    Systematic review: Do patient expectations influence treatment outcomes in total knee and total hip arthroplasty?

    <p>Abstract</p> <p>Objective</p> <p>This systematic review aims to summarise all the available evidence related to the association between pre-operative patient expectations (outcome expectations, process expectations and self efficacy expectations) and 5 different treatment outcomes (overall improvement, pain, function, stiffness and satisfaction) in patients with total knee or total hip arthroplasty at three different follow-op periods (>6 weeks; >6 weeks- ≤6 months; >6 months).</p> <p>Methods</p> <p>English and Dutch language articles were identified through PubMed, EMBASE.com, PsycINFO, CINAHL and The Cochrane Library from inception to September 2012. Articles assessing the association between pre-operative patient expectations and treatment outcomes for TKA/THA in either adjusted or unadjusted analysis were included. Two reviewers, working independently, determined eligibility, rated methodological quality and extracted data on study design, population, expectation measurements, outcome measurements and strength of the associations. Methodological quality was rated by the same reviewers on a 19 item scale. The scores on the quality assessment were taken into account when drawing final conclusions.</p> <p>Results</p> <p>The search strategy generated 2252 unique references, 18 articles met inclusion criteria. Scores on the methodological quality assessment ranged between 6% and 79%. Great variety was seen in definitions and measurement methods of expectations. No significant associations were found between patient expectations and overall improvement, satisfaction and stiffness. Both significant positive and non-significant associations were found for the association between expectations and pain and function.</p> <p>Conclusions</p> <p>There was no consistency in the association between patients’ pre-operative expectations and treatment outcomes for TKA and THA indentified in this systematic review. There exists a need for a sound theoretical framework underlying the construct of ‘patient expectations’ and consistent use of valid measurement instruments to measure that construct in order to facilitate future research synthesis.</p

    Splintless surgery using patient-specific osteosynthesis in Le Fort I osteotomies:a randomized controlled multi centre trial

    The accuracy of orthognathic surgery has improved with three-dimensional virtual planning. The translation of the planning to the surgical result is reported to vary by >2 mm. The aim of this randomized controlled multi-centre trial was to determine whether the use of splintless patient-specific osteosynthesis can improve the accuracy of maxillary translation. Patients requiring a Le Fort I osteotomy were included in the trial. The intervention group was treated using patient-specific osteosynthesis and the control group with conventional osteosynthesis and splint-based positioning. Fifty-eight patients completed the study protocol, 27 in the patient-specific osteosynthesis group and 31 in the control group. The per protocol median anteroposterior deviation was found to be 1.05 mm (interquartile range (IQR) 0.45-2.72 mm) in the patient-specific osteosynthesis group and 1.74 mm (IQR 1.02-3.02 mm) in the control group. The cranial-caudal deviation was 0.87 mm (IQR 0.49-1.44 mm) and 0.98 mm (IQR 0.28-2.10 mm), respectively, whereas the left-right translation deviation was 0.46 mm (IQR 0.19-0.96 mm) in the patient-specific osteosynthesis group and 1.07 mm (IQR 0.62-1.55 mm) in the control group. The splintless patient-specific osteosynthesis method improves the accuracy of maxillary translations in orthognathic surgery and is clinically relevant for planned anteroposterior translations of more than 3.70 mm

    Adaptation of a Vocabulary Test from British Sign Language to American Sign Language

    This study describes the adaptation process of a vocabulary knowledge test for British Sign Language (BSL) into American Sign Language (ASL) and presents results from the first round of pilot testing with twenty deaf native ASL signers. The web-based test assesses the strength of deaf children’s vocabulary knowledge by means of different mappings of phonological form and meaning of signs. The adaptation from BSL to ASL involved nine stages, which included forming a panel of deaf/hearing experts, developing a set of new items and revising/replacing items considered ineffective, and piloting the new version. Results provide new evidence in support of the use of this methodology for assessing sign language, making a useful contribution toward the availability of tests to assess deaf children’s signed language skills

    In potentie zou stadslandbouw leegstaande kantoorgebouwen een nieuwe bestemming kunnen geven. Op papier zijn er volop kansen: veel leegstand, productieruimte dicht bij stedelijke afzetmarkt, maatschappelijke belangstelling voor lokale producten en beschikbaarheid van geavanceerde teelttechnieken (LED, aquaponics). Uit verschillende hoeken (o.a. Provinciale Staten Utrecht en de gemeenteraad van Nieuwegein) komt de vraag of stadslandbouw leegstaande kantoren en industrieterreinen inderdaad weer een duurzame, productieve en maatschappelijke bestemming kan geven. Hoofdvraag voor de provincie Utrecht is: is stadslandbouw een substantieel alternatief voor leegstaande kantoorgebouwen? Het rapport richt zich op de plantaardige productie (incl. biobased productie), aquacultuur en de kweek van insecten, omdat hiervan de meeste expertise is en omdat deze vormen van stadslandbouw maatschappelijk geaccepteerd zijn. Benutting van daken, (buiten-) wanden of buitenruimten van gebouwen voor landbouw krijgt beperkt aandacht in deze studie

    Transient Grating Spectroscopy in Magnetic Thin Films:Simultaneous Detection of Elastic and Magnetic Dynamics

    Surface magnetoelastic waves are coupled elastic and magnetic excitations that propagate along the surface of a magnetic material. Ultrafast optical techniques allow for a non-contact excitation and detection scheme while providing the ability to measure both elastic and magnetic components individually. Here we describe a simple setup suitable for excitation and time resolved measurements of high frequency magnetoelastic waves, which is based on the transient grating technique. The elastic dynamics are measured by diffracting a probe laser pulse from the long-wavelength spatially periodic structural deformation. Simultaneously, a magnetooptical measurement, either Faraday or Kerr effect, is sensitive to the out-of-plane magnetization component. The correspondence in the response of the two channels probes the resonant interaction between the two degrees of freedom and reveals their intimate coupling. Unraveling the observed dynamics requires a detailed understanding of the spatio-temporal evolution of temperature, magnetization and thermo-elastic strain in the ferromagnet. Numerical solution of thermal diffusion in two dimensions provides the basis on which to understand the sensitivity in the magnetooptic detection
