Improving the outcomes of foot and ankle surgery through the audit cycle: a case study
The direct costs of managing adverse outcomes from Australian health care are estimated to be $2 billion. The audit cycle is considered an important tool to assist in the preventive management of adverse outcomes.Australian guidelines for audit cycle design allow for comparison of data sets derived from similar surgical specialities. However a lack of data set standardisation inhibits meaningful comparisons of foot and ankle surgical audits. This research will assist development of a best practice model for auditing foot and ankle surgery. Data derived from this model will improve the safety and quality of foot and ankle surgery. The preliminary phase of this process is to identify and understand the attitudes and behaviours of how and why surgeons participate in the audit cycle. A descriptive embedded multiple case study research design is planned to provide an intense focus on a single phenomenon (the audit cycle) within its real life context (clinical governance). The measures to be included in the case study have been identified by the Balanced Patient Safety Measurement Framework. These include: audit and peer review activity, provider attitudes to patient safety, safety learning, action and performance. A purposive sample of 6 to 8 surgeons (units of analysis) from 3 to 4 specialities (cases) will undergo semi-structured interview. This will investigate: current audit tools and processes; attitudes; and behaviours of surgeons to the audit cycle. Similarities in and differences between the units of analysis will indicate which identified measures function as barriers or enablers of the audit cycle. Reliability and validity (external and construct) will be assessed using established methods for case studies. The descriptive embedded multiple case study will reveal how and why foot and ankle surgeons participate in the audit cycle. This will inform further research to improve the outcomes of foot and ankle surgery through development of an audit tool

The final design of a hypersonic, SCRAMjet research aircraft, which is to be dropped from a carrier plane, is considered. Topics such as propulsion systems, aerodynamics, component weight analysis, and aircraft design with waverider analyses are stressed with smaller emphasis placed on aircraft systems such as cockpit design and landing gear configurations. Propulsion systems include analysis of the turbofanramjet for acceleration to low hypersonic speed (Mach 6.0) and analysis of the SCRAMjets themselves to carry the aircraft to Mach 10.0. Both analyses include the use of liquid hydrogen as fuel. Inlet design for both propulsion systems is analyzed as well. Aerodynamic properties are found using empirical and theoretical formulas for lift and drag on delta-wing aircraft. Multiple deficiencies of mitochondrial DNA- and nuclear-encoded subunits of respiratory NADH dehydrogenase detected with peptide- and subunit-specific antibodies in mitochondrial myopathies
AbstractAntibodies have been raised against synthetic peptides corresponding to several computer-predicted epitopes of three mtDNA-encoded subunits, ND4, ND5 and ND6, of the human respiratory chain NADH dehydrogenase (Complex I). Antibodies were characterized by a sensitive immunoblotting assay using proteins from human skeletal muscle mitochondria and by immunoprecipitation of radio-labeled HeLa cell mitochondrial translation products. Only antibodies against two of six selected peptides of the ND4 subunit, i.e., the C-terminal peptide and an internal peptide close to the C-terminus, reacted in both assays with the subunit. Antibodies raised against an internal peptide close to the N-terminus of the ND5 subunit and antibodies raised against an internal epitope of the ND6 subunit also reacted in both the immunoblotting and immunoprecipitation assays. The antibodies described above and other Complex I subunit- or holoenzyme-specific antibodies were used to investigate the subunit deficiencies of the respiratory NADH dehydrogenase in the skeletal muscle of patients affected by mitochondrial myopathies associated with Complex I defects. The reduction in enzyme activity correlated in an immunoblot assay with a decrease of four mtDNA-encoded subunits of the enzyme, as well as with a decrease of other subunits of Complex I encoded in the nDNA. The present work provides the first evidence of a decrease in NADH dehydrogenase subunits encoded in the mitochondrial genome in myopathy patients
The pregnant man: race, difference and subjectivity in Alan Paton’s Kalahari writing
In South African imaginative writing and scholarly research, there is currently an extensive
and wide-ranging interest in the ‘Bushman’, either as a tragic figure of colonial history, as
a contested site of misrepresentation, or even as an exemplary model of environmental
consciousness. Writing and research about ‘Bushmen’ has not only become pervasive in
the academy, but also a site of controversy and theoretical contestation. It is in this context
that this paper investigates the meaning and significance of ‘Bushmen’ for Alan Paton, one
of South Africa’s most well-known writers. Paton’s writing is not usually associated with
‘Bushman’ studies, yet this article shows that the ‘Bushman’ became a highly charged and
ambivalent figure in his imagination. Paton’s problematic ideas are contextualised more
carefully by looking at the broader context of South African letters. The article initially
analyses Paton’s representation of ‘Bushmen’ in his Lost City of the Kalahari travel narrative
(1956, published in 2005. Pietermaritzburg: KZN Press), and also discusses unpublished
archival photographs. A study of the figure of the ‘Bushman’ throughout the entire
corpus of his writing, ranging from early journalism to late autobiography, allows us to
trace the shift of his views, enabling us to reflect not only on Paton’s thinking about racial
otherness, but also gauge the extent to which his encounter with the Kalahari Bushmen

Stringent doxycycline-dependent control of gene activities using an episomal one-vector system
Conditional expression systems are of pivotal importance for the dissection of complex biological phenomena. Here, we describe a novel EBV-derived episomally replicating plasmid (pRTS-1) that carries all the elements for conditional expression of a gene of interest via Tet regulation. The vector is characterized by (i) low background activity, (ii) high inducibility in the presence of doxycycline (Dox) and (iii) graded response to increasing concentrations of the inducer. The chicken beta actin promoter and an element of the murine immunoglobin heavy chain intron enhancer drive constitutive expression of a bicistronic expression cassette that encodes the highly Dox-sensitive reverse tetracycline controlled transactivator rtTA2(S)-M2 and a Tet repressor-KRAB fusion protein (tTS(KRAB)) (silencer) placed downstream of an internal ribosomal entry site. The gene of interest is expressed from the bidirectional promoter P(tet)bi-1 that allows simultaneous expression of two genes, of which one may be used as surrogate marker for the expression of the gene of interest. Tight down regulation is achieved through binding of the silencer tTS(KRAB) to P(tet)bi-1 in the absence of Dox. Addition of Dox releases repression and via binding of rtTA2(S)-M2 activates P(tet)bi-1
Discovery of the Binary Pulsar PSR B1259-63 in Very-High-Energy Gamma Rays around Periastron with H.E.S.S
We report the discovery of very-high-energy (VHE) gamma-ray emission of the
binary system PSR B1259-63/SS 2883 of a radio pulsar orbiting a massive,
luminous Be star in a highly eccentric orbit. The observations around the 2004
periastron passage of the pulsar were performed with the four 13 m Cherenkov
telescopes of the H.E.S.S. experiment, recently installed in Namibia and in
full operation since December 2003. Between February and June 2004, a gamma-ray
signal from the binary system was detected with a total significance above 13
sigma. The flux was found to vary significantly on timescales of days which
makes PSR B1259-63 the first variable galactic source of VHE gamma-rays
observed so far. Strong emission signals were observed in pre- and
post-periastron phases with a flux minimum around periastron, followed by a
gradual flux decrease in the months after. The measured time-averaged energy
spectrum above a mean threshold energy of 380 GeV can be fitted by a simple
power law F_0(E/1 TeV)^-Gamma with a photon index Gamma =
2.7+-0.2_stat+-0.2_sys and flux normalisation F_0 = (1.3+-0.1_stat+-0.3_sys)
10^-12 TeV^-1 cm^-2 s^-1. This detection of VHE gamma-rays provides unambiguous
evidence for particle acceleration to multi-TeV energies in the binary system.
In combination with coeval observations of the X-ray synchrotron emission by
the RXTE and INTEGRAL instruments, and assuming the VHE gamma-ray emission to
be produced by the inverse Compton mechanism, the magnetic field strength can

First detection of a VHE gamma-ray spectral maximum from a Cosmic source: H.E.S.S. discovery of the Vela X nebula
The Vela supernova remnant (SNR) is a complex region containing a number of
sources of non-thermal radiation. The inner section of this SNR, within 2
degrees of the pulsar PSR B0833-45, has been observed by the H.E.S.S. gamma-ray
atmospheric Cherenkov detector in 2004 and 2005. A strong signal is seen from
an extended region to the south of the pulsar, within an integration region of
radius 0.8 deg. around the position (RA = 08h 35m 00s, dec = -45 deg. 36'
J2000.0). The excess coincides with a region of hard X-ray emission seen by the
ROSAT and ASCA satellites. The observed energy spectrum of the source between
550 GeV and 65 TeV is well fit by a power law function with photon index = 1.45
+/- 0.09(stat) +/- 0.2(sys) and an exponential cutoff at an energy of 13.8 +/-
2.3(stat) +/- 4.1(sys) TeV. The integral flux above 1 TeV is (1.28 +/- 0.17
(stat) +/- 0.38(sys)) x 10^{-11} cm^{-2} s^{-1}. This result is the first clear
measurement of a peak in the spectral energy distribution from a VHE gamma-ray
source, likely related to inverse Compton emission. A fit of an Inverse Compton
model to the H.E.S.S. spectral energy distribution gives a total energy in
non-thermal electrons of ~2 x 10^{45} erg between 5 TeV and 100 TeV, assuming a
distance of 290 parsec to the pulsar. The best fit electron power law index is

Observations of Mkn 421 in 2004 with H.E.S.S. at large zenith angles
Mkn 421 was observed during a high flux state for nine nights in April and
May 2004 with the fully operational High Energy Stereoscopic System (H.E.S.S.)
in Namibia. The observations were carried out at zenith angles of
60--65, which result in an average energy threshold of 1.5 TeV
and a collection area reaching 2~km at 10~TeV. Roughly 7000 photons from
Mkn~421 were accumulated with an average gamma-ray rate of 8 photons/min. The
overall significance of the detection exceeds 100 standard deviations. The
light-curve of integrated fluxes above 2~TeV shows changes of the diurnal flux
up to a factor of 4.3. For nights of high flux, intra-night variability is
detected with a decay time of less than 1 hour. The time averaged energy
spectrum is curved and is well described by a power-law with a photon index
\egamm and an exponential cutoff at \ecut~TeV and an average integral flux
above 2~TeV of 3 Crab flux units. Significant variations of the spectral shape
are detected with a spectral hardening as the flux increases. Contemporaneous
multi-wavelength observations at lower energies (X-rays and gamma-rays above
~GeV) indicate smaller relative variability amplitudes than seen

Very high energy gamma rays from the direction of Sagittarius A*.
We report the detection of a point-like source of very high energy (VHE) -rays coincident within 1' of Sgr A *, obtained with the HESS array of Cherenkov telescopes. The -rays exhibit a power-law energy spectrum with a spectral index of and a flux above the 165 GeV threshold of m -2 s -1. The measured flux and spectrum differ substantially from recent results reported in particular by the CANGAROO collaboration
