    Substitution, delegation or addition? Implications of workforce skill mix on efficiency and interruptions in computed tomography

    Objectives: This study evaluated multiple computed tomography (CT) workforce models to identify any implications on efficiency (length of stay, scan frequency and workforce cost) and scanning radiographer interruptions through substituting or supplementing with a trained CT assistant. Methods: The study was conducted in a CT unit of a tertiary Queensland hospital and prospectively compared four workforce models, including usual practice: Model 1 used an administrative assistant (AA) and one radiographer Model 2 substituted a medical imaging assistant (MIA) for the AA Model 3 was usual practice, consisting of two radiographers and Model 4 included two radiographers, with a supplemented MIA. Observational data were collected over 7 days per model and were cross-checked against electronic records. Data for interruption type and frequency, as well as scan type and duration, were collected. Annual workforce costs were calculated as measures of efficiency. Results: Similar scan frequency and parameters (complexity) occurred across all models, averaging 164 scans (interquartile range 160-172 scans) each. The median times from patient arrival to examination completion in Models 1-4 were 47, 35, 46 and 33 min respectively. There were between 34 and 104 interruptions per day across all models, with the 'assistant role' fielding the largest proportion. Model 4 demonstrated the highest workforce cost, and Model 2 the lowest. Conclusion: This study demonstrated that assistant models offer similar patient throughput to usual practice at a reduced cost. Model 2 was the most efficient of all two-staff models (Models 1-3), offering the cheapest workforce, slightly higher throughput and faster examination times. Not surprisingly, the additional staff model (Model 4) offered greater overall examination times and throughput, with fewer interruptions, although workforce cost and possible role ambiguity were both limitations of this model. These findings may assist decision makers in selecting the optimal workforce design for their own individual contexts. What is known about the topic?: Innovative solutions are required to address ongoing health workforce sustainability concerns. Workforce substitution models using trained assistants have demonstrated numerous benefits internationally, with translation to the Australian allied health setting showing promise. What does this paper add?: Building on existing research, this study provides clinical workforce alternatives that maintain patient throughput while offering cost efficiencies. This study also quantified the many daily interruptions that occur within the CT setting, highlighting a potential clinical risk. To the best of our knowledge, this study is the first to empirically test the use of allied health assistants within CT. What are the implications for practitioners?: Role substitution in CT may offer solutions to skills shortages, increasing expenditure and service demand. Incorporating appropriate assistant workforce models can maintain throughput while demonstrating implications for efficiency and interruptions, potentially affecting staff stress and burnout. In addition, the assistant's scope and accepted level of interruptions should be considerations when choosing the most appropriate model

    Magnetic unmixing of first-order reversal curve diagrams using principal component analysis

    We describe a quantitative magnetic unmixing method based on principal component analysis (PCA) of first-order reversal curve (FORC) diagrams. For PCA we resample FORC distributions on grids that capture diagnostic signatures of single-domain (SD), pseudo-single-domain (PSD), and multidomain (MD) magnetite, as well as of minerals such as hematite. Individual FORC diagrams are recast as linear combinations of end-member (EM) FORC diagrams, located at user-defined positions in PCA space. The EM selection is guided by constraints derived from physical modeling and imposed by data scatter. We investigate temporal variations of two EMs in bulk North Atlantic sediment cores collected from the Rockall Trough and the Iberian Continental Margin. Sediments from each site contain a mixture of magnetosomes and granulometrically distinct detrital magnetite. We also quantify the spatial variation of three EM components (a coarse silt-sized MD component, a fine silt-sized PSD component, and a mixed clay-sized component containing both SD magnetite and hematite) in surficial sediments along the flow path of the North Atlantic Deep Water (NADW). These samples were separated into granulometric fractions, which helped constrain EM definition. PCA-based unmixing reveals systematic variations in EM relative abundance as a function of distance along NADW flow. Finally, we apply PCA to the combined dataset of Rockall Trough and NADW sediments, which can be recast as a four-EM mixture, providing enhanced discrimination between components.     Source-to-sink magnetic properties of NE Saharan dust in Eastern Mediterranean marine sediments: review and paleoenvironmental implications

    We present a review of the magnetic properties of NE Saharan dust that was conducted, following a source-to-sink approach, to unravel the paleoclimatic significance of environmental magnetic records from Eastern Mediterranean marine sediments. Our synthesis indicates that pedogenic hematite, which formed during past wetter Green Sahara periods (GSPs), is the most common magnetic mineral in Eastern Mediterranean marine sediments as a result of its eolian transportation, along with smaller amounts of lithogenic hematite, from the NE Sahara. Coupled with the limited impact of reductive sedimentary diagenesis on hematite abundances in Eastern Mediterranean Sea sediments, this indicates that hematite concentrations provide reliable quantitative estimates of NE Saharan dust supply. Our results indicate that variations in NE Saharan dust supply record an on-off mechanism in which a key control on eolian input is provided by the monsoon-driven spread and retreat of savannah vegetation through the Sahara. Magnetite/maghemite is also a common magnetic mineral in NE Saharan dust, and also formed mainly pedogenically during GSPs but in much smaller amounts compared to hematite. Limited production of magnetite/maghemite in the source area during GSPs, along with the later imprint of diagenetic processes, indicates that magnetite/maghemite abundances cannot be used to estimate dust inputs from the NE Sahara. Goethite cannot be used either to estimate variations in NE Saharan dust supply, because its occurrence in Eastern Mediterranean marine sediments also appears to be linked to fluvial inputs.     Coupled microbial bloom and oxygenation decline recorded by magnetofossils during the Palaeocene-Eocene Thermal Maximum

    Understanding marine environmental change and associated biological turnover across the Palaeocene–Eocene Thermal Maximum (PETM; ~56 Ma)—the most pronounced Cenozoic short-term global warming event—is important because of the potential role of the ocean in atmospheric CO2 drawdown, yet proxies for tracing marine productivity and oxygenation across the PETM are limited and results remain controversial. Here we show that a high-resolution record of South Atlantic Ocean bottom water oxygenation can be extracted from exceptionally preserved magnetofossils—the bioinorganic magnetite nanocrystals produced by magnetotactic bacteria (MTB) using a new multiscale environmental magnetic approach. Our results suggest that a transient MTB bloom occurred due to increased nutrient supply. Bottom water oxygenation decreased gradually from the onset to the peak PETM. These observations provide a record of microbial response to the PETM and establish the value of magnetofossils as palaeoenvironmental indicators

    Supersymmetry of Tensionless Rotating Strings in AdS_5 x S^5, and Nearly-BPS Operators

    It is shown that a class of rotating strings in AdS_5 x S^5 with SO(6) angular momenta (J,J',J') preserve 1/8-supersymmetry for large J,J', in which limit they are effectively tensionless; when J=0, supersymmetry is enhanced to 1/4.     Developing research priorities for palliative care of people with intellectual disabilities in Europe: a consultation process using nominal group technique

    BACKGROUND: Empirical knowledge around palliative care provision and needs of people with intellectual disabilities is extremely limited, as is the availability of research resources, including expertise and funding. This paper describes a consultation process that sought to develop an agenda for research priorities for palliative care of people with intellectual disabilities in Europe. METHODS: A two-day workshop was convened, attended by 16 academics and clinicians in the field of palliative care and intellectual disability from six European countries. The first day consisted of round-table presentations and discussions about the current state of the art, research challenges and knowledge gaps. The second day was focused on developing consensus research priorities with 12 of the workshop participants using nominal group technique, a structured method which involved generating a list of research priorities and ranking them in order of importance. RESULTS: A total of 40 research priorities were proposed and collapsed into eleven research themes. The four most important research themes were: investigating issues around end of life decision making; mapping the scale and scope of the issue; investigating the quality of palliative care for people with intellectual disabilities, including the challenges in achieving best practice; and developing outcome measures and instruments for palliative care of people with intellectual disabilities. CONCLUSIONS: The proposal of four major priority areas and a range of minor themes for future research in intellectual disability, death, dying and palliative care will help researchers to focus limited resources and research expertise on areas where it is most needed and support the building of collaborations. The next steps are to cross-validate these research priorities with people with intellectual disabilities, carers, clinicians, researchers and other stakeholders across Europe; to validate them with local and national policy makers to determine how they could best be incorporated in policy and programmes; and to translate them into actual research studies by setting up European collaborations for specific studies that require such collaboration, develop research proposals and attract research funding

    Simulation of Remanent, Transient, and Induced FORC Diagrams for Interacting Particles With Uniaxial, Cubic, and Hexagonal Anisotropy

    The diagnostic power of first‐order reversal curve (FORC) diagrams has recently been enhanced by an extended measurement protocol that yields three additional FORC‐like diagrams: the remanent (remFORC), induced (iFORC), and transient (tFORC) diagrams. Here, we present micromagnetic simulations using this extended protocol, including numerical predictions of remFORC, iFORC, and tFORC signatures for particle ensembles relevant to rock magnetism. Simulations are presented for randomly packed single‐domain (SD) particles with uniaxial, cubic, and hexagonal anisotropy, and for chains of uniaxial SD particles. Noninteracting particles have zero tFORC, but distinct remFORC and iFORC signals, that provide enhanced discrimination between uniaxial, cubic, and hexagonal anisotropy types. Increasing interactions lessen the ability to discriminate between uniaxial and cubic anisotropy but reproduces a change in the pattern of positive and negative iFORC signals observed for SD‐dominated versus vortex‐dominated samples. Interactions in SD particles lead to the emergence of a bi‐lobate tFORC distribution, which is related to formation of flux‐closure in super‐vortex states. A predicted iFORC signal associated with collapsed chains is observed in experimental data and may aid magnetofossil identification in sediments. Asymmetric FORC and FORC‐like distributions for hexagonal anisotropy are explained by the availability of multiple easy axes within the basal plane. A transition to uniaxial switching occurs below a critical value of the out‐of‐plane/in‐plane anisotropy ratio, which may allow FORC diagrams to provide insight into the stress state of hexagonal minerals, such as hematite

    An Improved Algorithm for Unmixing First-Order Reversal Curve Diagrams Using Principal Component Analysis

    First‐order reversal curve (FORC) diagrams of synthetic binary mixtures with single‐domain, vortex state, and multidomain end‐members (EMs) were analyzed using principal component analysis (FORC‐PCA). Mixing proportions derived from FORC‐PCA are shown to deviate systematically from the known weight percent of EMs, which is caused by the lack of reversible magnetization contributions to the FORC distribution. The error in the mixing proportions can be corrected by applying PCA to the raw FORCs, rather than to the processed FORC diagram, thereby capturing both reversible and irreversible contributions to the signal. Here we develop a new practical implementation of the FORC‐PCA method that enables quantitative unmixing to be performed routinely on suites of FORC diagrams with up to four distinct EMs. The method provides access not only to the processed FORC diagram of each EM, but also to reconstructed FORCs, which enables objective criteria to be defined that aid identification of physically realistic EMs. We illustrate FORC‐PCA with examples of quantitative unmixing of magnetic components that will have widespread applicability in paleomagnetism and environmental magnetism.This work was supported financially by the Australian Research Council through grant DP160100805 and by the European Research Council under the European Union’s Seventh Framework Programme (FP/2007–2013)/ERC grant agreement 320750
