Vascular endothelial growth factor (VEGF ) receptor expression correlates with histologic grade and stage of colorectal cancer
Background: Colorectal carcinoma (CRC) is the seventh-most common malignancy and is the main cause of death in Iraq. The incidence of this cancer has increased sharply after the invasion of Iraq in 2003. Aim: To estimate immunohistochemical expression of vascular endothelial growth factor (VEGF) in CRC in relation to other parameters, such as grade and stage of tumour. Methods: Formalin fixed, paraffin-embedded blocks from 52 patients (27 male and 25 female) with CRC were included in this study. A group of 22 patients with non-cancerous colonic tissues were included as a control group. Avidin-biotin complex method was employed for immunohistochemical detection of VEGF. Results: VEGF immuno-expression was positive in 51.9% of CRC, while it was 18.2% in the normal colonic tissue (p<0.05). VEGF immunostaining was positively correlated with grade of colonic malignancy (p<0.05). Conclusion: These findings provide further evidence for the role of VEGF in the carcinogenesis of CRC. However, VEGF could not be well correlated with stage of tumour and hence may be a poor prognostic parameter of state of malignancy of colonic carcinoma.Keywords: colorectal carcinoma; VEGF; immunohistochemistr

Myographic evidence of polymyositis and dermatomyositis in COVID-19 patients
Idiopathic inflammatory myopathies, commonly known as myositis, are a diverse group of disorders defined clinically by persistent muscle weakness and reduced muscle endurance, as well as inflammatory cell infiltrates inside the muscle tissue. Myositis as a complication of coronavirus disease 2019 (COVID-19) has been described in an increasing number of reports. An analytical and cross-sectional study was undertaken in Basrah to analyze nerve conduction studies (NCS) and electromyographic (EMG) data in a COVID-19-affected patient. During the evaluation of 2240 patients, three cases of myositis were reported among the COVID-19 population, two of them with new clinical and EMG evidence of inflammatory myositis after the onset of COVID-19 infection, and one patient had a history of polymyositis before the COVID-19 pandemic, but a relapse was triggered by COVID-19, resulting in respiratory failure and death. The study found that the prevalence of myositis among the COVID-19 population was equal to 0.22%, which is 44 times higher than the prevalence of myositis (0.005%) worldwide before the onset of COVID-19 (P < 0.001)
Core competencies of the halal executive from the practitioners’ perspective
Malaysia strives to accelerate the growth of its halal industry and attain its vision of being a global halal centre
for innovation, production, and commerce. In attaining the vision, human competency and capability building through
competent halal executives is important. It is in fact, one of the success factors in supporting comprehensive and future
development of the Malaysian halal industry. Prior research, however, has revealed issues regarding halal executives’
competency. To fill the gap, 16 semi-structured in-depth interviews were conducted with halal executives to identify the
core competencies that were most related to their job performance. The identification of core competencies could aid in the development of guidelines that can be used to inform halal policymakers. These identified core competencies could
also inform the industry about the recruitment of halal executives, their placement, training, and succession planning
The factors that influence the reading habit among Malaysian: a systematic literature review
Reading is an action that a person does to obtain a wider knowledge
through his or her experience and this process may lead that person into a very
eye-opening self-discovery. In Malaysia, reading habits rate is still at a lower
level, Malaysian on average read only two books per year. About 80% of
Malaysian university students are reluctant readers. They can read but choose
not to read. Therefore, the objective of this study is to identify the factors
affecting the reading habit among Malaysia. To answer the research question of
this study we performed a systematic literature review (SLR). We defined a
mapping study process adapted from existing guidelines to categorize and to
structure the research evidence that has been published in the area of reading
habits among Malaysian and world-wide. Twenty-Four papers were used in our
synthesis of evidence, and five factors were identified. The results of this SLR
showed the five factors that can affect the reading habits among Malaysian:
(1) Internet (2) Environmental influence (3) Lack of motivation (4) Peer pressure
(5) Entertainment. The findings of this SLR would be beneficial for understanding the needs of Malaysian towards the reading habit by looking at the
factors, that might be a measurement of reading habit environment in other
specific scopes
High Resistance of Plasmodium falciparum to Sulphadoxine/Pyrimethamine in Northern Tanzania and the Emergence of dhps Resistance Mutation at Codon 581
BACKGROUND: Sulphadoxine-pyrimethamine (SP) a widely used treatment for uncomplicated malaria and recommended for intermittent preventive treatment of malaria in pregnancy, is being investigated for intermittent preventive treatment of malaria in infants (IPTi). High levels of drug resistance to SP have been reported from north-eastern Tanzania associated with mutations in parasite genes. This study compared the in vivo efficacy of SP in symptomatic 6-59 month children with uncomplicated malaria and in asymptomatic 2-10 month old infants. METHODOLOGY AND PRINCIPAL FINDINGS: An open label single arm (SP) standard 28 day in vivo WHO antimalarial efficacy protocol was used in 6 to 59 months old symptomatic children and a modified protocol used in 2 to 10 months old asymptomatic infants. Enrolment was stopped early (87 in the symptomatic and 25 in the asymptomatic studies) due to the high failure rate. Molecular markers were examined for recrudescence, re-infection and markers of drug resistance and a review of literature of studies looking for the 581G dhps mutation was carried out. In symptomatic children PCR-corrected early treatment failure was 38.8% (95% CI 26.8-50.8) and total failures by day 28 were 82.2% (95% CI 72.5-92.0). There was no significant difference in treatment failures between asymptomatic and symptomatic children. 96% of samples carried parasites with mutations at codons 51, 59 and 108 in the dhfr gene and 63% carried a double mutation at codons 437 and 540. 55% carried a third mutation with the addition of a mutation at codon 581 in the dhps gene. This triple: triple haplotype maybe associated with earlier treatment failure. CONCLUSION: In northern Tanzania SP is a failed drug for treatment and its utility for prophylaxis is doubtful. The study found a new combination of parasite mutations that maybe associated with increased and earlier failure. TRIAL REGISTRATION: ClinicalTrials.gov NCT00361114

Pliocene-Quaternary crustal melting in central and northern Tibet and insights into crustal flow
There is considerable controversy over the nature of geophysically recognized low-velocity-high-conductivity zones (LV-HCZs) within the Tibetan crust, and their role in models for the development of the Tibetan Plateau. Here we report petrological and geochemical data on magmas erupted 4.7-0.3 Myr ago in central and northern Tibet, demonstrating that they were generated by partial melting of crustal rocks at temperatures of 700-1,050°C and pressures of 0.5-1.5 GPa. Thus Pliocene-Quaternary melting of crustal rocks occurred at depths of 15-50 km in areas where the LV-HCZs have been recognized. This provides new petrological evidence that the LV-HCZs are sources of partial melt. It is inferred that crustal melting played a key role in triggering crustal weakening and outward crustal flow in the expansion of the Tibetan Plateau
Does true Gleason pattern 3 merit its cancer descriptor?
Nearly five decades following its conception, the Gleason grading system remains a cornerstone in the prognostication and management of patients with prostate cancer. In the past few years, a debate has been growing whether Gleason score 3 + 3 = 6 prostate cancer is a clinically significant disease. Clinical, molecular and genetic research is addressing the question whether well characterized Gleason score 3 + 3 = 6 disease has the ability to affect the morbidity and quality of life of an individual in whom it is diagnosed. The consequences of treatment of Gleason score 3 + 3 = 6 disease are considerable; few men get through their treatments without sustaining some harm. Further modification of the classification of prostate cancer and dropping the label cancer for Gleason score 3 + 3 = 6 disease might be warranted
Grand Challenges in global eye health: a global prioritisation process using Delphi method
Background: We undertook a Grand Challenges in Global Eye Health prioritisation exercise to identify the key issues that must be addressed to improve eye health in the context of an ageing population, to eliminate persistent inequities in health-care access, and to mitigate widespread resource limitations. Methods: Drawing on methods used in previous Grand Challenges studies, we used a multi-step recruitment strategy to assemble a diverse panel of individuals from a range of disciplines relevant to global eye health from all regions globally to participate in a three-round, online, Delphi-like, prioritisation process to nominate and rank challenges in global eye health. Through this process, we developed both global and regional priority lists. Findings: Between Sept 1 and Dec 12, 2019, 470 individuals complete round 1 of the process, of whom 336 completed all three rounds (round 2 between Feb 26 and March 18, 2020, and round 3 between April 2 and April 25, 2020) 156 (46%) of 336 were women, 180 (54%) were men. The proportion of participants who worked in each region ranged from 104 (31%) in sub-Saharan Africa to 21 (6%) in central Europe, eastern Europe, and in central Asia. Of 85 unique challenges identified after round 1, 16 challenges were prioritised at the global level; six focused on detection and treatment of conditions (cataract, refractive error, glaucoma, diabetic retinopathy, services for children and screening for early detection), two focused on addressing shortages in human resource capacity, five on other health service and policy factors (including strengthening policies, integration, health information systems, and budget allocation), and three on improving access to care and promoting equity. Interpretation: This list of Grand Challenges serves as a starting point for immediate action by funders to guide investment in research and innovation in eye health. It challenges researchers, clinicians, and policy makers to build collaborations to address specific challenges. Northern Eurasia Future Initiative (NEFI): facing the challenges and pathways of global change in the 21st century
During the past several decades, the Earth system has changed significantly, especially across Northern Eurasia. Changes in the socio-economic conditions of the larger countries in the region have also resulted in a variety of regional environmental changes that can
have global consequences. The Northern Eurasia Future Initiative (NEFI) has been designed as an essential continuation of the Northern Eurasia Earth Science
Partnership Initiative (NEESPI), which was launched in 2004. NEESPI sought to elucidate all aspects of ongoing environmental change, to inform societies and, thus, to
better prepare societies for future developments. A key principle of NEFI is that these developments must now be secured through science-based strategies co-designed
with regional decision makers to lead their societies to prosperity in the face of environmental and institutional challenges. NEESPI scientific research, data, and
models have created a solid knowledge base to support the NEFI program. This paper presents the NEFI research vision consensus based on that knowledge. It provides the reader with samples of recent accomplishments in regional studies and formulates new NEFI science questions. To address these questions, nine research foci are identified and their selections are briefly justified. These foci include: warming of the Arctic; changing frequency, pattern, and intensity of extreme and inclement environmental conditions; retreat of the cryosphere; changes in terrestrial water cycles; changes in the biosphere; pressures on land-use; changes in infrastructure; societal actions in response to environmental change; and quantification of Northern Eurasia's role in the global Earth system. Powerful feedbacks between the Earth and human systems in Northern Eurasia (e.g., mega-fires, droughts, depletion of the cryosphere essential for water supply, retreat of sea ice) result from past and current human activities (e.g., large scale water withdrawals, land use and governance change) and
potentially restrict or provide new opportunities for future human activities. Therefore, we propose that Integrated Assessment Models are needed as the final stage of global
change assessment. The overarching goal of this NEFI modeling effort will enable evaluation of economic decisions in response to changing environmental conditions and justification of mitigation and adaptation efforts
