    Small Footprint Multilayered Millimeter-Wave Antennas and Feeding Networks for Multi-Dimensional Scanning and High-Density Integrated Systems

    This paper overviews the state-of-the-art of substrate integrated waveguide (SIW) techniques in the design and realization of innovative low-cost, low-profile and low-loss (L3) millimeter-wave antenna elements, feeding networks and arrays for various wireless applications. Novel classes of multilayered antenna structures and systems are proposed and studied to exploit the vertical dimension of planar structures to overcome certain limita-tions in standard two-dimensional (2-D) topologies. The developed structures are based on two techniques, namely multi-layer stacked structures and E-plane corners. Differ-ent E-plane structures realised with SIW waveguide are presented, thereby demonstrating the potential of the proposed techniques as in multi-polarization antenna feeding. An array of 128 elements shows low SLL and height gain with just 200g of the total weight. Two versions of 2-D scanning multi-beam are presented, which effectively combine frequency scanning with beam forming networks. Adding the benefits of wide band performance to the multilayer structure, two bi-layer structures are investigated. Different stacked antennas and arrays are demonstrated to optimise the targeted antenna performances in the smallest footprint possible. These structures meet the requirement for developing inexpensive compact millimeter-wave antennas and antenna systems. Different structures and architectures are theoretically and experimentally studied and discussed for specific space- and ground-based appli-cations. Practical issues such as high-density integration and high-volume manufacturability are also addressed

    The sealing ability of biodentine and MTA as a root sealer in the management of open apices of permanent teeth

    Introduction: The root seal should provide an impermeable seal in different environments to prevent the egress of bacteria from the canal into the peri-radicular tissues and the ingress of periradicular fluid into the canal. Aim: The aim of this pilot study is to assess, by means of an in-vitro investigation using micro-CT and an optical microscope, the quality of the root apical seal achieved with either MTA® or Biodentine™ when placed in a moist environment that simulates the various clinical periapical wet environments. Materials and methods: A total of thirty-six freshly extracted human teeth were randomly allocated to 2 groups: MTA® and Biodentine™. Each group was subdivided into 3 subgroups containing 6 teeth each. Materials insertion and packing occurred while the teeth were immersed in the environmental fluids (Dry, SBF and Acid), following the standard apical divergence and instrumentation. Then 3 mm of the materials were scanned and analysed using the micro-CT scan (MCT) and an optical microscope was used to investigate the integrity of the root-apex at the surface interface seal. Results: The mean porosity percentage of MTA® and Biodentine™ in the 3 different environments; Dry: 24.08% and 45.42%, SBF: 38.28% and 56.03%, Acid: 46.78% and 50.43% subsequently. There was not any statistically significant difference between the three environments at a P-value=0.16. Conclusion: Moisture and acidic environment do not have a statistically significant effect on the sealing ability of both materials MTA® and Biodentine™. But they generate morphological changes in both materials

Logarithmic derivatives of densities for the solution process are studied, and the Bismut-Elworthy-Li type formulae can be obtained under the uniformly elliptic condition on the coefficients of the diffusion and jump terms.     Critical care nurses' experiences during the Illness of family members : a qualitative study

    Introduction: A loved one's hospitalization in a critical care unit is a traumatic experience for families. However, because of their status and professional competence, a family member who is also a critical care nurse has additional obstacles and often long-term consequences. Objectives: To describe the experiences of critical care nurse-family members when a loved one is admitted to a critical care unit at the Hotel-Dieu de France hospital. Methods: A qualitative path based on van Manen's hermeneutic phenomenology combining both descriptive and interpretive models were adopted. Results: The lived experience of critical care nurses in providing care for their family members admitted into the same critical care were summarized in five themes. Nurses were torn between roles, consisting of confounding roles, their registered nurse status, and watchfulness. The lived experience of critical care nurses in providing care for their family members admitted into the same critical care was summarized into specialized knowledge that included a double-edged sword of seeking information and difficulty delivering the information. Critical nurses compete for expectations, including those placed on self and family members, resulting in emotional and personal sacrifice while gaining insight into the experiences. Conclusions: Critical care nurse-family members have a unique experience compared to the rest of the family, necessitating specialized care and attention. Increased awareness among healthcare providers could be a start in the right direction

    A geospatial framework to support integrated biogeochemical modelling in the United Kingdom

    Anthropogenic impacts on the aquatic environment, especially in the context of nutrients, provide a major challenge for water resource management. The heterogeneous nature of policy relevant management units (e.g. catchments), in terms of environmental controls on nutrient source and transport, leads to the need for holistic management. However, current strategies are limited by current understanding and knowledge that is transferable between spatial scales and landscape typologies. This study presents a spatially-explicit framework to support the modelling of nutrients from land to water, encompassing environmental and spatial complexities. The framework recognises nine homogeneous landscape units, distinct in terms of sensitivity of nutrient losses to waterbodies. The functionality of the framework is demonstrated by supporting an exemplar nutrient model, applied within the Environmental Virtual Observatory pilot (EVOp) cloud cyber-infrastructure. We demonstrate scope for the use of the framework as a management decision support tool and for further development of integrated biogeochemical modelling

    Adherence to Drug-Refill Is a Useful Early Warning Indicator of Virologic and Immunologic Failure among HIV Patients on First-Line ART in South Africa

    Affordable strategies to prevent treatment failure on first-line regimens among HIV patients are essential for the long-term success of antiretroviral therapy (ART) in sub-Saharan Africa. WHO recommends using routinely collected data such as adherence to drug-refill visits as early warning indicators. We examined the association between adherence to drug-refill visits and long-term virologic and immunologic failure among non-nucleoside reverse transcriptase inhibitor (NNRTI) recipients in South Africa.In 2008, 456 patients on NNRTI-based ART for a median of 44 months (range 12-99 months; 1,510 person-years) were enrolled in a retrospective cohort study in Soweto. Charts were reviewed for clinical characteristics before and during ART. Multivariable logistic regression and Kaplan-Meier survival analysis assessed associations with virologic (two repeated VL>50 copies/ml) and immunologic failure (as defined by WHO).After a median of 15 months on ART, 19% (n = 88) and 19% (n = 87) had failed virologically and immunologically respectively. A cumulative adherence of <95% to drug-refill visits was significantly associated with both virologic and immunologic failure (p<0.01). In the final multivariable model, risk factors for virologic failure were incomplete adherence (OR 2.8, 95%CI 1.2-6.7), and previous exposure to single-dose nevirapine or any other antiretrovirals (adj. OR 2.1, 95%CI 1.2-3.9), adjusted for age and sex. In Kaplan-Meier analysis, the virologic failure rate by month 48 was 19% vs. 37% among adherent and non-adherent patients respectively (logrank p value = 0.02).One in five failed virologically after a median of 15 months on ART. Adherence to drug-refill visits works as an early warning indicator for both virologic and immunologic failure

    In Vitro Innovation of Tendon Tissue Engineering Strategies.

    Tendinopathy is the term used to refer to tendon disorders. Spontaneous adult tendon healing results in scar tissue formation and fibrosis with suboptimal biomechanical properties, often resulting in poor and painful mobility. The biomechanical properties of the tissue are negatively affected. Adult tendons have a limited natural healing capacity, and often respond poorly to current treatments that frequently are focused on exercise, drug delivery, and surgical procedures. Therefore, it is of great importance to identify key molecular and cellular processes involved in the progression of tendinopathies to develop effective therapeutic strategies and drive the tissue toward regeneration. To treat tendon diseases and support tendon regeneration, cell-based therapy as well as tissue engineering approaches are considered options, though none can yet be considered conclusive in their reproduction of a safe and successful long-term solution for full microarchitecture and biomechanical tissue recovery. In vitro differentiation techniques are not yet fully validated. This review aims to compare different available tendon in vitro differentiation strategies to clarify the state of art regarding the differentiation process
