    Rotura de cotilo en artroplastia total de cadera

    Describimos dos casos de rotura de cotilos de expansión de prótesis de cadera tipo CLS de origen no traumático. Ambos casos sucedieron a los seis años de cirugía y no se apreció desgaste o rotura del polietileno en ninguno de ellos. En nuestra opinión la hipótesis más probable es que la falta de contacto entre la zona polar del cotilo y el fondo del acetábulo produzca una zona de inestabilidad en los segmentos de la prótesis y rotura por fatiga del material. Creemos que es obligado su recambio aunque el paciente no presente sintomatología actual, ya que la rotura del acetábulo conducirá a un aflojamiento y a una osteolisis secundaria.We describe two cases of breakage of expansion cups in a CLS uncemented total hip arthroplasty wihtout traumatic etiologic. Both cases occurred six years after the surgery and none had breakage or polyethylene wear. We postulate that the lack of contact between the polar area of the cup and the acetabulum produces a zone of instability in the segments of the prosthesis and increases the risk of fatigue of the material. In our opinion acetabular revision is mandatory in this situation, even in asymptomatic cases, because cup rupture leads to implant loosening with secondary short-team osteolysis

    El objetivo de nuestro estudio es conocer las características fisiológicas de los jugadores juveniles de bádminton de alto nivel y comparar los parámetros fisiológicos obtenidos en el laboratorio y durante un partido. Se estudiaron 19 jugadores en edad juvenil, 12 varones y 7 mujeres. A todos se les realizó una prueba de esfuerzo máxima en el laboratorio y mediciones antropométricas. Durante la competición se les monitorizó la frecuencia cardiaca, se analizó la concentración de lactato y se valoró su percepción subjetiva al esfuerzo (RPE). El consumo máximo de oxigeno (VO2 máx.) medio se situaba en 56,07 +/- 6,5 ml/Kg/min. El pico de lactato en 3,18 +/- 1,78 mML-1 y la frecuencia cardiaca máxima media (FC máx.) era de 196,75 +/- 5,29 p. p. m., sin diferencias significativas en ninguno de los parámetros estudiados entre varones y hembras en el laboratorio y la competiciónThe aim of our study was to determine the physiological characteristics of young badminton high level players and to compare the physiological parameters obtained in the laboratory and during a match.     Periodontitis and vascular inflammatory biomarkers: an experimental in vivo study in rats

    The objective of this preclinical in vivo study was to determine changes in vascular inflammatory biomarkers in systemic circulation after injection of lipopolysaccharide (LPS) from Porphyromonas gingivalis (Pg) in rats. Experimental periodontitis was induced by injections of Pg-LPS. Gingival soft and hard tissues changes were analysed by means of magnetic resonance imaging and micro computed tomography. Serum levels of interleukin (IL)-6, IL-10, pentraxin (PTX) 3, and soluble fragment of tumor necrosis factor-like weak inducer of apoptosis (sTWEAK) were determined at baseline and 24 h, 7, 14, and 21 days after periodontal induction. Significant periodontal inflammation and alveolar bone loss were evident at the end of periodontal induction. Experimental periodontitis posed an acute systemic inflammatory response with increased serum levels of IL-6 and PTX3 at 24 h post-induction, followed by a significant overexpression of sTWEAK at 7 days. This inflammatory state was maintained until the end of the experiment (21 days). As expected, IL-10 serum levels were significantly lower during the follow-up compared to baseline concentrations. In the present animal model, experimental periodontitis is associated with increased systemic inflammation. Further studies are needed to confirm whether PTX3 and sTWEAK could be useful biomarkers to investigate potential mechanisms underlying the relationship between periodontitis and atherosclerotic vascular diseases

    The Tulczyjew triple for classical fields

    The geometrical structure known as the Tulczyjew triple has proved to be very useful in describing mechanical systems, even those with singular Lagrangians or subject to constraints. Starting from basic concepts of variational calculus, we construct the Tulczyjew triple for first-order Field Theory. The important feature of our approach is that we do not postulate {\it ad hoc} the ingredients of the theory, but obtain them as unavoidable consequences of the variational calculus. This picture of Field Theory is covariant and complete, containing not only the Lagrangian formalism and Euler-Lagrange equations but also the phase space, the phase dynamics and the Hamiltonian formalism. Since the configuration space turns out to be an affine bundle, we have to use affine geometry, in particular the notion of the affine duality. In our formulation, the two maps α\alpha and β\beta which constitute the Tulczyjew triple are morphisms of double structures of affine-vector bundles.     Quantification of inaccurate diagnosis of COPD in primary care medicine: An analysis of the COACH clinical audit

    Background: Inaccurate diagnosis in COPD is a current problem with relevant consequences in terms of inefficient health care, which has not been thoroughly studied in primary care medicine. The aim of the present study was to evaluate the degree of inaccurate diagnosis in Primary Care in Spain and study the determinants associated with it. Methods: The Community Assessment of COPD Health Care (COACH) study is a national, observational, randomized, non-interventional, national clinical audit aimed at evaluating clinical practice for patients with COPD in primary care medicine in Spain. For the present analysis, a correct diagnosis was evaluated based on previous exposure and airway obstruction with and without the presence of symptoms. The association of patient-level and center-level variables with inaccurate diagnosis was studied using multivariate multilevel binomial logistic regression models. Results: During the study 4,307 cases from 63 centers were audited. The rate of inaccurate diagnosis was 82.4% (inter-regional range from 76.8% to 90.2%). Patient-related interventions associated with inaccurate diagnosis were related to active smoking, lung function evaluation, and specific therapeutic interventions. Center-level variables related to the availability of certain complementary tests and different aspects of the resources available were also associated with an inaccurate diagnosis. Conclusions: The prevalence data for the inaccurate diagnosis of COPD in primary care medicine in Spain establishes a point of reference in the clinical management of COPD. The descriptors of the variables associated with this inaccurate diagnosis can be used to identify cases and centers in which inaccurate diagnosis is occurring considerably, thus allowing for improvement

    Variations of training load, monotony, and strain and dose-response relationships with maximal aerobic speed, maximal oxygen uptake, and isokinetic strength in professional soccer players

    This study aimed to identify variations in weekly training load, training monotony, and training strain across a 10-week period (during both, pre- and in-season phases); and to analyze the dose-response relationships between training markers and maximal aerobic speed (MAS), maximal oxygen uptake, and isokinetic strength. Twenty-seven professional soccer players (24.9±3.5 years old) were monitored across the 10-week period using global positioning system units. Players were also tested for maximal aerobic speed, maximal oxygen uptake, and isokinetic strength before and after 10 weeks of training. Large positive correlations were found between sum of training load and extension peak torque in the right lower limb (r = 0.57, 90%CI[0.15;0.82]) and the ratio agonist/antagonist in the right lower limb (r = 0.51, [0.06;0.78]). It was observed that loading measures fluctuated across the period of the study and that the load was meaningfully associated with changes in the fitness status of players. However, those magnitudes of correlations were small-to-large, suggesting that variations in fitness level cannot be exclusively explained by the accumulated load and loading profile

    Conserving Ecosystem Diversity in the Tropical Andes

    Documenting temporal trends in the extent of ecosystems is essential to monitoring their status but combining this information with the degree of protection helps us assess the effectiveness of societal actions for conserving ecosystem diversity and related ecosystem services. We demonstrated indicators in the Tropical Andes using both potential (pre-industrial) and recent (~2010) distribution maps of terrestrial ecosystem types. We measured long-term ecosystem loss, representation of ecosystem types within the current protected areas, quantifying the additional representation offered by protecting Key Biodiversity Areas. Six (4.8%) ecosystem types (i.e., measured as 126 distinct vegetation macrogroups) have lost >50% in extent across four Andean countries since pre-industrial times. For ecosystem type representation within protected areas, regarding the pre-industrial extent of each type, a total of 32 types (25%) had higher representation (>30%) than the post-2020 Convention on Biological Diversity (CBD) draft target in existing protected areas. Just 5 of 95 types (5.2%) within the montane Tropical Andes hotspot are currently represented with >30% within the protected areas. Thirty-nine types (31%) within these countries could cross the 30% CBD 2030 target with the addition of Key Biodiversity Areas. This indicator is based on the Essential Biodiversity Variables (EBV) and responds directly to the needs expressed by the users of these countries

    Ancient DNA reveals the lost domestication history of South American camelids in northern Chile and across the Andes

    The study of South American camelids and their domestication is a highly debated topic in zooarchaeology. Identifying the domestic species (alpaca and llama) in archaeological sites based solely on morphological data is challenging due to their similarity with respect to their wild ancestors. Using genetic methods also presents challenges due to the hybridization history of the domestic species, which are thought to have extensively hybridized following the Spanish conquest of South America that resulted in camelids slaughtered en-masse. In this study we generated mitochondrial genomes for 61 ancient South American camelids dated between 3,500 - 2,400 years before the present (Early Formative period) from two archaeological sites in Northern Chile (Tulán-54 and Tulán-85), as well as 66 modern camelid mitogenomes and 815 modern mitochondrial control region sequences from across South America. In addition, we performed osteometric analyses to differentiate big and small body size camelids. A comparative analysis of these data suggests that a substantial proportion of the ancient vicuña genetic variation has been lost since the Early Formative period as it is not present in modern specimens. Moreover, we propose a domestication hypothesis that includes an ancient guanaco population that no longer exists. Finally, we find evidence that interbreeding practices were widespread during the domestication process by the early camelid herders in the Atacama during the Early Formative period and predating the Spanish conquest
