    Rak płuca u pacjenta po resekcji gruźliczaka

    W pracy przedstawiono przypadek chorego, u którego 7 lat po resekcji gruźliczaka prawego płuca stwierdzono raka gruczołowego w tym samym płucu. Diagnozę postawiono w wyniku zaobserwowania powiększania się niebudzącej podejrzeń zmiany. Nie wiadomo, czy nowotwór był wynikiem przebudowy nowotworowej jednego z gruźliczaków, czy też zmianą pierwotną, uznaną za gruźliczak. Zastosowano leczenie operacyjne, z dobrym efektem w 4-letniej obserwacji.The paper presents a case of right lung carcinoma diagnosed 7 years after tuberculoma excision performed in the same lung. The diagnosis was based on observing the increase of a non suspicious mass. It is unclear whether the cancer developed from metaplasia, or was a primary tumor, mistaken for a tuberculoma. Patient underwent a successful excision of the neoplastic mass, with 4-year recurrence free survival

    WSTĘP. Celem niniejszej pracy była ocena występowania zaburzeń gospodarki węglowodanowej u krewnych I stopnia chorych na cukrzycę typu 2 zależnie od obecności innych czynników ryzyka tej choroby. MATERIAŁ I METODY. Ocenianą grupę tworzyło 42 krewnych I stopnia chorych leczonych w Przyklinicznej Poradni Diabetologicznej Szpitala Uniwersyteckiego w Bydgoszczy. Przeprowadzano badanie podmiotowe, przedmiotowe oraz test doustnego obciążenia 75 g glukozy. WYNIKI. U 13 osób (31%) rozpoznano zaburzenia gospodarki węglowodanowej (grupa ZGW), zaś u 29 ich nie stwierdzono (grupa BZGW). Badani z grupy ZGW byli istotnie statystycznie starsi od osób z grupy BZGW (59,8 &#177; 14,6 vs. 37,8 &#177; 15,5 roku; p < 0,001), cechowały ich znamiennie większe rodzinne obciążenie cukrzycą typu 2 (2,0 &#177; 1,1 vs. 1,1 &#177; 0,4; p < 0,001 - liczba krewnych w rodzinie) i istotnie wyższa liczba czynników ryzyka choroby (2,9 &#177; 1,2 vs. 2,0 &#177; 1,2; p < 0,01). Najczęstsze, istotne statystycznie okazało się występowanie w wywiadzie nieprawidłowego stężenia glukozy we krwi - 61,5% (BZGW - 6,9%). Obie grupy nie różniły się pod względem liczby chorych rodziców, natomiast rodzeństwo chore na cukrzycę miało 92,3% osób z grupy ZGW i tylko 13,8% z grupy BZGW. Przynajmniej jedno dziecko chore na cukrzycę miało 23% osób z grupy ZGW, natomiast nikt z BZGW. Nie wykazano istotnych różnic między grupami odnośnie wskaźnika masy ciała (28,3 &#177; &#177; 4,5 kg/m2 vs. 25,9 &#177; 4,9 kg/m2) oraz wskaźnika talia- biodro (0,86 &#177; 0,08 vs. 0,83 &#177; 0,07, odpowiednio). Cukrzyca ciążowa wystąpiła u 7,7% vs. 6,9% badanych, urodzenie dziecka z masą ciała powyżej 4000 g dotyczyło 7,7% versus 11,9%, a zespół policystycznych jajników stwierdzono u 7,7% versus 3,4%. WNIOSKI. U krewnych I stopnia chorych na cukrzycę typu 2 wystąpienie zaburzeń gospodarki węglowodanowej zależało od: wieku, sumy czynników ryzyka cukrzycy, a zwłaszcza stwierdzenia nieprawidłowych wartości glikemii w wywiadzie, liczby krewnych chorych na cukrzycę typu 2, w szczególności chorego rodzeństwa i dzieci.INTRODUCTION. The aim of the study was the evaluation of glucose metabolism disturbances in I° relatives of the patients with type 2 diabetes, depending on the other present risk factors. MATERIAL AND METHODS. The evaluated group consisted of 42 I° relatives of the patients treated in Outpatient Diabetology Clinic of the University Hospital in Bydgoszcz. A subjective, objective examination and the oral glucose tolerance test were carried out. RESULTS. In 13 patients (31%) glucose metabolism disturbances (ZGW group) were identified, in 29 they were not stated (BZGW group). The examined ZGW group patients were considerably statistically older than the BZGW (59.8 &#177; 14.6 vs. 37.8 &#177; 15.5 years; p < 0.001), had a remarkably greater inherited susceptibility to type 2 diabetes (2.0 &#177; 1.1 vs. 1.1 &#177; &#177; 0.4; p < 0.001 of the number of relatives in the family) and were characterized by a significantly higher number of the diabetes risk factors (2.9 &#177; 1.2 vs. 2.0 &#177; 1.2; p < 0.01). The occurrence of the abnormal glycaemia - 61.5% (BZGW - 6.9%) in the anamnesis turned out to be the most common and statistically important. Both groups did not differ in the number of parents with diabetes, whereas 92.3% patients of the ZGW and only 13.8% of the BZGW had siblings with diabetes. 23% people with ZGW had at least one child with diabetes while no-one thase with BZGW. No significant differences between the groups, regarding BMI were stated: 28.3 &#177; 4.5 kg/m2 versus 25.9 &#177; 4.9 kg/m2 and WHR: 0.86 &#177; 0.08 versus 0.83 &#177; &#177; 0.07 respectively. Gestational diabetes mellitus occurred in 7.7% versus 6.9%, giving birth to a newborn with birth weight above 4000 g: 7.7% versus 11.9%, polycystic ovary syndrome: 7.7% versus 3.4%. CONCLUSIONS. In the I° relatives of the patients with type 2 diabetes the occurrence of glucose metabolism disturbances depended on: age, the amount of diabetes risk factors, especially stating abnormal glycaemia values in the anamnesis, the number of relatives with type 2 diabetes, siblings and children in particular

    Kinesio taping: application and results on pain: systematic review

    Kinesio taping is being increasingly used under several conditions; however, its hypoalgesic effect and mechanism of action do not show scientific results. Therefore, the aims of this study were to describe the principles of the method and mainly to analyze the results of clinical trials with control groups, related to pain with Kinesio taping. The PubMed, SciELO, Lilacs, Scirus and Academic Google databases were consulted, using the keywords: Kinesio Taping and Kinesio Tape, either associated or not to pain. Ten clinical trials related to pain and Kinesio taping were selected, assessed by using the PEDro scale, and its results were analyzed. Kinesio taping provided higher, similar or lower pain reduction than in other groups (control, placebo or technique). The application continues to use the principles of Kenzo Kase, the gate control theory being the most described so far to justify the hypoalgesic effect; and the effects were only found in short-term (24 hours to 1 week). Pain relief provided by Kinesio taping was similar or slightly superior to other interventions, not representing a reason for it to be the main treatment of choice. However, it can be considered an adjunct or complementary technique

    Immune cell counts and risks of respiratory infections among infants exposed pre- and postnatally to organochlorine compounds: a prospective study

    <p>Abstract</p> <p>Background</p> <p>Early-life chemical exposure may influence immune system development, subsequently affecting child health. We investigated immunomodulatory potentials of polychlorinated biphenyls (PCBs) and <it>p,p'</it>-DDE in infants.</p> <p>Methods</p> <p>Prenatal exposure to PCBs and <it>p,p'</it>-DDE was estimated from maternal serum concentrations during pregnancy. Postnatal exposure was calculated from concentrations of the compounds in mother's milk, total number of nursing days, and percentage of full nursing each week during the 3 month nursing period. Number and types of infections among infants were registered by the mothers (N = 190). White blood cell counts (N = 86) and lymphocyte subsets (N = 52) were analyzed in a subgroup of infants at 3 months of age.</p> <p>Results</p> <p>Infants with the highest prenatal exposure to PCB congeners CB-28, CB-52 and CB-101 had an increased risk of respiratory infection during the study period. In contrast, the infection odds ratios (ORs) were highest among infants with the lowest prenatal mono-<it>ortho </it>PCB (CB-105, CB-118, CB-156, CB-167) and di-<it>ortho </it>PCB (CB-138, CB-153, CB-180) exposure, and postnatal mono- and di-<it>ortho </it>PCB, and <it>p,p'</it>-DDE exposure. Similar results were found for pre- and postnatal CB-153 exposure, a good marker for total PCB exposure. Altogether, a negative relationship was indicated between infections and total organochlorine compound exposure during the whole pre- and postnatal period. Prenatal exposure to CB-28, CB-52 and CB-101 was positively associated with numbers of lymphocytes and monocytes in infants 3 months after delivery. Prenatal exposure to <it>p,p'</it>-DDE was negatively associated with the percentage of eosinophils. No significant associations were found between PCB and <it>p,p'</it>-DDE exposure and numbers/percentages of lymphocyte subsets, after adjustment for potential confounders.</p> <p>Conclusion</p> <p>This hypothesis generating study suggests that background exposure to PCBs and <it>p,p'</it>-DDE early in life modulate immune system development. Strong correlations between mono- and di-<it>ortho </it>PCBs, and <it>p,p'</it>-DDE exposures make it difficult to identify the most important contributor to the suggested immunomodulation, and to separate effects due to pre- and postnatal exposure. The suggested PCB and <it>p,p'</it>-DDE modulation of infection risks may have consequences for the health development during childhood, since respiratory infections early in life may be risk factors for asthma and middle ear infections.</p

    Kinesio Taping reduces pain and improves disability in Low Back Pain patients: a randomised controlled trial

    Objectives: Investigate the effects of Kinesio Taping® (KT) on chronic nonspecific low back pain (LBP) Design: Randomised controlled trial with intention-to-treat analysis. Setting: University laboratory. Participants: One hundred eight women with chronic nonspecific LBP underwent an evaluation pre, three and ten days after intervention. Interventions: After randomization, participants were assigned in four groups: KT with tension group (KTT) applied Kinesio Taping® with tension in the region of the erector spinae muscles; KT no tension group (KTNT) applied Kinesio Taping® with no tension at the same region; Micropore® group (MP) applied Micropore® tape on the erector spinae muscles; and Control group (CG) did not receive any intervention. Main outcome measures: The primary outcome was pain sensation, measured by numerical pain rating scale. Secondary outcomes were: disability, trunk range of motion, strength and electromyographic amplitude, measured by Roland Morris Disability questionnaire, inclinometry, dynamometry and electromyography, respectively. Results: Pain relief was observed for KTT group (mean difference=1,963; CI 95%=0,501 - 3,425; p=0,003) and KTNT group (mean diference=1,926; CI 95%=0,464 - 3,388; p=0,004) compared to control group at 3 days after application of the tape. For disability there was difference between control group and KTT group at 3 (mean difference=3,481; CI 95%=0,825 – 6,138; p=0,004) and 10 days (mean difference=3,185; CI 95%=0,395 - 5,975; p=0,016). For all the others variables, there was no differences between group. Conclusion: KT with or without tension reduces pain 3 days after its application. Additionally, when applied with tension it improves disability after 3 and 10 days in LBP patients

    Niniejsza praca magisterska to porównanie korzystania z mediów na przełomie lat 80-tych i 90-tych z obecnymi czasami. Oparte jest ono na wielu źródłach, takich jak: autorska ankieta, książki, czasopisma, rządowe opracowania oraz materiały dostępne w sieci.W pierwszym rozdziale kompleksowo wyjaśniona jest definicja mediów masowych i ich przekazów. Drugi rozdział to charakterystyka korzystania z nich na przełomie lat 80-tych i 90-tych. Rozdział trzeci to metodologia i analiza badań przeprowadzanych za pomocą ankiety internetowej. W czwartym rozdziale znajduje się porównanie tego jak Polacy korzystali z mediów 25 lat temu, a jak korzystają obecnie na podstawie zgromadzonych informacji.Na podstawie przeanalizowanych źródeł oraz badań własnych nasuwa się wniosek, że Internet z biegiem czasu zastępuje tradycyjne media. Tempo tych zmian nie jest jednak bardzo dynamiczne i pozycja telewizji jako podstawowego medium pozostaje niezagrożona.The main purpose of this thesis is to compare the use of media at the turn of 80s and 90s to the present times. It is based on many sources such as: author's survey, books, magazines, governmental studies and also materials that can be found online. In the first chapter the definition of mass media and their transmission is comprehensively explained. The second chapter concerns the characteristics of their usa at the turn of 80s and 90s. The third chapter is the methodology and analisis of the research conducted by means of the Internet survey. In the fourth chapter we can find a comparision of tha way Polish people made use of media twenty-five years ago and at present – on the basis of information gathered. Taking into consideration all of analised own researches it can be concluded that as the time goes on, the Internet is replacing the traditional media The pace of these changes is not so dynamic and the television as a basic media stands unthreatened
