    Evaluation of Double Mesh Modification of Chevrel’s Technique in Management of Midline Incisional Hernia

    Background: Chevrel’s technique provides tensionfree repair of midline incisional hernia, but wide skin and subcutaneous dissection increases rate of complications. Here, we evaluate the double mesh modification of Chevrel’s technique in midline incisional hernia repair. Methods: 22 patients with midline incisional hernia underwent double mesh modification of Chevrel’s technique. After excision of hernial sac with minimal dissection of the skin and subcutaneous tissue, the anterior rectus sheath is incised on both sides to create medial flaps that are sutured toeach other. Both recti abdominis muscles were dissected off the posterior rectus sheath, opening the retrorectus space. Prolene mesh was fixed in the retrorectus space with prolene sutures, and tailored to cover the bare area of anterior surface of both recti muscles and fixed to the lateral flaps of the anterior rectus sheath with interrupted prolene sutures. Results: We observed no recurrences, no skin necrosis, two cases of seroma, one case of superficial wound infection and one case of temporarychronic pain. Conclusion: Double mesh modification of Chevrel technique is an efficient method for treatment of midline incisional hernia, with minimal surgical site occurrences.     CEMENT BRICKS BASED ON SHREDDED AUTOMOBILE TIRES

    The essential target of this is to make good, qualified bricks with economic background and to have several uses than ordinary brick that is utilized in construction of homes or any building. From these useful properties of those blocks are acoustic segregation and thermal insulation also with recovery of many wastes that don’t be benefit from them; create from them jobs to be reused and to make brick economically wise and to be applied for example the insulation. From those squandering, are the shredded used tires after being used and it can’t be used anymore. It was cleared that the ultimate percent that cement brick can withstand was 10%. After the test of acoustic and temperature measurements by EXTECH instrument, explored that automobile tire addition makes a decline in sound and temperature surrounding also decreased it. Which has a good effect on cement brick by enhancing the insulation of sound and thermal where sound insulation from 55 db to 42 db and make thermal insulation where temperature before is 37 oC and then after addition of 10 % which the best percent that cement brick can withstand; temperature becomes 26 oC which make an insulation of about 11 oC. In addition, that shredded used tires decreases cost of cement standard brick by 7.6 %

    The Impact of Antiepileptic Drugs on Thyroid Function in Children with Epilepsy: New Versus Old

     Abstract ObjectivesTo investigate the effects of traditional antiepileptic drugs (AEDs) versus newer AEDs on the thyroid hormone profile of children with epilepsy.Materials & MethodsA total of 80 children with epilepsy were included in this study and were divided into two groups. Group 1 included 40 children with epilepsy on traditional AEDs, and group 2 included 40 children with epilepsy on newer AEDs. Forty healthy children were also included as the control group (group 3). We analyzed the serum levels of free triiodothyronine (FT3), free thyroxine (FT4), and thyroid stimulating hormone (TSH).Results: Epileptic children treated with traditional drugs showed a statistically significant decrease in the serum fT4 and increase in TSH concentrations compared to the control group (P < 0.001). Whereas epileptic children treated with newer drugs showed no statistically significant changes in the serum fT3, fT4 and TSH concentrations compared to the control group Conclusions: Traditional AEDs have a significant impact in the thyroid hormone profile compared to the newer AEDs

    Abdominal aortic aneurysm is associated with a variant in low-density lipoprotein receptor-related protein 1

    Abdominal aortic aneurysm (AAA) is a common cause of morbidity and mortality and has a significant heritability. We carried out a genome-wide association discovery study of 1866 patients with AAA and 5435 controls and replication of promising signals (lead SNP with a p value < 1 × 10-5) in 2871 additional cases and 32,687 controls and performed further follow-up in 1491 AAA and 11,060 controls. In the discovery study, nine loci demonstrated association with AAA (p < 1 × 10-5). In the replication sample, the lead SNP at one of these loci, rs1466535, located within intron 1 of low-density-lipoprotein receptor-related protein 1 (LRP1) demonstrated significant association (p = 0.0042). We confirmed the association of rs1466535 and AAA in our follow-up study (p = 0.035). In a combined analysis (6228 AAA and 49182 controls), rs1466535 had a consistent effect size and direction in all sample sets (combined p = 4.52 × 10-10, odds ratio 1.15 [1.10-1.21]). No associations were seen for either rs1466535 or the 12q13.3 locus in independent association studies of coronary artery disease, blood pressure, diabetes, or hyperlipidaemia, suggesting that this locus is specific to AAA. Gene-expression studies demonstrated a trend toward increased LRP1 expression for the rs1466535 CC genotype in arterial tissues; there was a significant (p = 0.029) 1.19-fold (1.04-1.36) increase in LRP1 expression in CC homozygotes compared to TT homozygotes in aortic adventitia. Functional studies demonstrated that rs1466535 might alter a SREBP-1 binding site and influence enhancer activity at the locus. In conclusion, this study has identified a biologically plausible genetic variant associated specifically with AAA, and we suggest that this variant has a possible functional role in LRP1 expression

    The global burden of cancer attributable to risk factors, 2010-19 : a systematic analysis for the Global Burden of Disease Study 2019

    Background Understanding the magnitude of cancer burden attributable to potentially modifiable risk factors is crucial for development of effective prevention and mitigation strategies. We analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 to inform cancer control planning efforts globally. Methods The GBD 2019 comparative risk assessment framework was used to estimate cancer burden attributable to behavioural, environmental and occupational, and metabolic risk factors. A total of 82 risk-outcome pairs were included on the basis of the World Cancer Research Fund criteria. Estimated cancer deaths and disability-adjusted life-years (DALYs) in 2019 and change in these measures between 2010 and 2019 are presented. Findings Globally, in 2019, the risk factors included in this analysis accounted for 4.45 million (95% uncertainty interval 4.01-4.94) deaths and 105 million (95.0-116) DALYs for both sexes combined, representing 44.4% (41.3-48.4) of all cancer deaths and 42.0% (39.1-45.6) of all DALYs. There were 2.88 million (2.60-3.18) risk-attributable cancer deaths in males (50.6% [47.8-54.1] of all male cancer deaths) and 1.58 million (1.36-1.84) risk-attributable cancer deaths in females (36.3% [32.5-41.3] of all female cancer deaths). The leading risk factors at the most detailed level globally for risk-attributable cancer deaths and DALYs in 2019 for both sexes combined were smoking, followed by alcohol use and high BMI. Risk-attributable cancer burden varied by world region and Socio-demographic Index (SDI), with smoking, unsafe sex, and alcohol use being the three leading risk factors for risk-attributable cancer DALYs in low SDI locations in 2019, whereas DALYs in high SDI locations mirrored the top three global risk factor rankings. From 2010 to 2019, global risk-attributable cancer deaths increased by 20.4% (12.6-28.4) and DALYs by 16.8% (8.8-25.0), with the greatest percentage increase in metabolic risks (34.7% [27.9-42.8] and 33.3% [25.8-42.0]). Interpretation The leading risk factors contributing to global cancer burden in 2019 were behavioural, whereas metabolic risk factors saw the largest increases between 2010 and 2019. Reducing exposure to these modifiable risk factors would decrease cancer mortality and DALY rates worldwide, and policies should be tailored appropriately to local cancer risk factor burden.     Contemporary retrospective analysis of acute coronary syndrome. An Egyptian study

    Background: Cardiovascular disease is the leading cause of death in Egypt and worldwide, placing great strain on the world’s health systems. This is one of the few Egyptian registries dealing with patients with acute coronary syndrome admitted in critical care department, Cairo University. Methods: This is a retrospective non-controlled cohort study of patients with acute coronary syndrome admitted from January 2010 to December 2012. Retrospective analysis of these patients data were retrieved through reviewing written paper and electronic database. Results: A total number of 503 patients were enrolled in our study. The mean age was 57.2 ± 10.4 years. Their pain duration was 14 ± 24.4 h. Average length of stay was (7 ± 4.4 days). Primary percutaneous coronary intervention (PCI) was done to 154 patients (30.6%), while we had 105 elective PCI procedures (20.9%). Major adverse cardiac events (MACE) were higher in patients with higher age (60 years vs 56.7 years P value 0.021), STEMI (25.7% vs. 18% in UA/NSTEMI P value 0.002), higher CKMB levels (157iu/l vs 89iu/l P value0.019), and higher Killip class upon presentation (class III-IV 64.9% vs 2.2% class I-II p < 0.001). Patients with UA/NSTEMI who were treated conservatively developed statistically significant higher incidence of MACE as compared to those treated interventionally (23.4% vs. 13.5% P value 0.031). Patients with STEMI who were treated without intervention have significant higher incidence of MACE than those who were treated interventionally (15.4% vs. 5.5% p = 0.46). Conclusion: 1. Higher incidence of MACE was observed in the higher age group, higher levels of cardiac biomarkers, and higher Killip class. 2. Outcome was affected by early interventional treatment in all patient groups

    Using thermal analysis techniques for identifying the flash point temperatures of some lubricant and base oils

    The flash point (FP) temperatures of some lubricant and base oils were measured according to ASTM D92 and ASTM D93. In addition, the thermal stability of the oils was studied using differential scanning calorimeter (DSC) and thermogravimetric analysis (TGA) under nitrogen atmosphere. The DSC results showed that the FP temperatures, for each oil, were found during the first decomposition step and the temperature at the peak of the first decomposition step was usually higher than FP temperatures. The TGA results indicated that the temperature at which 17.5% weigh loss take placed (T17.5%) was nearly identical with the FP temperature (±10 °C) that was measured according to ASTM D92. The deviation percentage between FP and T17.5% was in the range from −0.8% to 3.6%. Keywords: Flash point, TGA, DS

    APPLICATION OF THE OVERLAY WEIGHTED MODEL AND BOOLEAN LOGIC TO DETERMINE THE BEST LOCATIONS FOR ARTIFICIAL RECHARGE OF GROUNDWATER

    With population increase, lack of conventional fresh water resources and uncertainties due to climate change, there is growing interest in the arid and semi-arid areas to increase groundwater recharge with recycled water. Finding the best locations for artificial recharge of groundwater in such areas is one of the most crucial design steps to guarantee the long life and the sustainability of these projects. This study presents two ways to go about performing analysis; creating a suitability map to find out the suitability of every location on the map and another way is querying the created data sets to obtain a Boolean result of true or false map. These techniques have been applied on Sadat Industrial City which is located in a semi arid area in the western desert fringes of The Nile delta in the north west of Egypt. Thematic layers for number of parameters were prepared from some maps and satellite images and they have been classified, weighted and integrated in ArcGIS environment. By the means of the overlay weighted model in ArcGIS a suitability map which is classified into number of priority zones was obtained and it could be compared with the obtained true-false map of Boolean logic. Both methods suggested mostly the northern parts of the city for groundwater recharge; however the weighted model could give more accurate suitability map while Boolean logic suggested wider ranges of areas. This study recommends Boolean logic as a first estimator for locating the best locations as it is easier and not time consuming, while the overlay weighted model for more accurate results
