Strukturierte Versorgung von Patient*innen mit atraumatischen Bauchschmerzen in der Notaufnahme
Background: Patients with atraumatic abdominal pain are common in the emergency department and have a relatively high hospital mortality, with a very wide spectrum of different causes. Rapid, goal-directed diagnosis is essential in this context.
Methods: In a Delphi process with representatives of different disciplines, a diagnostic treatment pathway was designed, which is called the Abdominal Pain Unit (APU).
Results: The treatment pathway was designed as an extended event process chain. Crucial decision points were specified using standard operating procedures.
Discussion: The APU treatment pathway establishes a consistent treatment structure for patients with atraumatic abdominal pain. It has the potential to improve the quality of care and reduce intrahospital mortality over the long term
Substance Use and Prevention Programs in Berlin's Party Scene: Results of the SuPrA-Study
Background: Berlin is internationally known for its nightlife. In a nation-wide and Europe-wide comparison, the use of legal and illegal substances is comparatively higher in Berlin than in other similar cities. However, few data exist about the drug use in the party scene. Objective: This study aims to assess the sociodemographic characteristics of Berlin's party scene and its patterns of substance use as well as expectations towards prevention in order to derive appropriate preventive measures. Methods: Using questionnaires, both online (n = 674) and in the field (n = 203), a total of 877 people of the Berlin party scene were interviewed. The questionnaires ascertained the demographic information of the participants and patterns of substance use in the scene. It also collected the demand for consulting services and personal assessments on the usefulness of prospective and existing prevention programs and offers. Results: The study participants were 29 years old (SD 7.5); 43% were female. Alcohol is the most common substance in the party scene, followed by cannabis, MDMA/Ecstasy, amphetamine, cocaine, and ketamine. In this particular cohort, methamphetamine and legal highs did not play a major role. The most demanded preventive measure was more education about drugs and the so called drug-checking. Conclusions: Prevention in this area is both needed and requested, and an expansion of the existing programs (e.g., by so far politically controversial drug-checking) should be considered
Gap-filling eddy covariance methane fluxes : Comparison of machine learning model predictions and uncertainties at FLUXNET-CH4 wetlands
Time series of wetland methane fluxes measured by eddy covariance require gap-filling to estimate daily, seasonal, and annual emissions. Gap-filling methane fluxes is challenging because of high variability and complex responses to multiple drivers. To date, there is no widely established gap-filling standard for wetland methane fluxes, with regards both to the best model algorithms and predictors. This study synthesizes results of different gap-filling methods systematically applied at 17 wetland sites spanning boreal to tropical regions and including all major wetland classes and two rice paddies. Procedures are proposed for: 1) creating realistic artificial gap scenarios, 2) training and evaluating gap-filling models without overstating performance, and 3) predicting halfhourly methane fluxes and annual emissions with realistic uncertainty estimates. Performance is compared between a conventional method (marginal distribution sampling) and four machine learning algorithms. The conventional method achieved similar median performance as the machine learning models but was worse than the best machine learning models and relatively insensitive to predictor choices. Of the machine learning models, decision tree algorithms performed the best in cross-validation experiments, even with a baseline predictor set, and artificial neural networks showed comparable performance when using all predictors. Soil temperature was frequently the most important predictor whilst water table depth was important at sites with substantial water table fluctuations, highlighting the value of data on wetland soil conditions. Raw gap-filling uncertainties from the machine learning models were underestimated and we propose a method to calibrate uncertainties to observations. The python code for model development, evaluation, and uncertainty estimation is publicly available. This study outlines a modular and robust machine learning workflow and makes recommendations for, and evaluates an improved baseline of, methane gap-filling models that can be implemented in multi-site syntheses or standardized products from regional and global flux networks (e.g., FLUXNET).Peer reviewe
A time-resolved proteomic and prognostic map of COVID-19
COVID-19 is highly variable in its clinical presentation, ranging from asymptomatic infection to severe organ damage and death. We characterized the time-dependent progression of the disease in 139 COVID-19 inpatients by measuring 86 accredited diagnostic parameters, such as blood cell counts and enzyme activities, as well as untargeted plasma proteomes at 687 sampling points. We report an initial spike in a systemic inflammatory response, which is gradually alleviated and followed by a protein signature indicative of tissue repair, metabolic reconstitution, and immunomodulation. We identify prognostic marker signatures for devising risk-adapted treatment strategies and use machine learning to classify therapeutic needs. We show that the machine learning models based on the proteome are transferable to an independent cohort. Our study presents a map linking routinely used clinical diagnostic parameters to plasma proteomes and their dynamics in an infectious disease

GWAS meta-analysis of over 29,000 people with epilepsy identifies 26 risk loci and subtype-specific genetic architecture
Epilepsy is a highly heritable disorder affecting over 50âmillion people worldwide, of which about one-third are resistant to current treatments. Here we report a multi-ancestry genome-wide association study including 29,944 cases, stratified into three broad categories and seven subtypes of epilepsy, and 52,538 controls. We identify 26 genome-wide significant loci, 19 of which are specific to genetic generalized epilepsy (GGE). We implicate 29 likely causal genes underlying these 26 loci. SNP-based heritability analyses show that common variants explain between 39.6% and 90% of genetic risk for GGE and its subtypes. Subtype analysis revealed markedly different genetic architectures between focal and generalized epilepsies. Gene-set analyses of GGE signals implicate synaptic processes in both excitatory and inhibitory neurons in the brain. Prioritized candidate genes overlap with monogenic epilepsy genes and with targets of current antiseizure medications. Finally, we leverage our results to identify alternate drugs with predicted efficacy if repurposed for epilepsy treatment
Risk, anxiety, search for information - patients with localized prostate cancer after their treatment decision
Einleitung: FĂŒr das lokal begrenzte Prostatakarzinom (LPCa) stehen
unterschiedliche Behandlungsstrategien zur VerfĂŒgung, die vom invasiven
Verfahren der Radikalen Prostatektomie (RP) bis zum observierenden Vorgehen
bei Active Surveillance (AS) reichen. Die krebsspezifische MortalitÀt
unterscheidet sich, laut aktuellen Studien, bei AS- und RP-Patienten nicht und
kann daher auch nicht das alleinige Zielkriterium zur Beurteilung der
Behandlungsstrategien sein. Auch die langfristige psychologische Situation von
Patienten ist von groĂer Bedeutung und sollte entsprechende Beachtung finden.
Hierbei spielen die Faktoren RisikoeinschÀtzung, Krankheitsangst und
Informiertheit eine SchlĂŒsselrolle. Das angenommene Risiko sowie die Angst vor
einer Erkrankung beeinflussen ganz wesentlich das Gesundheitsverhalten und die
LebensqualitÀt der Patienten, wÀhrend ausreichende Informiertheit die
Grundlage fĂŒr einen selbstbestimmten Umgang mit der Erkrankung bildet. Obwohl
AS und die RP sehr kontrÀre LPCa-Behandlungsstrategien sind, gibt es bisher
kaum Studien, welche die psychologische Situation von AS- und RP-Patienten in
der Zeit nach ihrer Therapieentscheidung vergleichen. Auch die wechselseitigen
ZusammenhÀnge zwischen RisikoeinschÀtzung, Krankheitsangst und Informiertheit
sind bei LPCa-Patienten weitestgehend unerforscht. Ziel der vorliegenden
Arbeit ist es, diese ForschungslĂŒcke zu schlieĂen. Methodik: Die vorliegende
Arbeit war Teil einer nicht-interventionellen, multizentrischen
Beobachtungsstudie. Sie basierte auf einer Stichprobe von 292 LPCa-Patienten
(nAS = 142, nRP = 150), deren Therapieentscheidung im Durchschnitt vier Jahre
zurĂŒck lag. Die Teilnehmer beantworteten Selbstbeobachtungsfragebögen zur
Erfassung von RisikoeinschÀtzung, Krankheitsangst, Informationsbedarf und
Informationsquellennutzung, sowie soziodemografischer und klinischer Daten.
Zur statistischen Auswertung kamen Mann-Whitney-U-Test, t-Test,
Rangkorrelation nach Spearman, chiÂČ-Test, sowie univariate Kovarianzanalyse
und Mediationsanalyse zum Einsatz. Ergebnisse: Es zeigte sich eine deutliche
ĂberschĂ€tzung des LPCa-MortalitĂ€tsrisikos sowohl durch AS-Patienten, als auch
im besonderen MaĂe, durch RP-Patienten. AS-Patienten schĂ€tzten das Risiko,
unter Active Surveillance an der Krankheit zu versterben, geringer ein als RP-
Patienten. Umgekehrt war die RisikoeinschĂ€tzung von RP-Patienten fĂŒr die
radikale Prostatektomie signifikant niedriger als die RisikoeinschÀtzung von
AS-Patienten. AS- und RP-Patienten zeigten keinen Unterschied in ihrer
Krankheitsangst. Gleichzeitig wurde ein Zusammenhang zwischen der EinschÀtzung
des LPCa-MortalitÀtsrisikos und der Krankheitsangst der Patienten deutlich.
Beide Gruppen zeigten einen hohen Informationsbedarf, wobei Patienten mit
einem höheren Informationsbedarf auch mehr unterschiedliche
Informationsquellen nutzten als Patienten mit niedrigem Informationsbedarf.
Schlussfolgerung: Die vorliegende Arbeit zeigt ZusammenhÀnge zwischen der
RisikoeinschÀtzung, der Krankheitsangst und der Informiertheit von LPCa-
Patienten. Insbesondere der Zusammenhang zwischen RisikoeinschÀtzung und
Krankheitsangst verdient Beachtung und gewinnt zusÀtzliche Relevanz angesichts
der deutlichen ĂberschĂ€tzung des LPCa-MortalitĂ€tsrisikos durch die Teilnehmer.
Die gewonnenen Daten liefern eine wissenschaftliche Basis fĂŒr die Entwicklung
von individualisierten Therapie- und Beratungskonzepten beim LPCa. Die sich
daraus ergebenden klinischen Implikationen werden in der Arbeit diskutiert.Background: For localized prostate cancer (LPCa), various treatment strategies
are available, ranging from the invasive method of radical prostatectomy (RP)
to the observational approach of active surveillance (AS). According to
current studies, the cancer-specific mortality does not differ in AS and RP
patients and therefore cannot be the sole target criterion for the assessment
of these treatment strategies. The long-term psychological situation of
patients is also of great importance and should be taken into consideration.
The factors of risk assessment, disease anxiety and information need play a
key role. The assumed risks, as well as the disease anxiety substantially
influence the health behavior and the quality of life of the patients, while
sufficient information provides the basis for a self-determined handling of
the illness. Although AS and RP are very contrary LPCa treatment strategies,
there are hardly any studies comparing the psychological situation of AS and
RP patients after their therapy decision. Also, the mutual correlations
between risk assessment, disease anxiety and information need are largely
unexplored in LPCa patients. It is the aim of this work is to close these
research gaps. Methods: This work was part of a non-interventional,
multicenter observational study. It was based on a sample of 292 LPCa patients
(nAS = 142, nRP = 150), who had an average of four years of therapy.
Participants responded to self-assessment questionnaires to evaluate risk
assessment, disease anxiety, information needs and information sources, as
well as socio-demographic and clinical data. For statistical analysis, the
Mann-Whitney-U-test, t-test, Spearman rank correlation, chiÂČ-test, as well as
univariate analysis of covariance and mediation analysis were used. Results:
There was a clear overestimation of the LPCa mortality risk by both AS
patients and, in particular, by RP patients. AS patients estimated the risk of
dying from the disease under Active Surveillance, lower than RP patients.
Conversely, the risk assessment of RP patients for radical prostatectomy was
significantly lower than the risk assessment of AS patients. AS and RP
patients showed no difference in their disease anxiety. At the same time, a
correlation between the assessment of the LPCa mortality risk and the disease
anxiety of the patients became clear. Both groups showed a high need for
information, whereby patients with a higher need for information also used
more different information sources than patients with low information
requirements. Conclusion: The present work shows correlations between the risk
assessment, the disease anxiety and the information need of LPCa patients. In
particular, the correlation between risk assessment and disease anxiety
deserves attention and is gaining additional relevance in view of the
significant overestimation of LPCa mortality risk by the participants. The
obtained data provide a scientific basis for the development of individualized
therapy and consultation concepts for LPCa. The resulting clinical
implications are discussed in the present work
