    Assessing hydromorphological and floristic patterns along a regulated Mediterranean river : The Serpis River (Spain)

    In the European context, several methodologies have been developed to assess the ecological status and, specifically, the hydromorphological status of running surface waters. Although these methodologies have been widely used, few studies have focused on hydrologically altered water bodies and the factors that may determine their status. In this study, the Serpis River was divided into 16 segments from the Beniarr'es dam (40 km from the river mouth) to the sea, all of which are affected by flow regulation, but with different severity. In each segment, an inventory of the flora was conducted, and hydromorphological indices (QBR, Riparian Habitat Quality Index; and IHF, River Habitat Index) were applied. The objectives of the study were (A) to identify relationships between floristic composition and QBR components and (B) between instream habitat characteristics and IHF components as well as (C) to determine the main factors controlling the floristic composition and riparian habitat quality (QBR) and the factors controlling instream habitat characteristics and heterogeneity (IHF). A cluster analysis allowed grouping sites according to their floristic composition and instream habitat characteristics, and non-metric multidimensional scaling (NMDS) was used to ordinate the sites, obtaining the biotic and instream habitat characteristics, as well as the QBR and IHF subindices, which better explained the spatial patterns. Finally, a canonical correspondence analysis (CCA) with predictor variables (geographical, hydrological, geomorphological and human pressures) indicated the main factors controlling the spatial patterns of the floristic composition, instreamhabitat characteristics, riparian habitat quality and instream habitat heterogeneity. A gradient of riparian and instream habitat quality was identified. Our results suggest that physical constraints (presence of a gorge) have protected sites from severe human impacts, resulting in good ecological quality, despite hydrological alteration. This area, where there is geomorphological control, could be potentially reclassified into a different ecotype because regular monitoring could be using incorrect references for index scores, and naturally high scores could be confused with recovery from hydrological alteration or other pressures. The sites with the worst quality were near the river mouth and were characterised by an artificial and highly variable flow regime (related to large autumnal floods and frequent human-induced periods of zero flow). This artificial flow variability as well as the presence of lateral structures in the river channel and geomorphological characteristics were identified as the main factors driving the hydromorphological and floristic pattern in this regulated river.Diversas metodologías han sido desarrolladas en el contexto europeo para evaluar el estado ecológico, y específicamente el estado hidromorfológico de las aguas superficiales. Aunque éstas han sido ampliamente utilizadas, pocos estudios se han centrado en masas de agua hidrológicamente alteradas y en los factores que condicionan su estado. En este estudio, el río Serpis fue dividido en 16 segmentos desde la presa de Beniarrés (a 40 km de la desembocadura) hasta el mar, todos ellos afectados por la regulación de caudales con distinta severidad. En cada segmento se realizó un inventario florístico y se aplicaron índices hidromorfológicos (QBR, Calidad del Bosque de Ribera, e IHF, Índice de Hábitat Fluvial). Los objetivos del estudio fueron (A) identificar relaciones entre la composición florística y los componentes del QBR, (B) entre las caracteríısticas del hábitat fluvial y los componentes del IHF, (C) determinar los principales factores que controlan la composición florística y la calidad del hábitat ripario (QBR), y las características del hábitat fluvial y su heterogeneidad (IHF). Un cluster permitió agrupar los puntos de muestreo según su composición florística y las características del hábitat fluvial, y un escalado multidimensional no-métrico (NMDS) fue usado para ordenar los puntos, obteniendo las variables bióticas y características del hábitat y los subindices del QBR e IHF, respectivamente, que explicaban mejor los patrones espaciales. Finalmente, un análisis de correspondencias canónicas (CCA) con variables predictoras (geográficas, hidrológicas, geomorfológicas y presiones humanas) indicó los principales factores que controlan los patrones espaciales de la composición florística, las características del hábitat fluvial, la calidad del hábitat ripario y la heterogeneidad del hábitat fluvial. Se identificó un gradiente de calidad del hábitat ripario y fluvial. Los resultados sugieren que las limitaciones físicas (presencia de un cañón) han protegido a los tramos de impactos humanos severos, resultando en una buena calidad ecológica a pesar de la alteración hidrológica. Esta zona podría potencialmente ser reclasificada en un ecotipo diferente, ya que un monitoreo regular podría estar usando referencias incorrectas para los índices y sus altas puntuaciones naturales se podrían estar confundiendo con una recuperación de la alteración hidrológica o de otras presiones. Los puntos de muestreo con peor calidad estuvieron cerca de la desembocadura y tuvieron un régimen de caudales alterado y muy variable. Esta variabilidad artificial del caudal, junto con la presencia de estructuras laterales en el cauce y las características geomorfológicas fueron identificadas como los principales factores determinantes del patrón hidromorfológico y florístico en este río regulado

    Bayesian reasoning with emotional material in patients with schizophrenia.

    Delusions are one of the most classical symptoms described in schizophrenia. However, despite delusions are often emotionally charged, they have been investigated using tasks involving non-affective material, such as the Beads task. In this study we compared 30 patients with schizophrenia experiencing delusions with 32 matched controls in their pattern of responses to two versions of the Beads task within a Bayesian framework. The two versions of the Beads task consisted of one emotional and one neutral, both with ratios of beads of 60:40 and 80:20, considered, respectively, as the “difficult” and “easy” variants of the task. Results indicate that patients showed a greater deviation from the normative model, especially in the 60:40 ratio, suggesting that more inaccurate probability estimations are more likely to occur under uncertainty conditions. Additionally, both patients and controls showed a greater deviation in the emotional version of the task, providing evidence of a reasoning bias modulated by the content of the stimuli. Finally, a positive correlation between patients’ deviation and delusional symptomatology was found. Impairments in the 60:40 ratio with emotional content was related to the amount of disruption in life caused by delusions. These results contribute to the understanding of how cognitive mechanisms interact with characteristics of the task (i.e., ambiguity and content) in the context of delusional thinking. These findings might be used to inform improved intervention programs in the domain of inferential reasoning.post-print700 K

    Human Melanoma-Associated Mast Cells Display a Distinct Transcriptional Signature Characterized by an Upregulation of the Complement Component 3 That Correlates With Poor Prognosis

    Cutaneous melanoma is one of the most aggressive human malignancies and shows increasing incidence. Mast cells (MCs), long-lived tissue-resident cells that are particularly abundant in human skin where they regulate both innate and adaptive immunity, are associated with melanoma stroma (MAMCs). Thus, MAMCs could impact melanoma development, progression, and metastasis by secreting proteases, pro-angiogenic factors, and both pro-inflammatory and immuno-inhibitory mediators. To interrogate the as-yet poorly characterized role of human MAMCs, we have purified MCs from melanoma skin biopsies and performed RNA-seq analysis. Here, we demonstrate that MAMCs display a unique transcriptome signature defined by the downregulation of the FcεRI signaling pathway, a distinct expression pattern of proteases and pro-angiogenic factors, and a profound upregulation of complement component C3. Furthermore, in melanoma tissue, we observe a significantly increased number of C3+^{+} MCs in stage IV melanoma. Moreover, in patients, C3 expression significantly correlates with the MC-specific marker TPSAB1, and the high expression of both markers is linked with poorer melanoma survival. In vitro, we show that melanoma cell supernatants and tumor microenvironment (TME) mediators such as TGF-β, IL-33, and IL-1β induce some of the changes found in MAMCs and significantly modulate C3 expression and activity in MCs. Taken together, these data suggest that melanoma-secreted cytokines such as TGF-β and IL-1β contribute to the melanoma microenvironment by upregulating C3 expression in MAMCs, thus inducing an MC phenotype switch that negatively impacts melanoma prognosis

    Design and evaluation of a treatment programme for Spanish adolescents with overweight and obesity. The EVASYON Study

    <p>Abstract</p> <p>Background</p> <p>The prevalence of overweight and obesity (OW/OB) among adolescents worldwide has increased since the 60 s. Spain has reached one of the highest OW/OB prevalence rates among adolescents from European countries. The aim of this methodological paper is to describe the design and evaluation in the EVASYON study (Development, implementation and evaluation of the efficacy of a therapeutic programme for adolescents with OW/OB: integral education on nutrition and physical activity).</p> <p>Methods/Design</p> <p>The EVASYON was planned by a multidisciplinary team to treat OW/OB in Spanish adolescents. The EVASYON is a multi-centre study conducted in 5 hospitals in 5 Spanish cities (Granada, Madrid, Pamplona, Santander and Zaragoza) and two hundred and four OW/OB Spanish adolescents were recruited for this intervention. The treatment was implemented for approximately one-year follow-up. The adolescents were treated in groups of a maximum of 10 subjects; each group had 20 visits during the treatment period in two phases: intensive during the first 2 months (1<sup>st </sup>to 9<sup>th </sup>visits), and extensive during the last 11 months (10<sup>th </sup>to 20<sup>th </sup>visits). In order to assess the efficacy of the treatment, 8 dimensions were measured: diet; physical activity and fitness; eating behaviour; body composition; haematological profile; metabolic profile; minerals and vitamins; immuno-inflammatory markers. Moreover, genetic polymorphisms were also determined.</p> <p>Discussion</p> <p>The treatment programme developed in the EVASYON study was designed as a national pilot study to be implemented as an effective treatment for adolescents with OW/OB into the Spanish Health Care Service.</p

The objective was to investigate the association between circulating plasma carotenoids and different cardiometabolic risk factors and the plasma fatty acid profile. This is a cross-sectional evaluation of baseline data conducted in a subcohort (106 women and 124 men) of an ongoing multi-factorial lifestyle trial for primary cardiovascular prevention. Plasma concentrations of carotenoids were quantified by liquid chromatography coupled to mass spectrometry. The associations between carotenoid concentrations and cardiometabolic risk factors were assessed using regression models adapted for interval-censored variables. Carotenoid concentrations were cross-sectionally inversely associated with serum triglyceride concentrations [-2.79 mg/dl (95% CI: -4.25, -1.34) and -5.15 mg/dl (95% CI: -7.38, -2.93), p-values = 0.0002 and <0.00001 in women and men, respectively], lower levels of plasma saturated fatty acids [-0.09% (95% CI: -0.14, -0.03) and -0.15 % (95% CI: -0.23, -0.08), p-values = 0.001 and 0.0001 in women and men, respectively], and higher levels of plasma polyunsaturated fatty acids [(0.12 % (95% CI: -0.01, 0.25) and 0.39 % (95% CI: 0.19, 0.59), p-values = 0.065 and 0.0001 in women and men, respectively] in the whole population.     Improved Survival Prediction by Combining Radiological Imaging and S-100B Levels Into a Multivariate Model in Metastatic Melanoma Patients Treated With Immune Checkpoint Inhibition

    Purpose: We explored imaging and blood bio-markers for survival prediction in a cohort of patients with metastatic melanoma treated with immune checkpoint inhibition. Materials and Methods: 94 consecutive metastatic melanoma patients treated with immune checkpoint inhibition were included into this study. PET/CT imaging was available at baseline (Tp0), 3 months (Tp1) and 6 months (Tp2) after start of immunotherapy. Radiological response at Tp2 was evaluated using iRECIST. Total tumor burden (TB) at each time-point was measured and relative change of TB compared to baseline was calculated. LDH, CRP and S-100B were also analyzed. Cox proportional hazards model and logistic regression were used for survival analysis. Results: iRECIST at Tp2 was significantly associated with overall survival (OS) with C-index=0.68. TB at baseline was not associated with OS, whereas TB at Tp1 and Tp2 provided similar predictive power with C-index of 0.67 and 0.71, respectively. Appearance of new metastatic lesions during follow-up was an independent prognostic factor (C-index=0.73). Elevated LDH and S-100B ratios at Tp2 were significantly associated with worse OS: C-index=0.73 for LDH and 0.73 for S-100B. Correlation of LDH with TB was weak (r=0.34). A multivariate model including TB change, S-100B, and appearance of new lesions showed the best predictive performance with C-index=0.83. Conclusion: Our analysis shows only a weak correlation between LDH and TB. Additionally, baseline TB was not a prognostic factor in our cohort. A multivariate model combining early blood and imaging biomarkers achieved the best predictive power with regard to survival, outperforming iRECIST

    Home parenteral nutrition (HPN) registry in spain for the years 2007, 2008 and 2009 (Nadya-SENPE group)

    Objetivo: Comunicar los datos del registro de Nutrición Parenteral Domiciliaria (NPD) del grupo de trabajo NADYA-SENPE de los años 2007, 2008 y 2009. Material y métodos: Recopilación de los datos del registro “on-line” introducidos por las Unidades responsables del seguimiento de la NPD desde el 1 de enero de 2007 al 31 de diciembre de 2009 dividido por años naturales. Resultados: Año 2007: Se registraron 133 pacientes con NPD (61 hombres y 72 mujeres), de 21 hospitales. La edad media de los 119 pacientes mayores de 13 años fue de 53,7 ± 14,9 años, y de 3,6 ± 3,6 años la de los 14 pacientes que no los superaban. La patología más frecuente fue la neoplasia (24%) seguida de las alteraciones de la motilidad intestinal y la enteritis posradiación (ambas 14%). En el 43% de los casos el motivo de indicación fue el síndrome de intestino corto, seguido de malabsorción (27%) y obstrucción intestinal (23%). Los catéteres más utilizados fueron los tunelizados (69%) y los reservorios subcutáneos (27%). Las complicaciones mas frecuentes fueron las sépticas relacionadas con el catéter con una tasa de 0,92 infecciones por cada mil días de NPD. La duración de la NPD fue superior a los dos años en el 50% de los casos. Al acabar el año seguía en activo el 71,4% de los pacientes; la muerte fue la principal causa de la finalización de la NPD (57,5%). El 26% de los pacientes se consideraron candidatos al trasplante intestinal. Año 2008: Se registraron 143 pacientes con NPD (62 hombres y 81mujeres), de 24 hospitales. La edad media de los 133 pacientes mayores de 13 años fue de 54,7 ± 13,9 años, y de 3,7 ± 0,6 años la de los 10 pacientes que no los superaban. La patología más frecuente fue la neoplasia (20%) seguida de la enteritis rádica (14%) y las alteraciones de la motilidad intestinal (13%). En el 44% de los casos el motivo de indicación fue el síndrome de intestino corto, seguido de malabsorción (28%) y obstrucción Nutriintestinal (20%). Los catéteres más utilizados fueron los tunelizados (60%) y los reservorios subcutáneos (29%). Las complicaciones mas frecuentes fueron las sépticas relacionadas con el catéter con una tasa de 0,50 infecciones por cada mil días de NPD. La duración de la NPD fue superior a los dos años en el 67% de los casos. Al acabar el año seguía en activo el 71,6% de los pacientes; la muerte fue la principal causa de la finalización de la NPD (52,4%). El 29% de los pacientes se consideraron candidatos al trasplante intestinal. Año 2009: Se registraron 158 pacientes con NPD (62 hombres y 96 mujeres), de 24 hospitales. La edad media de los 149 pacientes mayores de 13 años fue de 55,2 ± 13,0 años. La patología más frecuente fue la neoplasia (25%) seguida de la enteritis rádica (12%) y las alteraciones de la motilidad intestinal (11%). En el 42% de los casos el motivo de indicación fue el síndrome de intestino corto, seguido de malabsorción y obstrucción intestinal (ambas 23%). Los catéteres más utilizados fueron los tunelizados (60%) y los reservorios subcutáneos (36%). Las complicaciones mas frecuentes fueron las sépticas relacionadas con el catéter con una tasa de 0,67 infecciones por cada mil días de NPD. La duración de la NPD fue superior a los dos años en el 58% de los casos. Al acabar el año seguía en activo el 79,2% de los pacientes; el paso a alimentación oral fue la principal causa de la finalización de la NPD (48%). El 23% de los pacientes se consideraron candidatos a trasplante intestinal. Conclusiones: Se observa un aumento progresivo de los pacientes registrados respecto a años anteriores con una prevalencia muy variable según comunidades autónomas. La principal patología sigue siendo la neoplasia, que ocupa el primer lugar desde 2003. Se aprecia una disminución de las complicaciones sépticas relacionadas con el catéter en los dos últimos años, siendo la tasa de 2008 la más baja desde la creación del registroObjective: To report the data of the Home Parenteral Nutrition (HPN) registry of the NADYA-SENPE working group for the years 2007, 2008 and 2009. Tunnelled catheters were mostly used (69%), followed by implanted port-catheters (27%). Catheter related infections were the most frequent complications, with a rate of 0.92 episodes/103 HPN days. HPN was provided for more than two years in 50% of the cases. By the end of 2007, 71.4% of the patients remained active; exitus was the most frequent reason to end HPN (57.5%). 26% of the patients were eligible for intestinal transplant. 2008: 143 patients with HPN were registered (62 males and 81 females), belonging to 24 hospitals.     HLA association with the susceptibility to anti-synthetase syndrome

    Objective To investigate the human leukocyte antigen (HLA) association with anti-synthetase syndrome (ASSD). Methods We conducted the largest immunogenetic HLA-DRB1 and HLA-B study to date in a homogeneous cohort of 168 Caucasian patients with ASSD and 486 ethnically matched healthy controls by sequencing-based-typing. Results A statistically significant increase of HLA-DRB1*03:01 and HLA-B*08:01 alleles in patients with ASSD compared to healthy controls was disclosed (26.2% versus 12.2%, P = 1.56E?09, odds ratio?OR [95% confidence interval?CI] = 2.54 [1.84?3.50] and 21.4% versus 5.5%, P = 18.95E?18, OR [95% CI] = 4.73 [3.18?7.05]; respectively). Additionally, HLA-DRB1*07:01 allele was significantly decreased in patients with ASSD compared to controls (9.2% versus 17.5%, P = 0.0003, OR [95% CI] = 0.48 [0.31?0.72]). Moreover, a statistically significant increase of HLA-DRB1*03:01 allele in anti-Jo-1 positive compared to anti-Jo-1 negative patients with ASSD was observed (31.8% versus 15.5%, P = 0.001, OR [95% CI] = 2.54 [1.39?4.81]). Similar findings were observed when HLA carrier frequencies were assessed. The HLA-DRB1*03:01 association with anti-Jo-1 was unrelated to smoking history. No HLA differences in patients with ASSD stratified according to the presence/absence of the most representative non-anti-Jo-1 anti-synthetase autoantibodies (anti-PL-12 and anti-PL-7), arthritis, myositis or interstitial lung disease were observed. Conclusions Our results support the association of the HLA complex with the susceptibility to ASS

    Passive smoking alters circulating naïve/memory lymphocyte T-cell subpopulations in children

    While it has been indicated that exposure to second-hand smoke (SHS) can cause a local in vivo response, limited evidence exists on its possible systemic effects from population-based levels of exposure. We investigated into a possible systemic response in the immune parameters and lymphocyte subsets, i.e. B cell (CD19+), T cell (CD4+CD45RO+, CD4+CD45RA+, CD3+CD45RO+, CD3+CD45RA+) and natural killer (CD3+CD16CD56+) lymphocyte subsets relative to exposure to SHS. Blood was drawn from healthy, verified non-smoker, adolescent subjects (n = 68, mean age 14.2) and analysed for cotinine, antioxidants and lymphocyte immunophenotyping. SHS exposure was assessed using serum cotinine. Biomarker quantified exposure to SHS was correlated with a linear dose-response reduction in the percentages of memory CD4+CD45RO+ (p = 0.005) and CD3+CD45RO+ T-cell subsets (p = 0.005 and p = 0.003, respectively) and a linear increase in the percentage of naïve CD4+CD45RA+ and CD3+CD45RA+ T-cell subsets (p = 0.006 and p = 0.003, respectively). Additionally, higher exposure to SHS was associated with a higher CD4+CD45RA+ count (532 vs. 409 cells/ml, p = 0.017). Moreover, after controlling for age, gender, body mass index and plasma antioxidants, SHS exposure was found to be associated with the percentage of circulating naïve and memory CD4+ and CD3+ T-cell subpopulations, as revealed through a linear regression analysis. These findings indicate a systemic immunological response in healthy adolescents exposed to population-based levels of SHS exposure and imply an additional biological pathway for the interaction between exposure to SHS and its adverse effects on human health. © 2010 John Wiley & Sons A/S.Peer Reviewe

    Mediterranean Diet Reduces the Adverse Effect of the TCF7L2-rs7903146 Polymorphism on Cardiovascular Risk Factors and Stroke Incidence

    OBJECTIVE Transcription factor 7-like 2 (TCF7L2) polymorphisms are strongly associated with type 2 diabetes, but controversially with plasma lipids and cardiovascular disease. Interactions of the Mediterranean diet (MedDiet) on these associations are unknown. We investigated whether the TCF7L2-rs7903146 (C>T) polymorphism associations with type 2 diabetes, glucose, lipids, and cardiovascular disease incidence were modulated by MedDiet. RESEARCH DESIGN AND METHODS A randomized trial (two MedDiet intervention groups and a control group) with 7,018 participants in the PREvención con DIetaMEDiterránea study was undertaken and major cardiovascular events assessed. Data were analyzed at baseline and after a median follow-up of 4.8 years. Multivariable-adjusted Cox regression was used to estimate hazard ratios (HRs) for cardiovascular events. RESULTS The TCF7L2-rs7903146 polymorphism was associated with type 2 diabetes (odds ratio 1.87 [95% CI 1.62–2.17] for TT compared with CC). MedDiet interacted significantly with rs7903146 on fasting glucose at baseline (P interaction = 0.004). When adherence to the MedDiet was low, TT had higher fasting glucose concentrations (132.3 ± 3.5 mg/dL) than CC+CT (127.3 ± 3.2 mg/dL) individuals (P = 0.001). Nevertheless, when adherence was high, this increase was not observed (P = 0.605). This modulation was also detected for total cholesterol, LDL cholesterol, and triglycerides (P interaction < 0.05 for all). Likewise, in the randomized trial, TT subjects had a higher stroke incidence in the control group (adjusted HR 2.91 [95% CI 1.36–6.19]; P = 0.006 compared with CC), whereas dietary intervention with MedDiet reduced stroke incidence in TT homozygotes (adjusted HR 0.96 [95% CI 0.49–1.87]; P = 0.892 for TT compared with CC). CONCLUSIONS Our novel results suggest that MedDiet may not only reduce increased fasting glucose and lipids in TT individuals, but also stroke incidence
