    Cord blood cardiovascular biomarkers in left-sided congenital heart disease.

    Fetal echocardiography has limited prognostic ability in the evaluation of left-sided congenital heart defects (left heart defects). Cord blood cardiovascular biomarkers could improve the prognostic evaluation of left heart defects. A multicenter prospective cohort (2013-2019) including fetuses with left heart defects (aortic coarctation, aortic stenosis, hypoplastic left heart, and multilevel obstruction (complex left heart defects) subdivided according to their outcome (favorable vs. poor), and control fetuses were evaluated in the third trimester of pregnancy at three referral centers in Spain. Poor outcome was defined as univentricular palliation, heart transplant, or death. Cord blood concentrations of N-terminal precursor of B-type natriuretic peptide, Troponin I, transforming growth factor ÎČ, placental growth factor, and soluble fms-like tyrosine kinase-1 were determined. A total of 45 fetuses with left heart defects (29 favorable and 16 poor outcomes) and 35 normal fetuses were included, with a median follow-up of 3.1 years (interquartile range 1.4-3.9). Left heart defects with favorable outcome showed markedly increased cord blood transforming growth factor ÎČ (normal heart median 15.5 ng/mL (6.8-21.4) vs. favorable outcome 51.7 ng/mL (13.8-73.9) vs. poor outcome 25.1 ng/mL (6.9-39.0), p = 0.001) and decreased placental growth factor concentrations (normal heart 17.9 pg/mL (13.8-23.9) vs. favorable outcome 12.8 pg/mL (11.7-13.6) vs. poor outcome 11.0 pg/mL (8.8-15.4), p < 0.001). Poor outcome left heart defects had higher N-terminal precursor of B-type natriuretic peptide (normal heart 508.0 pg/mL (287.5-776.3) vs. favorable outcome 617.0 pg/mL (389.8-1087.8) vs. poor outcome 1450.0 pg/mL (919.0-1645.0), p = 0.001) and drastically reduced soluble fms-like tyrosine kinase-1 concentrations (normal heart 1929.7 pg/mL (1364.3-2715.8) vs. favorable outcome (1848.3 pg/mL (646.9-2313.6) vs. poor outcome 259.0 pg/mL (182.0-606.0), p < 0.001).     Total knee replacement from the patient's perspective. A qualitative study

    Total knee replacement (TKR) is effective in relieving pain and improving function in patients with severe osteoarthritis (OA) However, studies report that 14-30% of patients are dissatisfied with the result and do not achieve the expected benefits, especially in function. Objectives: To examine the factors influencing decision-making before TKR. Methods: We made a phenomenological study of the determinants that lead patients to accept, delay or reject TKR (this study is part of a larger study). Demographic and clinical data were collected and pain intensity measured by the WOMAC scale. Focus groups were conducted and the results transcribed and analysed using the 4-stage analysis of qualitative data according to Ritchie, Spencer and O'Connor (2003). Results: 12 patients (9 female, mean age 71.58 + 6.02 years, BMI 37.43 + 5.32, mean comorbidities 6.73 + 2.19 and mean WOMAC pain 14.9 + 8.89, function 15.77 + 8.6, total 15. 71 + 8. 22) were included: 6 had received, 4 were waiting for and 2 had rejected TKR. Focus groups identified widespread pain (pain intensity functional limitations), causal beliefs and perceptions (OA a natural process associated with age, obesity, physical work, sport) mood (importance of optimism), professional-user relationship (communication, experience of OA) expectations (need for surgery conditioned by pain, lost function, surgical risks), and social support (promotion of healthy habits and adherence conditioned by family and social support) as factors influencing attitudes to undergoing TKR. Conclusions: The process of deciding to undergo TKR is complex and influenced by multiple factors. TKR improves a small proportion of an aging painful body. Our results suggest the need of care before and after TKR, psychosocial support and preventive and educational programmes. References: Ritchie J, Spencer L, O'Connor W.     Genome-wide pathway analysis identifies VEGF pathway association with oral ulceration in systemic lupus erythematosus

    Background: Systemic lupus erythematosus (SLE) is a genetically complex rheumatic disease characterized by heterogeneous clinical manifestations of unknown etiology. Recent studies have suggested the existence of a genetic basis for SLE heterogeneity. The objective of the present study was to identify new genetic variation associated with the clinically relevant phenotypes in SLE. Methods: A two-stage pathway-based approach was used to identify the genetic variation associated with the main clinical phenotypes in SLE. In the discovery stage, 482 SLE patients were genotyped using Illumina Human Quad610 microarrays. Association between 798 reference genetic pathways from the Molecular Signatures Database and 11 SLE phenotypes was tested using the set-based method implemented in PLINK software. Pathways significantly associated after multiple test correction were subsequently tested for replication in an independent cohort of 425 SLE patients. Using an in silico approach, we analyzed the functional effects of common SLE therapies on the replicated genetic pathways. The association of known SLE risk variants with the development of the clinical phenotypes was also analyzed. Results: In the discovery stage, we found a significant association between the vascular endothelial growth factor (VEGF) pathway and oral ulceration (P value for false discovery rate (P FDR) < 0.05), and between the negative regulation signaling pathway of retinoic acid inducible gene-I/melanoma differentiation associated gene 5 and the production of antinuclear antibodies (P FDR < 0.05). In the replication stage, we validated the association between the VEGF pathway and oral ulceration. Therapies commonly used to treat mucocutaneous phenotypes in SLE were found to strongly influence VEGF pathway gene expression (P = 4.60e-4 to 5.38e-14). Analysis of known SLE risk loci identified a strong association between PTPN22 and the risk of hematologic disorder and with the development of antinuclear antibodies. Conclusions: The present study has identified VEGF genetic pathway association with the risk of oral ulceration in SLE. New therapies targeting the VEGF pathway could be more effective in reducing the severity of this phenotype. These findings represent a first step towards the understanding of the genetic basis of phenotype heterogeneity in SLE

    La llar d’infants Cobi es troba en una emplaçament que Ă©s bastant freqĂŒent al Poblenou de Barcelona: en la mateixa illa, limita amb l’escola dels nens mĂ©s grans, i per la banda del carrer dĂłna a un xamfrĂ  en el que hi ha un edifici industrial i els habitatges que ocupen l’illa dels carrers TurrĂł/Àvila/Alava i Llull, projectats per l’arquitecte Carlos Ferrater a la Vila OlĂ­mpica, tots aquests força alts. En definitiva, un teixit urbĂ  una mica caĂČtic i desigual. Potser per aixĂČ, la llar d’infants es tanca sobre si mateixa sense que s’apreciĂŻ gairebĂ© cap obertura a l’exterior. Malgrat aixĂČ no dĂłna la sensaciĂł de bĂșnquer –que es el que cabria esperar d’una actitud arquitectĂČnica com aquesta, i mĂ©s si pensem que l’acabat exterior Ă©s de formigĂł vist– sinĂł que, ben al contrari, el seu aspecte exterior Ă©s amable i agradable.
