and potent behavioral intervention for patients with drug-resistant epilepsy.     Stężenie cynku w surowicy krwi kobiet w wieku postmenopauzalnym

    Introduction: Demographic facts and forecasts about lengthening life expectancy motivate to systematize the knowledge of health problems experienced by women at the age of 50 and older. It refers to the whole health policy including health economics. Longer female life spans cause that an increasing number of women suffer from health problems associated with the perimenopausal period, and become health care recipients. Also a shift of retirement age is the reason to take interdisciplinary actions for women’s health and quality of life. This study describes a decline in the levels of many bioelements in hair, urine and blood serum, which progresses with age. It not only correlates with a decrease in the synthesis and secretion of estrogen, but also environmental pollution, unhealthy lifestyle and the use of substances. Aim of the study: The aim of this study was to determine the correlation between serum zinc levels in postmenopausal women and such variables as the use of substances (cigarettes, alcohol) and menopausal hormone therapy (MHT). Material and method: The study was conducted among 152 healthy women being 1-16 years after menopause. The women were divided into study group (MHT users) and control group (MHT non-users). A sub-division criterionwas the use of substances (cigarettes, alcohol). Serum zinc levels were determined in all women. Results: The use of substances significantly contributed to the lowering of serum zinc levels in postmenopausal women. MHT users had statistically higher average zinc levels in blood serum, which referred both to smokers and consumers of alcohol and those who did not use these substances. Conclusions: 1. The use of substances (cigarettes, alcohol) contributes to the lowering of zinc levels in blood serum. 2. MHT positively affects serum zinc levels in postmenopausal women regardless of whether they use substances (cigarettes, alcohol) or not.Wstęp: Fakty i prognozy demograficzne dotyczące stałego trendu wydłużania się średniej długości życia, stają się pilnym postulatem do opracowywania oraz usystematyzowania problemów zdrowotnych kobiet w wieku 50+. Dotyczy to całokształtu polityki zdrowotnej, także w aspekcie ekonomiki zdrowia. Wydłużanie czasu życia kobiet sprawia, że coraz liczniejsza ich populacja doświadcza problemów zdrowotnych związanych ze zjawiskami okresu około-menopauzalnego, co odzwierciedla się dynamiką wzrostu korzystania z opieki zdrowotnej. Opisywane przez Autorów procesy obniżania się stężenia wielu biopierwiastków we włosach, moczu i w surowicy krwi postępują wraz z wiekiem. Zmiany te korelują nie tylko z obniżeniem syntezy i sekrecji estrogenu, ale także zatruciem środowiska oraz niewłaściwym stylem życia, w którym pierwszoplanowym problemem jest rozpowszechnienie stosowania używek. Cel pracy: Określenie zależności pomiędzy stosowaniem używek (papierosy, alkohol) a stężeniem cynku w surowicy krwi u kobiet po menopauzie, oraz wpływu menopauzalnej terapii hormonalnej (MTH) na to stężenie. Materiał i metoda: Badanie przeprowadzone zostało wśród 152 zdrowych kobiet w wieku od 1 roku do 16 lat po menopauzie. Badane podzielono w zależności od tego czy paliły papierosy lub/i spożywały alkohol. U wszystkich oznaczono stężenie cynku w surowicy krwi. Kryterium podziału stanowiło także stosowanie, bądź nie MTH. Wyniki: Stosowanie używek wpływało istotnie na obniżenie stężenia cynku w surowicy krwi kobiet po menopauzie. Kobiety stosujące MTH miały statystycznie wyższe średnie stężenie cynku w surowicy krwi, zarówno te, które paliły papierosy i piły alkohol jak i te, które nie stosowały tych używek. Wnioski: 1. Intensyfikacja edukacji zdrowotnej, mającej na celu zaniechanie stosowania używek może wpłynąć korzystnie na stężenie cynku w surowicy krwi kobiet w wieku postmenopauzalnym, co za tym idzie poprawę ich stanu zdrowia. 2. Upowszechnienie stosowania MHT wpływać może korzystnie na stężenie cynku w surowicy krwi u kobiet po menopauzie zarówno u kobiet stosujących jak i nie stosujących używki (papierosy, alkohol)

    Self-control of epileptic seizures by nonpharmacological strategies

    Despite the unpredictability of epileptic seizures, many patients report that they can anticipate seizure occurrence. Using certain alert symptoms (i.e., auras, prodromes, precipitant factors), patients can adopt behaviors to avoid injury during and after the seizure or may implement spontaneous cognitive and emotional strategies to try to control the seizure itself. From the patient's view point, potential means of enhancing seizure prediction and developing seizure control supports are seen as very important issues, especially when the epilepsy is drug-resistant. In this review, we first describe how some patients anticipate their seizures and whether this is effective in terms of seizure prediction. Secondly, we examine how these anticipatory elements might help patients to prevent or control their seizures and how the patient's neuropsychological profile, specifically parameters of perceived self-control (PSC) and locus of control (LOC), might impact these strategies and quality of life (QOL). Thirdly, we review the external supports that can help patients to better predict seizures. Finally, we look at nonpharmacological means of increasing perceived self-control and achieving potential reduction of seizure frequency (i.e., stress-based and arousal-based strategies). In the past few years, various approaches for detection and control of seizures have gained greater interest, but more research is needed to confirm a positive effect on seizure frequency as well as on QOL
