    Erythropoietin as a protective factor in rat CNS cells receiving radiotherapy: An in vitro study

    Objetivo. Investigar el efecto de la eritropoyetina en cultivos celulares de corteza cerebral de ratas cuando se administra radioterapia. Materiales y métodos. El estudio se desarrolla con la obtención de corteza cerebral de embriones de 17 días de preñez de ratas Wistar. Las células cultivadas después de 72 horas de la extracción de la corteza se dividieron en dos grupos, a uno de ellos se le administró eritropoyetina alfa a una concentración de 30 pM y el otro era el grupo control. A los dos grupos de células se les radió con 6 Gy mediante un aparato Phoenix. Tras la radioterapia permanecieron 24 horas en la incubadora antes de fijarlas. Las células fueron fijadas con formaldehído al 4%. A continuación, con la técnica de TUNEL, se valoró el número de células apoptóticas en los cultivos radiados. Resultados. Se observó un porcentaje de apoptosis del 25,22% del grupo de cultivo sin eritropoyetina, mientras que en el grupo de células radiadas con eritropoyetina fue del 15,5%. Las variables cuantitativas se analizaron mediante el test t de Student y el resultado de la comparación entre los dos grupos fue estadísticamente significativo (p < 0,0001). Conclusión. En nuestro modelo experimental in vitro se comprobó que la eritropoyetina es eficaz en la prevención de la apoptosis en células del sistema nervioso central de ratas por radiación.     Gene expression analysis of aberrant signaling pathways in meningiomas

    Examining aberrant pathway alterations is one method for understanding the abnormal signals that are involved in tumorigenesis and tumor progression. In the present study, expression arrays were performed on tumor-related genes in meningiomas. The GE Array Q Series HS-006 was used to determine the expression levels of 96 genes that corresponded to six primary biological regulatory pathways in a series of 42 meningiomas, including 32 grade I, four recurrent grade I and six grade II tumors, in addition to three normal tissue controls. Results showed that 25 genes that were primarily associated with apoptosis and angiogenesis functions were downregulated and 13 genes frequently involving DNA damage repair functions were upregulated.     Pediatric televisits and telephone triage: impact on use of a hospital emergency department

    Objetivos: analizar las características de las teleconsultas y triajes telefónicos pediátricos atendidos por CatSalut Respon y describir su impacto sobre la actitud y la decisión final de los padres-usuarios de acudir o no a urgencias. Método: estudio observacional transversal. Durante la teleconsulta los pacientes se clasificaron según los niveles del sistema español de triaje. Aquellos que fueron derivados a urgencias se volvieron a clasificar en el hospital, y se compararon los niveles de triaje. Posteriormente, se realizó una llamada de verificación. Se recogieron variables sociodemográficas y clínicas. Resultados: se analizaron 370 teleconsultas, fundamentalmente no urgentes (n = 300; 81%). Un 20,3% (n = 75) fueron derivadas a urgencias. La fiebre (p = 0,002) y las dudas de medicación (p < 0,001) fueron motivos significativos de teleconsulta no urgente. Casi un 46% de los casos con niveles de gravedad altos en el triaje de la llamada también fueron clasificados con niveles de gravedad altos en el triaje posterior realizado en el servicio de urgencias hospitalario, mostrando una concordancia moderada. Más del 50% de los padres tenían intención de acudir a urgencias antes de la teleconsulta y un 46% cambiaron de actitud tras realizar esta llamada. Conclusiones: fiebre y dudas de medicación fueron motivos estadísticamente significativos de teleconsulta no urgente. La consulta telefónica produjo un cambio de actitud en casi la mitad de los padres

    Malformations of the craniocervical junction (chiari type I and syringomyelia: classification, diagnosis and treatment)

    Chiari disease (or malformation) is in general a congenital condition characterized by an anatomic defect of the base of the skull, in which the cerebellum and brain stem herniate through the foramen magnum into the cervical spinal canal. The onset of Chiari syndrome symptoms usually occurs in the second or third decade (age 25 to 45 years). Symptoms may vary between periods of exacerbation and remission. The diagnosis of Chiari type I malformation in patients with or without symptoms is established with neuroimaging techniques. The most effective therapy for patients with Chiari type I malformation/syringomyelia is surgical decompression of the foramen magnum, however there are non-surgical therapy to relieve neurophatic pain: either pharmacological and non-pharmacological. Pharmacological therapy use drugs that act on different components of pain. Non-pharmacological therapies are primarly based on spinal or peripheral electrical stimulation

    Multi-Centre, Multi-Vendor and Multi-Disease Cardiac Segmentation: The M&Ms Challenge

    The emergence of deep learning has considerably advanced the state-of-the-art in cardiac magnetic resonance (CMR) segmentation. Many techniques have been proposed over the last few years, bringing the accuracy of automated segmentation close to human performance. However, these models have been all too often trained and validated using cardiac imaging samples from single clinical centres or homogeneous imaging protocols. This has prevented the development and validation of models that are generalizable across different clinical centres, imaging conditions or scanner vendors. To promote further research and scientific benchmarking in the field of generalizable deep learning for cardiac segmentation, this paper presents the results of the Multi-Centre, Multi-Vendor and Multi-Disease Cardiac Segmentation (M&Ms) Challenge, which was recently organized as part of the MICCAI 2020 Conference. A total of 14 teams submitted different solutions to the problem, combining various baseline models, data augmentation strategies, and domain adaptation techniques. The obtained results indicate the importance of intensity-driven data augmentation, as well as the need for further research to improve generalizability towards unseen scanner vendors or new imaging protocols. Furthermore, we present a new resource of 375 heterogeneous CMR datasets acquired by using four different scanner vendors in six hospitals and three different countries (Spain, Canada and Germany), which we provide as open-access for the community to enable future research in the field

    Beyond Classic Anastomoses Training Models: Overview of Aneurysm Creation in Rodent Vessel Model.

    Nowadays, due to the decline in the number of microsurgical clippings for cerebral aneurysms and revascularization procedures, young neurosurgeons have fewer opportunities to participate and train on this type of surgery. Vascular neurosurgery is a demanding subspecialty that requires skills that can only be acquired with technical experience. This background pushes the new generations to be ready for such challenging cases by training hard on different available models, such as synthetic tubes, chicken wings, or placenta vessels. Although many training models for vascular neurosurgery have been described worldwide, one of the best is the rodent vessels model. It offers pulsation, coagulation, and real blood flow conditions in a physiologic atmosphere that mimics perfectly the intracranial human vessels environment, especially in terms of size. However, the current differences in governmental different regulations about the use of living animals in medical experimentation and the social awareness, as well as the lack of financial support, cause more difficulties for neurosurgeons to start with that kind of training. In this review, we describe the tools and techniques as basic steps for vascular microsurgery training by using rodent models, that provide an accurate copy of brain vessels environment under stable conditions. The initial three classical known microanastomoses for neurosurgeons are end-to-end, end-to-side, and side-to-side, but in literature, there have been described other more complex exercises for training and investigation, such as aneurysm models. Although there is still little data available, we aim to summarize and discuss aneurysm\u27s training models and reviewed the current literature on the subject and its applications, including a detailed description of the techniques

    Metástasis intramedular como primera manifestación de un carcinoma renal

    Presentamos el caso de un varón de 69 años sin patología previa de interés que presentó clínica de pérdida de fuerza distal en miembros inferiores de dos meses de evolución. La resonancia magnética (RM) mostró la presencia de una masa intramedular a nivel del cono. El paciente fue intervenido practicándose resección de la tumoración, mostrando el estudio histológico la presencia de un carcinoma de células claras correspondiente a una sospecha de metástasis de un tumor renal, que posteriormente se confirmó con una tomografía axial computarizada (TAC) abdominal. Cuando el estado del paciente es bueno, la cirugía puede subsanar el déficit neurológico producido por la lesión intramedular. Nuestro paciente, tras la intervención, mejoró su estado neurológico; y actualmente, tras 14 meses, se mantiene sin déficit neurológico en miembros inferiores
