    Teaching Foundational Aquatic Skills to Children in Open Water Environments

    Learning to swim in a swimming pool might not prepare water competence sufficiently for different aquatic environments. The aim of this study was to assess the effectiveness of teaching children water safety knowledge and skills in open water environments (i.e., harbor, river, and surf). The aquatic knowledge and skills of 98 children (7-11 years old) were tested in a swimming pool before, immediately after, and three months after receiving a three-day intensive education program. At pre-test, typically fewer than 50% of children achieved a high level of water safety competence. After the program, competency in each of the six tasks assessed had increased with up to 80% of participants completing the tasks unassisted. Three-month retention of these skills was generally high (i.e., competency levels were either maintained or improved). A key challenge for future research will be to untangle the influences of maturation, order effects, and the open water education

    MAP, Johne’s disease and the microbiome; current knowledge and future considerations

    peer-reviewedMycobacterium avium subsp. paratuberculosis is the causative agent of Johne’s disease in ruminants. As an infectious disease that causes reduced milk yields, effects fertility and, eventually, the loss of the animal, it is a huge financial burden for associated industries. Efforts to control MAP infection and Johne’s disease are complicated due to difficulties of diagnosis in the early stages of infection and challenges relating to the specificity and sensitivity of current testing methods. The methods that are available contribute to widely used test and cull strategies, vaccination programmes also in place in some countries. Next generation sequencing technologies have opened up new avenues for the discovery of novel biomarkers for disease prediction within MAP genomes and within ruminant microbiomes. Controlling Johne’s disease in herds can lead to improved animal health and welfare, in turn leading to increased productivity. With current climate change bills, such as the European Green Deal, targeting livestock production systems for more sustainable practices, managing animal health is now more important than ever before. This review provides an overview of the current knowledge on genomics and detection of MAP as it pertains to Johne’s disease

    Chemoreceptor responsiveness at sea level does not predict the pulmonary pressure response to high altitude

    The hypoxic ventilatory response (HVR) at sea level (SL) is moderately predictive of the change in pulmonary artery systolic pressure (PASP) to acute normobaric hypoxia. However, because of progressive changes in the chemoreflex control of breathing and acid-base balance at high altitude (HA), HVR at SL may not predict PASP at HA. We hypothesized that resting peripheral oxyhemoglobin saturation (SpO2) at HA would correlate better than HVR at SL to PASP at HA. In 20 participants at SL, we measured normobaric, isocapnic HVR (L/min·-%SpO2 -1) and resting PASP using echocardiography. Both resting SpO2 and PASP measures were repeated on day 2 (n=10), days 4-8 (n=12), and 2-3 weeks (n=8) after arrival at 5050m. These data were also collected at 5050m on life-long HA residents (Sherpa; n=21). Compared to SL, SpO2 decreased from 98.6 to 80.5% (P<0.001), while PASP increased from 21.7 to 34.0mmHg (P<0.001) after 2-3 weeks at 5050m. Isocapnic HVR at SL was not related to SpO2 or PASP at any time point at 5050m (all P>0.05). Sherpa had lower PASP (P<0.01) than lowlanders on days 4-8 despite similar SpO2. Upon correction for hematocrit, Sherpa PASP was not different from lowlanders at SL, but lower than lowlanders at all HA time points. At 5050m, whilst SpO2 was not related to PASP in lowlanders at any point (all R2=0.50), there was a weak relationship in the Sherpa (R2=0.16; P=0.07). We conclude that neither HVR at SL nor resting SpO2 at HA correlates with elevations in PASP at HA

    Inshore and offshore marine migration pathways of Atlantic salmon post-smolts from multiple rivers in Scotland, England, Northern Ireland and Ireland

    The migratory behavior of Atlantic salmon (Salmo salar) post-smolts in coastal waters is poorly understood. In this collaborative study, 1914 smolts, from 25 rivers, in four countries were tagged with acoustic transmitters during a single seasonal migration. In total, 1105 post-smolts entered the marine study areas and 438 (39.6%) were detected on a network of 414 marine acoustic receivers and an autonomous underwater vehicle. Migration pathways (defined as the shortest distance between two detections) of up to 575 km and over 100 days at sea were described for all 25 populations. Post-smolts from different rivers, as well as individuals from the same river, used different pathways in coastal waters. Although difficult to generalize to all rivers, at least during the year of this study, no tagged post-smolts from rivers draining into the Irish Sea were detected entering the areas of sea between the Hebrides and mainland Scotland, which is associated with a high density of finfish aquaculture. An important outcome of this study is that a high proportion of post-smolts crossed through multiple legislative jurisdictions and boundaries during their migration. This study provides the basis for spatially explicit assessment of the impact risk of coastal pressures on salmon during their first migration to sea

    Adjunctive rifampicin for Staphylococcus aureus bacteraemia (ARREST): a multicentre, randomised, double-blind, placebo-controlled trial.

    BACKGROUND: Staphylococcus aureus bacteraemia is a common cause of severe community-acquired and hospital-acquired infection worldwide. We tested the hypothesis that adjunctive rifampicin would reduce bacteriologically confirmed treatment failure or disease recurrence, or death, by enhancing early S aureus killing, sterilising infected foci and blood faster, and reducing risks of dissemination and metastatic infection. METHODS: In this multicentre, randomised, double-blind, placebo-controlled trial, adults (≥18 years) with S aureus bacteraemia who had received ≤96 h of active antibiotic therapy were recruited from 29 UK hospitals. Patients were randomly assigned (1:1) via a computer-generated sequential randomisation list to receive 2 weeks of adjunctive rifampicin (600 mg or 900 mg per day according to weight, oral or intravenous) versus identical placebo, together with standard antibiotic therapy. Randomisation was stratified by centre. Patients, investigators, and those caring for the patients were masked to group allocation. The primary outcome was time to bacteriologically confirmed treatment failure or disease recurrence, or death (all-cause), from randomisation to 12 weeks, adjudicated by an independent review committee masked to the treatment. Analysis was intention to treat. This trial was registered, number ISRCTN37666216, and is closed to new participants. FINDINGS: Between Dec 10, 2012, and Oct 25, 2016, 758 eligible participants were randomly assigned: 370 to rifampicin and 388 to placebo. 485 (64%) participants had community-acquired S aureus infections, and 132 (17%) had nosocomial S aureus infections. 47 (6%) had meticillin-resistant infections. 301 (40%) participants had an initial deep infection focus. Standard antibiotics were given for 29 (IQR 18-45) days; 619 (82%) participants received flucloxacillin. By week 12, 62 (17%) of participants who received rifampicin versus 71 (18%) who received placebo experienced treatment failure or disease recurrence, or died (absolute risk difference -1·4%, 95% CI -7·0 to 4·3; hazard ratio 0·96, 0·68-1·35, p=0·81). From randomisation to 12 weeks, no evidence of differences in serious (p=0·17) or grade 3-4 (p=0·36) adverse events were observed; however, 63 (17%) participants in the rifampicin group versus 39 (10%) in the placebo group had antibiotic or trial drug-modifying adverse events (p=0·004), and 24 (6%) versus six (2%) had drug interactions (p=0·0005). INTERPRETATION: Adjunctive rifampicin provided no overall benefit over standard antibiotic therapy in adults with S aureus bacteraemia.     Local Skin Temperature and Local Sweat Rate

    Local skin temperature may be a modulator of sweat gland function but the mechanism is unclear. PURPOSE: To examine the role of local skin temperature in modulating local sweating rate (LSR) using a model of sweat gland activation that produces a moderate, physiologically based sweating response. METHODS: Each subject (n=15) was instrumented with 7 skin temperature probes ( skin), an esophageal probe (Tcore), and a sweat rate capsule (dorsal forearm). A LSR was produced by 30 s of intradermal electrical stimulation (5 mA, 0.2 to 64 Hz) that resulted in release of ACh from the sudomotor nerve and produced a transient sweating response lasting 60 to 90 s. To modify local skin temperature the subjects sat in an environmental chamber for 60 min at three different ambient temperatures (Ta): 21, 27, and 34 °C, on the same day without moving the sweat capsule. Subjects acclimated to the chamber temperature for 45 min prior to producing the LSR. The LSR was quantified as the area-under the sweat rate-time curve (LSR AUC, normalized to peak LSR AUC at 21°C ) and curve fit using a four parameter logistic model. RESULTS: Local skin temperature averaged 29.3 ± 1.2, 31.3 ± 0.8, and 34.6 ± 0.8°C, baseline sweat rate (SR) averaged 0.127 ± 0.05, 0.142 ± 0.065, and 0.124 ± 0.083 mg • min-1 • cm-2 and Tcore averaged 37.1 ± 0.7, 36.8 ± 0.6, and 36.6 ± 0.5°C at 21, 27, and 34°C Ta, respectively. Comparison of the models indicated that the both the plateau LSR AUCX and the EC50 at 21, 27 and 34°C were not the same (p=0.0022). The onset of sweating occurred sooner at 34°C (4.6 ± 2 s) than at 21°C (7.3 ± 3.0 s) or 27°C (6.1 ± 2.0 s) and was linearly related to local skin temperature (p = 0.0002). Finally, the increase in sweating rate at 34°C (0.032 ± 0.027 ∆SR•s-1) was also faster than at 21 or 27°C (pCONCLUSION: The observed change in the stimulus-response characteristics of the LSR induced by intradermal electrical stimulation (decreased EC50 and increased plateau LSR AUC) supports the hypothesis that an increased local skin temperature improves the sweat gland responsiveness to ACh released by the sudomotor nerve during intradermal electrical stimulation

    Low-volume intense exercise elicits post-exercise hypotension and subsequent hypervolemia, irrespective of which limbs are exercised

    Introduction: Exercise reduces arterial and central venous blood pressures during recovery, which contributes to its valuable anti-hypertensive effects and to facilitating hypervolemia. Repeated sprint exercise potently improves metabolic function, but its cardiovascular effects (esp. hematological) are less well-characterized, as are effects of exercising upper versus lower limbs. The purposes of this study were to identify the acute (\u3c 24 h) profiles of arterial blood pressure and blood volume for (i) sprint intervals versus endurance exercise, and (ii) sprint intervals using arms versus legs. Methods: Twelve untrained males completed three cycling exercise trials; 50-min endurance (legs), and 5*30-s intervals using legs or arms, in randomized and counterbalanced sequence, at a standardized time of day with at least 8 days between trials. Arterial pressure, hemoglobin concentration and hematocrit were measured before, during and across 22 h after exercise, the first 3 h of which were seated rest. Results: The post-exercise hypotensive response was larger after leg intervals than endurance (AUC: 7540 ± 3853 vs. 3897 ± 2757 mm Hg·min, p = 0.049, 95% CI: 20 to 6764), whereas exercising different limbs elicited similar hypotension (arms: 6420 ± 3947 mm Hg·min, p = 0.48, CI: -1261 to 3896). In contrast, arterial pressure at 22 h was reduced after endurance but not after leg intervals (-8 ± 8 vs. 0 ± 7 mm Hg, p = 0.04, CI: 7 ± 7) or reliably after arm intervals (-4 ± 8 mm Hg, p = 0.18 vs. leg intervals). Regardless, plasma volume expansion at 22 h was similar between leg intervals and endurance (both +5 ± 5%; CI: -5 to 5%) and between leg and arm intervals (arms: +5 ± 7%, CI: -8 to 5%). Conclusions: These results emphasize the relative importance of central and/or systemic factors in post-exercise hypotension, and indicate that markedly diverse exercise profiles can induce substantive hypotension and subsequent hypervolemia. At least for endurance exercise, this hypervolemia may not depend on the volume of post-exercise hypotension. Finally, endurance exercise led to reduced blood pressure the following day, but sprint interval exercise did not
