El proyecto MPAR (Mejora del Proceso de Evaluación de Resultados en Atención Primaria)
El proyecto MPAR (Mejora de los Procesos de Evaluación de Resultados en Atención Primaria ) nació en 1996 como una iniciativa conjunta de un grupo de profesionales sanitarios con el propósito de profundizar en el análisis de los procesos de evaluación de la atención primaria de salud (APS). El proyecto se ha ido estructurando en grupos de trabajo con objetivos concretos, cuyas reflexiones y conclusiones se han ido recogiendo en una serie de documentos. En una primera fase, se definieron las líneas estratégicas para la evaluación: un mayor énfasis en los resultados, la trascendencia de la evaluación, la participación de los profesionales, la diversificación de las metodologías, la flexibilización de los calendarios, la adecuación de los sistemas e información, la evaluación del equipo de atención primaria (EAP) y de los profesionales, y la priorización de la evaluación evolutiva sobre la comparativa. Se elaboró un marco conceptual y estratégico común para todos los ámbitos implicados en la evaluación (financiador/comprador, proveedor de servicios, y profesionales), identificando sus prioridades, intereses y necesidades, de cuya intersección se deriva la evaluación posible y prioritaria. Este marco común no debía impedir que cada una de las partes pudiera desarrollar líneas de evaluación diferenciadas acordes con sus intereses concretos. Finalmente, en esta primera fase, de acuerdo con la idea básica de la diversificación metodológica, también se analizó el uso potencial de las técnicas cualitativas en evaluación, realizándose un estudio en 10 EAP, que determinó su utilidad y factibilidad para la evaluación de la APS. En una segunda fase, el proyecto se centró en los aspectos relacionados con la información. Inicialmente se elaboró un conjunto de 13 propuestas operativas sobre información y evaluación en APS. A partir de ellas, se definió un conjunto de 264 indicadores seleccionados, correspondientes a las diferentes áreas de evaluación de la APS, y unas recomendaciones relacionadas con la formulación y evaluación de objetivos en los procesos de planificación de APS. Otro grupo de trabajo abordó el tema del Conjunto Mínimo Básico de Datos en Atención Primaria (CMBD-AP), definiendo sus características y discutiendo las ventajas e inconvenientes de las diferentes opciones de unidades de registro, considerando como la más adecuada la de episodio de atención, en combinación con la de paciente atendido, y de los sistemas de codificación. Así mismo, se realizó una prueba piloto de evaluación de un EAP informatizado que combinara técnicas cuantitativas y cualitativas, centrada en los ámbitos de atención al cliente y de práctica clínica e incorporando la perspectiva del usuario. La valoración de la prueba fue muy positiva, ya que permitió hacerse una idea del funcionamiento del EAP y detectar puntos d mejora. La discusión de los resultados con el equipo directivo del EAP se consideró muy positiva. La línea actual de reflexión del proyecto MPAR se centra en el poder resolutivo, intentando responder a la cuestión de si la nueva APS ha dado respuesta suficiente a las expectativas generadas en el momento de su diseño y puesta en marcha, y si ha conseguido solucionar los problemas principales propios de la antigua asistencia médica ambulatoria pública. El punto de partida es que el poder resolutivo debe entenderse en el marco más amplio del conjunto del sistema y, por lo tanto, el análisis debe centrarse en la longitudinalidad e integración de los procesos asistenciales más que en la delimitación transversal y estructural de las competencias de cada nivel. El poder de resolución se podría definir como la capacidad que tiene el sistema sanitario, a través de sus organizaciones y profesionales, para dar respuestas asistenciales adecuadas a las necesidades y demandas de los ciudadanos, coherentes con el conocimiento científico. La resolución global del sistema mejora en función de las contribuciones de cada uno de sus niveles. En un primer documento sobre el tema, se identifican sus condicionantes y factores limitantes, y se realizan algunas visiones y aproximaciones sobre un contexto posible de su futura evolución. Un segundo documento se centra en la mejora del poder resolutivo del sistema de salud y la contribución de la APS a la misma, identificando los cambios necesarios en relación con la orientación de los servicios y los aspectos conceptuales del modelo. La principal línea actual de trabajo del proyecto MPAR se centra en el abordaje, tanto desde una perspectiva teórica como operativa, del problema de la medida del poder resolutivo
Attitudes and perceptions of medical students about family medicine in Spain: protocol for a cross-sectional survey
Background: Despite the fact that family medicine
(FM) has become established as a specialty in the past
25 years, this has not been reflected in the inclusion of
the specialty in the majority of medical schools in
Spain. Almost 40% of the students will work in primary
care but, in spite of this, most universities do not have
an assessed placement as such. There are only
specific practice periods in health centres or some
student-selected components with little weight in the
overall curricula.
Objectives: To evaluate the attitudes and perceptions
of medical students about FM in the health system and
their perception about the need for specific training in
FM at the undergraduate level. To explore change over
time of these attitudes and perceptions and to examine
potential predictive factors for change. Finally, we will
review what teaching activity in FM is offered across
the Spanish schools of medicine.
Methods: Descriptive cross-sectional survey. Each
one of the different analyses will consist of two
surveys: one for all the students in the first, third and
fifth year of medical school in all the Spanish schools
of medicine asking about their knowledge, perceptions
and attitudes in relation to primary care and FM. There
will be an additional survey for the coordinating faculty
of the study in each university about the educational
activities related to FM that are carried out in their
centres. The repetition of the study every 2 years will
allow for an analysis of the evolution of the cohort of
students until they receive their degree and the
potential predictive factors.
Discussion: This study will provide useful information
for strategic planning decisions, content and
educational methodology in medical schools in Spain
and elsewhere. It will also help to evaluate the
influence of the ongoing changes in FM, locally and at
the European level, on the attitudes and perceptions of

Cambios en los niveles de hormonas sexuales en el varón que envejece y su relación con el Síndrome Adam
En los últimos años está tomando un importante protagonismo el síndrome de ADAM ("Androgen Deficiency in Aging Males"), consecuencia del aumento de la expectativa de vida en los varones. Con la realización de este estudio pretendemos establecer la prevalencia de este síndrome en la población y establecer una relación entre los niveles androgénicos y el diagnóstico del síndrome de ADAM. Para ello escogeremos la muestra de la población de varones entre cincuenta y ochenta años del departamento 4 de Salud de la CCVV. La obtención de los datos se realizará mediante la realización de test clínicos fundamentalmente el cuestionario de ADAM.En els últims anys està prenent un important protagonisme el síndrome d' ADAM ("Androgen Deficiency in Agyng Males"), conseqüència de l'aument de l'expectativa de vida en els varons. Amb la realització d'aquest estudi se pretén establir la prevalència d'aquest síndrome en la població i establir una relació entre els nivells androgènics i el diagnòstic del síndrome de ADAM. La mostra de la població serà varons entre cinquanta i vuitanta anys del departament 4 de Salud de la CCVV. L'obtenció de les dades se realitzarà mitjançant la realització de test clínics fonamentalment el qüestionari d'ADAM
Psychometric properties of a test in evidence based practice: the Spanish version of the Fresno test
<p>Abstract</p> <p>Background</p> <p>Validated instruments are needed to evaluate the programmatic impact of Evidence Based Practice (EBP) training and to document the competence of individual trainees. This study aimed to translate the Fresno test into Spanish and subsequently validate it, in order to ensure the equivalence of the Spanish version against the original English version.</p> <p>Methods</p> <p>Before and after study performed between October 2007 and June 2008. Three groups of participants: (a) Mentors of family medicine residents (expert group) (n = 56); (b) Family medicine physicians (intermediate experience group) (n = 17); (c) Family medicine residents (novice group) (n = 202); Medical residents attended an EBP course, and two sets of the test were administered before and after the course. The Fresno test is a performance based measure for use in medical education that assesses EBP skills. The outcome measures were: inter-rater and intra-rater reliability, internal consistency, item analyses, construct validity, feasibility of administration, and responsiveness.</p> <p>Results</p> <p>Inter-rater correlations were 0.95 and 0.85 in the pre-test and the post-test respectively. The overall intra-rater reliability was 0.71 and 0.81 in the pre-test and post-test questionnaire, respectively. Cronbach's alpha was 0.88 and 0.77, respectively. 152 residents (75.2%) returned both sets of the questionnaire. The observed effect size for the residents was 1.77 (CI 95%: 1.57-1.95), the standardised response mean was 1.65 (CI 95%:1.47-1.82).</p> <p>Conclusions</p> <p>The Spanish version of the Fresno test is a useful tool in assessing the knowledge and skills of EBP in Spanish-speaking residents of Family Medicine.</p
Study protocol of psychometric properties of the Spanish translation of a competence test in evidence based practice: The Fresno test
<p>Abstract</p> <p>Background</p> <p>There are few high-quality instruments for evaluating the effectiveness of Evidence-Based Practice (EBP) curricula with objective outcomes measures. The Fresno test is an instrument that evaluates most of EBP steps with a high reliability and validity in the English original version. The present study has the aims to translate the Fresno questionnaire into Spanish and its subsequent validation to ensure the equivalence of the Spanish version against the English original.</p> <p>Methods and design</p> <p>The questionnaire will be translated with the back translation technique and tested in Primary Care Teaching Units in Catalonia (PCTU). Participants will be: (a) tutors of Family Medicine residents (expert group); (b) Family Medicine residents in their second year of the Family Medicine training program (novice group), and (c) Family Medicine physicians (intermediate group). The questionnaire will be administered before and after an educational intervention. The educational intervention will be an interactive four half-day sessions designed to develop the knowledge and skills required to EBP. Responsiveness statistics used in the analysis will be the effect size, the standardised response mean and Guyatt's method. For internal consistency reliability, two measures will be used: corrected item-total correlations and Cronbach's alpha. Inter-rater reliability will be tested using Kappa coefficient for qualitative items and intra-class correlation coefficient for quantitative items and the overall score. Construct validity, item difficulty, item discrimination and feasibility will be determined.</p> <p>Discussion</p> <p>The validation of the Fresno questionnaire into different languages will enable the expansion of the questionnaire, as well as allowing comparison between countries and the evaluation of different teaching models.</p
Fiabilidad de un cuestionario empleado para la aplicación en el seguimiento longitudinal de la opinión y percepciones de la medicina de familia de los estudiantes de medicina de la Universidad de Zaragoza
Objetivo
La medicina de familia aborda determinados aspectos y perspectivas que suelen quedar relegados en la formación de otros niveles de atención por lo que cada vez cobra mayor importancia la necesidad de que los estudiantes tomen contacto con la atención primaria. El objetivo de este estudio es el de evaluar la fiabilidad del cuestionario del grupo Unimedifam (FIS PI070975) para conocer la evolución longitudinal de las expectativas y conocimientos sobre medicina de familia de los estudiantes de medicina.
Diseño
Estudio de fiabilidad de un cuestionario.
Emplazamiento
Alumnos de medicina de la Universidad de Zaragoza.
Participantes
Contamos con un total de 371 alumnos de los cursos 1.°, 3.°, 5.° y 6.°.
Método
La consistencia interna del cuestionario se determinó con el coeficiente alfa de Cronbach y su estabilidad en una prueba test-retest.
Resultados
En la escala A del cuestionario encontramos que 3 ítems de 19 son poco estables; en la escala B 8 de 26 y en la escala C, 3 ítems de 38. El valor de alfa de Cronbach de la escala A es de 0,722, de la escala B de 0,861 y el de la escala C, 0,663.
Conclusiones
Las 3 escalas están entre los valores adecuados, excepto la escala C que aun así se encuentra muy próxima. Las conclusiones de este proceso de validación pueden servir de referencia y extrapolarse al cuestionario UNIMEDFAM a nivel estatal.
Aim
Family medicine deals with certain aspects and perspectives that are often left behind in the training of other levels of care, thus the need for medical students to make contact with Primary Care is of increasing importance. The aim of this study is to evaluate the reliability of the questionnaire of the UNIMEDIFAM group (FIS PI070975) for the long-term outcome of expectations and knowledge about family medicine.

Setting
University of Zaragoza.
Participants
371 students from the 1st, 3rd, 5th, and 6th years.
Method
The internal consistency of the questionnaire was determined using Cronbach's alpha coefficient, and the stability using the test-retest.
Results
On scale A of the questionnaire we found that three of 19 items were unstable, in 8 of 26 on scale B, and 3 of 38 items on scale C. The Cronbach's alpha value of scale A was 0.722, 0.861 on scale B, and 0,663 on scale C.
Conclusions
The 3 scales are within the appropriate values, except scale C, which is very close. The findings of this validation process can serve as a reference that may be extrapolated to the UNIMEDIFAM national questionnaire

Age and date for early arrival of the Acheulian in Europe (Barranc de la Boella, la Canonja, Spain)
The first arrivals of hominin populations into Eurasia during the Early Pleistocene are currently considered to have occurred
as short and poorly dated biological dispersions. Questions as to the tempo and mode of these early prehistoric settlements
have given rise to debates concerning the taxonomic significance of the lithic assemblages, as trace fossils, and the
geographical distribution of the technological traditions found in the Lower Palaeolithic record. Here, we report on the
Barranc de la Boella site which has yielded a lithic assemblage dating to ,1 million years ago that includes large cutting
tools (LCT). We argue that distinct technological traditions coexisted in the Iberian archaeological repertoires of the late
Early Pleistocene age in a similar way to the earliest sub-Saharan African artefact assemblages. These differences between
stone tool assemblages may be attributed to the different chronologies of hominin dispersal events. The archaeological
record of Barranc de la Boella completes the geographical distribution of LCT assemblages across southern Eurasia during
the EMPT (Early-Middle Pleistocene Transition, circa 942 to 641 kyr). Up to now, chronology of the earliest European LCT
assemblages is based on the abundant Palaeolithic record found in terrace river sequences which have been dated to the
end of the EMPT and later. However, the findings at Barranc de la Boella suggest that early LCT lithic assemblages appeared
in the SW of Europe during earlier hominin dispersal episodes before the definitive colonization of temperate Eurasia took
