    Role of magnetic resonance spectroscopy in grading of gliomas- a tertiary care centre study

    Background: Grading of gliomas is important for the determination of appropriate treatment strategies. MR spectroscopy has found increasing utility in grading of gliomas.Methods: MR spectroscopic imaging was done for the referred patients to obtain Cho/NAA and Cho/Cr ratios. After analyzing histopathology reports, gliomas were classified into low grade and high grade. Histologically proven 54 low grade and 54 high grade gliomas were included in the study. The mean and standard deviation of these variables were obtained and compared between two groups.  ROC curve analyses were performed in order to identify the optimal cut-off value for metabolite ratios for prediction purposes of high grade versus low grade gliomas.Results: The mean and SD of Cho/NAA ratio in low grade glioma was 1.93±1.19. The mean and SD of Cho /NAA ratio in high grade glioma was 3.16±1.73 was significantly higher. The optimal cut-off for differentiating low grade and high grade gliomas was 2.15 with a sensitivity of 74.07% and specificity of 66.67%. The mean and SD of CHO/Cr in low grade glioma was 2.05±0.76. The mean and SD of Cho/NAA in high grade glioma was 2.87±1.65 was significantly higher. The optimal cut-off for differentiating low grade and high grade gliomas was 1.98 with a sensitivity of 64.8% and specificity of 64.1%.Conclusions: MR spectroscopy imaging plays a pivotal role in prediction of glioma grade preoperatively and helps in deciding appropriate treatment strategies

    A framework for human microbiome research

    A variety of microbial communities and their genes (the microbiome) exist throughout the human body, with fundamental roles in human health and disease. The National Institutes of Health (NIH)-funded Human Microbiome Project Consortium has established a population-scale framework to develop metagenomic protocols, resulting in a broad range of quality-controlled resources and data including standardized methods for creating, processing and interpreting distinct types of high-throughput metagenomic data available to the scientific community. Here we present resources from a population of 242 healthy adults sampled at 15 or 18 body sites up to three times, which have generated 5,177 microbial taxonomic profiles from 16S ribosomal RNA genes and over 3.5 terabases of metagenomic sequence so far. In parallel, approximately 800 reference strains isolated from the human body have been sequenced. Collectively, these data represent the largest resource describing the abundance and variety of the human microbiome, while providing a framework for current and future studies

    Structure, function and diversity of the healthy human microbiome

The definitive version was published in Nature 486 (2012): 207-214, doi:10.1038/nature11234.Studies of the human microbiome have revealed that even healthy individuals differ remarkably in the microbes that occupy habitats such as the gut, skin and vagina. Much of this diversity remains unexplained, although diet, environment, host genetics and early microbial exposure have all been implicated. Accordingly, to characterize the ecology of human-associated microbial communities, the Human Microbiome Project has analysed the largest cohort and set of distinct, clinically relevant body habitats so far. We found the diversity and abundance of each habitat’s signature microbes to vary widely even among healthy subjects, with strong niche specialization both within and among individuals. The project encountered an estimated 81–99% of the genera, enzyme families and community configurations occupied by the healthy Western microbiome. Metagenomic carriage of metabolic pathways was stable among individuals despite variation in community structure, and ethnic/racial background proved to be one of the strongest associations of both pathways and microbes with clinical metadata.     Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

Embolization is the mainstay of treatment, however should be performed using proper techniques of selective embolization avoiding physiological blood vessels supplying normal structures

    A REVIEW ON ROLE OF ANTIOXIDANT PROPERTY OF HERBS IN HYPERTENSION

    Hypertension is the most common factor for cardiovascular disease and a major public health. Herbal medicines may protect against cardiovascular diseases by contributing to the total antioxidant defense system of the human body. Antioxidants are compounds that protect cells against the damaging effects of reactive oxygen species (ROS), such as superoxide, hydrogen peroxide, singlet oxygen, peroxyl radicals, hydroxyl and peroxynitrite radicals. The natural products should be considered as the best in primary healthcare because of better cultural acceptability, safety, potent, inexpensive and lesser side effect. This review describes the role of antioxidant property of herbs used in hypertension. Several herbal medicines and supplements have been studied as potential therapeutic agents in the management of hypertension and its complications. Available data suggests that the extracts of most of these herbs or compounds derived from them may provide a safe and effective adjunctive therapeutic approach for the treatment of hypertension

    Predictors of Clostridium difficile infection-related mortality among older adults

    BACKGROUND: Over 90% of annual deaths caused by Clostridium difficile infection (CDI) occur in persons aged ≥65 years. However, no large-scale studies have been conducted to investigate predictors of CDI-related mortality among older adults. METHODS: This case-control study included 540 CDI patients aged ≥60 years admitted to a tertiary care hospital in Detroit, Michigan, between January 2005 and December 2012. Cases were CDI patients who died within 30 days of CDI date. Controls were CDI patients who survived \u3e30 days after CDI date. Cases were matched to controls on a 1:3 ratio based on age and hospital acquisition of CDI. RESULTS: One-hundred and thirty cases (25%) were compared with 405 controls (75%). Independent predictors of CDI-related mortality included admission from another acute hospital (odds ratio [OR], 8.25; P = .001) or a long-term care facility (OR, 13.12; P = .012), McCabe score ≥2 (OR, 12.19; P \u3c .001), and high serum creatinine (≥1.7 mg/dL) (OR, 3.43; P = .021). The regression model was adjusted for the confounding effect of limited activity of daily living score, total number of antibiotic days prior to CDI, ileus on abdominal radiograph, low albumin (≤2.5 g/dL), elevated white blood cell count (\u3e15 × 1,000/mm3), and admission to intensive care unit because of CDI. CONCLUSIONS: Predictors of CDI-related mortality reported in this study could be applied to the development of a bedside scoring system for older adults with CDI
