    Periodontal parameters and BANA test in patients with chronic renal failure undergoing hemodialysis

    Objectives: The aim of this study was to analyze the periodontal parameters of patients with chronic renal failure. Material and Methods: The periodontal status of 16 Brazilian patients aged 29 to 53 (41.7±7.2) years with chronic renal failure (CRF) and another matched group of 14 healthy controls with periodontitis was assessed clinically and microbiologically. Probing pocket depth (PPD), gingival recession (GR), dental plaque index (PLI), gingival index (GI), and dental calculus index (CI) were the clinical parameters recorded for the entire dentition (at least 19 teeth), while the anaerobic periodontopathogen colonization in four sites with the highest PPD was evaluated using the BANA test (“PerioScan”; Oral B). Results: The results for the CRF group and control group, respectively were: PPD: 1.77±0.32 and 2.65±0.53; GR: 0.58±0.56 and 0.51±0.36; PLI: 1.64±0.56 and 1.24±0.67; GI: 0.64±0.42 and 0.93±0.50; CI: 1.17±0.54 and 0.87±0.52. Comparison between groups using the “t” test revealed a significantly increased PPD (p<0.001) in the control group. Comparison of the other clincial parameters by the Mann-Whitney test showed differences only for PLI, which was significantly higher (p<0.05) in the CRF group. Spearman’s test applied to each group showed a positive correlation among all clinical parameters, except for GR (p<0.05). None of the groups showed any correlation between GR and GI, while a significant negative correlation between GR and PPD was observed for the CRF group. The percentage of BANA-positive sites was 35.9% for the CRF group and 35.7% for the control group. The BANA test correlated positively with PPD only in the control group and with GR only in the CRF group. Conclusions: In spite of a higher PLI and dense anaerobic microbial population even in shallow PPD, patients with CRF exhibited better periodontal conditions than periodontitis patients, which is an evidence of altered response to local irritants

    FlĂșor e alumĂ­nio em chĂĄs e bebidas Ă  base de chĂĄs

    OBJECTIVE: To evaluate fluoride and aluminum concentration in herbal, black, ready-to-drink, and imported teas available in Brazil considering the risks fluoride and aluminum pose to oral and general health, respectively. METHODS: One-hundred and seventy-seven samples of herbal and black tea, 11 types of imported tea and 21 samples of ready-to-drink tea were divided into four groups: I-herbal tea; II-Brazilian black tea (Camellia sinensis); III-imported tea (Camellia sinensis); IV-ready-to-drink tea-based beverages. Fluoride and aluminum were analyzed using ion-selective electrode and atomic absorption, respectively. RESULTS: Fluoride and aluminum levels in herbal teas were very low, but high amounts were found in black and ready-to-drink teas. Aluminum found in all samples analyzed can be considered safe to general health. However, considering 0.07 mg F/kg/day as the upper limit of fluoride intake with regard to undesirable dental fluorosis, some teas exceed the daily intake limit for children. CONCLUSIONS: Brazilian and imported teas made from Camellia sinensis as well as some tea-based beverages are sources of significant amounts of fluoride, and their intake may increase the risk of developing dental fluorosis.OBJETIVO: Avaliar as concentraçÔes de flĂșor e alumĂ­nio em chĂĄs brasileiros de ervas, preto e bebidas a base de chĂĄ, como em chĂĄs preto importados considerando seus riscos para, respectivamente, a saĂșde oral e geral. MÉTODOS: Foram analisadas 177 amostras de chĂĄ de ervas e chĂĄ preto, 11 tipos de chĂĄ preto importados e 21 amostras de bebidas Ă  base de chĂĄ. FlĂșor e alumĂ­nio foram determinados apĂłs infusĂŁo dos chĂĄs de ervas e pretos. As determinaçÔes de flĂșor e alumĂ­nio foram feitas por eletrodo especĂ­fico e absorção atĂŽmica, respectivamente. RESULTADOS: A quantidade de flĂșor e alumĂ­nio encontrada nos chĂĄs de erva foi muito baixa, mas foi alta nos chĂĄs preto e nas bebidas Ă  base de chĂĄ. A quantidade de alumĂ­nio em todas amostras analisadas pode ser considerada segura para a saĂșde geral. Entretanto, considerando 0,07 mg F/kg/dia como o limite superior de risco de fluorose esteticamente comprometedora, alguns chĂĄs preto e algumas bebidas Ă  base de chĂĄ podem por si sĂł submeter crianças a uma dose diĂĄria superior ao limite. CONCLUSÕES: ChĂĄs preto brasileiros e importados, assim como bebidas Ă  base de chĂĄ sĂŁo fontes de quantidades significantes de flĂșor e sua ingestĂŁo diĂĄria pode aumentar o risco de fluorose denta

    Formação de flĂșor fracamente ligado ao esmalte apĂłs a aplicação tĂłpica profissional de flĂșor in vitro

    Since the efficacy of topical fluoride products is related to the fluoride (F) availability and its reactivity with enamel, this study was conducted. The F concentration of the following materials was verified: I- acidulated phosphate fluoride (APF) gel (1.23% F), II- APF foam (1.23% F) and III- Varnish (2.26% F). Forty blocks of bovine enamel were divided into 4 groups and treated according to the materials described, being one of them used as control. Loosely bound fluoride (''CaF2'') was determined on enamel after extraction with 1.0M KOH and analyzed by ion-selective electrode. Total F concentration found in gel was 12,642, in foam 12,755 and in varnish 23,183 mg F/g. All products formed statistically higher amounts of ''CaF2'' on enamel compared to the control group (p ; 0.05). Thus, ''CaF2'' formation was not proportional to the total F content in the products, suggesting that the pH and the vehicle used are more important.Considerando que a eficĂĄcia dos produtos para aplicação tĂłpica profissional de flĂșor (gĂ©is, espumas e vernizes) estĂĄ relacionada com a reatividade do flĂșor (F) com o esmalte e sendo esta dependente da disponibilidade do F em cada produto, este estudo foi conduzido. A concentração de F nos seguintes produtos foi estudada: I - FlĂșor Fosfato Acidulado (FFA) gel (1,23% F), II - FFA espuma (1,23% F) e III- Verniz fluoretado (2,26% F). Foram confeccionados 40 blocos de esmalte bovino, tratados de acordo com os grupos descritos, sendo um deles utilizado como controle. O F fracamente ligado (''CaF2'') ao esmalte foi determinado apĂłs a extração com 1.0 M KOH e analisado em eletrodo especĂ­fico. A concentração de F encontrada no gel foi de 12.642, na espuma 12.755 e no verniz 23.183 mg F/g.     Effect of Discontinuation of Fluoride Intake from Water and Toothpaste on Urinary Excretion in Young Children

    As there is no homeostatic mechanism for maintaining circulating fluoride (F) in the human body, the concentration may decrease and increase again when intake is interrupted and re-started. The present study prospectively evaluated this process in children exposed to F intake from water and toothpaste, using F in urine as a biomarker. Eleven children from IbiĂĄ, Brazil (with sub-optimally fluoridated water supply) aged two to four years who regularly used fluoridated toothpaste (1,100 ppm F) took part in the study. Twenty-four-hour urine was collected at baseline (Day 0, F exposure from water and toothpaste) as well as after the interruption of fluoride intake from water and dentifrice (Days 1 to 28) (F interruption) and after fluoride intake from these sources had been re-established (Days 29 to 34) (F re-exposure). Urinary volume was measured, fluoride concentration was determined and the amount of fluoride excreted was calculated and expressed in mg F/day. Urinary fluoride excretion (UFE) during the periods of fluoride exposure, interruption and re-exposure was analyzed using the Wilcoxon test. Mean UFE was 0.25 mg F/day (SD: 0.15) at baseline, dropped to a mean of 0.14 mg F/day during F interruption (SD: 0.07; range: 0.11 to 0.17 mg F/day) and rose to 0.21 (SD: 0.09) and 0.19 (SD: 0.08) following F re-exposure. The difference between baseline UFE and the period of F interruption was statistically significant (p < 0.05), while the difference between baseline and the period of F re-exposure was non-significant (p > 0.05). The findings suggest that circulating F in the body of young children rapidly decreases in the first 24 hours and again increases very fast after discontinuation and re-exposure of F from water and toothpaste

    The Exopolysaccharide Matrix Modulates the Interaction between 3D Architecture and Virulence of a Mixed-Species Oral Biofilm

    Virulent biofilms are responsible for a range of infections, including oral diseases. All biofilms harbor a microbial-derived extracellular-matrix. The exopolysaccharides (EPS) formed on tooth-pellicle and bacterial surfaces provide binding sites for microorganisms; eventually the accumulated EPS enmeshes microbial cells. The metabolic activity of the bacteria within this matrix leads to acidification of the milieu. We explored the mechanisms through which the Streptococcus mutans-produced EPS-matrix modulates the three-dimensional (3D) architecture and the population shifts during morphogenesis of biofilms on a saliva-coated-apatitic surface using a mixed-bacterial species system. Concomitantly, we examined whether the matrix influences the development of pH-microenvironments within intact-biofilms using a novel 3D in situ pH-mapping technique. Data reveal that the production of the EPS-matrix helps to create spatial heterogeneities by forming an intricate network of exopolysaccharide-enmeshed bacterial-islets (microcolonies) through localized cell-to-matrix interactions. This complex 3D architecture creates compartmentalized acidic and EPS-rich microenvironments throughout the biofilm, which triggers the dominance of pathogenic S. mutans within a mixed-species system. The establishment of a 3D-matrix and EPS-enmeshed microcolonies were largely mediated by the S. mutans gtfB/gtfC genes, expression of which was enhanced in the presence of Actinomyces naeslundii and Streptococcus oralis. Acidic pockets were found only in the interiors of bacterial-islets that are protected by EPS, which impedes rapid neutralization by buffer (pH 7.0). As a result, regions of low pH (<5.5) were detected at specific locations along the surface of attachment. Resistance to chlorhexidine was enhanced in cells within EPS-microcolony complexes compared to those outside such structures within the biofilm. Our results illustrate the critical interaction between matrix architecture and pH heterogeneity in the 3D environment. The formation of structured acidic-microenvironments in close proximity to the apatite-surface is an essential factor associated with virulence in cariogenic-biofilms. These observations may have relevance beyond the mouth, as matrix is inherent to all biofilms
