    Real-Time MRI of Continent and Stress Incontinent Male Patients after Orthotopic Ileal Neobladder

    Introduction: The aim of this study was to correlate anatomic differences with continence status in male patients after cystoprostatectomy and ileal neobladder using real-time magnetic resonance imaging. Patients and Methods: Anatomic differences of 14 male patients (7 daytime continent and 7 stress incontinent) with ileal neobladder were determined by measuring the orthogonal distance of the bladder neck to the pubococcygeal line (PCL) to correlate anatomic differences with continence status. Results: The median distance of the bladder neck to PCL was +5.4 mm in continent patients before voiding whereas in incontinent patients it was +2 mm (p = 0.012). During the Valsalva maneuver, the median distance in continent patients was +4 and in incontinent patients -3 mm (p = 0.003). At the end of micturition, the median distance was +2.3 mm in continent patients and -12 mm in incontinent patients (p = 0.002). Conclusions: The bladder neck in incontinent patients showed more pronounced mobility in relation to the PCL during micturition and the Valsalva maneuver as compared to continent patients. In addition, the ileal neobladder was positioned significantly lower in the pelvis of incontinent patients. These preliminary results suggest that a stable bladder neck may be an important factor to reach full continence in patients with ileal neobladder. Copyright (C) 2011 S. Karger AG, Base

Here, we describe a mechanism how dendritic mRNAs are transported and subsequently translated at activated synapses. Furthermore, we present the players involved in the regulation of local dendritic translation upon neuronal stimulation and their molecular interplay that maintain local proteome homeostasis.     Measurements of |Vub| and |Vcb| from CLEO

    I report results from the CLEO collaboration on semileptonic B decays, highlighting measurements of the Cabibbo-Kobayashi-Maskawa matrix elements |Vub| and |Vcb|.     Patients' perception of surgical outcomes and quality of life after retroperitoneoscopic and open pyeloplasty

    PURPOSE To report postoperative health-related quality of life (HRQoL) and patients' subjective evaluations of open pyeloplasty (OP) and retroperitoneoscopic pyeloplasty (RP) and influences on preoperative counselling. METHODS 107 patients (age 16-80 years, mean 31.5) with symptomatic primary ureteropelvic junction obstruction who underwent OP (32) or RP (75) were evaluated prospectively. HRQoL was evaluated using Short Form 36 (SF-36) questionnaires with 1 year follow-up. Operative outcomes were evaluated using a self-designed questionnaire regarding cosmetic outcomes, objective postoperative/current pain, convalescence and return to work. RESULTS The mean operative time was 174.4 vs. 161.4 min for RP versus OP, respectively, without intraoperative complications/conversions. There was an advantage for RP--except for two domains--without significance in any of the eight SF-36 domain scores. An advantage favouring RP in all aspects of the second questionnaire with significance in four aspects (cosmetic results, scar length, pain and convalescence) was found. Five weeks postoperatively, 58.7% (RP) vs. 25.8% (OP) were fully convalescent compared to 87.0% (RP) vs. 71.0% (OP) at 8 weeks. Similarly, 58.7 vs. 45.1% returned work 5 weeks postoperatively while 93.5 vs. 74.2% did so after 8 weeks, respectively. The small sample size, more questions on satisfaction/regret and mixed design are the main study limitations. CONCLUSION RP provides the same functional results beside earlier convalescence, better HRQoL and patients' convenience with surgery, which favours its inclusion in preoperative counselling providing patients with realistic postoperative expectations

    Radiocarbon distributions in Southern Ocean dissolved and particulate organic matter

    Dissolved organic carbon (DOC) is the largest actively exchanging pool of organic carbon in the ocean, yet its sources and sinks are not well constrained. The average C-14 ages of DOC in the deep N. Atlantic and N. Pacific Oceans are 4,000 [Bauer et al., 1992; Druffel et al., 1992] and 6,000 years [Williams and Druffel, 1987], respectively, and represent the beginning and end of the deep ocean conveyor [Broecker, 1991]. Here we report that the deep Southern Ocean DOC has a C-14 age (5,600 y) much closer to that of the deep N. Pacific, but its concentration in seawater (41 +/- 2 mu M) is nearly equal to that of the deep N. Atlantic. The radiocarbon and concentration data indicate that most, but not all, deep DOC is transported conservatively with the ocean\u27s conveyor. A younger (post-bomb) source of DOC to the N. Atlantic is the most likely explanation for the large age difference we observe between deep DOC in the Atlantic and Southern Oceans. Other possibilities are a source of older DOC or a smaller microbial sink in the S. Ocean, or perhaps a possible slowdown of S. Ocean deep water formation during the past century [Broecker et at, 1999]

    Clostridium difficile ribotypes in Austria: a multicenter, hospital-based survey

    A prospective, noninterventional survey was conducted among Clostridium difficile positive patients identified in the time period of July until October 2012 in 18 hospitals distributed across all nine Austrian provinces. Participating hospitals were asked to send stool samples or isolates from ten successive patients with C.difficile infection to the National Clostridium difficile Reference Laboratory at the Austrian Agency for Health and Food Safety for PCR-ribotyping and in vitro susceptibility testing. A total of 171 eligible patients were identified, including 73 patients with toxin-positive stool specimens and 98 patients from which C. difficile isolates were provided. Of the 159 patients with known age, 127 (74.3 %) were 65 years or older, the median age was 76 years (range: 9–97 years), and the male to female ratio 2.2. Among these patients, 73 % had health care-associated and 20 % community-acquired C. difficile infection (indeterminable 7 %). The all-cause, 30-day mortality was 8.8 % (15/171). Stool samples yielded 46 different PCR-ribotypes, of which ribotypes 027 (20 %), 014 (15.8 %), 053 (10.5 %), 078 (5.3 %), and 002 (4.7 %) were the five most prevalent. Ribotype 027 was found only in the provinces Vienna, Burgenland, and Lower Austria. Severe outcome of C. difficile infection was found to be associated with ribotype 053 (prevalence ratio: 3.04; 95 % CI: 1.24, 7.44), not with the so-called hypervirulent ribotypes 027 and 078. All 027 and 053 isolates exhibited in vitro resistance against moxifloxacin. Fluoroquinolone use in the health care setting must be considered as a factor favoring the spread of these fluoroquinolone resistant C. difficile clones

    Live cell imaging reveals 3 '-UTR dependent mRNA sorting to synapses

    mRNA transport restricts translation to specific subcellular locations, which is the basis for many cellular functions. However, the precise process of mRNA sorting to synapses in neurons remains elusive. Here we use Rgs4 mRNA to investigate 3'-UTR-dependent transport by MS2 live-cell imaging. The majority of observed RNA granules display 3'-UTR independent bidirectional transport in dendrites. Importantly, the Rgs4 3'-UTR causes an anterograde transport bias, which requires the Staufen2 protein. Moreover, the 3'-UTR mediates dynamic, sustained mRNA recruitment to synapses. Visualization at high temporal resolution enables us to show mRNA patrolling dendrites, allowing transient interaction with multiple synapses, in agreement with the sushi-belt model. Modulation of neuronal activity by either chemical silencing or local glutamate uncaging regulates both the 3'-UTR-dependent transport bias and synaptic recruitment. This dynamic and reversible mRNA recruitment to active synapses would allow translation and synaptic remodeling in a spatially and temporally adaptive manner

    New particle formation in the Front Range of the Colorado Rocky Mountains

    New particle formation is of interest because of its influence on the properties of aerosol population, and due to the possible contribution of newly formed particles to cloud condensation nuclei. Currently no conclusive evidence exists as to the mechanism or mechanisms of nucleation and subsequent particle growth. However, nucleation rates exhibit a clear dependence on ambient sulphuric acid concentrations and particle growth is often attributed to the condensation of organic vapours. A detailed study of new particle formation in the Front Range of the Colorado Rocky Mountains is presented here. Gas and particle measurement data for 32 days was analyzed to identify event days, possible event days, and non-event days. A detailed analysis of nucleation and growth is provided for four days on which new particle formation was clearly observed. Evidence for the role of sesquiterpenes in new particle formation is presented
