Remission of central fever with morphine post traumatic brain injury

ABSTRACT : After a brain injury, raised temperature may be due to a regulated readjustment in the hypothalamic ‘set-point’ in response to inflammation. The purpose of this report is to mention possible implications related to temperature and homeostasis of morphine treatment in a patient with brain injury. During the month previous to her hospitalization in our city she was treated for fever with paracetamol and metamizol without results. After 31 days with similar results, we changed to morphine IV considering the possibility of treating pain and fever. This option was successful and afterwards we changed to fentanyl patches, keeping fever absent. After 100 days of hospitalization, the patient was discharged to her home

Genetic characterizations and epigenetic interference to better understand and fight occult Hepatitis B virus infection

Occulte Hepatitis B virus-infectie (OBI) wordt gekenmerkt door ondetecteerbare expressie van oppervlakteantigen en lage viremia. Mutaties, cellulaire remming, afwijking in eiwit X (HBx) en epigenetische modificaties in het virale genoom zijn enkele voorgestelde mechanismen om OBI te verklaren, maar er is beperkte kennis van de verschillende aspecten van deze virale biologie. In dit werk hebben we de klinische en genetische condities die aanwezig zijn in HBV genen onderzocht bij OBI patiënten die zijn voorgedragen voor levertransplantatie. De evaluatie omvatte leversziekte-etiologie in relatie tot OBI, genotypering, genoom-mutaties en -integratie. We hebben ook DNA methylering geëvalueerd. Als een aanpak om het virale genoom te inactiveren, hebben we door epigenetische bewerking methylering op het virale genoom geinduceerd met CRISPR/dCas9. Tenslotte hebben we HBx als potentiele epigenetische editor onderzocht. Uit onze resultaten blijkt dat OBI hoofdzakelijk in cryptogene leverziekte en hepatitis C infecties belangrijk is, en dat deze aandoening kan worden geactiveerd door verschillende omstandigheden die niet noodzakelijkerwijs door alle gevallen worden gedeeld. Door gebruik te maken van CRISPR/dCas9 voor gerichte methylering, hebben we ook aangetoond dat methylering de activiteit van specifieke virale promotors kan remmen en deze epigenetische bewerkingsstrategie kan niet alleen beschouwd worden als een onderzoeksinstrument, maar ook als een strategie om de virale replicatie te verminderen en de infectie te controleren. Ten slotte, hoewel HBx in staat is om activatie van een transcriptie-inactieve promotor te induceren, kan dit eiwit waarschijnlijk niet effectief ingezet worden in epigenetische editing

Evaluación de la visualización por endoscopia en la identificación de lesiones gástricas premalignas en una población colombiana con prueba de referencia histopatológica

ResumenObjetivosevaluar el rendimiento de la visualización directa por endoscopia en la identificación de lesiones gástricas premalignas, utilizando como prueba de referencia el examen histopatológico de muestras obtenidas por biopsia.Emplazamientoliga de Lucha Contra el Cáncer, de Bucaramanga, Colombia.Diseñoestudio de evaluación de tecnologías diagnósticas con muestreo transversal.Mediciones principalesse calculó la sensibilidad, la especificidad, el valor predictivo positivo, el valor predictivo negativo, el likelihood ratio positivo, el likelihood ratio negativo, el índice kappa y la prevalencia.Resultadosse estudió a 155 pacientes con una media de edad de 45,43±14,15 años y una relación varón:mujer aproximada de 2:1. La visualización por endoscopia tuvo una sensibilidad del 87,84%, una especificidad del 55,56%, un valor predictivo positivo del 64,36%, un valor predictivo negativo del 83,33%, un likelihood ratio positivo de 1,98, un likelihood ratio negativo de 0,22 y un índice kappa de 0,4272, en la identificación de lesiones preneoplásicas del estómago en una población con una prevalencia de la enfermedad del 47,74%.Conclusionesla visualización mediante endoscopia de la mucosa gastrointestinal permite una rápida y moderada identificación de lesiones preneoplásicas tempranas, lo que da la posibilidad de su diagnóstico histopatológico precoz.AbstractAimTo evaluate the performance of endoscopic visualization in the identification of premalignant gastric lesions, with histopathological examination of biopsy samples as a reference test, in Bucaramanga, Colombia.LocationLeague for the Fight Against Cancer, Bucaramanga, Colombia.DesignDiagnostic technology evaluation with cross-sectional sampling.MeasurementsWe calculate the sensitivity, specificity, positive predictive value, negative predictive value, likelihood ratio (+), likelihood ratio (−), kappa index and the prevalence rate kappa.ResultsA total of 155 patients were studied, with a mean age of 45.43 (14.15) years and an approximate male:female ratio of 2:1. The endoscopic visualization had a sensitivity of 87.84%, a specificity of 55.56%, a positive predictive value of 64.36%, a negative predictive value of 83.33%, a likelihood ratio+of 1.98, a likelihood ratio−de 0.22 and a kappa index of 0.4272, in the identification of preneoplastic lesions of the stomach in a population with a disease prevalence of 47.74%.ConclusionsThe endoscopic visualization of the gastrointestinal mucosa, allows a moderate and fast identification of early preneoplastic lesions, providing an opportunity of its early histopathological diagnosis

Effects of three-day serial sodium bicarbonate loading on performance and physiological parameters during a simulated basketball test in female university players

The aim of this study was to investigate the effect of 3-day serial sodium bicarbonate ingestion on repeated sprint and jump performance. Fifteen female university basketball players (23.3 ± 3.4 years; 173.1 ± 5.8 cm; 65.8 ± 6.3 kg; 23.6 ± 4.9% body fat) ingested 0.4 g/kg body mass of sodium bicarbonate or placebo for 3 days (split in three equal daily doses), before completing a simulated basketball exercise. Sprint and circuit times, jump heights, performance decrements, and gastrointestinal side effects were recorded during the test, and blood lactate concentration wasmeasured pre- and posttest. Sodiumbicarbonate supplementation led to significant decreases in mean sprint times (1.34 ± 0.23 vs. 1.70 ± 0.41 s, p = .008, 95% confidence intervals [−0.54, −0.10 s]) and mean circuit times (30.6 ± 2.0 vs. 31.3 ± 2.0 s, p = .044) and significantly greater mean jump height (26.8 [range 25.2–34.2] vs. 26.0 [range 25.6–33.6] cm, p = .013) compared with placebo. Performance decrement was significantly less for sprints with sodium bicarbonate compared with placebo (9.9 [range 3.4–37.0]% vs. 24.7 [range 4.1–61.3]%, p = .013), but not different for jumps (13.1 ± 4.5% vs. 12.5 ± 3.1%, p = .321) between conditions. No differences in gastrointestinal side effects were noted between conditions. Significantly greater postexercise blood lactate concentrations were measured in the sodium bicarbonate condition compared with the placebo condition (8.2 ± 2.8 vs. 6.6 ± 2.4 mmol/L, p = .010). This study is the first to show that serial loading of sodium bicarbonate is effective for basketball players to improve repeated sprint and jump performance during competition, or withstand greater training load during practice sessions without any gastrointestinal side effects

Molecular characterization of occult hepatitis B virus infection in patients with end-stage liver disease in Colombia.

ABSTARCT: Hepatitis B virus (HBV) occult infection (OBI) is a risk factor to be taken into account in transfusion, hemodialysis and organ transplantation. The aim of this study was to identify and characterize at the molecular level OBI cases in patients with end-stage liver disease. METHODS: Sixty-six liver samples were obtained from patients with diagnosis of end-stage liver disease submitted to liver transplantation in Medellin (North West, Colombia). Samples obtained from patients who were negative for the surface antigen of HBV (n = 50) were tested for viral DNA detection by nested PCR for ORFs S, C, and X and confirmed by Southern-Blot. OBI cases were analyzed by sequencing the viral genome to determine the genotype and mutations; additionally, viral genome integration events were examined by the Alu-PCR technique. RESULTS: In five cases out of 50 patients (10%) the criteria for OBI was confirmed. HBV genotype F (subgenotypes F1 and F3), genotype A and genotype D were characterized in liver samples. Three integration events in chromosomes 5q14.1, 16p13 and 20q12 affecting Receptor-type tyrosine-protein phosphatase T, Ras Protein Specific Guanine Nucleotide Releasing Factor 2, and the zinc finger 263 genes were identified in two OBI cases. Sequence analysis of the viral genome of the 5 OBI cases showed several punctual missense and nonsense mutations affecting ORFs S, P, Core and X. CONCLUSIONS: This is the first characterization of OBI in patients with end-stage liver disease in Colombia. The OBI cases were identified in patients with HCV infection or cryptogenic cirrhosis. The integration events (5q14.1, 16p13 and 20q12) described in this study have not been previously reported. Further studies are required to validate the role of mutations and integration events in OBI pathogenesis

De igual manera en el marco teórico se desarrolla contenidos que permite realizar un análisis concreto de la investigación, estando organizada de la siguiente forma: antecedentes, sustento teórico y finalmente el glosario de términos. El diseño de investigación que se empleo fue el descriptivo simple o de diagnóstico de una sola variable, tomándose como población y muestra a la totalidad de docentes, director y personal administrativo. Se realiza una encuesta a través del cuestionario a los mencionados anteriormente. Finalmente se concluye que el 64% de los profesores poseen una actitud regular frente a los factores que generan el clima institucional puesto que su incidencia directa es de vital importancia en las interacciones pedagógicas
