    Does physical activity change following hip and knee replacement? Matched case-control study evaluating Physical Activity Scale for the Elderly data from the Osteoarthritis Initiative

    Objectives: To determine whether physical activity measured using the Physical Activity Scale for the Elderly (PASE), changes during the initial 24 months post-total hip (THR) or knee replacement (TKR), and how this compares to a matched non-arthroplasty cohort. Design: Case-controlled study analysis of a prospectively collected dataset. Setting: USA community-based. Participants: 116 people post-THR, 105 people post-TKR compared to 663 people who had not undergone THR or TKR, or had hip or knee osteoarthritis. Cohorts were age-, gender- and BMI-matched. Main outcome measures: Physical activity assessed using the 12-item PASE at 12 and 24 months post-operatively. Results: There was no significant difference in total PASE score between pre-operative to 12 months (mean: 136 vs. 135 points; p=0.86) or 24 months following THR (mean: 136 vs 132 points; p=0.95). Whilst there was no significant difference in total PASE score from pre-operative to 12 months post-TKR (126 vs. 121 points; p=0.93), by 24 months people following TKR reported significantly greater physical activity (126 vs. 142 points; p=0.04). There was no statistically significant difference in physical activity between the normative matched and THR (p≥0.14) or TKR (p≥0.06) cohorts at 12 or 24 months post-joint replacement. Conclusions: Physical activity is not appreciably different to pre-operative levels at 12 or 24 months post-THR, but was greater at 24 months following TKR. Health promotion strategies are needed to encourage greater physical activity participation following joint replacement, and particularly targeting those who undergo THR

    Cluster X-ray luminosity-temperature relation at z>~1.5

    The evolution of the properties of the hot gas that fills the potential well of galaxy clusters is poorly known, since models are unable to give robust predictions and observations lack a sufficient redshift leverage and are affected by selection effects. Here, with just two high redshift, z approx 1.8, clusters avoiding selection biases, we obtain a significant extension of the redshift range and we begin to constrain the possible evolution of the X-ray luminosity vs temperature relation. The two clusters, JKC041 at z=2.2 and ISCSJ1438+3414 at z=1.41, are respectively the most distant cluster overall, and the second most distant that can be used for studying scaling relations. Their location in the X-ray luminosity vs temperature plane, with an X-ray luminosity 5 times lower than expected, suggests at the 95 % confidence that the evolution of the intracluster medium has not been self-similar in the last three quarters of the Universe age. Our conclusion is reinforced by data on a third, X-ray selected, high redshift cluster, too faint for its temperature when compared to a sample of similarly selected objects. Our data suggest that non-gravitational effects, such as the baryon physics, influence the evolution of galaxy cluster. Precise knowledge of evolution is central for using galaxy clusters as cosmological probes in planned X-ray surveys such as WFXT or JDEM.Comment: MNRAS, in press.     <p>Abstract</p> <p>Background</p> <p>The first cases of influenza A/H1N1 (swine flu) were confirmed in the UK on 27th April 2009, after a novel virus first identified in Mexico rapidly evolved into a pandemic. The swine flu outbreak was the first pandemic in more than 40 years and for many, their first encounter with a major influenza outbreak. This study examines public understandings of the pandemic, exploring how people deciphered the threat and perceived they could control the risks.</p> <p>Methods</p> <p>Purposive sampling was used to recruit seventy three people (61 women and 12 men) to take part in 14 focus group discussions around the time of the second wave in swine flu cases.</p> <p>Results</p> <p>These discussions showed that there was little evidence of the public over-reacting, that people believed the threat of contracting swine flu was inevitable, and that they assessed their own self-efficacy for protecting against it to be low. Respondents assessed a greater risk to their health from the vaccine than from the disease. Such findings could have led to apathy about following the UK Governments recommended health protective behaviours, and a sub-optimal level of vaccine uptake. More generally, people were confused about the difference between seasonal influenza and swine flu and their vaccines.</p> <p>Conclusions</p> <p>This research suggests a gap in public understandings which could hinder attempts to communicate about novel flu viruses in the future. There was general support for the government's handling of the pandemic, although its public awareness campaign was deemed ineffectual as few people changed their current hand hygiene practices. There was less support for the media who were deemed to have over-reported the swine flu pandemic.</p

    INTRODUCTION: Cabotegravir and rilpivirine (CAB+RPV long-acting (LA)) is recommended as a treatment for HIV-1 allowing people living with HIV to receive 2 monthly injectable treatment, rather than daily pills. Providing injectable therapy in a system designed to provide and manage study participants on oral treatments poses logistical challenges namely how resources are used to accommodate patient preference within constrained health economies with capacity limitations. In this pragmatic multicentre study, we aim to understand the implementation of CAB-RPV-LA administration in two settings via mixed methods to explore perspectives of participants and the clinical team delivering CAB+RPV LA. METHODS AND ANALYSIS: Women, racially minoritised people and older people are chronically under-represented in HIV clinical trials so the ILANA trial has set recruitment caps to ensure recruitment of 50% women, 50% ethnically diverse people and 30% over 50 years of age to include a more representative study population. Using a mixed-methods approach, the primary objective is to identify and evaluate the critical implementation strategies for CAB+RPV LA in both hospital and community settings. Secondary objectives include evaluating feasibility and acceptability of CAB+RPV LA administration at UK clinics and community settings from the perspective of HIV care providers, nurses and representatives at community sites, evaluating barriers to implementation, the utility of implementation strategies and adherence. ETHICS AND DISSEMINATION: Ethical approval has been obtained from the Health Research Authority Research Ethics Committee (REC reference: 22/PR/0318). The dissemination strategy has been formulated with the SHARE Collaborative Community Advisory Board to maximise the impact of this work on clinical care and policy. This strategy draws on and leverages existing resources within the participating organisations, such as their academic infrastructure, professional relationships and community networks.     Apportioning sources of organic matter in streambed sediments: An integrated molecular and compound-specific stable isotope approach

    We present a novel application for quantitatively apportioning sources of organic matter in streambed sediments via a coupled molecular and compound-specific isotope analysis (CSIA) of long-chain leaf wax n-alkane biomarkers using a Bayesian mixing model. Leaf wax extracts of 13 plant species were collected from across two environments (aquatic and terrestrial) and four plant functional types (trees, herbaceous perennials, and C3 and C4 graminoids) from the agricultural River Wensum catchment, UK. Seven isotopic (δ13C27, δ13C29, δ13C31, δ13C27–31, δ2H27, δ2H29, and δ2H27–29) and two n-alkane ratio (average chain length (ACL), carbon preference index (CPI)) fingerprints were derived, which successfully differentiated 93% of individual plant specimens by plant functional type. The δ2H values were the strongest discriminators of plants originating from different functional groups, with trees (δ2H27–29 = − 208‰ to − 164‰) and C3 graminoids (δ2H27–29 = − 259‰ to − 221‰) providing the largest contrasts. The δ13C values provided strong discrimination between C3 (δ13C27–31 = − 37.5‰ to − 33.8‰) and C4 (δ13C27–31 = − 23.5‰ to − 23.1‰) plants, but neither δ13C nor δ2H values could uniquely differentiate aquatic and terrestrial species, emphasizing a stronger plant physiological/biochemical rather than environmental control over isotopic differences. ACL and CPI complemented isotopic discrimination, with significantly longer chain lengths recorded for trees and terrestrial plants compared with herbaceous perennials and aquatic species, respectively. Application of a comprehensive Bayesian mixing model for 18 streambed sediments collected between September 2013 and March 2014 revealed considerable temporal variability in the apportionment of organic matter sources. Median organic matter contributions ranged from 22% to 52% for trees, 29% to 50% for herbaceous perennials, 17% to 34% for C3 graminoids and 3% to 7% for C4 graminoids. The results presented here clearly demonstrate the effectiveness of an integrated molecular and stable isotope analysis for quantitatively apportioning, with uncertainty, plant-specific organic matter contributions to streambed sediments via a Bayesian mixing model approach

    Discovery of the X-ray selected galaxy cluster XMMU J0338.8+0021 at z = 1.49 - Indications for a young system with a forming brightest galaxy

    We report on the discovery of a galaxy cluster at z = 1.490 originally selected as an extended X-ray source in the XMM-Newton Distant Cluster Project. Further observations carried out with the VLT-FORS2 spectrograph allowed the spectroscopic confirmation of seven secure cluster members, providing a median system redshift of z = 1.490 +/- 0.009. The color magnitude diagram of XMMU J0338.8+0021 reveals the presence of a well populated red sequence with z-H ~ 3, albeit with an apparent significant scatter in color. Since we do not detect indications for strong star formation activity in any of the objects, the color spread could indicate different stellar ages of the member galaxies. In addition, we found the brightest cluster galaxy in a very active dynamical state, with an interacting, merging companion located at a physical projected distance of d ~ 20kpc. From the X-ray luminosity we estimate a cluster mass of M200 ~ 1.2 x 10^(14) Msun. The data seem to suggest a scenario in which XMMU J0338.8+0021 is a young system, possibly caught in a moment of active ongoing mass assembly.Comment: 6 pages, 4 figures, 2 Table

    Baddies in the classroom: media education and narrative writing

    When teachers allow pupils to write stories that include elements of popular media, we must ask what to do with media once it has entered the classroom. This article relates findings from a classroom study which focuses on children’s media-based story writing. The study looks at children as producers of new media texts and describes their activities as a form of ‘media education’. The research shows that through their production of media-based stories, children are reflecting on their consumption of media. Furthermore, children’s media-based stories make explicit some of their implicit knowledge of new media forms. Finally, children’s stories provide ample opportunities for teachers to engage in important discussions about media within the framework of existing writing programmes

    New times, new politics: history and memory during the final years of the CPGB

    This article examines the relationship between collective memory, historical interpretation and political identity. It focuses on the dissolution of the Communist Party of Great Britain (CPGB) as constructed through collective narrative memory, and on Marxist interpretations of history. The divisions within the party and the wider Marxist community, stretching from 1956 until 1991, were often framed around questions of historical interpretation. The events of 1989–1991 created an historical and mnemonic crisis for CPGB members who struggled to reconcile their past identities with their present situation. Unlike the outward-facing revisionism of other political parties, this was an intensely personal affair. The solution for many was to emphasise the need to find new ways to progress socialist aims, without relying on a discredited grand narrative. In contrast, other Communist parties, such as the Communist Party of Britain, which had been established (or ‘re-established’) in 1988, fared rather better. By adhering to the international party line of renewal and continued struggle, the party was able to hold its narrative together, condemning the excesses of totalitarian regimes, while reaffirming the need for international class struggle

    The star formation history of mass-selected galaxies from the VIDEO survey

    © 2014 The Authors Published by Oxford University Press on behalf of the Royal Astronomical SocietyWe measure star formation rates (SFRs) and specific SFRs (SSFRs) of Ks-selected galaxies from the VISTA Deep Extragalactic Observations survey by stacking 1.4 GHz Very Large Array data.We split the sample, which spans 0 < z<3 and stellar masses 108.0 < M*/M⊙ < 1011.5, into elliptical, irregular or starburst galaxies based on their spectral energy distributions. We find that SSFR falls with stellar mass, in agreement with the 'downsizing' paradigm. We consider the dependence of the SSFR-mass slope on redshift: for our full and elliptical samples the slope flattens, but for the irregular and starburst samples the slope is independent of redshift. The rate of SSFR evolution reduces slightly with stellar mass for ellipticals, but irregulars and starbursts co-evolve across stellar masses. Our results for SSFR as a function of stellar mass and redshift are in agreement with those derived from other radio-stacking measurements of mass-selected passive and star-forming galaxies, but inconsistent with those generated from semi-analytic models, which tend to underestimate SFRs and SSFRs. There is a need for deeper high-resolution radio surveys such as those from telescopes like the next-generation MeerKAT in order to probe lower masses at earlier times and to permit direct detections, i.e. to study individual galaxies in detail.Peer reviewe
